Toxigenic strains produce toxin A and toxin B. The pathogenicity of C. difficile is due to the production of these two exotoxins. This study aimed to evaluate diagnostic value of two enzyme immunoassay by comparison of concordance rate to diagnose C. difficile-associated infection. Methods: C. DIFF QUIK CHEK (TECHLAB, USA) that detect glutamate dehydrogenase antigen and VIDAS C. difficile Toxin A&B (Bio Mérieux, France) that detect toxin A and toxin B were done in 122 fecal specimens to detect C. difficile. Results: In the total 122 stool specimens, 17 cases showed positive results in both tests. One specimen showed discrepancy that positive result in VIDAS C. difficile Toxin A&B (relative fluorescence value, RFV=2.93) but negative result in C. DIFF QUIK CHEK. Therefore, the concordance rate between two tests was 95.1% (116/122). Both anaerobic culture and in-house PCR for toxin B were negative in the discrepant fecal specimen and there was no clinical evidence that support C. difficile-associated diarrhea, so we concluded result in VIDAS C. difficile Toxin A&B as false positive. Conclusions: Although these two enzyme immunoassays targeted different antigen, they showed high concordance rate. The discrepant case was concluded to false positive in VIDAS C. difficile Toxin A&B test because it showed negative results in culture and PCR for toxin B and there were no clinical evidences of C. difficile-associated infection. It could be needed for analysis about conditions that cause false positive result in enzyme immunoassays to detect C. difficile toxin. C. difficile 연관설사의진단에는대변검체에서배양을통해직접균을확인하거나균이생성하는독소를검출하는방법이권장된다 [5]. 현재진단의표준방법은세포독성검사법 (cytotoxic assay) 으로서세포를배양하여독소 B 의세포병변효과 (cytopathic effect) 를확인하는방법이다 [6,7]. 그러나이방법은배양법처럼시간이오래걸리고 ( 적어도 48시간이상 ) 표준화하기어려운단점을가져최근신속한진단을위해효소면역법이많이사용되고있다. 효소면역법의주요검출대상은독소 A 또는독소 A와 B, 그리고독소생성균주나비생성균주가모두생성하는글루탐산탈수소효소 (glutamate dehydrogenase, GDH) 등이다 [8]. 대변검체에서효소면역법을통하여독소 A를검출한국내의연구에따르면 % 의검출률을보였으며임상상혹은세균배양을기준으로할때 80% 정도의일치율을나타내었다 [9,10]. 최근에는독소 A를생성하지않는변이주로인한감염의보고가증가하고있어독소 A만을검출하는검사법은한계를가진다 [8]. 