The approaches can conveniently be divided into external and intranasal techniques. We prefer the external approach to the intranasal approach because the former provides better operative fields and more complete correction. ObjectivesThe purpose of this report is to analyze the experiences of rhinoplasty through the external approach. Materials and MethodsA retrospective study was performed in 119 patients who underwent rhinoplasty using the external approach from January of 1994 through March of ResultsThis procedure has been used in all kinds of deformities even though most of them overlapped54 cases of crooked nose, 57 cases of saddle nose, 5 cases of hump nose and 3 cases of tip surgery. ConclusionThe exact procedures of cosmetic nasal tip surgery can be performed more easily by the external approach. The disadventages of this procedure, which is the columellar scar, longer operation time and post-operative edema, are neglible. (Fig. 1, 2, 3, 4)(Table 4). 비외접근법에 의하여 발 생한 부작용으로는 비교부위 반흔이 2례에서 문제시 되었는데, 1례는 술후 감염으로 인해 이식물의 유실과 함께 비교 반흔이 커졌으며, 나머지 1례는 재수술의 경우로써 1차 수술시 Silastic 삽입을 위해 비교에 넣 은 종절개의 반흔을 없애기 위해 Z-plasty를 시행하 였으나 술후 반흔이 좀더 눈에 띄었다(Fig. 5, 6). 1례에서 피판을 너무 천층박리하여 2차 수술후 혈 액공급의 감소로 피부위축이 발생하였으며, 1례에서 피 판을 올리는 과정에서 피부 천공이 유발 되었으나 큰 문제는 없었다(Fig. 7, 8). 수술직후 비첨부의 혈종이 c. after healing, unacceptable columellar scar. 4례에서 발생하였으나 처치없이 치유되었다(Table 5). 5 Korean J Otolaryngol 40 8, August 1997 고 찰 외비성형술시 접근법은 비내접근법과 비외접근법으 로 분류된다. 비외접근법은 1934년 Emile Rethi가 비첨부에 대한 접근법으로 경비교 절개술(transcolumellar incision)을 처음 소개하면서 알려지게 되었 으나1) 그동안 비내접근법으로 시술이 어려운 경우에 서만 사용되어 왔으며 비외접근법에 대한 연구는 활 발하지 않았다. A case showing alar rim retraction(arrow). 에서 비외접근법을 사용하여 좋은 결과를 얻었다고 발표하였으나 비교부위의 반흔때문에 쉽게 받아들여 지지 않았으며 특히 미용 외비성형술시에는 더욱 그 러하였다2). 그러나 1980년초부터 비교부위의 반흔이 큰 문제가 되지않으며 더 만족스러운 결과를 얻을 수 있다는 보고와 함께 현재는 비외접근법을 선호하는 것 이 세계적인 추세이다. 이 접근법의 가장 중요한 장점 은 첫째, 술자에게 좋은 시야를 제공한다는 것이다. 상, 하비익연골과 비골 그리고 최대 비근점까지의 비 배부를 한 시야에서 관찰할 수 있으므로 비대칭적인 구조와 다양한 비변형의 완전한 교정이 가능하고, 융 a. immedate postoperative basal view. 비나 비첨성형술시 이식물의 정확한 삽입과 고정이 가 능하다3)4)5). 둘째, 재수술 이나 심한 외상을 받았던 코에서 볼수있는 반흔을 쉽게 조작할 수 있으며, 비내 접근법으로 조작이 어려운 두꺼운 피부의 경우 이 접 근법을 사용하여 보다 강력한 효과를 줄 수 있는 술식 의 사용이 가능하여 보다 정교한 교정을 가능하게 한 다4). 셋째, 비내접근법중 가장 많이 사용되는 연골간 절개는 비 발브부위에 절개가 가해지므로 반흔으로 인 한 비폐색의 문제가 발생할 수 있으나 비외접근법에 서는 이러한 문제의 발생소지가 적다4). 넷째, 출혈부 위의 정확한 지혈이 가능하며, 다섯째는 양손을 사용 할 수 있다는 것이다6). 그외에 비중격에 대한 완전한 b. postoperative basal view. 노출이 가능하여 비중격 미부(caudal septum)의 조 Fig. 8.
