No. 11, pp , 2020 e-issn DOI : 부적응병사를위한인지행동치료및수용전념치료집단상담프로그램개발및효과검증 조주성 1, 최재광 2, 강요한 3, 유한별 4, 송원영 5* 1 건양대학교심리상담치료학과강사, 2 건양대학원상담심리학과박사과정, 3 건양대학원상담심리학과석사졸업, 4 건양대학원상담심리학과석사졸업, 5 건양대학교심리상담치료학과교수 Development and Effectiveness Verification of and Group Counseling Programs for Maladjusted Soldiers Ju Sung Cho 1, Jaegwang Choi 2, Yohan Kang 3, Han Byeol Yu 4, Wonyoung Song 5* 1 Lecturer, Dept. of Counseling & Psychotherapy, Konyang Univ. 2 Doctoral Student, Dept. of Counseling Psychology, Graduate School of Konyang Univ. 3 Graduate, Dept. of Counseling Psychology, Graduate School of Konyang Univ. 4 Graduate, Dept. of Counseling Psychology, Graduate School of Konyang Univ. 5 Professor, Dept. of Counseling & Psychotherapy, Konyang Univ. 요약본연구는국군장병들의부적응문제해결을위한집단상담프로그램을개발하고프로그램의효과를검증하기위해진행하였다. 정서및행동문제를경험하는군장병을위한 12 회기 및 프로그램을개발하고충남권소재육군부대장병을대상으로, 각 2 집단씩주 1 회프로그램을실시해효과성을검증하였다. 결과분석을위해사전 - 사후성인용행동평가척도 (ASR) 을시행하였고, 기술통계분석, 교차분석, 대응 t 검증, 독립 t 검증을실시하여분석하였다. 나타난결과는다음과같다. 첫째, 집단간사전동질검사결과두집단은인구통계학적특성과주요변인의특성이동질한것으로나타났다. 둘째, 사전 - 사후비교검증결과 집단과 집단모두문제행동총점및내재화, 외현화의변화가유의하였다. 셋째, 집단과 집단의사전 - 사후검사의차이는나타나지않았다. 본연구는군부대내부적응문제에개입한점, 두이론간의효과성을비교했다는점에서의미가있으며, 향후연구를위한방법론및연구대상확대를제안하였다. 주제어 : 심리교육, 군상담, 집단상담, 인지행동치료, 수용전념치료 Abstract This study was conducted to develop a group counseling program to solve the problem of maladjustment of military personnel and to verify the effectiveness of the program. The 12 session and program for military soldiers experiencing emotional and behavioral problems was developed and the effectiveness was verified by conducting a program once a week for soldiers in the army units based in South Chungcheong Province. Adult Self Report scale(asr) was implemented for pre-post test and analysis, and descriptive statistical analysis, cross-analysis, paired sample t-test, and independent sample t-test were performed to analyze the results. The results indicated are as follows: First, the preliminary homogeneity test between groups showed that the two groups had homogeneous demographic characteristics and the characteristics of the variables. Second, as a result of the pre-post test verification, both and groups showed significant changes in problem behavior totals, internalizations, and externalizations. Third, there was no difference between the pre-post-examination of the group and the group. This study is meaningful in that it was involved in the maladjustment problem within the military unit, and compared the effectiveness between the two theories, and proposed the study of control groups and further studies on more diverse and diverse subjects that were not implemented under the circumstances of the unit. 2 225 융합정보논문지제 10 권제 11 호 1. 