Methods: Fifty female patients (ASA physical status 1, 2) scheduled for thyroidectomy (Group I) or mastectomy (Group II) received total intravenous anesthesia (TIVA) with propofol and remifentanil. The anesthesia was maintained with target-controlled infusion (TCI) of propofol and remifentanil under bispectral index (BIS) monitoring. The effect site concentration of propofol was controlled for maintaining BIS values within the range of The effect site concentration of remifentanil was controlled for maintaining blood pressure and heart rate within 20% of baseline values. Blood pressure, heart rate, BIS, effect site concentration of propofol and remifentanil were measured before induction, during induction, at the beginning of operation, at the end of operation, and during recovery. The time from discontinuation of propofol to eye opening and regaining of orientation, total amount of propofol and remifentanil used, and the amount of drug per body weight kilogram per hour were investigated. Results: There were no significant differences between Group I and II on the amount of remifentanil used. Although the amount of propofol consumed per body weight kilogram per hour was larger in Group II, there were no significant differences of recovery index between Group I and II. Conclusions: There were no significant differences in the amount of propofol and remifentanil consumed and recovery index in thyroidectomy and mastectomy under TIVA. The larger amount of propofol consumed per body weight per hour in Group II was thought as a result of shorter operation time than Group I. 2 대한마취과학회지 : 제 53 권제 2 호 2007 단순히수술종류와절개의크기만으로수술적자극의경중도를예측하기는어려울것이다. Propofol과 remifentanil을이용한 TIVA의적용에있어수술종류에따른약물의소모량이나마취후각성에대한연구는아직보고된바없다. 이에저자들은근치적갑상선절제술과유방절제술에서 BIS (Bispectral Index System) 감시하에 propofol과 remifentanil을이용한목표농도조절주입마취시약물의소모량과회복지수에차이가있는지를비교해보았다. 대상및방법전신마취하에갑상선절제술혹은유방절제술을시행받을예정인미국마취과학회신체분류등급 1 또는 2에해당하는 20세이상여자환자 50명을대상으로하였다. 대상환자중고혈압이나심혈관계질환, 갑상선기능이비정상인경우, 에스트로겐호르몬치료를받는경우, 임산부, 신경계질환, 수면제나신경안정제의상습복용력이있는경우는대상에서제외하였다. 대상환자의나이, 체중, 신장, 체질량지수 (Body Mass Index, BMI) 는두군간유의한차이가없었고, 수술시간은갑상선절제술에서더길었다 (P = 0.002)(Table 1, 2). 갑상선절제술과유방절제술은각기동일한외과의사에의해시행되었다. 