We could diagnose a case of glycogen storage disease type Ia with molecular genetic analysis. A 17-year-old man visited Yeungnam university hospital because of abdominal discomfort. Clinical features were characterized by short stature, hepatosplenomegaly, accompanying hypoglycemia, hypercholesterolemia, hyperuricemia. Liver needle biopsy disclosed compatible findings of glycogen storage disease. Molecular genetic analysis of the G6PC gene was performed by direct sequencing method. We identified two mutations within the exon 5 of the G6PC gene, 727G>T and 743G>A. We report this rare case with a review of the literature.(korean J Med 71:91-96, 2006) Key Words : Glycogen storage disease type Ia, Gene analysis 서론당원병은 glucose-6-phosphatase의부족혹은결핍에의해 glycogen이간이나다른장기에축적됨으로써초래되는선천성탄수화물대사장애질환으로 1929년 von Gierke가처음으로보고하였고, 국내에서는 1972년서등이최초로증례보고하였다 1). 당원병은우리나라에서도간헐적으로보고되었으나 2-6) 임상에서당원병을경험할기회는그리많지않다. 과거에는간조직검사에서 효소분석을통해진단하였으나최근에는유전자분석으로당원병을진단하고, 환자의유전자형와표현형간의상관관계를연구하여환자의치료에이용하려는노력을하고있다 6-8). 저자들은영남대학병원내과에상복부불쾌감으로내원한 17세남자환자에서간비종대, 고지혈증, 저혈당증, 고요산혈증및저신장이있어간조직검사및유전자분석을시행하여당원병 Ia형을진단하였기에문헌고찰과함께보고하는바이다. 접수 : 2005 년 11 월 28 일 통과 : 2006 년 2 월 7 일 교신저자 : 이헌주, 대구시남구대명동 317-1, 영남대학교의료원소화기내과 ( ) hjlee@med.yu.ac.kr

Liver ultrasonography showed hepatomegaly and moderate degree of fatty liver. 증례환자 : 17세남자주소 : 상복부불쾌감과거력및현병력 : 나이에비해키가작고성장이느렸으나별다른검사를하지않고지내다최근상복부불쾌감이있어내원함. 가족력 : 어머니가간암으로사망하였고, 형이군대에서원인미상의간질환으로사망하였다고함. 진찰소견 : 신장 144 cm, 체중 41 kg으로각각 3백분위미만이었고, 혈압은 120/80 mmhg, 맥박수 90회 / 분, 호흡수 20회 / 분, 체온 36.5 였다. 전신상태는양호하였고, 의식은명료하였으며외형상기형은없었다. 심음, 호흡음모두정상이었고, 복부는팽창되어있었으며간이우측늑골밑으로약 8 cm, 비장이좌측늑골밑으로 2 cm 정도만져졌다. 액모및음모가관찰되지않았다 ( 그림 1). 검사실소견 : 말초혈액검사상백혈구 6,010/uL, 혈 색소 11.2 g/dl, 혈소판 309,000/uL였고, 생화학검사에서공복시혈당 66 mg/dl, 요산 mg/dl, 총콜레스테롤 368 mg/dl, HDL/LDL 콜레스테롤 24/137 mg/dl, 중성지방 1443 mg/dl, AST/ALT 98/62 IU/L, alkaline phosphatase 527 IU/L, γ-gtp 228 IU/L, 총빌리루빈 0.4 mg/dl, 총단백 / 알부민 8/4.92 g/dl, lactic acid 6.7 mmol/l, Na/K/Cl 140/5.6/96 mmol/l, BUN/creatinine 12.4/0.6 mg/dl 였다. 혈액응고검사상 PT/aPTT 10.7/ 30.8초, 바이러스표지자는 HBsAg, HBsAb, HBcAb (IgG), anti-hcv 모두음성이었고, AFP 2.11 ng/ml였다. 자가항체검사에서 anti-smooth muscle Ab, antimitochondrial Ab, anti-lkm Ab 음성이었고, 혈청 ceruloplasmin은 22 mg/dl 였다. 24시간소변구리는 1.1 ug이었고, 혈청구리는 75.4 ug/dl 였다. Fe/TIBC 117/ 525 ug/dl, ferritin ng/ml 였고, 호르몬검사에서 FSH/LH 2.22/1.88 IU/L, testosterone 0.4 ng/ml, E2 10 pg/ml, TSH 2.93 mu/l, free T pmol/l, prolactin 2.09 ng/ml, ACTH 1.3 pg/ml, cortisol Table 1. So the bony age was estimated to be 10 years old (arrows). ug/dl 였다. 종합적뇌하수체전엽기능검사 (Combined anterior pituitary test) 에서 insulin, TRH, LHRH 세가지호르몬주입후 glucose, hgh, ACTH/TSH, prolactin, LH, FSH 반응은정상이었다 ( 표 1). 방사선검사 : 흉부사진은정상이었고, 복부초음파검사상간종대및비장종대와중등도의지방간소견이관찰되었다 ( 그림 2). 두개골 Sella cone down view 및뇌자기공명영상은정상소견이었고 ( 그림 3), 방사선학적골연령은요골과척골의골단 (epiphysis) 이아직닫치지않아 10세정도로추정되었다 ( 그림 4). 간조직검사 : H&E 염색에서경한 macrovesicular steatosis, 얇은 fibrosis septa, hepatocyte swelling, rarefied cytoplasm, nuclear glycogenosis 소견이관찰되었고, PAS 염색에서양성소견, DPAS 염색에서는음성으로나타나므로당원병에합당하였다 ( 그림 5). 염색체검사 : 46XY로정상이었다. 유전자분석 : 유전자분석은삼성서울병원진단검사의학과유전검사실에의뢰하여시행하였다. 임상적으로가장의심이되고빈도가높은당원병 Ia형에대한염색체 17q21에서 G6PC gene에대한돌연변이분석을시행하였는데, 환자말초혈액백혈구로부터 genomic DNA를분리하여 G6PC gene을분석하였고, 모두 5개 exons의 direct sequencing method로하였다. 이중 5번 exon에서 727G>T (Leu216Leu) splice mutation과 743G>A

