The current study investigated the changes in nnos, NO and NE upon the needle insertion at varying depths at the connecting point of three yin meridians of the hand. Methods : Needles were inserted into rats, on both left and right sides of the connecting point, including the LU7, HT5 and PC6 acupoints which are three yin meridians of the hand. After insertion, needles were retained for five minutes. Each acupuncture groups were treated acupuncture at each acupoint and at the depths of superficial, middle and deep layer. After the retention, blood was drawn via cardiac puncture, and tissues of each point near meridian vessel was extracted to examine the changes in the expression of nnos, NO and NE. Results : Compared with the superficial layer group, nnos production significantly decreased in the middle and deep layer at LU7 acupoint group and in the deep layer at HT5, PC6 acupoint group. The tissue NE decreased in the deep layer on PC6 acupoint and the plasma NE increased at the middle layer at LU7 acupoint group but decreased at the deep layer on at LU7 acupoint group. Conclusions : Acupuncture at connecting points of three yin meridians of the hand can regulate the activities of nnos, and NE. 2 신욱ㆍ이유미ㆍ이경인ㆍ최동희ㆍ김미래ㆍ나창수ㆍ김선민ㆍ표병식ㆍ윤대환 인 絡穴 은 靈樞經脈篇 에의하면十二正經에하나씩의絡穴이존재하며그이외에督脈, 任脈, 脾之大絡에각각하나씩의락혈이존재한다고하였다 4). NO는 L-arginine 이 nitric oxide synthase(nos) 에의해생성되며, NOS 는 neuronal NOS(nNOS), inducible NOS(iNOS), endothelial NOS(eNOS) 3가지로알려져있으며, 특히 nnos 는신경조직에서 NO를생성하며신경전달을주로담당하는것으로알려져있다 5). 이러한 NO는원형질막간 (plasma membrane) 을이동하여중추신경계와말초신경계및혈관계에작용하며, 이는평활근에작용하여피부, 피하근육등의말초혈관의긴장을증가시키는 norepinephrine 과관련성을가진다 6). 락혈에관한최신연구로는수삼양경락혈의깊이별침자가혈위및자침깊이에따른신경전달물질기전이다름을나타내었고 7), 光明配太衝에刺鍼이腦神經의抗細胞枯死및保護效果에영향을주고中風初期治療에效果가있으며 8), 열결, 여구에대한용량별 He-Ne 레이저침조사가고지혈증에서레이저파장대별로체중, 식이섭취량, 식이효율, 혈중지질함량, 간기능에효과를보이기도하였다 9). 또한외관전침이뇌부위에서측정된뇌파의네가지영역중 δ, θ, β파의전력스펙트럼평균에유의성이있는변화와외관에가한전침자극이사람의뇌활동에영향을주며 10), 공손-내관자침은스트레스상태의정상성인자율신경계에서미주신경을활성화시키고교감신경활동을억제함으로써스트레스로인해무너진자율신경계의균형을회복시키고 11), 풍륭전침이생체내산화억제에유효한것등여러보고가되었다 12). 또한, nnos 에대한기존연구에서는 Choi 13) 는침자가교감신경의흥분을억제하는효과를보고하였으며, LEE 등 14) 은족삼리침자가대뇌피질에서저하된 NOS 의활성도를조절함을보고하였다. 현재신경계와침자의상호관계에대한연구로는 Yeom 등 15) 은금연치료요법으로의침자극에대한신경학적기전을보고하였으며, 족궐음간경의火穴과水穴에대한침자의보사에따른신경활성 16) 이연구되었으나, 침자의중요요건중의하나인자입깊이에대한체계적인연구보고는미흡한실정이다. 