As described in the Korea and Chinese Pharmacopoeia, rhubarb consists of the roots and rhizomes of Rheum palmatum, R. officinale, and R. tanguticum. However, the pharmacological differences among rhubarb have not been scientifically established. In the present study, we investigated and compared the inhibitory effects of 70% ethanolic extracts from R. palmatum (RPE), R. officinale (ROE), R. nobile (RNE), and R. franzenbachii (RFE) on the production of inflammatory mediators, nitric oxide (NO), prostaglandin E 2 (PGE 2 ), interlukin-1beta (IL-1β), and tumor necrosis factor-alpha (TNFα) in RAW macrophage cells. ROE, RNE, and RFE significantly inhibited the release of NO, PGE 2, IL-1β, and TNF-α. RPE significantly reduced the release of IL-1β, but not NO, PGE 2, and TNF-α. Overall, RFE was found to inhibit the release of PGE 2 and IL-1β, to a far greater degree than RPE, ROE, and RNE. Our results indicate that RFE possess the strongest anti-inflammatory activity among 4 tested rhubarb. Key Words : Anti-inflammation; Inflammatory Mediators; Rheum franzenbachii; Rheum nobile; Rheum officinale; Rheum palmatum 서 염증은 조직의 손상을 비롯한 외부로부터의 자극 등 다양한 감염원에 대한 생체 내 방어 반응 중 하나로서 (Mariathasan and Monack, 2007; Zedler and Faist, 2006), 국소혈관과 다 양한 면역세포가 유기적인 상호작용을 하게 된다. 이 과정에 서 대식세포와 같은 염증세포들이 활성화되면서 nitric oxide (NO)와 prostaglandin (PG), interlukin-1beta (IL-1β), tumor necrosis factor-alpha (TNF-α) 등 다량의 염증 매개 인자를 분비하게 된다 (Guha and Mackman, 2001; Nathan, 1992). 이러한 염증 반응이 지속적으로 또는 과도하게 일어나면 과민 성 알러지 질환이나 만성 염증 질환, 순환기 장애, 암 등과 같은 다양한 질환의 원인을 제공하게 된다 (Gracie et al., 1999; Kinne et al., 2000). 대황 ( 大黃 )은 사하약( 瀉下藥 ) 중 공하약( 攻下藥 )에 속하는 한약으로, 사열통장( 瀉熱通腸 ) 양혈해독( 凉血解毒 ) 축어통 경( 逐瘀痛經 )하는 효능이 있으며, 실열변비( 實熱便秘 ) 습열 언 황달( 濕熱黃疸 ) 혈열토뉵( 血熱吐 ) 옹종정창( 癰腫瘡 ) 질타손상( 跌打損傷 ) 수화탕상( 水火傷 ) 등의 증상을 치료하 는데 쓰인다. 대한약전 (제9개정)과 중화인민공화국약전 (2005년판)은 모두 여뀌과 식물인 장엽대황( 掌葉大黃, Rheum palmatum L.), 약용대황( 藥用大黃, R. officinale Baill.), 탕 구트대황( 唐古特大黃, R. tanguticum Maxim. ex Balf.) 등 세 종류만 대황으로 수재하고 있다. 그런데 이 세 가지는 모두 국내에 자생하지 않는 식물이므로 전량을 중국으로부 터의 수입에 의존하고 있는 실정이며, 다년간 국내 재배실험 이 이루어져 왔으나 아직 성공적으로 재배가 이루어지지 않 고 있다. 일선 한방의료기관에서는 장엽대황에 대한 선호도가 가장 높은 것으로 알려져 있으나, 현행 품질검사 기준에 따르면 중 국산 장엽대황은 성분 함량 부적합 판정을 받는 경우가 많다. 또한 약용대황은 절단면에 대황 특유의 금문 ( 錦紋 )이 없어 저 급으로 인식되고 있으며, 약전에 수재되어 있지 않은 다른 대 황속 (Rheum) 식물의 뿌리가 성분 함량 적합 판정을 받고 유 Corresponding author: (Phone) ( ) hkkim@kiom.re.kr Received March 6, 2009 / Revised April 1, 2009 / Accepted April 9,

