We compared the Vitek ESBL test with the NCCLS ESBL phenotypic confirmatory test by disk diffusion (NCCLS ESBL test) and double disk synergy test (DDST). Methods : For a total of 316 clinical isolates composed of Escherichia coli (184), Klebsiella pneumoniae (120) and Klebsiella oxytoca (12), we performed the Vitek ESBL test and the NCCLS ESBL test. For sixty-eight ESBL producing isolates, the Vitek ESBL test was compared with the NCCLS ESBL test and the DDST. The ESBL producer was defined as an organism showing an increase in the inhibited zone diameter of 5 mm for either cefotaxime or ceftazidime in combination with clavulanic acid versus its single test. The DDST was performed with 20 mm and 30 mm for interdisk diameter. For seven false negative isolates in the Vitek ESBL test, the DDST of cefepime was performed. Results : Compared with the NCCLS ESBL test, the Vitek ESBL test showed one false positive (specificity; 99.6%), seven false negatives (sensitivity; 89.7%) and 97.5% agreement. Seven false negative isolates of the Vitek ESBL test were the cefoxitin-resistant ESBL producer. In positivity for the NCCLS ESBL test of 68 ESBL producing isolates, cefotaxime/clavulanic acid and ceftazidime/ clavulanic acid were 94% and 91%. Cefotaxime, ceftazidime, aztreonam and ceftriaxone showed 95/90%, 100/55%, 100/85% and 95/80% positivity in double-disk synergy with amoxacillin/clavulanic acid (AMC) for 20/30 mm of the interdisk diameter respectively. For seven false negative isolates of the Vitek ESBL test, cefepime showed a distinct synergic effect with AMC. Conclusions : The Vitek ESBL test may be a useful method for clinical laboratories due to its easy, rapid and sensitive method but its method was less sensitive to cefoxitin-resistant ESBL. For these cases, if the NCCLS ESBL test or DDST with cefepime are added, the detection rate of the ESBL producer can be augmented. 현재사용되고있는 ESBL 검출법은이중디스크확산법 (double disk synergy test, DDST), E-test, Vitek ESBL 검사법, National Committee for Clinical Laboratory Standards (NCCLS) 의디스크확산법또는최소억제농도측정에의한확인법및분자생물학적방법이있다 [9-14].

