1 REVIEW ARTICLE Chemotherapy induced Cadiotoxicity 항암제 심독성 Division of Cardiology, Department of Internal Medicine, College of Medicine, The Catholic University of Korea, Seoul, Korea Ho-Joong Youn, M.D. 가톨릭대학교 의과대학 순환기내과 윤호중 서 론 암 발생률이 증가하면서 항암치료를 하는 환자들이 많이 증가 하고 있으며 이에 여러가지 항암제가 개발되어 현재 임상에서 쓰 이고 있다. 항암제의 기전에 따라 여러가지 부작용이 나타나게 되 는데 몇몇 항암제는 그 부작용으로 심독성을 나타내며 그 중 가장 유명한 약제는 doxorubicin, epirubicin 등으로, anthracycline계열의 약물들이다. 이러한 심독성은 치료 중에 발생되어 항 암제 투여 시 그 용량이나 투여 기간에 제한이 있어 치료성적을 낮추게 되고 또한, 항암치료 후 환자에게 심각한 후유증을 남길 수 있으므로 임상적으로 중요하다. 1) 항암제가 심독성을 일으키는 기전이 각각 다르고 또한 아직 밝혀지지 않은 부분이 많아서 심독 성의 예방에 큰 걸림돌이 되고 있다. 이 글에서는 심독성을 일으 키는 항암제와 그 약제의 심독성의 기전과 예방법에 대해 알아보 고자 한다. 1. Anthracyclines Anthracyclines에 대한 심독성은 비교적 널리 알려져있다. Anthracyclines는 심근병증, 울혈성 심부전증과 심전도 변화 (비 특이적인 ST-T 변화, QRS voltage의 감소, QT간격의 연장)를 유발할 수 있다. 조기에 심독성이 발생하는 경우는 치료 후 1년 이 내에 발생하지만 투약한지 20년이 지난 후에도 발생할 수 있다. 2) 적인 ST-T 변화나 QT간격의 연장과 같은 심전도 변화가 많으며 약의 투여를 중지하면 사라진다. 2) 만성 심독성 만성 심독성은 약의 투여 중지 이후에도 지속되는 것으로 심근 병증으로 인해 나타날 수 있는 모든 증상과 징후들이 관찰된다 (좌심실의 기능 저하, 운동능력 감소, 울혈성 심부전의 증상 등). 3) 3) 위험인자 누적 용량(cumulative dose), 나이, 방사선 치료의 과거력, 다른 항암 치료제와의 동시 투여, 기저 심질환 등이 중요한 위험 인자로 알려져 있다. 4) 이 위험인자 중에서 가장 중요한 위험인자 는 누적 용량으로, doxorubicin의 경우에는 450~550mg/m 2 이상이 투여되면 심근병증이나 울혈성 심부전이 빈번하게 발생한 다. 그러나 이 수치가 절대적인 것은 아니고 개인마다 차이가 있 어 300mg/m 2 이하로 투여되었을 때에도 심독성의 증상이 나타 날 수 있다. 그리고 약제마다 심독성을 일으키는 정도가 다른데 epirubicin의 경우, 치료 용량에서 발생하는 심독성은 doxorubicin의 그것보다 발생 빈도가 낮으며 mitoxantrone이나 새로 운 anthracycline제제(men 10755)는 심독성의 발생 빈도가 더 낮은 것으로 보고되고 있다. 5) 1) 급성 심독성 Anthracyclines의 투약 도중이나 투약 직후에 발생할 수 있으 나 비교적 드물고 발생하게 되면 대개 일시적이다. 대부분 비특이 책임저자 : 윤호중 , 서울시영등포구여의도동62 가톨릭대학교성모병원순환기내과 Tel : , Fax : , younhj@catholic.ac.kr 접수일 : 2009년 4월 26일 ; 게재승인일 : 2009년 5월 19일 4) 심독성의 발견 및 감시 주기적인 심초음파 검사는 약제로 인한 심기능의 저하의 발견 및 경과관찰에 매우 유용하다. 심내막심근생검(endomyocardial biopsy)은 anthracycline으로 인한 심근의 섬유화 여부와 그 정 도를 평가할 수 있으나 침습적 검사라는 점, 정확한 조직학적 분석 이 필요하다는 점, 비용이 많이 든다는 점이 제한점이다. 혈청학적 검사를 통한 방법으로는 troponin T를 이용한 방법이 있는데, 6) 약 제 투여 후 troponin T의 상승이 관찰되는데 소아 환자에게 있어 Chemotherapy induced Cadiotoxicity 9

항암제의 기전에 따라 여러가지 부작용이 나타나게 되 는데 몇몇 항암제는 그 부작용으로 심독성을 나타내며 그 중 가장 유명한 약제는 doxorubicin, epirubicin 등으로, anthracycline계열의 약물들이다.

