연하곤란 (Dysphasia) 의치료 치료목표는독립적인식사수행과적절한영양섭취이다. 치료는식사전단계, 구강기, 인두기단계로나누어각단계의문제를정확히파악하여행해진다. 그에따른치료은크게 2가지으로분류된다. INDIRECT THERAPY VS. DIRECT THERAPY COMPENSATORY STRATEGIES VS.

1 연하곤란 (Dysphasia) 의치료 치료목표는독립적인식사수행과적절한영양섭취이다. 치료는식사전단계, 구강기, 인두기단계로나누어각단계의문제를정확히파악하여행해진다. 그에따른치료은크게 2가지으로분류된다. INDIRECT THERAPY VS. DIRECT THERAPY COMPENSATORY STRATEGIES VS. REHABILITATION TECHNIQUE A. INDIRECT THERAPY 음식물을사용하지않는으로서 oral motor ex, sensory stimulation, swallowing reflex를촉진시키는운동, vocal cord adduction ex, 여러가지삼킴에관여하는기능의향상을목적으로한다. B. DIRECT THERAPY 음식물을직접환자에게투여하여환자의삼키는능력을평가하고흡인 (aspiration) 의위험없이환자가적절한단계의음식을삼킬수있도록하는치료이다. 음식물의분류 - 점조성에따라 solid food - pureed food ( 반유동식-갈은음식 ) liqid food - thick( 크림스프 ) mechanical soft 다진음식 semithick( 토마토 ) chopped food 연한음식 thin 1, thick등걸쭉한음식을먼저시행 - 저작의어려움, reflex의지연시천천히흘러감 sensory저하시중량으로 pressure가하여 relex유도 1. 소량의음식물을 (1/3 teaspoon) 혀의중앙에놓는다. 3cc-5cc-10cc 2. 환자에게적절한자세와기법으로음식을삼키게한다. 이때후두상승을촉지하기위하여 2번째손가락은 mandible 바로아래에 3번째는 hyoid bone, 4번째,5번째는 thyroid bone의위, 아래에두어갑상연골과설골이상승하는것을느낀다. 이것의강도와속도로인두기에서의기능이평가될수있다. 3. 각단계에서의 transit time이측정될수있다 (OTT /PTT) ott: bolus형성하여식괴가구강뒤로이동, ant faucial arch에닿는순간-1초 ptt: ant faucial potch- upper esophageal sphinter 통과불수의적 1초. 4. 일반적으로 lateral sulci. under the tongue, base of tongue, hard palate에음식이남게되며 (oral pocketing) 이것은불충분한 oral transit skill로인해생긴다.

2 5. 충분히삼키게한후 (double swallow) 음질을평가하여 residual material이있는지와 aspiration의여부를평가한다. (valleculae pouth, pyriform sinus) *gurgly voice -지연된 swallow response의가능성과 larynx에음식물이모여있을가능성을배제할수없다. 6. Throat clearing과 coughing을유도한다. 심한기침, 사래등은흡인의증상- 중단한다. 1점조성음식물에서반응좋을경우여러가지단계의음식으로시도 (esp thin liqid alterative하게고형음식과액체를번갈아서시도 C. COMPENSATORY STRATEGIES 음식물이구강, 인두로이동하는을달리한것으로환자의자세조정, 음식물의점조성, 식괴의크기등을변경하여삼키게하는이다. 1. postural modification 1. upright position - 이의목적은식사시근긴장도를정상화함으로향상된운동조절능력과안면근육의기능, 턱과혀의운동성그리고 swallowing process를촉진시키기위함이다. 또한 aspiratrion에대한 potetial을최소화하기위함이다. 자세가불가능한환자를제외하고는음식을삼킬때다음과같이앉아있도록유도한다. 머리, 목, 몸통, 골반의정중위대칭유지양발은바닥에붙이고무릎과고관절은 90 굴곡한다. 턱은앞으로붙인자세에서고개는중앙에들고있어야한다 head control이어려운경우자세유지를위하여적용또한 smandible의운동성이떨어지는경우적용 2. chin tuck - 지연된삼킴반사로인하여음식물이반사가일어나기전에 valleculae에고여있고 aspiration을보이는경우, 이자세는 vallecular space를넓혀주고기도를좁혀주는효과를얻는다. 3. turn head to the affected side - unilateral phayngeal paresis의경우affecting side로고개를돌리면음식물이약한쪽의 valleculae나 pyriform sinus를통과하여그반대편으로가는효과를얻는다. 4. tilt head to the sound side - unilateral pharyngeal paresis나 lingual paresis를보이는경우고개를 stronger side로기울이게하여양호한쪽의 channel을이용하면효과적인경우가있다. 5. turn head to the affeced side with chin tuck - unilateral pharyngeal paresis나 paralysis, delayed swallowing reflex를함께보이는경우 affected side로고개를돌리게하고