병원임상연구위원회의연구승인후, 모든환자들에게연구의목적과방법에대한충분한설명을하고동의를받았다. α = 0.05, β = 0.2, 연구의 power = 0.8에서적절한대상환자수는 18명이었으며컴퓨터를이용한난수표를이용하여여자환자중에서갑상선절제술을받는환자 25명과유방절제술을받는환자 25명을무작위선별하여 I군과 II군으로하였다. 모든대상환자들에게마취전투약으로 midazolam mg을근주하였고수술실로이동후혈압, 심전도, 맥박산소포화도계측기와 BIS 감시장치를부착하였다. 마취유도는 100% 산소 6 L/min로마스크흡입상태에서 목표농도조절주입기 (Orchestra R, Fresenius vial. France) 를이용하여 propofol 4μg/ml, remifentanil 1.5 ng/ml를효과처농도로설정하고투여를시작하였다. 구두명령에대한환자의반응이소실되면 rocuronium 0.6 mg/kg를투여한후안면마스크를통해 100% 산소로손환기를한후기관내삽관하였다. 마취유지는공기를혼합한흡입산소농도 50% 에서 BIS가 사이로유지되도록 propofol의효과처목표농도를 0.5μg/ml씩가감시켜조절하였고, 혈압및심박수의변화가마취전값의 20% 이내로유지되도록 remifentanil의효과처목표농도를 0.5 ng/ml씩가감조절하였다. 4) 수술이종료되기 5분전 propofol투여를중단하였고, remifentanil은마지막봉합이끝나면중단하였다. 마취유도전과마취유도시작 1분후, 기관내삽관직전, 기관내삽관 1분후, 피부절개직전, 피부절개직후, propofol 투여중단시점, 수술종료시점 (remifentanil 투여중단시점 ) 에각각혈압, 심박수, BIS를기록하였다. 환자의마취후회복지수는 propofol을중단한때부터구두명령에반응하여눈을뜨기까지의시간, 이름및장소와수술상황에대한지남력이회복되기까지의시간으로정하여관찰기록하였고활력징후를기록하였다. 수술중사용된 propofol과 remifentanil의총량과단위시간당체중 kg별투여량을조사하였다. 모든측정값은평균 ± 표준편차로표시하였다. 측정값들은 SPSS WIN version 12.0을이용하여 t 검정으로분석하였고, 평균동맥압, 심박수, BIS는반복측정분산분석과사후분석의 Tukey test를시행하였다. T0: before the induction of general anesthesia, T1: 1 minute after induction, T2: right before the endotracheal intubation, T3: 1 min after the endotracheal intubation, T4: right before the beginning of operation, T5: right after the beginning of operation, T6: discontinuation of propofol infusion, T7: the end of operation, T8: when the patient opens eyes, T9: when the patient regains orientation. 기관내삽관 1분후, 수술시작직전, 수술시작직후, propofol 투여중단시점, 수술종료시점, 구두명령에눈을뜰때, 지남력회복시점의평균동맥압, 심박수, 및 BIS도두군간유의한차이가없었다 (Fig. 1). 구두명령에반응을했을때의 BIS 평균은 I군에서 80 ± 5.9, II군에서 79.3 ± 6.9로두군간유의한차이가없었으며, 지남력회복시의 BIS도 I군에서 83.8 ± 7.4, II군에서 84.2 ± 4.3으로역시군간유의한차이가없었다. 분당체중 kg당사용량에있어 remifentanil은 I군에서 72.3 ± 19.2 ng/kg/min, II군에서 81.0 ± 28.8 ng/kg/min으로두군간차이가없었으나 propofol의사용량은 I군에서 ± 19.8μg/kg/min, II군에서 ± 26.3μg/kg/min으로 II군에서더많았다 (P = 0.034)(Fig. 2). Group II에서 propofol의단위시간별체중당소모량이더많았으나 propofol 투여를중단한후부터구두명령에눈을뜰때까지의시간은 563초, 지남력회복시간은 708초로 Group I의 463초, 638초와유의한차이가없었다. Propofol과 remifentanil의용량, 회복지수와환자의나이, Fig. 2. 4 대한마취과학회지 : 제 53 권제 2 호 2007 BMI 간에는유의한상관관계를나타내지않았다. 또한 propofol과 remifentanil의총사용량과회복지수사이에도유의한상관관계를보이지않았다. 