2 서는 비교적 잘 나타나지만 성인에게 있어서는 연구에 따라 그 결 과가 상이하여 심독성의 지표로 사용하기는 어렵다. 다른 혈청학적 표지자로서는 BNP(B-natriuretic peptide)가 있는데 anthracycline의 투여 후 의미있는 BNP의 상승을 보였다. 7) 그러나 BNP도 심독성에 대한 예측을 정확하게 할 수는 없으며 다만, 무증 상의 좌심실 기능저하를 발견하는데 유용한 것으로 보인다. 향후 이 분야에 대한 연구가 더 진행되어야 할 것으로 보인다. 8) 4 면역계의 역할 Oxidative stress 후에 발생하는 면역학적 반응이 심독성에 영향을 끼칠 것이라는 이론인데 몇몇 연구에서 doxorubicin의 투여 후 심근 세포 주위에 염증 반응이 활성화 되는 것을 관찰하 였고 수상돌기세포(dendritic cell)가 많이 분포하는 것도 발견하 여 doxorubicin의 투여가 항원의 발현을 자극하는 것으로 예상 하였다. 16) 5) 심독성의 기전 1 Oxidative Stress 가설 가장 널리 알려진 가설로 활성 산소(oxygen free radicals)와 과산화물(superoxide)에 의한 심독성을 의미한다. 9) 이 가설은 in vitro 실험에 그 근거를 두고 있는데 anthracycline이 체내에 들어가서 효소에 의해 환원된 후 철 이온과 결합하여 결합체를 형 성하게 되는데 이 때 형성된 결합체가 분리될 때 많은 활성 산소 및 과산화물이 생성되게 된다. 심근세포에는 항산화제가 상대적 으로 부족하므로 이러한 활성 산소에 대해 손상을 더 잘 받게 된 다. 또한 활성 산소에 의해 손상 받은 세포는 세포자멸사(apoptosis)를 통해 심독성을 나타낼 수 있다는 가설도 있는데, 고용량 의 doxorubicin에 노출된 세포는 괴사(necrosis)를 일으키지만 저용량의 doxorubicin에 노출된 세포는 세포자멸사를 일으키는 것이 몇몇 실험에서 밝혀진 바가 있다 ) 2 대사 산물(metabolite)에 의한 손상 이론 Doxorubicin은 심근 세포의 지질 과산화(lipid peroxidation) 과정과 연관이 있다고 알려져 있다. 지질 과산화는 활성 산 소에 의해 세포막에 있는 지질이 전자를 잃는 과정을 의미하며 이 과정으로 인해 세포막이 손상되게 된다. Doxorubicin은 지질 과 산화 과정에 연관되어 세포막 손상 및 활성 산소 발생에 깊은 관 여를 한다. 14) 3 칼슘 항상성(homeostasis)에의 영향 Anthracycline의 또 다른 심근 독성의 기전으로 칼슘 항상성 의 변화를 들 수 있다. 칼슘 이온 이동의 변화는 세포의 손상을 초 래하며 심장의 수축 기능을 저해할 수 있다. In vitro 실험에서 doxorubicin의 투여로 인해 미토콘드리아의 칼슘 처리 기능의 손상이 비가역적으로 발생되었다. 15) 또한 세포내 sarcoplasmic reticulum에서 칼슘의 유리를 촉진시키는 것으로 알려졌다. 이에 verapamil을 투여하여 세포내 칼슘의 과축적을 억제하여 doxorubicin의 미토콘드리아에 대한 반응을 감소시키는 연구보 고가 있다. 그러나 아직 이 기전을 규명하기에는 증거들이 부족하 여 정설로 받아들이기까지는 많은 연구가 더 필요하다. 6) 예방 누적 용량을 철저히 지키는 것과는 별도로 anthracycline에 대한 심독성 예방을 위한 여러 예방법들을 시도해왔다. 17) 활성 산 소에 의한 가설에 의해 항산화제를 투여하는 것을 여러 차례 시도 하였으나 주목할만한 성과를 거두지 못했다. 연구들마다 그 효과 에 대한 보고가 제각각이어서 일관된 치료방침은 없는 실정이다. 1 Dexrazoxane 철과 doxorubicin의 결합체에 대한 치료로, iron chelator가 심독성을 예방하기 위한 방법으로 도입되었다. 