본연구에서는독소 A와 B를검출대상으로하여독소생성균주만을대상으로하는방법과독소생성균주와비생성균주가모두생성하는 GDH 효소를검출대상으로하는두가지종류의효소면역검사법간일치율비교를통해두검사법의진단적효용성을알아보고병원내 C. difficile 의독소생성균주와비생성균주분포비를추정해보고자하였다. 대상및방법항균제연관설사증및기타원인에의한설사를주소로하여 2008 년 1월과 2월사이에 C. difficile 검사가의뢰된 122 례의대변검체에대해 C. difficile 이생성하는효소인 GDH 를검출하는 C. DIFF QUIK CHEK (TECHLAB, Blacksburg, VA, USA) 검사와독소 A와 B를검출하는 VIDAS C. difficile Toxin A&B (Bio Mérieux, Marcy-I Etoile, France) 검사를동시에시행하였다. GDH 의검출은 rapid membrane immunoassay 원리를이용하여검체내에 C. difficile의존재를확인하는방법으로키트내에들어있는희석제와접합제의혼합용액에분변검체를가하여희석된검체를반응창 (reaction window) 에떨어뜨린후 15분후에판독하였다. 독소 A와 B의검출은 enzyme linked fluorescent immunoassay (ELFA) 원리를이용하여 450 nm에서형광강도를측정하는방법으로제품설명서에기술된대로 relative fluorescence value (RFV) 가 0.37이상이면양성, 0.13 이상 미만이면 equivocal, 0.13 미만이면음성으로판정하였다. 두검사사이에서불일치결과를보인경우에대해서는항원검사와동일한검체로 C. difficile selective agar (CDSA) 에서 48시간동안 C. difficile의혐기성배양을시행하였고또한독소 B에대한자가제조 PCR 을시행하였다 [11]. VIDAS 에서 equivocal 로나타난검체에대해서는배양이나 PCR 등의추가검사를시행하지못하였다. 평균값의비교는 Medcalc 12.0 (Medcalc Software, Mariakerke, Belgium) 에서 t-test 를통하여확인하였고 P < 0.05 일때통계적으로유의하다고판단하였다. 결과환자의평균연령은 62.6 세였으며남녀비는 1.2:1 이었다. 총환자수는 87명이었고 122 개의검체중 21개에서는 1회이상검사가의뢰되었다. 122 개의대변검체중두검사에서모두양성을보인경우는 17례로전체의 13.9% 였다. C. DIFF QUIK CHEK 에서음성을보인 5 예에서 VIDAS C. difficile Toxin A & B에서는 equivocal 한결과를나타내었다. 단 1례에서만 C. DIFF QUIK CHEK 에서음성, VIDAS C. difficile Toxin A&B 에서양성 (relative fluorescence value, RFV=2.93) 으로불일치결과를보여두검사간결과의일치율은 95.1% (116/122) 였다 (Table 1). 검사가가장많이의뢰된과는신경외과 (23.0%), 내과 (22.1%), 중환자실 (5.7%) 순이었으며특히신경외과에서의뢰된검체에서는 39.3%(11/28) 에서두검사가모두양성을보였다. 기저질환별분류에서는뇌경색과뇌출혈, 무산소성뇌손상 (anoxic brain damage) 등의중추신경계질환이 41.7% 를차지하였고악성종양환자가 13.9% 로두번째높은빈도를나타내었다. 두검사에서모두양성을보인 17명중 8명에서추적검사가시행되었고모두에서음성으로전환되었다. 두검사에서모두양성을보인군 (17 명 ) 과음성을보 Table 1. DIFF QUIK CHEK and VIDAS C. difficile Toxin A&B 인군 (99 명 ) 사이에 C반응단백질 (C-reactive protein, CRP) 평균값은통계적으로유의한차이가없었다 (Fig. 1). 불일치를보인대변검체에대해서는 CDSA 에서 48시간동안혐기성배양을시행하였고또한독소 B에대한자가제조 PCR 를시행하였으며그결과는모두음성으로나타났다. 불일치를보였던환자는뇌수술을받은후 3차세팔로스포린, 반코마이신, 피페라실린-타조박탐등의항균제를장기간사용중이었다. 발열이나복통등의특이한임상증상은없었으며혈변이나점액변이아닌무른변을보여 2008년 2월 C. difficile 검사가의뢰되었다. 이후환자는대장염과관련된특별한증상을호소하지않아추가적인치료가시행되지않았으며 C. difficile 에관련된추가검사도요청되지않았다. 