The CT attenuation density of pituitary glands was also evaluated with coronal scans of 103 normal Korean adults who had no clinical evidence of pi tuitary abnormalities The pituitary g land was classified into 4 types based on the contours of superior margin of the glands; slightly convex (type 1 & type II ), slightly concave (type rn) and deeply concave (typen). Among them type rn was the most common (44.4 %) The size and shape of each lobe of gland were varied. The mean length and height of gland were 10.9 ::l::: 1.4 mm and 4.6 ::l::: 1.2 mm. Rathke s cleft cysts over 0.5 mm in diameter was found in 54.8 %, but it rarely exceeded 3 mm in diameter. Rathke s cleft cysts were commonly located at the middle one third between anterior lobe and pars intermedia of pituitary gland The CT attenuation density of pituitary gland varied depending on the anatomic location and was roughly corresponding to the compactness of cellularity. CT는 GE 9800으로조영제 Conray 60(60 % meglumine iothalamate) 150 ml를정맥주사한후직접판상연 (coronal plane) 으로촬영하였마. 이해촬영연이얀결결 ( tu bercul um sellae) 와안배 (dorsum sellae) 를잇는선에수직이되도록하였다. 1. 5mm절펀두께로촬영한 CT 영상을 window wid 1. 사체의정상뇌하수체의형태조직학적검사와 CT 및 MRI th Hounsfield unit(hu) 와 Window level 25-35HU 에서판찰하였마. 뇌하수체각부위의음영 대상은연세의대, 한양의대및 원주의대해부학교 을구별하기위하여, 뇌하수체찰데기부위를기준으로 실에서학생실습용으로사용한성인사체 63 구중뇌하 수체선종을동반한 1 예를제외한 62 예로나이는 세 ( 평균 57.2 세 ) 사이였고, 이중남자는 34 영 (54.8 3mm 앞의영상을전엽무로, 1. 5 mm 둬의영상을후 엽으로이를기준하여각부위의음영의차이와균일 성을조사하였다. %) 이었다. 총 62 예중 8 예는뇌하수체누두와뇌하수체를포함 한접형골을절개분리하여, 6 에는 CT, 2 예는 MRI 를 ill. 걸 과 시상연 (sagittal plane ) 으로시행한후, 얼런조직을 CT 및 MRI촬영같은연으로절펀을만들어 hematoxylin-eosin염색하에저배율에서광학현미경으로판찰하였고, 냐머지 54예는뇌하수체를석출한후뇌하수체누두가위치한중앙부의시상연으로조직절펀을만들어같은땅법으로판찰하였다. CT는 GE 9800을이용하여 1. 5mm두께로뇌하수체를 6-107~ 의시상촬영을하였으며, 이해 FOV (field of view) 는 9. 6 cm, matrix는 512X512로하였 1. 