철 이온을 제거함 으로서 활성 산소의 발생을 예방하는 것이 그 목적이다. Dexrazoxane(ICRF-187)은 이 분야의 가장 주목할만한 약제로 여러 연구에서 심독성을 예방하는데 괄목할 만한 성과를 보여주 었다. 한 연구에서는 이 제제를 투여한 군과 투여하지 않은 군에 서 심독성의 발생률이 각각 4%와 24%를 보였다. 18) 이러한 효과 를 보이지만 심독성의 완벽한 예방은 불가능한 것으로 보이며 특 히 후기 심독성에 대한 예방효과에 대해서는 아직도 미지수이다. 또한 소아에서의 효과는 크지않아 미국 암학회에서는 소아 환자 에게 이 제제의 투여를 권하지 않고 있다. 19) FDA에서는 doxorubicin의 누적 용량 300mg/m 2 이상의 성인 환자의 경우에만 인정하고 있다. 2 MonoHER 또 다른 활성 산소 제거제로 연구되는 것이 monoher이다. MonoHER은 쥐 실험에서 항암제의 효과를 떨어뜨리지 않고 심 보호 효과가 있는 것이 알려졌다. 20) 이 제제는 potency가 낮아 고용량을 투여해야한다는 단점이 있다. 최근에는 monoher의 유도체인 frederine이 개발되어 monoher에 비해 5배 강한 potency를 지니게 되었다. 이 제제에 대한 임상적 실험은 현재 진행중이다. 3 지질 저하제 지질 저하제 또한 anthracycline에 의한 심독성을 예방하는 것으로 알려져 있다. 쥐에게 doxorubicin과 probucol을 병용 10 Ho-Joong Youn

3 Korean Journal of Clinical Oncology Summer 2009 ; Vol.5, NO.1: 9-14 투여하면 항산화 효소의 발현이 촉진되고 지질 과산화 작용이 감 소하는 것이 밝혀졌다. 21) 최근 한 연구에서는 statin이 항암 효과 상승 및 심보호 효과가 있는 것으로 밝혀져 주목을 받고 있다. 22) 4 투여 제형(formulation)의 변형 Anthracycline의 투여 제형을 liposome의 형태로 변화하는 것이 심독성을 감소하는 것과 관련이 있음이 알려졌다. 23) Liposome은 주로 간이나 비장의 세망내피계(reticuloendothelial system)에 분포하게 되어 심장에의 노출을 줄일 수 있게 된다. 한 retrospective study에서는 liposome제제를 투 여한 치료군에서 울혈성 심부전이 전혀 나타나지 않았고 심초음 파상 좌심실 구혈률이 2% 감소하였으나 전혀 증상은 나타나지 않 았다. 2. Taxoids Paclitaxel이나 docetaxel과 같은 taxoids는 여러가지 암 치 료에 사용되는 중요한 항암제이다. 이 약제도 심독성을 일으킬 수 가 있는데 서맥이나 빈맥, 방실 차단이나 다발갈래 차단(bundle branch block), 심근허혈 등이 발생할 수 있다. 한 연구에서는 최대 용량(110~250mg/m 2 )을 투여한 경우에 5% 정도의 심한 부 정맥이 발생하고 29% 정도에서 무증상의 서맥이 관찰 되었다. 24) 이러한 현상들은 약의 투여를 중지하는 경우 대부분 사라졌다. Taxoids와 관련된 심독성은 대부분의 경우 그 정도가 심하지 않 은 것으로 알려져있으나, 기존의 심질환을 가지고 있는 환자에서 의 연구 결과는 아직 정립되어 있는 것이 없어 이러한 경우에는 주의를 요한다. 1) 심독성의 기전 Paclitaxel은 Cremophor EL라는 가용화제와 결합하여 체내 에 투여되는데 Cremophor EL이 히스타민의 과다 분비를 유도 하여 심독성을 일으키는 것으로 보고 있다. 또한 paclitaxel의 심 근 세포의 직접적인 독성도 하나의 가능성으로 보고있다. Paclitaxel이 anthracycline과 병용 투여될 때 심독성의 위험 성이 커지는데, paclitaxel과 doxorubicin이 병용 투여될 때에 Table 1. 한 retrospective study에서는 liposome제제를 투 여한 치료군에서 울혈성 심부전이 전혀 나타나지 않았고 심초음 파상 좌심실 구혈률이 2% 감소하였으나 전혀 증상은 나타나지 않 았다. 2. Taxoids Paclitaxel이나 docetaxel과 같은 taxoids는 여러가지 암 치 료에 사용되는 중요한 항암제이다.