3 동시에고개를앞으로숙이게하면 material 이양호한쪽으로천천히흘러드는효과가있다. 2. change in food consistency : 환자가안전하게삼킬수있는점도의음식물을평가하여삼키기를용이하게도와준다 3. change in the volume of the bolus : bolus의 size를조절하여안전한삼키기를유도한다. D. REHABILITATION TECHNIQUES 삼킴에관여하는기능의향상을목적으로한다. 1. Mendelsohn Maneuver - laryngeal elevation과 cricopharyngeal opening이저하된환자에게이를개선하기위해사용한다. 1. As you swallow, rest your hand on your neck and feel your adam's apple raise up and your throat squeeze together. 삼킬때손을목에대고후두가올라가는것과꽉조여지는느낌을느낀다. 2. Hold the voice box up for a couple of seconds during the swallow, by contracting the neck (and/or tongue) muscles. 다시삼킬때후두가올라간시점에서후두를꽉조이듯이하고그상태로몇초간유지한다 ( 소량의물이용 ). 이때손가락으로후두가올라가는것을아래로부터밀어주듯이하여올려주기도한다. 3. Relax the tongue and neck muscles. 4. Do steps 1-3, 10 times in a row, 4 times per day 2. Supraglottic swallow - mild reduced laryngeal elevation이있는환자에게삼키기전 ( 기도폐쇠법 ) 과삼키는동안 vocal fold를 close할목적으로시행 1. Take a breath in and hold it숨을들이마시고참았다가 2. Keep holding your breath while you swallow. 삼킨다 3. After you swallow, cough before your next swallow. 숨쉬기전에기침 4. If you have a tracheostomy tube, cover it while you swallow. 3. Supersupraglottic swallow - 삼키기를시작할때 laryngeal elevation을증가시켜 ( 특수기도폐쇠법 ) airway protection 시킨다.

4 1. Take a breath in and hold your breath very tightly. 최대한숨을들이마신상태에서숨을멈추고아래로누르듯이하면서가슴에단단히힘을준다이때두팔로위자를누르거나당기면효과적이다. 2. Keep holding your bearing down as you swallow. 숨을멈추고삼킨다. 3. Cough when you are finished. 기침후숨내쉬기 4. Effortful swallow-valleculae에남아있는 material을제거하기위하여 posterior tongue movement를강화하는. swallow, while... pushing harder, 천장에혀를밀어붙여단단히조이듯이삼킨다. squeezing harder. 5. Double swallow - residual material 의제거를목적으로음식물또는 wet swallow에이어서 dry swallow등반복하여 2-3회삼키는이다. 이것은단순하지만감소된설근수축, 감소된 laryngeal elevation, 인두근육의약수축이있는환자에적당하다. 6. Thermal tactile stimuation - swallow response의 speed증진을위한치료 1. Place laryngeal mirror, in ice for at least 10 secend to cool 2. Firmly stroke the faucial arches. 3. Do stimulation 4-5/day, 5-10min, 2-4weeks 7. Vocal cord adduction ex - vocal fold closure의증진 1. Say 'ah' several seconds as you push down with your arms on the seat of your chair. Bear your weight on your arms as you push. Repeat 10 times, making your voice as loud and as smooth as you can each time. 두팔로의자를누르면서몸을약간들어올리듯이하고 아 를가능한크게높이가같도록몇초동안지속하도록한다.10회반복 2. Say 'ah' for several seconds as you pull up hard on the seat of your chair with your arms. repeat this 10 times, making your voice as loud and smooth as you can each time. 두팔로의자를당기면서가능한크고일정한소리로 아 를발성. 10회반복 3. Take a deep breath and hold your breath tight. Then say 'ah' for several seconds in a chair, loud voice. Repeat this 10 times. 숨을깊게들이쉰상태에서숨을유지하고아래로누르듯이가슴에힘을준다. 몇초간크게 아 발성한다. 10회반복 4. Say 'ah-ah-ah' as loudly and clearly as you can. Repeat 10 times. 아-아-아-하고짧은발성을가능한크고또렷하게 10회 8. Oro- motor ex