조직표본시상면에서의뇌하수체의형태뇌하수체의중앙시상연 ( mid-sagi ttal plane) 에서의형태는아주다양하였으며, 이는비교적일정한형태와크기를갖고있는후엽보다는전엽의형태에짜- 라서로다른모양을나타내었마. 62예의형태를네가지유형부로분류하였다 (Fig. 1). 제 1형파 2행은뇌하수체의상연이볼록하나상연의뇌하수체깔데기 (pituitary sta lk ) 와연결부의함몰유무에짜라분류하였고, 제 3형파 4형은전엽상연의함몰정도에따라분 다. 또한 MRI 는한국과학원의 2.0 tesla prototype 기 류하였다. 구두모양 (boot shape) 의제 3 형이가장많 를이용하여 3mm 두께로시상촬영을하였고, 촬영방 았으며, 전체의 44 % 이었다. n < U

F our different configurat. ions of the 원gittal section of normal pi tui tary g lands and their relati ve incidence in 62 specimens 제 4형은공허얀 (empty se lla e) 을동반한형태로 7. 4% 에서판찰되었다. 각형은사체의성멸에따른차이점은없었으나, 제 4형의전예가 60세이후에서나타났으여, 제 1 형의 % 가 50세이전에서보였마. 뇌하수체전엽은후엽에비하여진한염색파다소높은조직밀도로구분이가능하였으며 34예 (54.8 %) 에서는다양한크기의 Rathke써낭이중간엽의전엽사이에서관철되었다. 천엽의모양은둥근형이 %, 반달형이 22.2%, 삼각형이 % 이였다. 또한후엽의모양은반달형이가장많은 77. 8%, 둥근행파초생달형이각각 % 이었다. 전엽과후엽의경계연은전엽을향하여오옥한것이 66. 7%, 봉록한것이 18.5%, 펑펀한것이 14.8 % 였다. 뇌하수체윗연의형태는시상연에서천엽의상연이오옥하고후엽의상연이볼록한 S" 형이 27.8%, 전체적으로볼록형, 펑펀형이각각 18.5%, 뇌하수체 깔데기와연결부위가함올된 3 " 자형이 18.5%, 오옥형이 % 이었다 (Fig. 2A,B) 2. 조직표본시상면에서의뇌하수체의크기뇌하수체, 전엽및후엽의전후장경과전엽, 후엽의중심부및깔데기의연결부에서상하높이를계측하였다 (Fig.3). 정중시상연에서뇌하수체의평균장경은 10. 9i:1. 4 mm, 전엽은 6. 7i:1. 3 mm, 후엽은 4.2 士 0. 8 mm 였으며, 성별, 연령별로통계학적으로유의한차이는없었으냐, 후엽의장경은비교적알정한것에매하여, 전엽의장경의차이에의하여전체장경의크기가좌우되었마. 뇌하수체의평균높이는천엽이 4. 4 土 1. 1 mm, 후엽이 4. 8i:1. 1 mm, 그러고깔데기연결부에서 4.6 i: 1. 2mm 이었r::.HTab!e 1). Rathke씨낭의위치와크기저배율광학현미경석검색에서중간엽과전엽의사이에서 0.5 rnm크기이상의 Rathk e씨낭은 34예 (54. 8 % ) 가관찰되었다. Rathke써낭은뇌하수체를상하 3 둥분할혜, 상부에위치한 1. 6 %, 하부에 9. 7 % 외에 % 는주로가운데 1 /3에위치하거나상하부에결처있었다. 또한낭의크기는가장큰것이직경 3.3 mrn이였으며, 2mm 이상이 18.5 % 이었다 (Table 2) (Fig.4)