4 는 doxorubicin의 누적 용량이 380mg/m 2 이상 투여되면 심독 성의 위험성이 높아진다. 이는 paclitaxel이 doxorubicin의 간 배설을 억제시켜 혈중 농도를 높이기 때문이라고 보고있다. 25) Docetaxel의 경우에는 doxorubicin과의 병용 투여에도 심독성 을 증가시키지 않는 것으로 보고되고 있다. 2) 심독성의 예방 Anthracycline과 병용 투여 시에는 anthracycline의 투여 용량이 감소되어야 한다. 그리고 doxorubicin보다는 epirubicin이 taxoids와의 병용 투여 시 심독성 발생의 확률이 낮다. Taxoids의 심독성 발생은 아직까지 제한적이어서 심독성을 예방 하는 약제는 아직 개발되지 않았다 Fluorouracil(5-FU) 5-fluorouracil(5-FU)는 대사길항물질(antimetabolite) 로, 조혈기능 억제, 점막염, 피부염, 설사 등의 부작용이 있다. 심 독성은 적게는 1~5%, 많게는 18% 정도로 보고되고 있다. 5-FU 의 심독성은 주로 continuous injection보다는 bolus injection시에 더 많이 발생한다. 최근에 5-FU의 전구체로서 경구 투 여가 가능한 약제인 capecitabine의 심독성이 보고된 바가 있다. 5-FU의 심독성에 의한 증상은 부정맥, 협심증, 울혈성 심부전, 심지어 돌연사까지 나타날 수 있다. 5-FU에 의한 심독성의 위험 인자로는 기존의 관상동맥 질환과 방사선 치료를 동시에 받을 때 이다. 5-FU의 심독성의 기전은 아직 명확히 밝혀진 것은 없지만 관상동맥의 연축이나 심근에 직접적인 손상이 가설로 제시되고 있다. 26) 이 약제로 인한 심독성을 예방하는 방법이 아직은 없다. 따라서 심독성의 증거가 있는 경우에는 약의 투여를 중지하고 약 을 재차 투여하면 부작용이 발생할 가능성이 더 커지므로 약을 다 시 투여해서는 안될 것으로 보인다. 4. Cyclophosphamide and ifosfamide Cyclophosphamide와 ifosfamide는 알킬화 제제(alkylating agent)로 간에서 활성화된 형태로 대사된다. 고용량의 cyclophosphamide는 치명적인 심근병증과 같은 급성 심독성과 관련이 있다. 27) 또한 가역적인 심실 수축 기능 저하도 보고되고 있 다. Ifosfamide에 의한 심독성은 한 차례 보고된 적이 있다. 28) 이 약제의 심독성 기전도 현재는 알려진 것이 없으나 활성 산소와 관 련이 있을 것으로 생각한다. 5. Cisplatin Cisplatin은 여러가지 암에 투여되는 항암제로 cisplatin치료 한 후 급성 심근 경색이 발생한 몇몇 경우들이 보고되었다. 29) Cisplatin을 투여하는 경우, 심혈관계 관련 질병의 발생 빈도가 의미있게 높아지게 된다. 1) 이는 혈관의 손상, 혈소판 응집력의 변 화, 저마그네슘혈증을 유발하여 발생하는 것으로 생각된다. 6. Trastuzumab Trastuzumab은 HER2 단백질에 대한 단일클론항체로 주로 유방암의 치료제로 사용되고 있다. Trastuzumab에 의한 심독 성은 anthracycline의 심독성과 그 양상이 비슷하다. Anthracycline과의 병용 투여 시에 심독성의 발생가능성이 높 아지며 여기에 cyclophosphamide까지 투여하게 되면 28%의 가능성으로 심독성이 발생할 수 있다. 30) 결 론 항암화학요법은 많은 부작용이 있을 수 있는데 심독성도 예외 는 아니다(Table 1). 