Clinicians have relied on published institutional experience for carotid duplex ultrasound interpretation. Duplex parameters for interpreting carotid artery stenosis severity consist of peak systolic velocity (PSV) and end-diastolic velocity of the internal carotid artery (ICA), the ratio of ICA PSV and end-diastolic velocity (EDV) to common carotid artery (CCA) PSV (PSV ICA/CCA) and EDV (EDV ICA/CCA). This study was performed to elucidate which duplex parameter can predict the severity of the carotid artery stenosis more accurately. Methods: The carotid duplex ultrasound and angiographic results of 121 carotid arteries were analyzed. Receiver operater characteristic (ROC) curves were used to compare PSV, EDV, and both ratios in detecting 50%, 60%, and 70% stenosis of the internal carotid artery. Results: The PSV ICA/CCA ratio can accurately detect 50%, 60%, and 70% stenosis of the internal carotid artery. To detect 50% angiographic stenosis, a PSV ICA/CCA ratio of 1.2 has a sensitivity of 83% and specificity of 77%. A PSV ICA/CCA ratio of 1.8 can detect 60% angiographic stenosis with a sensitivity of 80% and specificity of 79%. Finally, a PSV ICA/CCA ratio of 2.2 can detect 70% angiographic stenosis with a sensitivity and specificity of 70 and 89%, respectively. Conclusion: The PSV ICA/CCA ratio can be used for detecting internal carotid artery angiographic stenosis. PSV ICA/CCA ratios of 1.2, 1.8, and 2.2 are proper criteria for interpreting 50%, 60% and 70% stenosis, respectively. 2 Jin Hyun Joh, et al: Duplex Criteria for Carotid Artery Stenosis 109 가장 중요한 기준이 되고 있다. 협착정도를 알기 위해 시행하는 수술 전 영상검사로 혈관조영술, computed tomography 혈관조영술, magnetic resonance imaging 혈관조 영술, 초음파검사 등이 시행되고 있다. 초음파검사는 비 침습적으로 혈류역학적 정보와 해부학적 정보를 통해 협착정도를 측정할 수 있다는 장점이 있어 가장 일반적 으로 시행되고 있다(3,4). 협착정도를 파악하기 위한 초음파 인자로는 B 모드 영상과 peak systolic velocity (PSV), end-diastolic velocity (EDV), 내경동맥과 총경동맥의 PSV 비(peak systolic velocity ratio of internal carotid artery to common carotid artery, PSV ICA/CCA ratio)가 이용되고 있다. 