5 % 뻐김총익외 한국인정상뇌하수체의형태조직학적소견및영상진단 뇌하수체조직단연과 CT 및 MRI 영상과의비교 CT 는 Hounsfield unit 의 window width 에 서촬영하였으며, 천예에서천엽과후엽의경계가불 분명하였마. 같은변의조직학적소견에의하여확인 펀천엽의 CT 음영은, 출혈성경색에의하여국소적 인조직결손이있는 1 예를제외하고, 터키안저부에 연하는전엽의외연을따라고음영을나타내고그내 부는배교석저음영으로보였다. 이러한 C T 읍영의 차이는조직에서의세포의밀집정도와상판판계를나 타내었다. 중간엽 (pars!tl termed i a) 은 CT 에서다양한음영으 로구옐이어렵고, 후엽은 3 예에서중간엽에인정한 부위가비교석높은 CT 음영을보인반연냐머지 3 예 는균일한저음영으로조직에서도 >1 ] 슷한세포의밀집 도의분포를냐타내였다 (Fig. 5A,B ). CT 에서 뇌하수체각엽의펑균 Hounsfield 번호와 그범위는마음과같마 ( Tabl e 3). MRI(magentic resonace im ag ing) 을시행한 2 예의 Tl 강조영상에서 l 예는뇌하수체각부위에짜른차이 를발견할수없었으나, 1 예에서는후엽이확연히구 분되는고신호강도를나타내고, 전엽은중간엽과의 경계부와외연이비교적고신호강도를보인반연내부 는저신호강도를보였다. 관성이없었다 ( Fi g.6). 이는조직의세포밀도와연 5. 조영제주입후정상인뇌하수체의관상전산화단 층촬영에의한뇌하수체의음영 뇌하수체전엽의 CT 음영은 10 3 예중 39 예 (3 7.9 %) 에서서로마른음영의차이를판찰하였고, 이중 3 1 예 (30.1 %) 는고음영파저음영의부분이 2-3 개의수직 줄무늬형태로냐타났무며, 8 예는국소저음영의부위 가전엽의한쪽에치우쳐판찰되었다 ( Fig 7). 뇌하수 체깔데기의연결부위에서는칼데기와수직으로연결 되는고음영이 2 1 예에서보였으며, 29 예 (28.2 %) 에서보였다. 국소적저음영이 뇌하수체후엽은기저부에고음영부분이보였던 12 예 (11. 7 % ) 를제외하고, 대부분이전엽보다낮은균 A Fig. 5. N. 고찰뇌하수체는발생학적기원과구성하는조직및기능이서로마른전엽파후엽 ( 신경엽 ) 으로이루어지여, 따라서두부분이만나서말육되는과정에서다양한행태를보얼수있마 5-7) 여러부검결과에서도이러한다양성으로인하여정상뇌하수체의형태를정형화하여기술하기가매우어캡다고알려져있다 8, 9-11 ) 뇌하수체천엽의용석은신체의내분매계의활성도, 즉성별, 연령, 월경주기둥의여러요인이판여되 고 1 0) 전엽에서말생하는미세선종의진단을위한정상법위를설정하기위한많은연쿠논운이보고되었마 1, 3, 12-19) 그러냐, 적출된뇌하수체와관상전산화단층촬영에의한것이대부분으로, 전엽과후엽의판계를보기위한시상연에서의판찰은없는실정이며, 이는최근 MRI의이용이가능해짐에따라더욱필요하게되었다. Fuji saw a동 8) 은 60예의정상인의뇌하수체에대한 MRI에서시상촬영을한전예에서전엽과후엽의쿠렬이가능하였고, 천엽의높이가남자에서평균 5.1 mm, 여자에서 7.1 mm 였고, 후엽은각각 4. 1 mm, 5. 3 mm로전엽이후엽보마그러고여자에서크며천엽의상연은남자의 40 %, 여자의 67 % 에서

7 - 김동익외 : 한국인정상뇌하수체의행태조직학석소견및영상진단... - 볼록하다고보고하였다. 후엽 ( 신경엽 ) 이전엽에대하여후연의가운데위치하는것이 % 로가장않으냐 3 ) 24 % 에서만이가운데위치할뿐대부분이중앙에서벗어나있으며 14 % 에선측연에위치한마는보고도있마 1 3) 저자의정중시상연의염색한조직표본의전예에서두엽의관계를볼수있었으여, 천체석인모양은전엽의형태, 특히상연의모양에따라차이를나타내었마. 후엽의상연은대부분에서볼록하거냐평펀한반연, 전엽은반수에서오옥하였고, 공허안을동반한싱한오옥형은 60세이후에서발견되었다. 그러냐성벨에따른유의한차이점은발견할수없었으여, 이는대상성별연령군의차이에따은것으로추정된마 20 ) 또한전엽과후엽각각의모양은상연과두엽이만냐는연의형태에따라마양하여, 후엽은 % 에서반달형으로보였고, 두엽사이의연은 82 % 에서전방 로약간오옥하거나평펀하였다. 터어키안의뇌하수체전엽에서말생하는가장흔한질환은뇌하수체선종A로, 부검시 lcm 이하의미세선종의발견반도가 27 % 까지보고되고있으며 ) 이러한마세선종의영상잔난을위하여 CT와 MRI 가이용된다. 