1 심혈관질환에서저고밀도지단백 (HDL) 콜레스테롤혈증치료의중요성 서재빈 정우영 서울대학교의과대학내과학교실, 서울대학교서울특별시립보라매병원내과 The Importance of Treatment of Low HDL Cholesterolemia in Cardiovascular Disease Jae-Bin Seo, Woo-Young Chung Seoul National University College of Medicine, Department of Internal Medicine, Boramae Medical Center 서론 심혈관질환의치료와예방에많은발전이있었음에도불구하고심혈관질환은여전히주요한사망원인이다. 주지하듯이이상지혈증은심혈관질환의핵심적인위험인자중하나로서이의치료는동맥경화증환자의예후에있어서중요한부분이다. 특히심혈관질환예방을위한이상지혈증의치료는 LDL 콜레스테롤 (LDL-C) 의저하를최우선목표로삼고있다. 이는 LDL-C이심혈관질환의명확한독립적위험인자임이잘알려져있으며, 많은연구에서도 LDL-C의저하가심혈관질환의예방에있어서중요한역할을한다는것이입증되어왔기때문이다. 그러나근래에들어서 LDL-C의중요성에가리워졌던 HDL-C의중요성이점점부상하게되었다. 대표적으로 Framingham 연구를살펴보면 HDL -C이높을수록남녀에상관없이심혈관질환의 발병률이낮게나타났고 LDL-C이낮은집단에서도 HDL-C이높을수록심혈관질환의발생이 HDL-C이낮은집단에비해감소하는경향을보였다. 1 또한 Cannon 등이시행한연구에서는 LDL-C를낮추면위험도는감소하지만관상동맥질환은계속나타나게된다고보고하였다. 2 이러한사실들은 LDL-C만이이상지혈증에서심혈관질환의위험인자로작용하는것이아니고 HDL-C 또한독립적인위험인자이고치료목표가되어야함을시사하고있다. 이같은관점은 HDL-C의기능을통해서도유추할수있다. 밝혀진 HDL-C의기능에는혈전생성억제, 접촉분자 (adhesion molecule) 의발현억제, 프로스타사이클린 (prostacyclin) 의생성촉진, 내피세포의세포자살 (apoptosis) 차단, 항산화효과, 콜레스테롤의역수송 (reverse cholesterol transport) 등이있는데이러한기능들은항동맥경화효과에있어서중요한역할을하므로낮은 HDL-C은동맥경화의위험인자가될수있을것임을짐작

2 심혈관질환에서저고밀도지단백 (HDL) 콜레스테롤혈증치료의중요성 하게된다. 3~8 따라서 HDL-C 자체가심혈관질환의위험인자로인식되고있고저 HDL 콜레스테롤혈증의치료의필요성에공감하고있다. 일반인에게도 HDL-C는좋은콜레스테롤로인식되고있다. 특히 NCEP ATP III 지침에서는 HDL-C가높으면위험요인을하나감해주고있다. 9 이에본글에서는이러한 HDL-C의의의와그치료방법에대하여살펴보고자한다. 본론 1. 저 HDL 콜레스테롤혈증의유병률저 HDL 콜레스테롤혈증은그혈중농도가 40 mg/dl 미만으로정의된다. 9 미국의자료를보면 20세이상의성인에서남자 39%, 여자 15%, 전체 26.4% 의유병률을보일정도로비교적흔한질환이다. 10 대개의경우저 HDL 콜레스테롤혈증은고중성지방혈증이나고 LDL 콜레스테롤혈증을수반하기때문에그중요성이크다고할것이다. 11 한국의자료로는국민건강영양조사를들수있다 년조사결과를보면 30세이상에서저 HDL 콜레스테롤혈증은남자 47.9%, 여자 26%, 전체 36.7% 의유병률을보였다. 최근에발표된 30세이상을대상으로한 2007년조사결과를보면 40대에서유병률이제일낮아 42.5% 이고 60대에서유병률이가장높아 56.1% 이다. 