DDD) 과세균의내성률 (%) 과의상관성을보고자하였다. 항균제내성은최근다제내성균들의증가로인하여감염증의치료가점점더어려워져임상적으로중대한문제가되고있다. 그리고, 여러요인들중에서항균제오남용은항균제내성의주요원인이되고있다. 중부권지역의일개 2 차대학병원의입원환자를대상으로병원전산통계자료와진단검사의학과의미생물프로그램을이용하여 2010 년 1 월부터 2014 년 12 월까지혈액배양에서분리동정된 Acinetobacter baumannii, Pseudomonas aeruginosa, Klebsiella pneumoniae, Escherichia coli 균주에대하여항균제사용량에따른균주의내성률과의상관성을분석하기위하여균주내성률과항균제사용량을각각조사하였다. 사용량에따른항균제내성률은 pearson 상관분석을실시하였다. 그결과 cefepime 사용량과 cefepime 내성 E. coli 에서유의한상관관계가있었고 (P<0.033;r=0.907), tobramycin 사용량과 tobramycin 내성 E.coli 에서도유의한음의상관관계가있었다 (P<0.028;r=-0.917). Aminoglycoside 계열내성 A.baumannii 가유의한음의상관관계를나타냈고 (P<0.048;r=-0.881), aminoglycoside 계열내성 E. coli 도유의한음의상관관계를나타냈다 (P<0.001 ;r=-0.992). 결론적으로항균제사용량은일부세균에서내성률과상관성이있었다. Abstract The purpose of the study is to investigate the correlation between the amount of antimicrobial agent (Defined Daily Dose, DDD) and antimicrobial resistance rate (%)..The treatment of infectious diseases is becoming increasingly difficult, due to the increase in the number of multi-drug resistant bacteria, making it a clinically significant problem. Among the various factors, antimicrobial abuse is a major cause of antimicrobial resistance. The study was conducted on inpatients in a secondary university hospital in the central region utilizing the hospital's computerized statistical data and microbiological program of laboratory medicine from January 2010 to December 2014 pertaining to the dose of antimicrobial drugs for Acinetobacter baumannii, Pseudomonas aeruginosa, Klebsiella pneumoniae, and Escherichia coli strains isolated from blood culture. We analyzed the antimicrobial resistance rate per dose with the Pearson correlation coefficient. A significant (positive?) correlation was detected between the cefepime dose and the resistance of E. coli (P<0.033; r=0.907), while a significant negative correlation was found between the tobramycin dose and the resistance of E.coli. (P<0.028; r=-0.917). The aminoglycoside resistance of A. baumannii showed a significant negative correlation (P<0.048; r=-0.881), and the aminoglycoside resistance of E. coli showed a significant negative correlation as well (P<0.001; r=-0.992). In conclusion, the amount of antimicrobial agent (Defined Daily Dose, DDD) (is partly related to) the bacterial strain and its antimicrobial resistance rate (%). 2 한국산학기술학회논문지제 17 권제 5 호, 서론지난 60년동안많은종류의항균제가개발되어임상에이용되고있으나, 이들항균제에대한내성균주가점차증가하고있어심각한문제가되고있다 [1]. 항균제의개발로감염증에의한사망이현저하게감소하였으나, 부적절한항균제의사용은내성균의출현과이로인한의료비의증가및입원기간의연장등의료및사회경제적인문제를야기하기도한다 [2]. 