2 전명숙 윤태숙 최고야 김승주 이아영 문병철 추병길 김호경 통되는 일이 잦다. 따라서 정확한 기원종 정립과 효능이 우수 한 기원종을 확인하기 위해 동속근연종간의 효능 비교가 필요 하다. 따라서 본 연구에서는 공정서 기원종인 장엽대황과 약용대 황, 그리고 대황속에 속하는 동속근연종인 탑황 ( 塔黃, Rheum nobile Hook. f. et Thoms.)과 산대황 ( 山大黃, Rheum franzenbachii Muent.)의 항염증 효능을 비교함으로써 그 단초를 제공하고자 하였다. 이를 위해 염증 반응에 대한 장엽대황 (RPE)과, 약용대황 (ROE), 탑황 (RNE), 산대황 (RFE)의 70% 에탄올 추출물의 활성 비교 연구를 진행하였다. 이를 위 해 마우스 대식세포주인 RAW 세포에서 염증조절 인자 인 NO와 PGE 2, IL-1β, TNF-α 분비에 대한 RPE와 ROE, RNE, RFE의 억제 효과를 측정하였다. 재료 및 방법 1. 시료 및 추출 본 실험에 사용한 장엽대황과 약용대황, 탑황, 산대황은 중 국 간쑤 ( 甘肅 )와 스촨 ( 四川 )에서 구입하여 한국한의학연구 원 한약자원연구센터에서 검정하고 정선한 것을 사용하였다. 각각의 약재 200 g에 5배량의 70% 에탄올을 가한 다음 2시 간씩 2회 환류 냉각 추출하여 여과한 후 농축하고 동결 건 조하여 분말을 얻었다. 시료의 농도는 동결 건조 분말의 중 량을 기준으로 표시하였다. 수율은 장엽대황과 약용대황, 탑 황, 산대황의 수율은 각각 21.44%, 24.19%, 17.96%, 36.60% 였다. 2. 시약 Dulbecco's modified Eagle medium (DMEM)과, fetal bovine serum (FBS), penicillin-streptomycin은 Invitrogen Co. (Grand Island, 미국)에서 구입하였다. PGE 2, IL-1β, TNF-α 측정을 위한 ELISA kits와 NO 측정을 위한 Griess reagent는 R&D Systems (Minneapolis, 미국)에서 구입하였다. Lipopolysaccharide (LPS, Escherichia coli 0111: B4)와 3- (4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT), 그 외의 시약들은 Sigma (St. Louis, 미국)에서 구입하였다. 3. 세포배양 마우스 대식세포주인 RAW 세포를 American Type Culture Collection (ATCC, Rockville, 미국)로부터 구입하여 10% FBS, 100 units/ml penicillin, 100 μg/ml streptomycin을 첨가한 DMEM 배지로 37, 5% CO 2 환경에서 배양하였다. 4. 세포독성 시험 RPE와 ROE, RNE, RFE의 세포독성을 확인하고자 MTT assay를 실시하였다. 96-well plate에서 5 10 cells/well의 농 4 도로 배양된 RAW 세포에, LPS 1 μg/ml와 RPE 또는 ROE, RNE, RFE를 100 μg/ml로 처리하고 24시간 동안 배 양한 후, MTT를 최종농도 500 μg/ml이 되도록 각 well에 가 하고 추가로 1시간 동안 37 C에서 반응시켰다. 배양액을 모두 제거하고 DMSO 용액 1 ml을 첨가하여 생성된 formazan을 용해시킨 뒤, SpectraMax 340 reader (Molecular Devices, Silicon Valley, 미국)를 이용하여 570 nm에서의 흡광도를 측정 하고 대조군과의 비교를 통해 상대적인 세포생존율 (% of control)을 계산하였다. 5. NO 생성량 측정 96-well plate에서 5 10 cells/well의 농도로 배양된 4 RAW 세포에, LPS 1 μg/ml와 RPE 또는 ROE, RNE, RFE 를 100 μg/ml로 처리하고 20시간 동안 배양한 후, 배지에 분 비된 nitric oxide을 Griess reaction에 기초하여 측정하였다. 100% 활성은 LPS만 처리한 군의 분비량으로 정의하여 상대 적인 생성량 (% of control)을 계산하였다. 