협착정도의 판정 기준은 검사실마다 다양해서 표준화하기가 쉽지 않다. 따라서 2002년 San Francisco에서 개최된 Society of Radiologists for Ultrasound (SRU) 회의에서 표준화를 위해 제 시한 판정기준이 많은 검사실에서 이용되고 있다(4). 그 러나 이 판정기준을 모든 검사실에서 그대로 적용하기 에는 무리가 있다. 각 검사실에서는 자체의 초음파결과 를 표준검사법(gold standard)과 비교한 검증과정이 필요 하고, 이미 보고된 판정기준도 필요하다면 수정이 필요 하다(5-7). 본 연구의 목적은 본원에서 시행한 경동맥초음파검사 의 결과를 표준검사법인 혈관조영술 영상의 결과와 비 교하여 초음파 인자 중에서 가장 정확도가 높은 것을 찾 고, 50%, 60%, 70% 협착증을 시사하는 가장 적정한 판 정기준을 분석하는 것이다. 방 법 대상환자는 2009년 3월부터 2010년 2월까지 강동경희 대학교병원에 경동맥협착증 혹은 두개강 내 혈관질환으 로 입원하여 경동맥초음파검사와 혈관조영술을 동시에 시행한 환자를 대상으로 하였다. 총 61명의 환자에서 121 경동맥검사를 후향적으로 조사하였다. 경동맥초음파는 경동맥초음파검사 경력이 2년 이상인 숙련된 검사자와 registered vascular technologist (RVT) 자 격을 가진 혈관외과 의사에 의해 시행되었다. 검사는 본 원 경동맥초음파검사 지침에 의해 시행되었다. 총경동맥 은 기시부부터 경동맥 분기부까지 스캔하여 병변 유무 를 확인한 후, 병변이 관찰되면 병변부위와 병변의 전후 에서 도플러파형 분석을 통해 혈류속도를 측정하였고, 병변이 없는 경우에는 총경동맥의 기시부, 중간부, 경동 맥 분기부 2 cm 근위부에서 혈류속도를 측정하였다. 내 경동맥은 경동맥분기부부터 내경동맥 말단부까지 스캔 Fig. 1. 3 110 Korean J Vasc Endovasc Surg Vol. 27, No. 3, 2011 하여 병변이 없는 경우에는 내경동맥의 팽대부보다 원 위부의 3부위에서 혈류속도를 측정하였다. 외경동맥은 분지된 직후에서 혈류속도를 측정하였고, 척추동맥은 혈 류방향과 혈류속도를 측정하였다. 초음파인자로 PSV, EDV, PSV ICA/CCA ratio, EDV ICA/CCA ratio를 측정하였다. PSV ICA/CCA ratio, EDV ICA/CCA ratio에서 내경동맥의 혈류속도는 가장 측정값이 높은 것을 사용 하였고, 총경동맥의 경우에는 경동맥 분기부 2 cm 근위 부의 혈류속도를 사용하였다. 초음파검사 결과를 분석하기 위한 표준검사법은 혈관 조영술 영상을 이용하였다. 혈관조영술은 뇌혈관 중재적 시술을 담당하는 영상의학과 의사에 의해 시행되었다. 모든 혈관조영술 영상은 검사 방향에 따라 협착정도가 달라지는 오류를 피하기 위해 여러 방향 획득하였다. 협 착증의 정도는 North American Symptomatic Carotid Endarterectomy Trial (NASCET) 연구에서 이용되었던 방법으로 측정하였다(8,9). NASCET 연구의 협착정도는 {1-(병변 부위 내경동맥의 혈류개존 직경/병변보다 원위 부의 정상 내경동맥 직경)} 100 (%) 와 같이 측정한 것이다. 내경동맥의 50%, 60%, 70% 협착증의 적정한 판정기준 을 분석하기 위한 통계적 방법은 receiver operator characteristic (ROC) 곡선분석을 이용하였다. 각 협착증 정도에 대해 PSV, EDV, PSV ICA/CCA ratio, EDV ICA/CCA ratio 전체를 ROC 곡선분석을 통해 가장 정확한 초음파인자를 찾고, 찾아진 초음파인자에 대해 다시 ROC 곡선분석을 통해 가장 적합한 민감도와 특이도를 보이는 기준점을 분석 하였다. 