1 ORIGINAL ARTICLE DOI: /jkap 차필선 정인영 조성암 * 경북대학교치의학전문대학원치과보철학교실 연구목적 : 본연구의목적은경북대학교치과병원보철과에서치료받은환자들의부분무치악의패턴과가철성국소의치의 framework 디자인의빈도를조사하는것이다. 연구재료및방법 : 경북대학교치과병원보철과에서 년사이에수련의가진료한 63 명의환자들을대상으로조사하였다. 각각의부분무치악은 Applegate 가주장한지침을포함한 Kennedy 분류로기록되었고 Kennedy 분류에서추가결손부 (modification area) 는 5 그룹으로다시나누었다. 결과 :Kennedy 분류에따라 Class I 이 63.63% 로가장많이설계되었으며상, 하악모두치아지지국소의치보다후방연장국소의치가더많았다. 또한, 추가결손부가없는가철성국소의치가 83.33% 로가장많았다. 주연결자의분포로는상악에서전후방구개스트랩이 67.57% 로가장많았고하악에서는설측바가 71.79% 로가장많았다. 사용된직접유지장치들중 73.91% 가 RPI 또는 RPA 였고간접유지장치는교합면레스트와견치레스트가 93.83% 로대다수를차지하였다. 결론 : Kennedy Class I 가철성국소의치가가장흔하게사용되었고대부분추가결손부가없는국소의치로디자인되었다. 상악에서는전후방구개스트랩, 하악에서는설측바와설측판이가장흔하게사용되었고직접유지장치로는 RPI 와 RPA 가가장흔하게사용되었으며간접유지장치는교합면레스트와설면레스트가주로사용되었다. Kennedy 분류에따른가철성국소의치 framework 의분포 Class I 이 63.6% 로가장많이설계되었으며상악의 class IV 의수가가장적었다. 상악과하악의비율은거의동일하였으며상, 하악모두치아지지국소의치보다후방연장국소의치가더많았다 (Table 2). 3. 추가결손부의패턴에따른가철성국소의치의분포 추가결손부가없는가철성국소의치가 83.3% 로가장많았다. 추가결손부위는모두 class I 과 II 였으며상악보다하악에서더흔하였다 (Table 3). 4. 나이와성별에따른가철성국소의치의분포 (Table 4) Table 4. U-shape 구개스트랩과설하바와순측바의사용은관찰되지않았다 (Table 5). 6. 사용된직접유지장치의분포 사용된직접유지장들중 73.9% 가 RPI 또는 RPA 였다. 후방연장국소의치에서 RPI 가제일많이사용되었고그다음으로 RPA 가사용되었으며 Aker s clasp 는 20.5% 가사용되었다. Embrasure clasp 는 3.7% 가사용되었으며모두 class II 에서사용되었다 (Table 6). 7. 사용된간접유지장치 간접유지장치는거의모든후방연장국소의치에사용되었으며절단면레스트가 6.2% 였고교합면레스트와견치레스트가 93.8% 로대다수를차지하였다 (Table 7). 고찰 이번연구에서가철성국소의치로치료받은환자는남자가여자보다많았다. 연령이증가될수록치아상실이많아지므로 40 대이후의환자에서가철성국소의치의수가증가됨을보여주었고 60 대이후에서는감소되다가 70 대이상에서는다소증가되는양상을보였다. 이는치아상실로인해부분부치악상태로이행되는시기가 세사이라는 1985 년미국의 NIH 연구결과에서알수있듯 대에부분무치악환자의수가증가하며이시기에가철성국소의치로치료받는환자가많아지는 것으로생각된다. 9 또한후방연장국소의치 (Class I, II) 가치아지지국소의치 (Class III, IV) 보다더많았다. Kennedy class III, IV 부분무치악환자들은고정성국소의치로수복이가능하다. 환자들은비용문제가아니라면고정성국소의치로수복하거나임플란트식립치료를원하기때문이다. 특히 Kennedy class IV 인경우에서는심미적인이유로환자들은고정성수복치료를더선호한다. 이번연구에서도하악전치부에서는 Kennedy class IV 가철성국소의치로디자인된경우가 3 증례가있었으나심미적으로더중요한상악전치부에서는전혀관찰되지않았다 이전연구들은하악의후방연장국소의치 (Class I, II) 가상악의후방연장국소의치보다더흔하다고보고했다. 24 그러나이번연구에서는그차이를관찰할수없었다. 전체의 83.3% 가추가결손부가없는국소의치로디자인되었다. 2 명의환자를제외하고대부분추가결손부위는무치악부가길거나지대치의치주지지가좋지않은경우였다. 이는악궁의연속성을유지하고국소의치의디자인을단순화하기위해고정성보철물로수복하기때문인것같다. 상악에서가장흔한주연결자는전후방구개스트랩이었고하악에서는설측바와설측판의사용이가장많았다. 이전연구에서도 U-shape 구개스트랩보다는전후방구개스트랩의사용빈도가더높다고보고하고있다. 25,26 상악에서주연결자는전후방구개스트랩의사용이가장많았는데이는 U-shape 구개스트랩보다견고하며, 구개판보다는조직피개량이더적으며혀의움직임을더적게방해하기때문인것같다. 하악에서는설측바와설측판이가장흔히사용되는주연결자였는데설측치조열구의설소대부착의높이, 골융기의존재가하악주연결자선택에가장큰영향을미친다. 순측바는설측바나설측판을사용할수없을정도의골융기나설측언더컷이있을경우사용할수있다. 그러나심한골융기는외과적으로제거하기때문에심미적으로좋지않은순측바의사용빈도는적은것으로생각된다. 설하바또한잘못설계시혀의움직임을방해하거나이물감이증가될수있기때문에환자들이꺼려할것으로생각된다. 후방연장국소의치인경우직접유지장치는 94.8% 가 RPI 또는 RPA clasp 를사용하였으며간접유지장치는교합면레스트와견치레스트가대다수를차지하였다. 이는환자조사가대학병원에서이루어진것때문으로추정된다. 이창균등은 RPA 와 RPI 간의지대치에대한응력분산차이를인정하지않았고이석현등은 RPA 가 RPI 보다응력분산에유리한디자인임을보고한바심미적인이유만아니하면 RPA 디자인사용이권장된다. 결론 1. Kennedy class I 가철성국소의치가가장흔하게사용된반면 class III, IV 가철성국소의치가가장적게사용되었다. 대한치과보철학회지 2010 년 48 권 3 호 191

Materials and methods: 63 patients who were treated with removable partial denture in Kyungpook National University hospital for were selected. A total of 76 removable partial denture frameworks were investigated. Kennedy classification was used to identify the class of partial edentulism. Results: Results indicated that Kennedy class I removable partial dentures were the most frequently constructed. Most patients cases were designed without modification areas. Conclusion: The most common type of direct retainer were the RPI clasp and RPA clasp in both maxilla and mandible. Lingual bar, linguoplate and anterior posterior palatal straps were the more frequently used mandibular and maxillary major connectors respectively.