6. PGE 2, IL-1β, TNF-α 생성량 측정 96-well plate에서 5 10 cells/well의 농도로 배양된 4 RAW 세포에, LPS 1 μg/ml와 RPE 또는 ROE, RNE, RFE 를 100 μg/ml로 처리하고 20시간 동안 배양한 후, 배지에 분 비된 PGE 2 또는 IL-1β, TNF-α를 ELISA kits의 실험방법에 따라 측정하였다. 100% 활성은 LPS만 처리한 군의 분비량으 로 정의하여 상대적인 생성량 (% of control)을 계산하였다. 7. 통계처리 모든 측정 결과는 3반복 이상의 독립적인 실험에서 도출된 대표값의 평균 (mean)과 표준편차 (standard deviation; SD)로 나타내었다. 각 실험군 간의 차이는 Microsoft Office Excel의 Student's t-test를 사용하여 통계학적 분석을 수행하였으며, p < 0.05 값인 경우에 통계적으로 유의성이 있는 것으로 판정하 였다. 결 1. RAW264.7 세포에 대한 장엽대황과 약용대황, 탑황, 산대 황 추출물의 독성 RAW264.7 세포에 대한 RPE와 ROE, RNE, RFE의 독성 을 알아보기 위해, LPS 1 μg/ml로 활성화된 RAW 세 포에 RPE 또는 ROE, RNE, RFE를 100 μg/ml로 처리하여 24시간 후 세포생존율을 측정하였다. Fig. 1에서 나타난 것처 럼, RPE와 ROE, RNE, RFE는 100 μg/ml에서 대조군에 비 해 세포 생존율의 유의한 차이가 없었다. 과 110

NO는 배지 내에 축적된 nitrite를 통해 측정하 였다. ROE와 RNE, RFE는 NO의 분비량을 현저하게 억제한 반면, RPE는 NO 분비에 영향을 미치지 않았다 (Fig. 2). Fig. 3. ROE와 RNE, RFE는 PGE 2 의 분비량을 통계적으로 유 의하게 억제하였으나, RPE는 PGE 2 분비억제 효과를 보이지 않았다. 특히 RFE의 PGE 2 분비억제 효과는 매우 뛰어났으며, RPE와 ROE, RNE 3가지 추출물 모두에 비하여 탁월한 효능 을 나타내었다. 4. 장엽대황과 약용대황, 탑황, 산대황 추출물이 IL-1β 분비 에 미치는 영향 RPE와 ROE, RNE, RFE의 4가지 추출물 모두 LPS에 의 해 활성화된 RAW 세포의 IL-1β 분비를 현저하게 감 소시킨 것으로 나타났다. 특히 RFE의 IL-1β 분비억제 효과는 월등했으며, RPE와 ROE, RNE 3가지 추출물보다 IL-1β 분 비에 있어 현저하게 뛰어난 억제효과를 보이는 것으로 나타났 다 (Fig. 4) 5. 장엽대황과 약용대황, 탑황, 산대황 추출물이 TNF-α 분비 에 미치는 영향 RPE는, LPS에 의해 활성화된 RAW 세포가 분비하 는 TNF-α에 영향을 미치지 않았으나, ROE와 RNE, RFE는 유의한 TNF-α 분비억제 효과를 나타내었다. 특히, RFE는 RPE와 ROE보다 TNF-α 분비억제 효과가 뛰어난 것으로 나 타났다 (Fig. 5). 111

(RPE: 70% ethanolic extracts from R. palmatum; ROE: 70% ethanolic extracts from R. officinale; RNE: 70% ethanolic extracts from R. nobile; RFE: 70% ethanolic extracts from R. franzenbachii). 고 대황은 가장 널리 알려져 있는 약재 중 하나로, 변비, 염증, 암 등의 치료에 쓰이고 있으며 (Xio et al., 1984; Duke, 2002), 사하, 항산화, 항암 효과 뿐 아니라 (Miyamoto et al., 1967; Matsuda et al., 2001; Jang et al., 1997; Lee et al., 2002), 항염, 항균, 혈액순환 증진 등에도 효과적인 것으로 보 찰 고되었다 (Darias, 1987; Paik et al., 1998; Li et al., 2005; Kim et al., 2002). 대황속 ( 屬 )에는 약전수재종 이외에도, 탑황, 산대황, 하투대 황 ( 河套大黃, R. hotaoense), 장변대황 ( 藏邊大黃, R. emodi) 등 수십여 가지 종이 포함된다. 이러한 대황 유사종 중에서 탑황은 고산대황 ( 高山大黃 )의 뿌리줄기로서, 중국의 시장 ( 西藏 ) 등지에서 생산되며, 대황과 약효가 유사한 것으로 알려져 있다. 또한 산대황은 화북대황 ( 華北大黃 )으로 불리기도 하는데, 파엽대황 ( 波葉大黃 )의 뿌리줄기로서, 중국의 허베이 ( 河北 ) 산시 ( 山西 ) 네이멍구 ( 內蒙古 ) 등지에서 생산되며, 대황과 약효가 유사하나 사하력 ( 瀉下力 )은 없는 것으로 알려져 있다 (Xio et al., 1984). 