5 1. Ex for facial expression. 2. Ex. for the lip 3. Ex for cheek control 4. Ex for jaw control 5. Ex to increase Sucking 6. Ex for head control 7. Ex for abnormal oral reflex * Bite reflex : tongue에 rubber seizure stick으로지속적인 pressure를가하고정중위자세에서턱을당겨반사를억제할수있다. * Tongue thrust : tongue retration muscle을위해 chin아래 pressure를가한다. tongue아래에 manual vibration을준다. * Gag reflex facilitation of hypoactive gag : 설압자, ice Q-tip 또는 laryngeal mirror 를이용해 upward/outward로 sofh palate arch를빠르게자극해준다. 만약구역반응이나타나지만 torerance가좋지않으면 ah' 소리를내어 soft palate의거상을촉진시킨다. (faucial arch) inhibition of hyperactive gag : 반사의정상화를위해손가락이나설압자를이용해혀의뒷부분을지긋이눌럿다가살짝뗀다. 환자가구역질을느낄때멈추고다시동작을반복한다. * Palate reflex : Gag reflex나 'ah' 소리를낼때 palate가올라가는지관찰한다. 만약이의정도가약하다면 ice Q-tip이나 laryngeal mirror를가지고 palate를 3-5회두드려주고 A-h-h-h 소리를내게한다. gag reflex가나타나면 soft palate를자극하는것을중지한다. 또다른은환자에게코를막게한후 blow 시킨다. 다양한 blow ex를이용한다. 9. Strengthening ex : * Larynx strengthening : diagonal, rotation pattern으로 head를돌려어깨위를쳐다본후반대편팔꿈치를보게한다. 이때 neck은 extension, rotation된다. 처음엔천천히시작하여나중엔혼자서할수있게한다. 이것은 head control 뿐아니라 larynx elevation의 strengthening을위해효과적이다. * Pharynx strengthening 10. respiratory and coughing ex 모든환자에게위의치료을적용하는것은아니다. 연하곤란환자의 severity scale (steefel 1981) severe : 음식섭취느 tude나 npo specialist에의주의깊게시도해보는정도 mod to severe : oral intake시도하나성공률이저조하고비일관적임 moderate : 시험적으로 tube feeding 중단하고 oral intake 시도조제된특정 diet로어느정도성공적인섭취가능. 감독이요구됨 mild-mod : 주어진점조성의음식에서는안전도가어느정도확정적임맑은액체나고체는어려움 mild : 일반식섭취가가능, 특정음식에서제한적임. 안전한을위하여일정을사용. 반드시감독하지않아도됨 minimal : 정상적인 diet 섭취가능, 감독필요하지않음. 그러나액체나고형물에서간혹기침

6 normal : 모든음식에완전한삼킴기능위의을적용하였으나나타날수있는합병증이있다. 주의를요함 Complication of dysphagia 1 Aspiration pneumonia 2 Malnutrition 3 Dehydration Aspiration : material이 airway로흘러들어가 vocal cord를통과함 *. 구강기에서혀의움직임이저조 bolus를형성하고 faucail arch까지보내기전에사래들림 *. 지연된삼킴반사로음식물이먼저 valleculae로떨어짐 during swallowing : reduced laryngeal closure * 삼키는동아지연된후두거상으로기도보호가되지않는경우 after swallowing : residue in valleculae or pyriform sinus * 삼킨후완젆삼켜지지않은그이기도로흘러들어감 Penetration : material이 airway로흘러들어가나 vocal cord를통과하지않음 Residue : swallowing후에도 material이 mouth 나 pharynx에남아있슴 Backflow : swallowing후 material이 esophagus로부터 pharynth로또는 nasal cavity로역류됨 Aspiration 의증상 1. 기침과재채기 2. 가래, 분비물이인후두강내증가됨 3. 음질의변화 (gurgly voice, wet voice) 4. 호흡패턴의변화 5. 지연되거나감소된삼킴반사 6. 입과코로음식물의역류 silent aspiration( 침묵성흡인 ) 무의식적흡인이란뜻으로 syptomatic aspiration과는반대의개념이다.. 즉흡인이되고있으나증상은전혀관찰되지않는경우를뜻한다.