서론 1.1 연구의필요성한국은징병제를실시하고있다. 헌법 39조는 모든국민은법률이정하는바에의하여국방의의무를진다 고하여, 모든국민들이국방의의무에참여해야함을의무하고있다. 반면군생활로인한스트레스는매우높은수준이다. 군실태조사에따르면일반장병의 22.7% 가군생활에부적응을호소하고있는것으로나타났다 [1]. 이러한부적응은군대의수직적구조와사회와의단절등생활전반의변화를경험하면서발생하여, 심해지면심리적정서적어려움을초래하며,[2] 극단적으로는군생활의질과개인의자아존중감이낮아져살인이나자살사건까지이르기도한다 [2-5] 국방부는자살을방지하고복무부적응을완화하고자그린캠프, 힐링캠프등의프로그램을운영중이다 [6, 7]. 하지만이러한프로그램을실시하여도수료후회복되어자대로복귀하는비율이지속적으로감소하고있는데다가프로그램참여자들이 효과가없다 고응답한비율이 효과가있다 고응답한비율보다 10% 이상더많은등한계점을드러내고있다. 해당프로그램의효과를증가시키기위하여이를보완하거나다른대안이필요한실정이다 [1, 8]. 대안으로는부적응병사들을따로격리하여진행하는캠프형식의프로그램이아닌정상적으로부대생활을하면서비일과시간에참여하는심리지원프로그램을제시할수있다. 정상적으로부대생활을하면서진행하기에군복무적응이라는측면에서보다나은효과가기대되기때문이다. 선행연구를통해보고된프로그램형태의집단상담은심리학을비롯한다양한이론을근거로개발되었다. 예를들어, 심리학이론에근거한프로그램으로는, 인지행동적분노관리프로그램 [9], 수용-전념치료기반 [10, 11], 멘탈휘트니스긍정심리프로그램 [12] 등이있었으며, 심리치료기법또는표준화된심리검사를적용한것으로는마음챙김명상프로그램 [13], MBTI를활용한집단상담프로그램 [14], 사이코드라마프로그램 [15] 등이있었다. 이외에도예술치료와관련하여점토중심미술치료 [16], 무용동작치료프로그램 [17] 역시수행되었다. 그러나이프로그램들이지속적으로군부대에서활용되는지의여부는불확실하다. 집단상담프로그램이군부대에서지속적으로적용 가능하려면두가지요소가필요하다. 첫째는검증된심리학이론에근거한프로그램이어야한다. 공식적으로사용가능한심리프로그램은그효과성이담보되어야할것이기때문이다. 기법들을단순히연결하여만들어진프로그램도몇번의프로그램에서효과성을발휘할수는있다. 그러나, 여러상황에서지속적으로효과성을검증할수있을지여부는검증된심리학이론이뒷받침하는프로그램에비해불투명하다하겠다. 두번째로는표준화된심리검사를통해서문제의완화가검증가능해야할것이다. 내적, 외적으로경험되는병사들의스트레스를측정하기위해서선행연구들에서사용한검사들은문제의심각도에대한국내기준의절단점이나와있지않아서사전사후검사외에병사들의문제심각도를알수없거나우울등특정요인에대해서만알수있었다. 병영에서경험하는문제는다양하므로국내에서표준화되고병사들의보다다양한내적, 외적스트레스행동수준을알수있는검사를활용하여검증하여야목적에부합하는프로그램인지알수있을것이다. 선행연구프로그램중에서근거가되는이론으로는인지행동치료 (Cognitive Behavioral Therapy; ), 수용전념치료 (Acceptance Commitment Therapy; ), 긍정심리학 (Positive Psychology) 정도가있었다. 이중에서도 와 프로그램은행동주의의 2 세대와 3세대프로그램으로, 미국심리학회 (American Psychological Association; APA) 의근거기반프로그램으로서도인정받고있다 [18]. 는정보처리과정의특정한오류와역기능적신념들을수정하도록고안된다양한기법들을사용하여바람직하지않은사고들을약화시키거나제거시키는것을목표로한다 [19]. 또한 는심리적고통을제거하려고노력하거나변화시키고통제하려는행동이오히려고통을증가시킬수있다는내용을바탕으로고통을기꺼이경험해보고수용함으로서의식이있는인간으로서현재의순간과더접촉할수있는능력을강화하고, 행동을변화시키거나지속하면서가치있는목적을달성할수있는능력즉심리적유연성을촉진하는것을목표로한다 [20]. 따라서본연구에서는인지행동치료와수용전념치료를기반으로부적응병사들을위한심리프로그램을실시하고효과를검증하고자하였다. 효과성검증에사용하는척도는국내에서표준화되고문제수준을파악할