이에수삼음경의락혈을대상으로락혈의깊이별침자시행이신경전달관여물질중의하나인 nnos, NO 및 NE의생성및발현에미치는영향을관찰하고자한다. 450 g, Samtaco, Korea) 을준비하여, 동신대학교실험윤리위원회승인을받았고이에따라사육하고처치하였다. 사육되는동안조명을통해낮과밤이 12시간간격으로교대되도록하였고, 21±2 o C, 50±10% 의항온항습이유지되는사육실에서사육하며물과사료는자유롭게섭취하도록하였다. 2. 실험군각군들은총 45마리의흰쥐를대상으로무작위로배정하였으며, 수태음폐경의絡穴인列缺에 superficial layer 에침자한군 (LU7-A, n=5), middle layer 에침자한군 (LU7-B, n=5), deep layer 에침자한군 (LU7-C, n=5), 수소음심경의絡穴인通里에 superficial layer 에침자한군 (HT5-A, n=5), middle layer 에침자한군 (HT5-B, n=5), deep layer 에침자한군 (HT5-C, n=5), 수궐음심포경의絡穴인內關에 superficial layer 에침자한군 (PC6-A, n=5), middle layer에침자한군 (PC6-B, n=5), deep layer에침자한군 (PC6-C, n=5) 등총 9개군으로분리하였으며, 사육단계부터호흡마취까지의모든과정을모두동일하게처치되었다. 3. 취혈및침자극手三陰經의絡穴에해당하는 3개혈위에대한흰쥐의침자는구등 17) 의기준을참고하여인체에상응하게취혈하였다. 수태음폐경의絡穴인列缺은 LU7 은 elbow joint 와 wrist joint 사이를 10등분하여 wrist join 위 1.5/10 되는부위로 radius 의 styloid process 내측에인체와상응되는곳으로취혈하였다. 수소음심경의絡穴인通里는신문혈위 1촌으로경거와평행한부위에, 수궐음심포경의絡穴인內關는대릉혈위 2촌으로 flexor carpi radialis 와 palmaris longus 사이에각각취혈하였다. 침자전에실험동물을 80% O 2 와 20% N가혼합된가스에 5% isoflurane(choongwae, Korea) 을이용하여흡입마취유도를한후 2% isoflurane 으로마취를계속유지시켰다. 마취를유지시킨후 superficial layer 군은피하 1 mm, middle layer 군은피하 3 mm, deep layer 군은피하 5 mm로각락혈좌우모두에수직으로자침하고, 보사법을시행하지않은상태에서 3분간유침하였다. 침자에사용된침은호침 ( mm, Dongbang, Korea) 이었으며, 13년침자를시행한경험이있는전문가 1인이시술하였다. 재료및방법 1. 동물실험동물은 Sprague dawley계 16주령의수컷흰쥐들 ( 혈장준비 3분간의유침과정이끝난후심장천자하여 6 ml의혈액을채혈하였다. 채혈된혈액은 K3 EDTA(Green VAC-tube EDTA K3 liquid 3.0 mm 13 75, Greencross, Korea) 를사용하였고 15,

3 수삼음경의깊이별침자 rpm에서 20분동안 4 o C에서원심분리 (Vision Scientific, Korea) 하여혈장을얻었으며, NE content 의분석을위해액화질소를사용하여급속냉동시킨후 70 o C에보관하였다. 5. 조직준비심장천자를통하여채혈한후각군들에해당하는手三陰經의경혈부위에서 deep layer 에해당하는두께로조직을적출한후 superficial layer, middle layer 및 deep layer의 3개부분으로나누었다. NO assay 를얻기위한조직은 1X Protease Cocktail inhibitor(bd, USA) 가포함된 NP40 lysis buffer 500 μl를넣어 homogenization(next Advance, USA) 하였다. 이 sample을 20 분간 ice상태에놓아둔후 12,000 rpm에서 20분동안 4 o C에서원심분리 (Eppendorf, Centrifuge 5415R, Germany) 한후 supernatants 을분리하였다. 이중 50 ml sample 을 NO assay 에이용하고남은 sample은 nnos assay에사용하였다. NE production 을얻기위한조직은해당경혈부위를포함하는조직을분리하여 0.1 M perchloric acid 0.5 ml와함께 homogenization 하였고, 이를 10,000 rpm에서 20분동안 4 o C에서원심분리 (Eppendorf, Centrifuge 5415R, Germany) 하여 supernatants 만분리하였다. 6. Assay of nnos 해당경혈부위에서조직절편을적출하여 bouin 용액에 6시간동안고정한후, 2.5% nitric acid 용액 (Duksan, Korea) 에서 10일동안질산액을교환하면서조직절편내의 bone 등에대한탈회 (decalcification) 과정을진행하였다. 