특히우리나라는항균제의과다사용및체계적인관리부족으로항균제내성균의발현율이높은것으로알려져있다 [3]. 항균제내성의여러요인중선택적압력으로작용하는항균제의오남용은내성의주요요인이될수있는데 [4], 사용미달, 환자의약물순응도불량, 부적절한항균제의사용등이항균제오남용의좋은예이다 [5]. 주로다약제내성을보이고원내감염을일으키는균주들은 Acinetobacter baumannii, Pseudomonas aeruginosa, Klebsiella pneumoniae, Escherichia coli 등이있는데, 90년대이후 Pseudomonas aeruginosae와더불어 Acinetobacter baumannii가원내감염의주요원인균으로특히급격히증가하고있다 [6-9]. 더불어 Escherichia와 Klebsiella역시외막에위치하는 porin의구조적변화로인해서항균제의투과성을감소시켜 β-lactam계및 Aminoglycoside 계항균제에자연내성을가지고있다 [10-12]. 항균제의사용증가가항균제내성증가에영향을미친다는일반적인추론은병원과지역에서이루어진다양한연구들을통해서그동안입증되어왔다 [13-15]. 그러나외국에서여러가지객관적인항균제사용량의척도를이용한항균제사용관리에관한많은연구가있었던반면, 아직까지국내에서의연구는많지않은실정이다 [16]. 그리고최근다제내성균들의증가로인하여감염증의치료가어려워져 [9] 점점더임상적으로중대한문제가되고있으며, 이에대한내성연구의중요성이부각되고있다 [17]. 항생제내성에영향을미치는것은항생제의사용량이다 [27]. 이에항균제사용량에따른내성률과의관계를분석하여항균제의제한적사용과오남용방지를위한자료로활용되고자본연구를실시하였다. 2. 연구방법 1. 대상및기간 A.baumannii, P.aeruginosa, K.pneumoniae, E.coli 4 개균주를대상으로 2007년 1월부터 2011년 12월까지중부권지역의 1개 2차대학병원을대상으로병원전산통계자료와진단검사의학과의미생물프로그램을이용하여입원환자의혈액배양검사에서분리된균주를수집하였다. 수집된균주는각각 A. baumannii 1,935주, P.aeruginosa 2,696주, K.pneumoniae 17,550주, E.coli 49,626주로총 71,807 균주가수집되었고, 항균제사용량 (Defined Daily Dose. DDD) 과내성률 (%) 을분석하기위하여 cephalosporin, aminoglycoside, Carbapenem, quinolone, monobactam계열의총 12종항균제에대해조사하였다. 2. 조사방법및내용항균제사용량에따른균주의내성율과의상관성을분석하기위하여항균제사용량과균주의내성율 (%) 을각각조사하였다. 항균제사용량분석은항균제용법에따른기간내소비된의약품의양에대하여약제의고유사용량기준과투약일수, 그리고일평균입원환자수를이용하여일일약제사용량 (Defined Daily Dose. DDD) 으로계산하였다. 세계보건기구 (World Health Organization, WHO) 의 ATC/DDD system을이용한약품별일일사용량 (DDD) 은의약품의소비량및경향을파악할수있게해주며, 이를통하여서로다른국가나기관간의비교를가능하게한다. 일일약제사용량 (Defined Daily Dose. DDD) 의계산식은다음과같다. DDD/1000 habitants/day=[ 기간내소비된의약품의양 (mg) / DDD(mg) * 일수 * 일평균입원환자수 ] * 1000 즉, 항균제사용량은개별항균제와계열별항균제로구분하여각각년단위로분석하였으며, 혈액배양검사에서조사균주가분리된입원환자를대상으로분리시점부터소비된항균제의양과일수, 그리고 DDD (mg) 를더이상분리되지않을때까지조사하였다. 균주의내성률 (%) 은환자가입원한후혈액배양검사에서병원내분리균주로알려져있고분리빈도가높은균주를대상으로하였는데, A.baumannii 1,935주, P.aeruginosa 2,

3 혈액배양에서분리된임상적주요균주의항균제사용량에따른내성률변화 주, K.pneumoniae 17,550주, E.coli 49,626주가년단위로조사되었다. 입원환자에게서중복분리된균주와내성은제외하였으며, 균주내성률의조사는 CLSI (Clinical and Laboratory Standards Institute) 의기준을이용하여감수성 (susceptibility, S), 중등도내성 (intermediate resistance, I), 내성 (resistance, R) 으로하였고, 일반적으로사용되는표준디스크확산법의결과로부터감수성을제외한모든결과를내성으로간주하여균주의항균제내성률을분석하였다. 약제에대한내성률의계산공식은다음과같다. 항균제내성률 (%) = ( 내성균주수 / 분리배양된균주수 ) * 100 주요균주의항균제사용량에따른내성률변화는 Pearson 상관성분석을실시하였다. 자료처리및분석은 Excel 2007 프로그램을이용하여전산자료를취합하였고, 통계분석은 SPSS(PASW) ver.18 통계프로그램을사용하였으며, 유의수준은 P<0.05로하였다. 항균제사용량과내성률은평균과백분율로표현하였다. 3. 연구결과항균제사용량에따른내성률과의상관성을보기위해항균제사용량과균주의내성률을파악하였다. 