3 부적응병사를위한인지행동치료및수용전념치료집단상담프로그램개발및효과검증 226 수있는검사를활용하여보다엄격하게검증하고자하였다. 1.2 연구목적이연구는인지행동치료와수용전념치료에기반한두프로그램이병사들의군부적응문제를완화하는지를검증하고, 두프로그램중어느프로그램의효과성이높은지를확인하는것에그목적이있다. 1) 병사의군부적응문제에대한인지행동치료기반프로그램의효과를파악한다. 2) 병사의군부적응문제에대한수용전념치료기반프로그램의효과를파악한다. 3) 인지행동치료기반프로그램과수용전념치료기반프로그램의효과의차이를확인한다. 2. 연구방법 2.1. 연구대상및절차이연구는충청남도소재군부대소속군장병 145 명을대상으로선별검사를실시하여점수에따라대상자 27명을선정하여실시하였다. 구체적인절차는다음과같다. 먼저지역보건소를통하여대상군부대소속군장병 145명에게표준화검사를실시하여병사들의심리적문제수준을확인하였으며, 해당검사결과를바탕으로심리지원프로그램대상자를선정하였다. 프로그램개발의객관성을확보하기위하여널리사용되고있는프로그램개발방법을활용하였다. 변창진 [21] 의프로그램개발모형을따라프로그램을개발하였다. 이과정에서 와 를이론적바탕으로하는총 12회기의프로그램을개발하였다. 이프로그램에서초기 5회기는두이론이공유하는프로그램이었으며나머지 7회기는각이론고유의프로그램이었다. 내용타당도의검증을위하여해당분야에서 15년이상의경력을가진임상심리전문가의검토를받았으며, 이에맞추어세부내용수정및보완이이루어졌다. 이후개발한프로그램을 2집단, 2집단으로나누어주 2회기씩총 6주간에걸처실시하였다. 사전-사후검사를실시하여프로그램의효과성을검증한뒤에 와 프로그램의효과성비교역시실시 하였다. 통계분석에는근무일정상집단에서중도탈락 하게된 2 명의자료를제외한총 25 명의자료가사용 되었다. 변창진의프로그램개발단계및그에해당하 는본연구에서의활동내용은 Table 1 에제시하였다 프로그램내용및실행 개발한프로그램은총 12 회기로, 초기 5 회기는두 이론이공유할수있는부분을담았다. 6 회기부터 12 회 기는각이론고유의프로그램으로구성되었다. 개발한 프로그램의내용은 Table 2 에제시하였다. 프로그램의진행은프로그램개발에참여하고훈련 을받은주진행자 1 명과보조진행자 1 명이함께진행 하였다. 진행자대비적합한인원을구성하기위하여 각이론별로 2 집단씩구성하였다. 프로그램은 1 집 단 7 명, 2 집단 7 명을대상으로실시하였으며, 프로 그램은 1 집단 8 명, 2 집단 5 명을대상으로실시하였다. Creating a 'Cognitive Conceptualization Chart' Understanding the concept of experience avoidance through the metaphors of 'window prison' and 'pink elephant'. practice of acceptance attitude 2.3 연구도구 성인행동평가척도자기보고용 (Adult Self Report, ASR) Achenbach와 Rescorla[22] 가개발한척도의한국어판을사용하였다. 이척도는자기보고식평가로개인문제행동총점과내재화 외현화점수를구분하여측정하는검사도구이며, 국내표준화점수에따라증상의수준과정도를확인할수있어이연구에적합하였다. 내재화척도에는불안 / 우울, 위축, 신체증상이있고, 외현화척도에는공격행동, 규칙위반, 과잉관심유도가있다. 특정증후군으로묶이지않는척도는기타척도로보고된다. 총 126문항이고지난 6개월을기준으로문제행동에대해 0점 ( 전혀해당되지않는다.), 1점 ( 가끔그렇거나그런편이다.), 2점 ( 자주그런일이있거나많이그렇다.) 으로평가하도록한다. 한국판 ASR의내적일치도 (Cronbach s α) 는.96이었다 Understanding ABC of cognitive therapy. argue against one's cognitive distortion by distinguishing between automatic thinking, middle belief, and core belief Searching automatic thogut about negative interpersonal realtionship. Inspection of obstructions and training techniques. building up hope Post-test 2.4 분석방법이연구는총 27명을대상으로프로그램을실시하였고, 프로그램진행중 2명의대상이근무환경으로인하여중도탈락하여총 25명으로부터수집된자료를분석하였다. 수집된자료는 SPSS 22를사용하여분석을진행하였다. 첫째, 각집단의인구통계학적특성을탐색하기위해기술통계분석을하였다. 둘째. 처치집단의동질성을검증하기위해 χ 2 -test와독립 t검증을실시하였다. 셋째, 각프로그램에따른집단내차이를검증하기위해대응 t검증을실시하였다. 마지막으로각집단에서나타난프로그램의효과를비교하기위해독립 t 검증을실시하였다. 3. 연구결과 3.1. 연구대상의인구통계학적특성 집단과 집단의인구통계학적특성을파악하고집단간사전동질성을확인하기위해 χ 2 -test를진행하였다. 분석한결과두집단에서나타난연령의범주는 19세-25세로나타났고, 두집단의연령간차이는유의하지않았다. 해당내용을 Table 3에표기하였다.