파라핀고정을위해탈회된조직은 tissu processor(tissue-texii, Japan) 을사용하여조직탈수과정을거친후, microtome(pika, Japan) 을사용하여자침방향과수직으로 10 μm 두께로절편하였다. 조직절편을 2% BSA와 15% 의 Goat Serum 으로 3시간동안반응시킨후 Rabbit anti-nnos(invitrogen, USA), anti-inos(1:100, Abcam, England), anti-enos(1:200, Abcam, England) antibody를 0.05% Sodium azide 와 2% BSA 가함유된 PBS 로희석시켜조직을 3시간동안 4 o C 에서반응을시킨후 50 mm PBS 에세척하였고, Biotinylated universal Secondary Antibody(Quick Kit: Vector Laboratories, Burlingame, CA, USA) 에실온에서 10분동안반응시켰다. 그리고 50 mm PBS에세척하고 Streptavidin peroxidase preformed complex (Quick Kit: Vector Laboratories, Burlingame, CA, USA) 에 10 분동안반응시킨후다시 50 mm PBS 에세척하고조직을 3,3'-diaminobenzidine 발색용액 (Vector Laboratories, Burlingame, USA) 으로반응시킨후, 반응이끝난후조직을 PBS 로세척하 여 hematoxyline 으로대조염색후슬라이드를 dehydration 한후에 100배의광학현미경 (Eclipse 80i, Nikon, Japan) 으로관찰하였다. 염색된각각의 NOS-positive cell 은육안으로관찰하여연구자 2인이개수를 counting 하였다. 7. Assay of NO 경혈부위의조직내 NO 생성량을측정하기위하여 Griess 시약 (Sigma, USA) 을이용하여안정된마지막산물인 NO 2 와 NO 3 에대해서분석하였다. 조직의단백질시료 40 μl와 MOPS buffer(50 mm MOPS/1mM EDTA, ph 7.0) 45 μl 를혼합하고 reconstituted E.coli nitrate reductase 5 μl 를가한후 2 mm NADH 10 μl 를가하여 20분동안실온에서반응시킨후 50 μl Griess reagent A(1% sulfanilamide in 3N HCl) 와반응시키고, 50 μl Griess reagent B(0.1% N-(1-naphthyl) ethylenediamine, 2HCl) 와혼합하여실온에서 5분간반응시킨후 purple azo dye 가생성되면 20분이내에 Micro plate reader (Spectra max m2e, Moleculardevices, USA) 를이용하여 540 nm에서흡수율을측정하였다. 8. Assay of NE production Norepinephrine 분석은 LC-MS/MS를활용하였다 series LC(Agilent, USA) 와연결된 6410 MS(Agilent, USA) 를사용하며, positive electrospray ionization(esi) mode로분석을실시하고, column 은 YMC-Pack Pro C8( mm, 3 μm) 을사용하며, 이동상으로 5 mm ammonium acetate 및 0.1% formic acid가혼합된증류수 (A) 와 5 mm ammonium acetate 및 0.1% formic acid 가혼합된 methanol(b) 을 0.3 ml/min 의유속으로사용하였다. Norepinephrine 과 epinephrine 을검출하기위해 selected ion monitoring 조건으로각각 m/z 와 m/z 을사용하고 neubulizer 압력과 capillary voltage 는각각 20 psi와 4 kv로설정하였으며, gas flow 와온도는 10 l/min 과 320 o C로유지하였다. 각물질의함량은표준물질 0.125, 0.50, 2.5 ppm(μg/ml) 을각각 Table 1. 수삼음경의 깊이별 침자 layer군에 비해, LU7군에서는 middle layer군, deep layer군이, pinephrine 농도 변화에 미치는 영향을 관찰한 결과, LU7군의 HT5에서는 deep layer군이, PC6군에서는 deep layer군이 유의하 superficial layer는 0.86±0.15 mg/l, middle layer는 0.