조영증강 CT에서미세선종은정상뇌하수체조직에비하여주로저음영의벙소로판찰되고 12, 23-25) 그외종괴효과로뇌하수체상연이볼록해지고, 높이가증가하여, 뇌하수체칼데기의천이등이각각 47 % 26 1, 72 %23), 92 %28 ) 에서보고되고있지만, 저음영명소의존재가가장통계학적으로의의있는소견이다 29 ) 그러나정상뇌하수체조직도구성세포의밀도에짜라서서로마른 CT음영의차이를보알수있고 ), Rathke씨낭파표파양당종 ( epidermoid cyst) 가비교적흔히말견되는저음영의영소로감벨 이필요하마 13, 1 5, , 3 1 ) Rathke씨열낭 ( cleft cyst) 은 Rathke s p o uch가말육기의뇌하수제전엽파중간엽사이에남아형성되며, 낭벽은원주형 (co lumn a r ) 혹은입방형 (c ub o idal ) 세포로구성되며간혹성모 (cilia ) 와 gob l et세포가존재한다 ) 뇌하수체낭, 콜로이드낭혹은터어키안내상피낭 (i ntrase llar epithelial cys t ) 둥으로기술되어지기도하며 ) 신경상파세포에서발생하거냐 37) 또는뇌하수체전엽세포의화생 ( m e taplasia ) 에의하여생성된마는 3 5) 학설도있마. 부검시뇌하수체에서 % 의빈도로말견되며그크기는대부분 이 7 mm 를넘지않는것으로알려져있다 38, 39 ) 저자 의관찰에선다소높은밴도 ( %) 로천엽파중간 엽 사이의경계에서 mm 의크기로냐타났으 며, 직경 3. 3 mm 가가장큰것이였다. 뇌하수체전엽은세포말도가높으며, 진한과렵으 로구성된 acidophi li c 세포군의높은 C T 음영에 여 배하 상대적 로세포빌도와과립이석은 chromopho bi c 파 basophilic 세포군이낮은음영으로, 판상 CT 에 서고음영파처음영이반복적으로보알수있우며, 간엽은 R a thk e 써낭의크기에짜라저음영부로보이 게되고, 또한후엽은천반적으로전엽보마도처음영 으로판찰되나부분석우로후엽의앞부분은높은밀도 의헬판분포가많은신경엽조직으로고음영으로보인 다 1 2, 29) 저자의 10 3 예의정상인 CT 에서전엽의 30 % 에서상기처음영파고음영의반복되는소견이관찰되 었으며, Rathk e 써낭으로추정되는국소저음영이 28 % 에서보였다. 또한뇌하수체의시상연 C T 와조직 표본의비교에서세포의밀도와 CT 음영이상판관계 를보였으나, 중 조직의밀도와 CT 음영의분포가불규 칙하게나타냐는예가많기때문에전형적인소견 로 규정하는데어려움이있으며, 실제소영증강후 CT 에서뇌하수체의헬판분포와주입후촬영시간에따라 음영의밴화를호래할수있는요안으로작용할수있 마. 석출된뇌하수체에대한 MRI 의 Tl 강조영상파조 직소견과의 tl1 교에서 1예는전엽과후엽의신호강도 에차이가없었으나, 1 예는후엽이고신호강도로나 타났으맥, 같은부위의조직에서지방의첨착이냐마 른특기할만한소견이판찰되지않았 여, 천엽에비 하여중간엽이더낮은신호강도를보였다. 이러한신 호강도의차이는조직의세포밀도와유의한상관판계 를볼수없었마. Mark 동은뇌하수체의시상연 MRI 의 Tl 강조영상에서냐타냐는터어키안후부의고신호 강도는뇌하수체보마는뇌하수체와안배 (dorsum sel. lae) 사이의 fat pad 에의한다고보고한 40 ) 반연, Co l omb 둥은후엽자체가 87 % 에서 고신호강도로 냐타난다고하였다 8, 4 1, 42 ) 조직학적으로신경엽 ( 후 엽 ) 은신경섬유, 신경분비물 ( n eurose c r etory prod. u cts ) 파뇌하수체세포 ( pituicyt es) 로구성되여 ) 이 뇌하수체세포에는정상석으로지망성분이존재한 다 ) 또한뇌하수체세포는항이뇨홀온 (a ntid. iureti c h o rm o n e) 이축석된축삭돌기종말 (ax o n term