고 C. difficile 는항균제나항암요법등장내정상균무리에변화를줄수있는치료를받은환자에게서나타나는병원획득성대장염의주요원인균으로인식되고있다 [12]. 환자들은단순히유발약제를끊는것에반응을보이나대부분은 metronidazole 이나 vancomycin 으로치료를받아야한다 [13]. 격리나소독등의방법으로균의전파를최소화하려는노력이있으나포자의경우완전한제거가어렵기때문에집단발병이수년간지속되기도한다 [14]. 다행스럽게도최근이에대한인식의증가와접근성이향상된진단법의등장으로인해임상의들은빠르고정확한진단과이를바탕으로한정확한치료적접근이가능하게되었다 [15]. C. difficile 에대한검사에는혐기성배양법, 세포독성법 찰 그리고일반적항원이나독소에대한항체를이용한검사등이있으며결과는모두환자의병력과연관해서해석해야한다 [8]. 배양법은시간이오래걸리고비용이높다는단점을가진다 [16]. 또한독소생성균주인지를확인해줄수있는추가적검사가요구된다. 독소생성균주대비생성균주의비는잘알려져있지않지만 Tracy 와 David[8] 등은환자에서분리된 C. difficile의 75% 에서 80% 가독소를생성하였다고보고하였다. 이에따라본연구에서도 GDH 를검출대상으로하는검사에서더많은양성이나올것으로예상하였으나 Diff Quik 에서양성이나온 17례모두독소 A와 B를검출하는 VIDAS 검사에서양성이나와모두독소생성균주로나타났다. 불일치를나타낸경우는 1례뿐이었고 VIDAS 에서 equivocal 을보인 5례를제외하여도 VIDAS 와 Diff Quik 은높은일치율을보여주었다. 이러한결과의원인은다음과같이추정해볼수있다. 첫째, 검체를수집할당시원내에독소생산 C. difficile 균주의 outbreak 가있었을가능성이다. 그러나균주들을대상으로역학조사가이루어지지않았기때문에이러한추론은한계를가진다. 둘째, Diff Quik 에위음성을나타내는검체들이있었을가능성이다. 그러나 Diff Quik 이 93.5% 의민감도를나타냈다고보고한 Reyes 등 [4] 의연구를볼때이러한가능성에도큰비중을두기는어렵다. 셋째, C. difficile 검사가의뢰된대부분의환자들이항균제를투여받은후설사증상을보이는경우였으므로독소생산균주에감염된환자들만선택되어검사가의뢰된선택오차 (selection bias) 가개입하였을가능성이다. 이렇게두검사의높은일치율에대한정확한해석을위해서는긴기간동안좀더다양하고많은검체를통해추가적인연구가필요하다고사료된다. Hardt 등 [17] 은진단시의혈청 CRP 농도는심한 C. difficile 연관설사의독립적인예측인자라고보고하였다. 본연구에서두검사에서모두양성을보인군과음성을보인군사이에 CRP 평균값은통계적으로유의한차이가없었다. 또한신경외과에서의뢰된검체에서특히높은양성률을보인것은신경외과의높은항균제사용량그리고장기입원환자가많은특징과관련있을수있다고생각된다. 불일치를보인한예는 CDSA 배지에서시행한 C. difficile 의혐기성배양과독소 B에대한자가제조 PCR 검사에서모두음성을보여 C. DIFF QUIK CHEK 에서나타난결과와일치하였으므로검사적으로는 VIDAS C. difficile Toxin A&B의위양성일가능성이높아보이나임상적으로는항생제를장기간사용중인설사환자였고또한환자에게사용된세팔로스포린은 C. difficile 연관설사를잘유발하는항균제중하나로보고된바있으므로 [13,18] C. difficile 연관설사를완전히배제할수는없다. 향후 C. difficile 독소를검출하는효소면역검사의위양성유발조건에대한분석이필요할것으로사료되었다. 또한 C. 4 284 조선영 유재권 김명희 이희주 음성을보이고 VIDAS C. difficile Toxin A&B에서는 equivocal 한결과를나타낸 5례에대해서는두검사의민감도의차이에서기인한결과로해석할수도있겠으나재검을하거나배양, PCR 등의추가적검사로확진하는과정이진행되지않아이후연구에서는이점에대한보완이필요할것으로사료된다. 요약배경 : Clostridium difficile는항균제유발설사의가장흔한원인균이다. 독소생성균주는독소 A, B를생성하며이두종류의독소는 C. difficile 의주요한독성인자이다. 본연구에서는 C. difficile 감염증의진단을위한두종류의효소면역법결과의일치율비교를통해두검사법의진단적의의를평가하고자하였다. 방법 : C. difficile 검사가의뢰된 122 개의대변검체에대해 glutamate dehydrogenase 를검출하는 C.