08 mg/l을, PC6군의 superficial layer는 1.12±0.08 手三經絡穴의 깊이별 침자가 NO발현에 미치는 영향을 관찰한 mg/l, middle layer는 0.93±0.1 mg/l, deep layer는 0.47±0.05 결과, LU7군의 superficial layer는 0.14±0.018 μm, middle mg/l을 나타내었다.

6 신 욱ㆍ이유미ㆍ이경인ㆍ최동희ㆍ김미래ㆍ나창수ㆍ김선민ㆍ표병식ㆍ윤대환 deep layer는 0.043±0.002 mg/l을, PC6군의 superficial layer 2) 라 불린다. 經脈과 관계를 가지고 있어 本經의 病證을 主治하는 는 0.056±0.001 mg/l, middle layer는 0.051±0.002 mg/l, 作用과 經脈과 소속된 臟腑의 病證에도 반영된다. 혈위는 질병과 deep layer는 0.057±0.002 mg/l을 나타내었다. 각 군들 간의 비 치료 자극점으로 진단과 치료에 중요한 내용이다18). 교에서 superficial layer군에 비해, LU7군에서는 middle layer군 현재 經穴에 대한 연구는 피부전기특성, X-ray, CT, PET, 생체 이 유의하게 증가함을 나타내었고, deep layer군이 유의하게 감소 전기저항법, 발광자극과 가시화, 생리신호, 온도유발에 의한 등온 하였다(p 0.05, Fig. 5, 6). 분포, 방사선 동위원소 추적, 뇌파연구, 저저항 양도성 연구, 血壓과 心搏動의 관계, 주파수 측정, MF-BIA Analyze, 신경물질, 광학적 특성 등의 다양한 각도에서 연구가 활발하게 진행 중이며, 소리, 고 찰 빛, 전기, 열, 자기 등의 다양한 방법을 이용하여 증명하려고 노력하 였으며 갖가지 다양한 가설들을 제시하였으나, 오로지 경맥의 형상 經穴은 經絡의 氣가 體表에 통하는 곳이며, 인체 내부와 관련되 며, 十二經脈, 任脈 및 督脈에 존재하는 穴을 十四經穴 로 經穴 이 적 면에서 경락을 연구하고 있어서 구체적인 성과를 나타내지 못하 19) 고 있는 실정이다. 7 수삼음경의깊이별침자 NO 와 NOS, NE 의최신연구로는 NO 와 NO 를생성하는각 NOS 의특징과작용에대한기전을다양하게연구되고있으며뇌손상을통한연구도활발한추세이다 20). NE는뇌손상후행동복구프로세스에중요한역할을가지고있으며중추신경계기능회복에중요한분자이며신경전달물질로연구되고있다 21). 침자가 NOS, NO, NE 발현에미치는영향에관한연구로 Kim 22) 은五輸穴중에서陽經의經火穴의특성을통하여혈위의작용을신경전달과관련된지표를통하여해당혈위국소작용과전신적인작용을통하여관찰한결과, 신경전달계에미치는국소적인작용발현이발휘됨을보여주었고, 이의작용은전신적인작용과일정하게연관될수있음을제시하였으며, 또 Kim 23,24) 각각의혈위마다의특성이신경전달물질생성에다른결과를가져온다고유추해볼수있으며, 모든혈위에서 NOS 의증가와감소율이 NO 의생성에일정한영향을미치는것은아니라는것을관찰하였다. 이는기존연구에서는침자가신경전달물질의생성과관련된 nnos 에일정한영향을주는것으로나타났으며침자가 nnos 에미치는영향으로는 nnos 를증가시키는것으로나타났다. NO에대한기존연구에서 Choi 14) 는자침을통한각각의 nnos, enos, inos 의감소는 NO 생성의억제의결과를가져와교감신경의흥분을안정화시켜자율신경실조를완화시키는작용이있다고하였으며, Kim 22) 은혈위자극이주요하게 NO의기능발현을촉진하는작용이발휘됨을보았으며, NO의생성에관여하는 nnos, enos, inos 에일정한영향을미침을보았다. 