(Gly222Arg) missense mutation 의 2개돌연변이가관찰되었다 ( 그림 6). 진단및치료 : Glycogen storage disease Ia로진단하고 allopurinol 300 mg/day, fibric acid 160 mg/day, uncooked cornstarch diet로치료를시작하고외래경과관찰중이다. 고찰당원병 Ia형은 glucose-6-phosphatase의결핍에의해 glycogen이간, 신장, 장점막등에축적되는상염색체열성유전질환으로 1929년 von Gierke에의해처음보고되었고, 국내에서는 1972년서등이최초로증례보고 하였다 1). 당원병은우리나라에서도간헐적으로보고되었으나 2-7) 여전히임상에서흔히볼수있는질환은아니다. 간내 glycogen이축적되어소량의포도당만이간에서유리되므로저혈당이발생하고, 2차적으로고유산혈증, 고요산혈증, 고지혈증과같은생화학적변화가나타나며, 적절히치료하지않았을때저신장, 심한간종대등이생긴다 10, 11). 장기합병증으로저신장외에골다공증, 통풍, 신질환, 폐고혈압, 간선종, 간세포암등이있으며이에대한증례보고가이루어졌다 4-6, 12). 간조직검사상 glycogen의축적으로간세포는커져있으며 medium to large sized lipid vacuole 및당원핵이관찰된다 15). 과거에는간조직에서의 glucose

5 - 이헌주외 6 인 : 유전자분석을시행한당원병 Ia 형 1 예 - 대한동질접합체는경한표현형을보이지만간세포암의발생위험은증가한다고한다 8). 그러나아직당원병의유전자형과표현형간의상관관계가명확히밝혀진것은없다. 당원병환자를보다효과적으로관리하고치료하기위해서유전자분석및유전자형-표현형의상관관계에대한보다많은연구가이루어져야할것으로생각한다. 중심단어 : 당원병 Ia 형, 유전자분석 REFERENCES Figure 6. Molecular gene analysis showed two mutations within the exon 5 of the G6PC gene, 648G>T (727G>T*) and 664G>A (743G>A*). *The nucleotide positions were 648, 664 bp from the ATG translation start site based on the reference sequence; however, 727G>T, 743G>T were used in the text in line with the previous reports to avoid confusion. phosphatase 활성도측정에의해확진하였으나최근유전자검사가가능해지면서여러형태의 1형당원병의진단에유전자분석이간조직검사에서효소분석을대치하고있다 7-9). 1993년 Lei 등이염색체 17q21에 G6Pase 를 encoding하는유전자 (G6PC) 가존재함을확인한후당원병의다른형태들이발견되어현재당원병 1형 (von Gierke 병 ) 에서 11형까지명명되었다 14). Matern 등의연구에서 G6PC mutation 의빈도는 exon 2의 326C>T (Arg83Cys), exon 5의 727G>T (Leu216Leu), exon 5의 1118C>T (Gln347Stop) 순으로각각 29.0%, 16.4%, 14.3% 였고, 일본인의경우 727G>T (Leu216Leu) 가 84.6% 로가장흔한돌연변이였다 8). 한국인에대한연구로는기등이 Ia형의당원병환자 13명을분석하였는데 727G>T mutation 이 81% 로가장많았다. 이와같이유전자분석을통한연구가많이진행되고있는데당원병을진단하는데침습적인간조직검사를대체할수있고, 산전진단을통해잠재적환자를찾아조기에치료할수있으며, 유전자형과표현형간의상관관계를밝혀환자의임상경과및예후를예측할수있을것이라는기대를하고있다 7-9). 일부에서는유전자형과표현형간의상관관계가존재하는데예를들면, G6PC 돌연변이에대한동질접합체 (homozygosity) G188R의경우당원병비 Ia표현형과관련있고, 727G>T 돌연변이에 1) 서동진, 신무현, 고창무, 이문호.