결 50% 이상의 협착증을 가장 정확하게 예측하는 초음파 인자를 찾기 위한 ROC 곡선분석에서 PSV ICA/CCA ratio가 가장 우수한 예측도를 보였다(Fig. 1). PSV ICA/CCA ratio를 1.2로 설정했을 때, 민감도는 83%, 특이도는 77%로 가장 우수한 정확도를 보였다(Fig. 2). 60% 협착증의 정도를 가장 정확하게 예측할 수 있는 인자도 역시 PSV ICA/CCA ratio였다(fig. 3). 60% 이상의 협착증은 PSV ICA/CCA ratio를 1.8로 설정했을 때, 민감도는 80%, 특이도는 79%로 가장 정확도가 높았다(Fig. 4). 70% 협착증의 정도를 가장 정 확하게 예측할 수 있는 인자도 PSV ICA/CCA ratio였고(fig. 5), 2.2에서 민감도는 70%, 특이도는 89%로 가장 정확도 가 높았다(Fig. 6). 과 Fig. 3. PSV ICA/CCA = peak systolic velocity of internal carotid artery to common carotid artery. 고 찰 경동맥초음파는 수술 전 협착증의 정도를 파악하는데 많이 이용되는 검사법이다. 그렇지만 초음파가 검사자의 숙련도와 장비, 그리고 각 검사실마다 다른 검사지침을 가지고 있기 때문에 다양한 판정기준이 보고되고 있다 (10-13). 미국 내 많은 혈관검사실이 Intersocietal Commission for the Accreditation of Vascular Laboratories 인증을 획득한 후 경동맥초음파에 대해 비교적 정형화된 검사 법을 따르고 있지만, 여전히 다양한 검사지침이 존재하 고 있다. 이에 여러 분과의 초음파 전문가들이 경동맥초 음파의 표준검사법과 판정기준을 마련하기 위해 SRU의 주체로 2002년 San Francisco에서 회의를 가졌다. 이 회 의에서는 병변의 정도를 정상, 50% 미만, 50-69%, 70% 이상, 폐색 전, 완전폐색으로 구분하였다. 판정기준에 이 용된 초음파인자 주요인자로 내경동맥의 PSV와 B 모드 에서 50% 이상 혹은 이하의 협착정도를 추천하였다. 이 주요인자로 판정하기 어려운 경우에 부수적인 인자로 PSV ICA/CCA ratio와 내경동맥의 EDV가 제시되었다(4). 죽 상판이 관찰되지 않고, 내경동맥의 PSV가 125 cm/s 이하 일 때에는 정상으로 판독하고, 50% 미만의 협착정도와 내경동맥의 PSV가 125 cm/s 이하일 때에는 50 미만의 협착증, 내경동맥의 PSV가 cm/s이고 병변이 50% 이상의 협착소견을 보일 때에는 50-69%의 협착증으 로 판독할 것을 권장하고 있다. 70% 이상의 협착증은 내경동맥의 PSV가 230 cm/s 이상인 경우, 혈류가 관찰되 지 않고, 죽상판이 관찰된 경우는 완전폐색으로 판독할 것을 권장하고 있다. 그러나 AbuRahma 등(7)은 SRU에서 제시한 판독기준 에 수정이 필요하다고 보고하였다. 376예의 경동맥초음 파검사 결과를 혈관조영술과 비교하였다. 50% 미만의 협착증에 대한 기준은 민감도 88%, 특이도 99%로 정확 하고, 70% 이상의 협착증은 민감도 99%, 특이도 86%로 SRU에서 제시된 기준을 이용하는데 문제가 없었다. 그 러나 50-69% 협착증은 민감도 93%, 특이도 68%로 조사 되었다 % 협착증에 대한 PSV 기준을 cm/s 로 수정하면 민감도 94%, 특이도 92%, 정확도 92%로 증 가한다고 보고하였다. 따라서 SRU 판독기준에 대한 재 검증이 필요하다고 결론지었다. Shakhnovich 등(14)은 중 등도 혹은 고도의 경동맥협착증을 혈관조영술과 비교하 면서 각 혈관검사실에서 적용하고 있는 판독기준을 표 준검사법인 혈관조영술과 비교하여 검증해야 한다고 보 고하였다. Beach 등(15)은 각 검사실에서 검사를 시행할 때 설정한 도플러 각에 따른 차이가 있기 때문에 검사의