이는기존연구에서는침자가신경전달물질의생성과관련된 NO에일정한영향을주는것으로나타났으며침자가 NO에미치는영향으로는 NO를감소시키거나아무변화를주지않는것으로사료된다. NE에대한기존연구에서 Choi 14) 와 Kim 22) 은자침후혈액에서의 norepinephrine 및 epinephrine 의변화가경혈의위치및자침시행과관련이있다고하였고, 혈위침자가국소적인 NO와전신적인 NE에미치는영향이혈위별로다르게발현될수있음을제시하였으며, 신경전달계에미치는국소적인작용발현이전신적인작용과일정하게연관될수있음을제시하였다. 이는기존연구에서는침자가신경전달물질의생성과관련된 NE에일정한영향을주는것으로나타났으며침자가 NE에미치는영향으로는조직내 NE 증가, Plasma내 NE를감소시키는것으로사료된다. 絡穴에대한깊이별침자가 NOS, NO, NE 발현에미치는영향에관한연구로 Lee 25) 의수삼양경과본연구의수삼음경연구를보았을때, nnos 발현변화는수삼양경혈이 superficial layer 군에 비해, SI7군에서는 middle layer 군, deep layer 군이유의하게증가한반면, 수삼음경혈은 superficial layer 군에비해, LU7 군에서는 middle layer군, deep layer군이, HT5에서는 deep layer군이, PC6 군에서는 deep layer 군이유의하게감소하였으며, NO 발현변화는수삼양경과수삼음경모두실험군들간의유의한변화를보이지않았다. 혈위조직내의 norepinephrine 농도변화는수삼양경혈이 superficial layer군에비해, LI6군에서는 middle layer군이, TE5군에서는 middle layer 군, deep layer 군이유의하게증가한반면, 수삼음경혈은각군의 superficial layer 에비해, PC6 군에서는 deep layer 군이유의하게감소하였고, 혈장의 norepinephrine 농도변화는수삼양경혈이각군들간의비교에서 superficial layer 군에비해, LI6군에서는 middle layer 군, deep layer 군이, SI7군에서는 deep layer 군이유의하게감소하였으며, 수삼음경혈은각군의 superficial layer 에비해, LU7 군에서는 middle layer 군이유의하게증가함을나타내었고, deep layer 군이유의하게감소하였다. 絡穴에대한깊이별침자가 nnos, NO, NE에미치는영향을관찰한결과수삼양경과수삼음경침자의깊이가심층면에서변화하는것과일치하였다. NOS 발현변화는수삼양경과수삼음경이중간및심층부위에서대조적인결과를나타냄으로양경과음경의특징적인차이인지, 족경에서도같은양상을보이는지다른기전이있는지차후심도있는연구가필요할것으로사료된다. NO발현변화는手三陽經, 手三陰經모든실험군들에서유의한변화를보이지않았다. 이는手經絡穴에서는 NO에유의한변화를주지않는것으로사료된다. 혈위조직내의 norepinephrine 변화에선 NOS 와같이대조적인결과를나타내었으며, NE의농도변화에대해서는심도있고꾸준한연구가필요하다고사료된다. 혈장의 norepinephrine 변화에선이는수삼양경과수삼음경이심층면에서감소한것과대체적으로일치하였다. 본연구를통해자침깊이와경혈의위치에따른 NO, nnos, NE 관련성에대해이론을제시하기에부족함을느꼈으나혈위및자침깊이에따른신경전달물질변화가다름을확인함으로차후연구를통해경혈조직의위치및깊이에따른 NO, nnos, NE의변화, 그리고자침후혈액에서의 NE의변화가경혈의위치및자침깊이와어떠한관련성이있으며, 경혈학적으로어떠한의미가있을것인지밝히며신경전달관여물질에및신경활성기전대한영향에대해정확한이론을제시할수있을것으로본다. 81