이러한결과를통해 2005년에비해저 HDL 콜레스테롤혈증의유병률이증가하였음을알수있고한국에서의저 HDL 콜레스테롤혈증은상당히흔한질환임을알수있다. 2. HDL-C의심혈관보호효과 HDL-C의심혈관보호효과의기전으로는콜레스테롤에기반한기전과콜레스테롤에기반하지않은기전이있다. 전자에해당하는것이콜레스테롤의역수송효과이다. 13 이는 HDL-C이말초조 직으로부터잉여의콜레스테롤을간으로수송하고이것이담도를통해배설되는효과를지칭하는것이다. 후자에해당하는것은항산화효과, 항염증효과, 항혈전효과, 내피세포기능향상효과등이다. HDL-C은활성산소 (reactive oxygen species) 를청소 (scavenge) 하고또한산화된 LDL 인지질의분해를촉매하는효소인 paraoxonase를통하여직접 LDL의산화를억제한다. 4 그리고내피세포의 sphingosine kinase를억제함으로써염증의매개체인 sphingosine 1-phosphate의생성을감소시키고접촉분자의발현을감소시킨다. 5 또한 HDL-C은트롬빈에의해유발되는혈소판집합현상과혈소판섬유소원결합을억제한다. 6 마지막으로내피세포의기능향상에도도움을주는데이에관여하는기전으로는산화질소의생성자극, 프로스타사이클린의생산유도, 내피세포의세포자살차단이있다. 7,8,14 3. 역학적연구를통해본심혈관질환의위험인자로서의 HDL-C 많은역학적연구를통하여저 HDL 콜레스테롤혈증이 LDL-C과상관없이심혈관질환의중요한위험인자임이밝혀졌다. Framingham Heart Study 결과를보면 HDL-C이 40 mg/dl 미만인환자의 44% 에서관상동맥사건이발생하였다. 1 또한 HDL-C이 <35 mg/dl인환자에서는 HDL-C이 >65 mg/dl인환자에비하여관상동맥질환의발생이 8배였다. HDL-C<35 mg/dl인집단이 HDL-C >54 mg/dl인집단에대한관상동맥질환에의한사망률의상대위험도는남자에서는 4.1 여자에서는 3.1이었다. 1989년 Gordon 등이 Framingham Heart Study를포함한 4개의연구결과를종합하여발표한논문에의하면 HDL-C의농도가 1 mg/dl 증가할때마다관상동맥질환의위험도는 2~3% 감소하였다. 15 또한 1991년 Genest 등이발표한논문에의하면혈관조영술을통해확인된관상동맥질환을가진 321명의환자에서거의절반에이르는환자가총콜레스테롤의증가를

3 韓國脂質動脈硬化學會誌 18 卷 2 號 보이지않았지만총콜레스테롤이 200 mg/dl미만인환자에서 75% 정도는 HDL-C이 35 mg/dl 미만이었다. 16 그리고 2007년에발표된 TNT연구에서 LDL-C이 70 mg/dl 미만인집단만을따로분석해보았을때 HDL-C이낮은집단은 HDL-C 이높은집단에비하여주요심혈관사건에대한위험도가더높았다. 17 따라서이러한결과들을종합해볼때저 HDL 콜레스테롤혈증이 LDL-C 과상관없이심혈관질환의위험인자임은자명하다. 4. HDL-C 를증가시킴으로써얻는임상적이득에대한연구 1) 1차예방 Statin 이전의시대에 40~55세의 4,081명을대상으로 5년추적관찰을시행한 Helsinki Heart Study는 gemfibrozil과 placebo를비교하였다. 18 Gemfibrozil 을복용한경우 HDL-C이 11% 증가하였고중성지방이 35% 감소하였으며 LDL-C이 11% 감소하였다. 이러한변화와더불어새로운관상동맥질환의발생이 gemfibrozil군에서 34% 적었고이러한결과는 LDL-C의감소만으로는설명하기어려웠다 ( 상대위험도 0.66). AFCAPS/TexCAPS 연구에서는평균적인농도의중성지방과 LDL-C, 그리고평균이하의 HDL-C 농도를갖는 5608명의남자와 997명의여자를대상으로 lovastatin과 placebo를비교하였다. 