5 112 Korean J Vasc Endovasc Surg Vol. 27, No. 3, 2011 정확성을 높이기 위해서는 표준화된 검사법을 적용해야 한다고 보고하고 있다. 초음파결과에 대한 판독기준은 대부분 혈류속도에 의 존한다. 혈류속도는 혈관의 직경에 따라 차이가 있다. 서 양과 동양의 인종에 따른 경동맥직경의 차이 및 혈류속 도의 차이에 대한 보고는 찾을 수 없지만, 아시아 국가 에서 미국과 유럽에서 보고된 판정기준을 그대로 적용 하기에는 무리가 있다. 아시아 국가에서 발표된 보고를 보면, 2002년 대만의 Hwang 등(16)은 134명의 환자를 대 상으로 경동맥초음파검사와 혈관조영술 결과를 비교하 였다. 70% 이상의 협착증은 내경동맥의 PSV를 200 cm/s, EDV를 65 cm/s, PSV ICA/CCA ratio를 2.5로 적용했을 때 가 장 적정한 판정기준이 된다고 보고하였다. 싱가포르의 Chua 등(17)은 50%, 60%, 70% 협착증에 대해 PSV ICA/CCA ratio를 각각 1.5, 2.6, 3.1로 설정할 때 가장 적합하다고 보고하였다. 이와 같은 결과가 본 연구와 상이한 이유는 각 검사실마다 표준화되지 않은 검사방법을 적용하고 있기 때문으로 판단된다. 따라서 각 검사실마다 표준화 된 검사방법과 프로토콜이 필요하다. 협착정도의 구간은 여러 연구에서 다르게 설정하였다. University of Washington 연구에서는 정상, 1-15%, 16-49%, 50-79%, 80-99%, 완전폐색으로 구간을 설정하였다 (18). 다른 연구에서는 60% 이상과 이하, 혹은 70% 이상 과 이하로 구분하여 연구하였다(11,13). SRU에서는 정상, 50% 미만, 50-69%, 70% 이상, 폐색 전, 완전폐색으로 구 분하여 설정하였다(4). AbuRahma 등(10)은 30% 이상, 50% 이상, 60% 이상, 70% 이상으로 세분하여 연구하였 다. 본 연구에서는 50% 이상, 60% 이상, 70% 이상으로 구분하여 연구하였다. 이렇게 구분하여 연구한 이유는 경동맥내막절제술 혹은 스텐트삽입술 등 치료방법을 결 정할 때 지침을 주기 위해서였다. 2008년에 미국 혈관외 과학회에서 발표된 경동맥협착증에 대한 치료 지침에 의하면 유증상의 50% 이상의 협착증, 무증상의 60% 이 상의 협착증은 경동맥내막절제술의 적응증이 된다고 하 였다(19). 따라서 본 연구에서는 50% 이상과 60% 이상 으로 구분하여 연구하였고, 70% 이상의 협착증은 고도 협착증으로 구분하기 위해서 세분하였다. 본 연구에서는 PSV ICA/CCA ratio가 가장 우수한 판정기 준이었다. 다른 연구에서도 본 연구에서와 같이 PSV ICA/CCA ratio가 협착증을 가장 적정하게 판정할 수 있었다(20). 그렇지만 PSV ICA/CCA ratio가 판정에 많은 도움을 주지 못 하는 연구도 있다(21). PSV가 협착증의 정도를 판정하는 데 가장 적정하다는 연구도 있다(5,22). 이 외에도 EDV ratio가 의미 있는 초음파 인자라는 연구도 있다(6). 대부 분의 연구에서 협착증의 정도를 판정하는데 혈류속도에 의한 도플러인자가 가장 우수하고, B 모드 영상에 대한 중요성은 언급되지 않았다. 그러나 MacKenzie 등(23)의 연구에서는 B 모드 영상도 협착정도를 판정하는데 중요 한 요소라고 언급하고 있다. 이와 같이 각 연구마다 다 른 결과를 보여주는 이유는 표준화된 검사법이 없기 때 문으로 판단된다. 따라서 결과의 정확성을 높이기 위해 서는 협착정도를 판정할 때 하나의 초음파 인자로 판정 하는 것 보다는 여러 가지 초음파인자의 결과를 고려하 여 판정하는 것이 바람직하다. 또한 초음파가 자체가 가 지고 있는 한계점으로 검사자의 경험이나 숙련도에 따 라 다른 결과를 보인다는 점을 인식해야 한다(24). 결 결론적으로 두개강 외 경동맥협착증의 정도를 가장 정확하게 예측할 수 있는 초음파인자는 PSV ICA/CCA ratio 였고, 이 수치를 50% 이상의 협착증에서는 1.2, 60% 이 상의 협착증에서는 1.8, 70% 이상의 협착증에 대해서는 2.2로 설정했을 때 가장 정확도가 높았다. 론 REFERENCES 1) Faries PL, Chaer RA, Patel S, Lin SC, DeRubertis B, Kent KC.