그결과조사기간 5년동안연구기관의평균항균제사용량은 2007년 18.63, 2008년 26.96, 2009년 24.40, 2010년 28.07, 2011년 DDD/1000 habitants/day이었다. 2007년부터 2008년에급격한사용량의증가를보이다가 2009년부터는완만한사용량의증가를보였다 (Table 1). 단일항균제로서 5년평균사용량을분석해본결과 ciprofloxacin이 DDD/1000 habitants/day로가장많은사용량을보였고, Ceftriaxone 34.72, cefepime 24.36, tobramycin 22.74, amikacin DDD/1000 habitants/day 순으로사용량을보였다. 항균제사용량을 ATC(Anatomic Therapeutic Chemical) 분류별로조사 해본결과 cephalosporin 계열의사용량이가장많았고, quinolone 계열, aminoglycoside 계열, carbapenem 계열순으로사용량을보였다. Monobactam 계열의 aztreonam은 2008년이후로사용이중지되어조사대상에서제외하였다. Cephalosporin 계열은 2007년대비 2008년두배이상의증가세를보이다가 2011년도에는세배이상의증가세를보였다. 특히 3세대항균제인 Ceftriaxone은총 DDD/1000 habitants/day을사용하여가장많은사용량을보였다. Cefotaxime은총 DDD/1000 habitants/day을사용하였고, ceftazidime은총 39.42, 4세대항균제인 cefepime은총 DDD/1000 habitants/day을사용하였다. ceftazidime은총 DDD/1000 habitants/day을사용하여 Cephalosporin 계열중에서가장낮은사용률을보였다. aminoglycoside 계열은 2007년부터점차감소하여 2011년도에는 2007년대비무려 50% 이상사용량의감소를보였다. tobramycin은총 DDD/1000 habitants/day을사용하여 Aminoglycoside 계열중에서가장많은사용량으로조사되었고, amikacin은총 DDD/1000 habitants/day를사용하였다. gentamicin은총 DDD/1000 habitants/day을사용하여 Aminoglycoside 계열중에서가장낮은사용량을보였다. carbapenem 계열은 2007년부터 2009년까지비슷한사용량을보이다가 2010년부터 2007년대비거의두배이상증가하였다. Meropenem은총 DDD/1000 habitants/day을사용하여 imipenem의총 7.95 DDD/1000 habitants/day보다많은사용량을보였다. Quinolone 계열매년비슷하거나소폭증가하는추세를보였다. ciprofloxacin은총 DDD/1000 habitants/ day을사용하였고, Levofloxacin 총 8.98 DDD/1000 habitants/day을사용하였다. levofloxacin보다 ciprofloxacin이절대적인사용량을보였다 (Table 2). 균주별항균제내성률분석에서는, A.baumannii는 cefepime내성률이 2007년 35.7% 에서 2011년 83.3% 로증가하였고, ceftazidime도 21.4% 에서 2011년 50.0% 로 Table 1. Ciprofloxacin과 levofloxacin은 2배이상으로증가하였다. P.aeruginosa는 amikacin, gentamicin, Tobramycin 의내성률이 2007년에비해 2011년에대폭감소하였다. ciprofloxacin, levofloxacin역시대폭감소추세를보였다. amikacin과 gentamicin에대한 K.pneumoniae 내성률은 2007년각각 14.8%, 18.0% 에서 2011년 7.2%, 8.7% 로대폭감소하였다. imipenem, meropenem은조사기간동안에처방되지않았으며, 다른항균제들은 5년동안비슷한내성률을보였다. E.coli에서는 Cefepime, cefotaxime, ceftazidime, ceftriaxone과 Gentamicin, tobramycin내성률이 2007년보다 2011년에 2배이상증 가하였고, imipenem, Meropenem은거의처방이발생하지않았다. Ciprofloxacin, levofloxacin도 2007년에비해지속적으로증가추세를보였다 (Table 3). 항균제사용량과균주내성률과의상관성을분석해본결과단일항균제에서는 cefepime 사용량과 cefepime 내성 E. coli에서유의한양의상관관계가있었다 (P<0.033;r=0.907). 그리고 Tobramycin 사용량과 tobramycin 내성 E.coli에서도유의한음의상관관계가있었다 (P<0.028;r=-0.917). 