항암치료로 인한 생존률이 증가되고 있는 시 점에서 이러한 부작용에 대한 관심이 더욱 증대되고 있다. Anthracycline에 의한 심독성이 가장 잘 알려져 있으며 활성 산 소에 의한 손상이 가장 가능성이 높은 이론으로 제시되고 있다. 이에 항산화제와 iron chelator인 dexrazoxane의 투여가 좋은 성적을 보이고 있다. Paclitaxel은 anthracycline과의 병용 투 여 시 anthracycline의 대사에 영향을 주므로 심독성을 조장하 게 된다. 5-FU, cisplatin, cyclophosphamide등고 심독성과 관련이 있으나 이에 관련된 기전이나 예방법에 대해서는 좀 더 많 은 연구가 필요하다. REFERENCES 1. 6 Chemotherapy induced Cadiotoxicity Division of Cardiology, Department of Internal Medicine, College of Medicine, The Catholic University of Korea, Seoul, Korea Ho-Joong Youn, M.D. Abstract Chemotherapy induced cardiotoxicity is a well-known side effect of several cytotoxic drugs, especially of the anthracyclines and may cause severe morbidity in surviving cancer patients. The mechanism of anthracycline induced cardiotoxicity, relatively well established, seems to involve the free radicals formation and the apoptosis of myocytes. Prevention of cardiac toxicity can be achieved by control of the cumulative dose. Furthermore, addition of the iron chelator to anthracycline therapy has shown to be helpful. Other cytotoxic drugs such as the taxoids, 5-fluorouracil, cyclophosphamide and trastuzumab are associated with cardiotoxicity as well, although little is known about the possible mechanisms. The pathophysiology of these adverse effects is similarly heterogeneous and the identification of potential mechanisms is frequently difficult since the majority of cancer patients is not only treated with multiple anti-cancer drugs but might also be exposed to potentially cardiotoxic radiation therapy. Further research is warranted to understand the mechanism of chemotherapy induced cardiotoxicity and to develop strategies to avoid this side effect.