현재 중국에서, 그 외형이 서로 유사하여 관능적으로 구분 이 어려운 여러 가지 대황 유사종이 사용되고 있으므로, 국내 에서도 정품 이외의 대황 유사종이 유통될 가능성이 높다. 따 라서 정품 대황과 대황 유사종 간의 약리학적 차이를 과학적 으로 규명할 필요가 있으며, 이에 본 연구에서는 LPS로 활성 화된 RAW 마우스 대식세포를 이용하여 염증 매개 인 자들의 분비에 대한 장엽대황과 약용대황, 탑황, 산대황의 억 제효과를 조사하여, 이들이 염증 반응에 미치는 영향을 비교 하고자 하였다. LPS는 그람음성 세균의 세포벽에서 분리된 생물학적 독소 로서 (Roth et al., 1997), 염증 매개 인자들을 분비하도록 대 식세포를 자극하는 대표적 성분 중 하나이다 (Fujiwara and Kobayshi, 2005; Kundu and Surh, 2005; Paul et al., 1999). 과량으로 분비된 NO는 염증반응에 있어서 중요한 역 할을 담당하며, PGE 2 와 IL-1β, TNF-α 분비는 염증 자극에 대한 면역반응에서 핵심적 위치를 차지한다 (McCann et al., 2005). 따라서, 항염증 생리활성 평가를 위해 LPS로 활성화된 RAW 마우스 대식세포가 널리 쓰이고 있다 (Shen et al., 2008; Yun et al., 2008; Lee and Cho, 2007). 본 연구에서는 장엽대황을 제외한 약용대황, 탑황, 산대황이 NO 및 PGE 2, IL-1β, TNF-α, 4가지의 모든 실험 인자의 분 비를 현저하게 억제함을 관찰할 수 있었다. 특히 산대황의 PGE 2 와 IL-1β에 대한 분비억제 효과는 매우 뛰어난 것으로 나타났으며, 이러한 억제 효과에는 PGE 2 생성을 조절하는 cyclooxygenase-2 (COX-2)와, IL-1β 생성을 조절하는 IL-1β converting enzyme의 발현 및 활성 수준이 연관되어 있을 것 으로 예상된다. 그러나 장엽대황의 경우, IL-1β 분비에 대해서 는 뚜렷한 억제효과를 보였으나, NO와 PGE 2, TNF-α에 대해 서는 전혀 억제효과를 나타내지 않았다. 예상과 달리 본 연구결과에서는, 동일하게 대황 약전수재종 에 속하는 장엽대황과 약용대황의 항염효과가 유사하지 않았 으며, 또한 대황 유사종인 산대황의 억제효과가 가장 뛰어난 것으로 나타났다. 장엽대황과 약용대황의 성분 비교에서 나타 112

5 4종 대황류의 항염증 효능 난 sennosides A 이성질체 타입과 anthraquinone glycosides 의 함량의 많은 차이에 의해 이러한 결과의 원인을 일부 설명 할 수 있을 것이다 (Ye et al., 2007). 또한 대황 약전수재종 은 대황 유사종에 비해 stilbenes 함량이 미미하며, 대황 유사 종 중에서도 특히 산대황의 stilbenes 함량이 매우 높다는 사 실 역시 산대황의 뛰어난 억제효과를 뒷받침하고 있다 (Ye et al., 2007). Stilbenes은 free anthraquinones와 함께, 항염 효 과를 나타내는 대황의 주요 활성 성분이다 (Xiao et al., 1984). Stilbenes 유도체가 COX-2의 효소 활성을 저해하고 (Ko et al., 2003), carrageenan 유발 족부종에 대한 억제효과 를 보인 연구결과를 통해 (Ko et al., 2004), 산대황의 염증 매개 인자 억제 효과가, 적어도 부분적으로는, 높은 stilbenes 함량에서 비롯된 것이라 짐작할 수 있다. 이상의 결과를 종합하면, 본 연구에 사용된 2종의 대황 약 전수재종인 장엽대황 및 약용대황과 대황 유사종인 탑황 및 산대황 중에서, 염증 매개 인자인 NO 및 PGE 2, IL-1β, TNF-α에 대해 산대황이 가장 뛰어난 분비억제 효과를 나타냈 으며, 따라서 산대황의 항염 효과가 가장 강력한 것으로 보여 진다. 본 연구는 염증반응에 대한 대황류의 억제 활성을 비교 한 첫 번째 보고이며, 따라서 본 연구에서 도출된 결과는 염 증 질환의 치료에 있어서 산대황이 가장 효과적으로 이용될 수 있음을 의미하는 것이다. 이는 향후 산대황이 염증억제제 개발에 응용될 수 있음을 암시한다. 그러나 더욱 명확한 비교 를 위해서는 대황 약전수재종 중 본 연구에 사용되지 않은 탕 구트대황에 대한 연구도 향후 필요할 것으로 보인다. 또한 산 대황의 항염 효과에 대한 작용 기전이나, 항염 효과를 나타내 는 유효 성분 등에 대한 추가적인 연구가 이루어져야 할 것으 로 사료된다. 감사의 글 본 연구논문은 한국한의학연구원 한의본초 활용기반 구축사 업 (L08020)의 지원으로 이루어졌으며, 이에 감사드립니다.