19 그결과 lovastatin군에서일차주요관상동맥사건의발생이 37% 적었다 ( 상대위험도 0.63). 이러한결과는 HDL-C이심혈관질환의위험인자로포함되어야함을지지하는것이다. 2) 2차예방대표적으로 Veterans Affairs High Density- Lipoprotein Cholesterol Intervention Trial (VA-HIT) 연구가있다. 20 이연구는낮은 HDL-C 농도 ( 평균 32 mg/dl), 중등도로증가되어있는 LDL-C 농도 ( 평균 111 mg/dl), 그리고관상동맥질환력을가 지고있는 2,531명의환자를대상으로하였다. Gemfibrozil을치료약제로하였고이는 HDL-C를 6% 증가시켰고중성지방을 31% 낮추었다. 그결과 gemfibrozil군에서관상동맥질환사, 비치명적심근경색, 중풍을종합한발생률이 24% 적었다. 또한이러한결과는 LDL-C의변화를동반하지않은것이었다. 회귀분석을시행해보았을때에도 HDL-C의임상적이득은확인되었다. 3) 혈관조영술상효과저 HDL 콜레스테롤혈증환자를포함하여혈관조영술상분석을시행한 2개의연구를살펴보면다음과같다. 395명을대상으로시행한 Lopid Coronary Angiography Trial (LOCAT) 에서는저 HDL 콜레스테롤혈증이주된지질이상인경우 gemfibrozil치료가관상동맥경화의진행을늦출수있다는결론을내릴수있었다. 21 정상 LDL-C 농도와낮은 HDL-C 농도의 160명을대상으로시행한 HDL Atherosclerosis Treatment Study (HATS) 에서는 statin+niacin을치료약제로이용하였다. 22 이경우 LDL-C이 42% 감소하였고 HDL-C 이 26% 증가하였으며동맥경화반의퇴축을가져왔다. 따라서관상동맥질환과저 HDL 콜레스테롤혈증을가진환자에서 simvastatin과 niacin의투여는동맥경화반의퇴축을가져온다고결론내릴수있었다. 5. 치료 1) 생활습관개선 (1) 운동규칙적인유산소운동은건강하지만주로앉아서생활하면서운동량이많지않은사람들에있어서 HDL-C 농도를 3~9% 증가시킨다. 23 개선효과는빈도및강도와밀접한관련이있는데자주하되저강도일때더좋은효과를볼수있다. 일례로 30분씩규칙적으로 1주일에 5회운동하는것을권고한다 (60분씩 3일하는것보다좋은효과를낸다 )

4 심혈관질환에서저고밀도지단백 (HDL) 콜레스테롤혈증치료의중요성 (2) 금연흡연은낮아진 HDL-C과연관성이있으며금연을하게되면평균 4 mg/dl의상승한다. 24,25 이러한결과는여자보다는남자에서그리고기본적인 HDL-C농도가높은사람에서잘나타나게된다. (3) 체중감량체질량지수와 HDL-C사이에는역관계가성립된다. 24~26 안정된감량을이룬사람에서는 1 kg 감량당 0.35 mg/dl의 HDL-C의증가가나타난다. 26 그러나한참감량중인상황에서는 1 kg 감량당 0.27 mg/dl의 HDL-C의감소가나타난다. 바람직한목표는주당 0.45 kg의감량이며최종목표체질량지수는 25 kg/m 2 미만이다. (4) 알코올섭취적당한알코올섭취는 HDL-C의상승을가져오게되는데하루에 30 gram의알코올섭취는평균 4 mg/dl의 HDL-C 상승을가져온다. 그러나중독이될수도있고간기능저하환자에서는합병증을유발할수도있다. 24,27 (5) 식이지방질식이의감소는 LDL-C과 HDL-C의감소를동시에가져온다. 