고찰저자들은유방절제술이갑상선절제술에비해피부절개부위가크고피부및근육을많이견인하기때문에통증자극이더심하고따라서 remifentanil의소모량이더많을것으로예상하였다. 그러나본연구결과갑상선절제술보다근치적유방절제술에서체중당 propofol의소모량이많았으며 remifentanil 소모량은차이가없었고마취후회복지수에서도두군간유의한차이가없었다. 적정마취의깊이를유지하기위해서는혈역학적변화를일반적지표로삼아왔으나최근에는뇌파의시그널을이행시킨 power spectrum을이용하는 bispectral index system (BIS) 이많이사용되고있다. BIS는수술로인한통증자극에민감하며, 마취에서진통요소의부족을반영해주는것으로알려져있다. 5) 따라서 BIS를혈역학적반응과함께이용하여마취의적정깊이를조절하는경우가일반적이며본연구에서도마취깊이의측정에 BIS값과혈압및심박수를지표로하였다. 본연구에서는 BIS 수치가 사이로유지되도록 propofol의효과처목표농도를조절하였고, 혈압과심박수가마취전평균의 20% 이내로유지되도록 remifentanil의목표농도를조절하였다. Propofol에의한진정과기억상실효과는 BIS 값의변화와밀접한상관관계가있으며이관계는진통제에의해변하지않는것으로되어있으나, 통증자극이가해질경우엔 BIS 수치의증가가진통제에의해감소될수있다. 5-7) Propofol은또한수술중아편유사제의요구량을감소시킨다. Propofol은일반적으로진통효과가없는것으로알려져왔으나최근, 갓태어난쥐에서 GABA A 수용체를활성화시켜척수수준에서진통효과를나타내는것으로보고된바있다. 8) 따라서아편유사제의병용사용이마취의적정깊이측정을위해 BIS를사용할때혼란을일으킬가능성을배제할수는없다. Propofol과 remifentanil을이용한전정맥마취후의각성에는남녀간성별차이가있는것으로보고되고있다. 9) Propofol과 remifentanil 주입을중단했을때여성에서각성이더빠르고 9) EEG 감시하에시행한 TIVA에서여성의 propofol 소모량이남성보다많은것으로보고되었으며 10) 이러한차이는주로약력학적차이로설명되고있다. 11) Remifentanil의요구량또한남성에비해더많다는보고도있으 나 12) remifentanil 의약력학과약동학의성별에따른차이가 밝혀진바는없다. 11) 따라서본연구에서는성별차이에따 른영향을배제하기위해여자환자를대상으로하는수술종류를선택하였다. 본연구에서는유방절제술의경우에서단위시간및체중 kg 당 propofol의소모량이더많은결과를보였으며 propofol의소모량만을본다면유방절제술이갑상선절제술에비해수술적자극이더크다고생각할수있다. 그러나제통이주된작용인 remifentanil의용량에있어군간차이가없었으며 propofol이혈중치료농도범위내에서는제통효과가없다는점으로미루어본다면 13) 두수술군간통증정도의차이가유의하다고보기는어렵다. 이단위시간별체중당소모량은두군간수술시간의차이로인한결과로도생각된다. 즉효과처목표농도를올리기위해짧은시간내에많은유도량이투여되고이후로는목표농도가유지되는범위내에서유지량이낮게조절되므로수술시간이긴경우엔총사용량에대한단위시간및체중당투여량이수술시간이짧은경우보다더적은결과를얻게된다. 이러한결과로수술시간이길었던근치적갑상선절제술의경우가유방절제술에비해단위시간및체중당 propofol의용량이적었던것으로생각되며이러한점은본연구의결과해석에제한을두는요인이라할수있다. 본연구결과의해석에또다른제한점은갑상선수술의경우이다. Group I의경우갑상선호르몬수치에이상이있는환자는대상에서제외했지만수술도중수술적조작으로갑상선을자극해서호르몬분비가증가된다면이로인해혈역학적변화가생기고이에따라약물의용량을조절했을가능성이다. 그러나갑상선수술중호르몬이분비 될가능성은적다고하며 14) 본연구에서도두군간 remifentanil의용량은별차이를나타내지않은점으로미루어갑상선호르몬분비에의한혈역학적반응의가능성은배제될수있다. 대부분의약물이그렇듯 propofol 또한고령일수록그소모량이감소하고이에반해회복시간은연장된다. 15,16) 본연구대상환자들의연령이 20세에서 65세사이로다양하였으나평균연령은 40대였고군간연령의차이도없었다. 환자들을연령별로분류하여 propofol과 remifentanil의소모량과각성시간의비교에서도연령과약물소모량및각성시간사이에서상관관계를발견할수없었다. 결론적으로, propofol과 remifentanil을사용한 TIVA에서여자환자의근치적갑상선절제술과근치적유방절제술에있어약물의소모량과회복지수의차이는없었다. 그러나수술종류별자극의정도와그에따른약물소모를알기위해서는더다양한종류의수술을대상으로하는연구가필요할것으로생각된다. 172