NCCLS 디스크확산법에의한 ESBL 확인법 Mueller-Hinton 한천 (Asan, Korea) 에 McFarland 0.5 탁도의균액을바른후 cefotaxime (BBL, Microbiology Systems, Cockeysville, MD., USA, 30 g) 과 cefotaxime/clavulanic acid (BBL, 30/10 g; 이하 CTC) 그리고 ceftazidime (BBL, 30 g) 과 ceftazidime/clavulanic acid (BBL, 30/10 g; 이하 CZC) 디스크를놓고 35 에서 16-18시간배양후억제대를측정하여 CTC 또는 CZC의억제대가 cefotaxime과 ceftazidime 보다 5 mm 이상클경우 ESBL로판정하였다 [13]. 그리고정도관리를위하여양성대조로 K. pneumoniae ATCC , 음성대조로 E. coli ATCC 25922를사용하였다. 4. NCCLS ESBL 확인법에의한 ESBL 생성현황 시험대상균주중 E. coli의 2.8% (5/181), K. pneumoniae 의 29.9% (26/87) 및 K. oxytoca의 8.3% (1/12) 가 NCCLS ESBL 확인법에서양성반응을나타내었다. 2. Vitek ESBL test 의평가 총 316주를 NCCLS ESBL 확인법과동시에시행하여비교한결과위음성 7예, 위양성 1예를보여예민도 89.7%, 특이도 99.6% 그리고 NCCLS ESBL 확인법과 97.5% 의일치율을보였다 (Table 1). Vitek ESBL test에서위음성결과를보인 7주모두는 cefoxitin에내성이었다. Table 1. ESBL 검출방법의비교 ESBL 생성 68주중 ESBL 확인법에서 CTC의양성률은 94.1% (64/68) 이었고, CZC의양성률은 91.2% (62/68) 이었다. DDST에서양성률은디스크간거리가 20 mm인경우 cefotaxime (95.5%), ceftazidime (100%), aztreonam (100%), ceftriaxone (95.5%) 이었으며 30 mm인경우 cefotaxime (90.0%), ceftazidime (54.5%), aztreonam (84.9%), ceftriaxone (80.3%) 이었다 (Table 2). ESBL 생성 68주로 cefoxitin 감수성을시험한결과 15주 (22.1%) 가억제대직경이 18 mm 미만으로내성이었으며, 15개의내성균주중 7주가 Vitek ESBL 시험에서위음성을보였다. 그리고위음성을보인 7주는 cefepime 과의이중디스크확산법에서모두명확한상승효과를보였다 (Table 3, Fig. 1). 고찰장내세균에서 ESBL 생성균주는흔히이들감염의실패와감염의확산을야기할수있으므로 ESBL 생성유무를신속하고정확하게검출할필요가있다 [8, 15-18]. ESBL 생성균주의검출방법에는 3세대 cephalosporin이나 aztreonam으로선별하는방법과 -lactamase inhibitor에의해 ESBL 활성이억제되는것을이용하는 Vitek ESBL 시험, E-test 및 DDST 등이있으며, NCCLS에서는디스크확산법과액체배지희석법으로 clavulanic acid의첨가에의해억제대나최소억제농도의변화를확인하는것이참고방법으로되어있다 [9-13]. 그외에확진방법으로분자유전학적방법이있다 [14]. 액체배지희석법이나분자유전학적방법은술식의번거로움과특별한기기의필요성으로인해검사실에서통상적으로시행하기에부적합하며, DDST은다양한형태의상승효과로인해주관적인해석의문제가있으며, E-test 는검사료가고가라는단점이있다. Vitek ESBL 시험은검출시간이 4-15시간소요되는데 [12] 실제적으로대부분의균들은 10시간이내에검출되어 16-18시간배양후판독하는대부분의검사방법에비하여신속하게검출할수있다는것은큰장점이라고할수있다. 본연구에서임상검체에서분리한 E. coli 181주, K. pneumoniae 87주, K. oxytoca 12주등총 316주를대상으로 Vitek ESBL 시험과 NCCLS ESBL 확인법을시행하였는데, Vitek ESBL 시험에서 1예의위양성 (K. oxytoca) 과 7예의위음성결과를보여특이도 99.6%, 검출예민도 89.7% 그리고 97.5% 의일치도를보였다. Sanders 등 [12] 은분자유전학적으로확인된 ESBL 균주를 Vitek ESBL 시험으로비교한결과 1예의위음성만보여 99.5% 의예민도와 100% 의특이도를보인다고하여본실험과예민도에서차이를보였다. AmpC type -lactamase는 oxyimino-cephalosporin는물론 cefoxitin과같은 cephamycin도분해하나 cefepime과같은 4세대 cephalosporin은분해하지못한다. 그리고 clavulanic acid에의해억제되지않는데, Steward 등 [19] 은 clavulanic acid 의한억제로 ESBL을검출하는 NCCLS ESBL test에서억제대가 5 mm 이하이며 cefoxitin에내성을보일경우 AmpC type - lactamase 또는 porin의변화에의한막투과성의감소등과더불어 ESBL의존재가능성이있다고하였다. AmpC type -lactamase와 TEM 혹은 SHV 형의 ESBL을동시에생성하는균주가존재할수있는데 [20, 21], Tzouvelekis 등 [21] 은 Vitek ESBL test (Vitek GNS-506 panel) 에서검출되지못한 ESBL 생성균주가 AmpC -lactamase와 SHV-5 -lactamase를동시에생성하였으며 cefepime과 DDST에서상승효과를보였다고보고하였다. 