계열별항균제에서는 Aminoglycoside 계열사용량과 aminoglycoside 내성 A.baumannii에서유의한음의상관관계를나타냈고 (P<0.048;r=-0.881), aminoglycoside계열사용량과 aminoglycoside 내성 E. coli에서도유의한음의상관관계를나타냈다 (P<0.001;r=-0.992) (Table 4). Correlation between the antimicrobial usage and bacterial resistance Bacteria Correlation Analysis Assembly Coefficient P-value E. coli Cefepime( R )*-cefepime DDD Tobramicin( R )*-tobramicin DDD A.baumannii Aminoglycoside(R)*-aminoglycoside DDD E. coli Aminoglycoside(R)*-aminoglycoside DDD *( R ) : resistance 4. 고찰본연구는항균제사용량과내성률의상관성을연구하고자입원환자들의혈액배양검사결과에따른항균제의종류와사용량, 그리고내성률을분석하였다. 항균제에대한노출을줄임으로써내성을감소시킬수있는일은항균제내성의가역성을제시하고있어주요연구쟁점이되고있다. 그러나이전소수의연구들에서그관계를입증한바있을뿐, 국내연구결과를찾기힘든실정이다 [18-20]. 그이유는항균제에대한내성은지역이나질환, 병원의환자군, 그리고사용량과노출된시간에따라서매우다양하게나타나기때문이다. 본연구에서통계적으로유의하게조사된 E.coli균주와 A.baumannii 균주는중요한원내감염균 [21] 일뿐만아니라, 여러가지항균제에대해내성률이높은균주들이다. 특히 A.baumannii는전세계적으로증가하고있고, 다제내성 A.baumannii 역시나타나고있다 [22]. 본연구에서항균제사용량을조사한결과 Quinolone계열의 ciprofloxacin사 용량이가장많이사용된것으로파악됐다. 그다음으로 Cephalosporin계열의 ceftriaxone이많이사용되었는데, 이는 cephalosporin계의사용량이특히높았으며, 그외 quinolone계의사용량도다른국가에비해높은편으로나타났다 [28] 는결과와일치한다. 조사균주에대한상관성분석에서는 E. coli균주는 cefepime항균제사용량에서양의상관관계로 (P<0.033;r=0.907), tobramycin사용량에서유의한음의상관관계로조사되었다 (P<0.028 ;r=-0.917). 또한 aminoglycoside 계열사용량에서 A.baumannii가유의한음의상관관계를나타냈고 (P<0.048;r=-0.881), E.coli 균주도 aminoglycoside 계열사용량에대하여유의한음의상관관계를나타냈다 (P<0.001;r=-0.992). 양의상관성을보인 E.coli는 2010 년 5.71 DDD/1000 habitants/day에서 2014년 DDD/1000 habitants/day로증가한 Cefepime사용량으로인해내성률이증가한것으로생각된다. 이는항균제의사용량증가는선택적압력으로작용하여내성의발생을증가시킨다 [23] 는연구와일맥상통한다. 또한 A.baumannii 657

6 한국산학기술학회논문지제 17 권제 5 호, 2016 역시 2010년 tobramycin이 DDD/1000 habitants/ day에서 2014년 DDD/1000 habitants/day로사용량감소를보여내성률이감소한것으로추측된다. 이또한항균제사용량을줄임으로써내성을감소시킬수있다는연구가뒷받침된다 [24]. 그러나 aminoglycoside사용량이감소하였음에도불구하고내성률이 2010년 13.7% 에서 2014년 28.8% 로증가하여음의상관관계를나타낸 E.coli와, 2010년 35.7% 에서 2013년 66.0% 까지두배이상의내성증가율을보였지만 2014년 16.7% 로감소한 A.baumannii의음의상관관계는눈여겨볼만하다. 이들균주의음의상관관계는사용된양과는반대의결과를보였는데, 이는관찰기간과세균의항균제내성획득기간이다르기때문인것으로파악된다. 항균제사용과해당항균제에대한내성발생간에시간차이가있다면, 시간을고려하지않은분석에서음의상관관계가확인될수있다 [25] 는연구로설명할수있다. 세균의항균제내성발생의기전은선택의압력, 내성의출현및전파등여러가지로이해되고있기때문에그접근방법또한세포학적, 유전학적, 임상적, 역학적연구등다양하다 [26]. 즉, 항균제내성에영향을끼칠수있는요소들을포괄적으로접근하고파악하여야한다. 그러나본연구는각균주별항균제사용량과항균제내성에대한시간적, 기타다른인자의영향을고려하지못하고, 항균제사용량과내성과의상관관계만을분석했다는제한점이있다. 이러한제한점을고려하여추가연구가필요할것으로사료된다. 그럼에도불구하고일부세균에대하여항균제사용량이내성률과상관성이있음을확인할수있었는데, 항균제내성감소를위한방안으로항균제의제한적사용과오남용방지를위한노력이절실히수행되어야하겠다. 5. 결론다약제내성을보이고원내감염을일으키는 A.baumannii, P.aeruginosa, K.pneumoniae, E.coli 균주를대상으로항균제사용량과내성률과의상관성을조사한결과다음과같은결론을얻을수있었다. 1. E. coli균주가 cefepime항균제사용량과상관관계가있었다. (P<0.033;r=0.907) 2. E.coli균주가 tobramycin항균제사용량과상관관계 가있었다. (P<0.028 ;r=-0.917) 3. A.baumannii가 aminoglycoside 계열항균제사용량과상관관계가있었다. (P<0.048; r=-0.881) 4.