Comparision of difference between two groups variable problem behavior total Internalization Externalization group (N=25) pre-post M SD 논의 t p 1.05 NS 1.77 NS.99 NS 본연구는국군장병들의심리적부적응문제를해결 하기위한병영적용형태의집단상담프로그램을개발 하고그효과성을검증하는것을목적으로하였다. 연 구를진행하기위해선행연구를조사하여타당한이론 적배경을확인하고, 해당지역사회의보건소그리고 군부대의요구를반영하여각각 와 를기반으 로한군병사대상집단상담프로그램을구성하여사 전사후검사결과를분석하였다. 해당결과를바탕으로 프로그램의효과성을논의하면다음과같다. 먼저 집단결과, 문제행동총점 (t=4.71, p<.001), 내재화 (t=5.17, p<.001), 외현화 (t=3.03 p<.01) 수준을 유의미하게낮추었다. 이는인지행동치료적개입이우 울과불안, 신체화등의정신병리적특성에효과적이라 는선행연구를지지하는결과로볼수있다 [23-26]. 이 는인지행동치료가병사의군생활적응력향상에도움 이된다는선행연구와일치하는것이다 [9]. 또한, 집단프로그램시행결과, 사전사후비교 분석에서문제행동총점 (t=4.78, p<.001), 내재화 (t=3.70, p<.01), 외현화 (t=2.19, p<.05) 수준을유의미 하게낮춘것이확인되었다. 이는수용전념치료에기반 한개입이수용과탈융합, 가치와전념행동이우울, 불 안을해결하고 [27-30], 군사병의적응문제수준을감 소시킨다는선행연구결과와일치하는것이다 [10, 11]. 본연구의의의를살펴보면다음과같다. 본연구는 국방의의무를수행하면서스트레스를받는장병들의 심리적어려움에도움을주는프로그램이다. 입대후 개인은일반사회에비해상대적으로통제되거나사생

6 229 융합정보논문지제 10 권제 11 호 활이보장되지않은생활을하게되는데, 이러한생활은스트레스를야기하며개인의삶의질을낮추는요인이된다 [9, 31]. 전세계유일의분단국가에서국방의의무를다하기위하여시간을들이는청년장병들의심리적어려움을해결해주기위하여해당집단프로그램을적용하는것은도움이될것이다. 두번째, 심리적어려움을해결하기위해검증된이론을기반으로한프로그램을구성하고개입하며그효과를비교하였다는데의의가있다. 와 는모두실생활에서의효과가검증된증거기반치료들 (evidencebased therapy) 이다. 본연구는 와 를함께고려하였으며군장병들의내재화와외현화변인에서유의한치료적효과를검증하였다. 이러한과정은군장병들을대상으로진행되었던기존의, 그리고다양한예술치료등의단일화된하나의프로그램만을고려한선행연구들에서더나아가두프로그램을함께진행하고차이를비교하였다는데의의가있다 [13, 32]. 또한동일집단에각각행동주의의 2세대, 3세대에해당하는두이론을적용하고효과차이를분석한선구자적인연구라하겠다. 두이론모두효과성을나타내었으므로상황과환경에따라선택적으로적용할수있을것이다. 와 는각기다른접근으로볼수도있으나학자에따라서는이둘은매우유사한기법이며그차이는주로그초점이외현적주의인지내현적주의인지에대한것과해당이론가들의독특한용어사용이라지적하는경우도있다 [33]. 따라서현장에서는 5회기까지의공통부분을실시한후진행자의숙련도나이론적입장에따라이후부분을선택하여이프로그램을융통성있게활용할수있다. 세번째, 점차필요성이증가하고있는군병사들을대상으로한집단상담프로그램에대한연구였다는점이다. 2000년이후군부적응프로그램과관련된연구가수행되고는있지만그수가부족하며군장병들의부적응문제를해결하기위한더많은프로그램연구가요구되고있다 [33]. 본연구는이미발견된문제가있는장병들을찾아가서검사를제공한것이아니라, 스크리닝을통해심리적문제가있는장병들을찾아내어집단을제공했다는점에서더욱의미가있다하겠다. 이연구의한계점은다음과같다. 첫째, 특정부대의사병들을대상으로했다는점이다. 지역보건소에검사의뢰가들어온부대원 145명을대상으로선별검사를실 시한다음에, 그중최종적으로 25 명에대해서만프로그램 및통계분석을실시한것이다. 따라서계급, 부대, 지역별 차이에따라결과가달라질수있다. 둘째, 대기통제집단 을설정하지않았다는한계점을지닌다. 모집의대상이된 군부대의근무환경및인원관리의특수성으로인하여통 제집단을설정하지못하여차이를확인할수없었다. 5. 결론및제언 본연구를통해서내재화, 외현화문제를가진군장 병들의문제행동수준이 및 집단상담프로 그램을통하여유의미하게감소하는결과를확인할수 있었다. 해당프로그램을적용함으로써군생활부적응 문제를완화하는데도움이될것으로예상되며, 특히 다른두이론간효과비교를통하여더나은개입방법을 찾고자시도하였으며, 두이론모두유사한효과를가 지고있음을확인했다는점에서의미가크다. 추후에는 통제집단을설정한연구설계환경에서다양한군구성 원들을대상으로적용하여효과를검증하는연구가이 루어질것을제언한다.