본연구에서 Vitek ESBL 검사에서위음성을보인 7균주는모두 cefoxitin에내성이었으며 cefotaxime, ceftazidime, aztreonam 및 ceftriaxone 디스크와 DDST에서다양한결과를보였으나 cefepime에의한상승효과는모든균주에서명확하게 나타났다 (Table 3). 이는 AmpC -lactamase가동시에존재하여 clavulanic acid에의한억제효과를감소시켰을가능성을시사한다. ESBL 생성 68주중 15주 (22.1%) 가 cefoxitin에내성이었고이중7주 (46.6%) 가 Vitek ESBL 검사상위음성을보였으나 cefoxitin 감수성 ESBL은 Vitek ESBL 검사에서모두검출되었다. 따라서 cefoxitin 내성이면서 Vitek ESBL 검사에서음성이면다른방법을첨가해야할것으로생각되었다. NCCLS에서 ESBL 확인법으로권장하고있는방법은기존의 cefotaxime과 ceftazidime에 clavulanic acid 10 g을첨가한디스크로검사하여억제대직경이 5 mm 이상차이날때 ESBL로판정한다 [13]. 최근에이 ESBL 디스크가상품화되어 (BBL, Oxoid) 임상검사실에서도쉽게사용할수있게되었는데, ESBL 생성 68주로 ESBL 확인법을시행한결과 CTC의양성검출률은 94.1% 이었으며 CZC의양성검출률은 91.2% 로우수하였는데이는황등 [22] 의결과와유사하였다. DDST은주관적인해석에의한문제와더불어결과의신빙성을높이기위해서는 clavulanic acid와검사디스크간의적당한거리가필요한데, 억제대가클경우는디스크간거리가넓어야하며, 억제대가작은경우에는디스크간거리가작아야한다 [10, 12]. 본연구에서는디스크간거리를 20 mm와 30 mm로하여동시에시행하였데, 20 mm로시행한경우의예민도가 30 mm 보다현저하게높았다. 특히 ceftazidime 디스크에서두거리사이에큰차이를보였는데, 디스크간거리를 30 mm로하였을때 20 mm로한경우보다양성률이 45% 낮았다. 그러나디스크간거리를 20 mm로하였을때억제대가넓어상승효과의유무를판단하기에곤란한경우도있었다 (Fig. 1). 디스크간거리를 30 mm로하였을때 4종류항생제에모두상승효과가관찰되지않았던 2예가이었는데, 20 mm 거리에서는상승효과가관찰되어항생제의종류또는균종에따른디스크간거리선정이매우중요하였다. DDST는 ESBL 검출에표준화된술식이없으며 ESBL 생성 E. coli의검출예민도가낮은것이문제가된다고하였는데 [23, 24], 본연구에서는 ESBL 생성 E. coli의예가적

5 ESBL 생성균주검출법의비교 25 어서판단할수없었다. 결론적으로 DDST은 ESBL 생성균의검출에예민한방법이었으나주관적인해석과균주또는항생제에따른적당한디스크간거리의선정문제가있었으며, NCCLS ESBL 확인법은가격이나사용방법이간편해통상적인검사에적당한것으로사료되었다. 그리고 Vitek ESBL 검사는빠르고사용하기편하며대부분의 ESBL 생성균주를예민하게검출할수있는방법이나 cefoxitin에내성인 ESBL 생성균주의검출력에문제가있으므로이경우 NCCLS ESBL 확인법이나다른방법을첨가한다면검출률을보다높일수있을것이다. 요약배경 : Extended-spectrum -lactamase ( 이하 ESBL) 생성균주는이들감염의치료실패와원내감염을야기시킬수있으므로이들균주를신속하고정확하게검출하는것이중요하다. 저자들은임상검사실에서 ESBL 균주를검출하기위하여통상적으로사용되고있는 Vitek ESBL 시험과 NCCLS의디스크확산법을이용한 ESBL 확인법 ( 이하 NCCLS ESBL 확인법 ) 그리고이중디스크확산법 (double disk synergy test, DDST) 을비교하여보았다. 방법 : 총 316주를대상으로 Vitek ESBL 시험과 NCCLS ESBL 확인법을시행하였으며, ESBL 생성 68주로 Vitek ESBL 검사를 NCCLS ESBL 확인법그리고 DDST와비교하였다. Vitek ESBL 시험상위음성을보인 7균주에대하여 cefepime으로 DDST를시행하였다. 결과 : NCCLS ESBL 확인법과비교하여 Vitek ESBL 시험은 1예의위양성 ( 특이도 ; 99.6%), 7예의위음성 ( 예민도 ; 89.7%) 그리고 97.5% 의일치도를보였다. 위양성을보인 7주는모두 cefoxitin 내성이었다. NCCLS ESBL 확인법에서 cefotaxime/clavulanic acid와 ceftazidime/clavulanic acid를이용한경우의양성률은각각 94% 과 91% 이었다. 20 mm와 30 mm의디스크간격을둔 DDST에서각각의양성률은 cefotaxime (95% 및 90%), ceftazidime (100% 및 55%), aztreonam (100% 및 85%) 및 ceftriaxone (95% 및 80%) 이었다. Vitek ESBL 검사상위음성을보인 7균주는 cefepime을이용한 DDST 에서명확한상승효과를보였다. 결론 : Vitek ESBL 검사방법은쉽고검출시간이빠르며예민한검사법으로임상검사실에서유용할것으로사료되나 cefoxitin 내성 ESBL의검출률은낮았다. 이런경우 NCCLS ESBL 확인법등을첨가한다면검출률을보다높일수있을것이다. 참고문헌 1.