또한식이의당부하 (glycemic load) 정도는 HDL-C농도와역관계가성립된다. 28 따라서 n-3 다가불포화지방산이풍부하면서탄수화물은제한된식이가 HDL-C의상승을위해권고될수있다. 13 2) 약물적치료 HDL-C농도를증가시키기위한약제에는 niacin 과 fibrate, 그리고 statin 등이있다. (1) Niacin (nicotinic acid) Niacin은가장효과적인약제로서 HDL-C농도를 20~35% 증가시킨다. 29 Coronary Drug Project의 연구결과는심근경색의병력이있는환자에게 5 년간 niacin을투여하였을때사망과심근경색의발생률에유의한감소를가져왔다고보고하고있다. 30 또한내피세포기능의향상을가져오고 nitric oxide synthase의활성도를증가시킨다. 31 (2) Fibrates Fibrate치료는 HDL-C농도를 10~25% 증가시킨다. 32 전술하였던 Helsinki Heart Study, VA-HIT 연구, LOCAT 연구에서사용하였던약제가 fibrate로서그효과가증명되어있다. 이계열의약제에는 Fenofibrate, clofibrate와 gemfibrozil이있다. (3) Statin Statin은 LDL-C농도를낮추는주효과에더불어 HDL-C의농도를 2~15% 상승시킨다. 33 Statin 의사용이주된관상동맥사건의발생을낮춘다고보고하고있지만이러한효과에이약제의 HDL-C상승효과가관련이있는지는분명치않다. 19 (4) 혼합요법저 HDL 콜레스테롤혈증환자에서다양한약제의혼합이유용할수있다. 전술한 HATS 연구가대표적이다. 이연구에서는저용량의 simvastatin 과고용량의 niacin 혼합요법이유의하게 HDL-C 농도를높였음 (26%) 을보고하고있다. 혼합요법을이용한또다른연구로는 nicotinic acid와 clofibrate을혼합한 Stockholm Heart Disease Study, niacin과 statin을혼합한 Arterial Biology for the Investigation of the Treatment Effects of Reducing Cholesterol (ARBITER) 2 연구를들수있다. 34,35 결론 심혈관질환은이환율이높고중요한사망원

5 韓國脂質動脈硬化學會誌 18 卷 2 號 인이기때문에위험인자의적절한관리를통하여이환율의감소와막대한의료비용을감소시키는것은국민건강을위하여꼭필요하다. 근래에들어서 HDL-C이심혈관질환에있어서독립적인위험인자로서중요하다는점이널리인식되게되었다. 외국의자료뿐아니라국내에서의통계에서도저 HDL 콜레스테롤혈증이흔하다는점을잘알수있다. 따라서 LDL-C의치료에주안점을두었던이상지혈증의치료에있어서 HDL-C이임상적으로얼마나중요한지에대해서는재론의여지가없다. HDL-C의심장혈관보호효과에대해서는기초의학적연구에의해서제시되었고역학적연구와임상연구를통하여그관리의필요성이잘증명되었다. 실제임상에서는 National Cholesterol Education Program 에서제시하듯이생활습관을그첫번째접근으로하며필요시약물요법을통하여적절한관리를하여야할것이다. 최근에발표된 EPIC-Norfolk연구에의하면 HDL-C이 70 mg/dl가넘거나 HDL particle의크기가큰경우에는그렇지않은사람보다심혈관위험률이 2배나높았다. 36 이러한결과는 HDL-C 이높다고꼭좋은것은아닐지모른다라는문제의식을우리에게던져주고있다. 따라서앞으로저 HDL 콜레스테롤혈증과더불어고 HDL 콜레스테롤혈증의의미에대해서많은연구가이루어지기를기대해본다. 참고문헌 1.
