Treatment of acute promyelocytic leukemia with all-trans retinoic acid or arsenic trioxide in association with anthracyclin has improved outcomes in this disease. We report the results of all-trans retinoic acid and arsenic trioxide-based treatment regimens in acute promyelocytic leukemia patients diagnosed in a single center. Methods : Thirty-seven treatment events in 26 patients with acute promyelocytic leukemia were retrospectively reviewed. Results : Complete remission was achieved in 29 (78.4%) of 37 treatment events. One-year overall survival rate and 1-year event-free survival rate were 74.3% and 66.3%, respectively. Among 16 patients treated with an arsenic trioxide-based regimen, 14 patients (87.5%) achieved complete remission. The 1-year overall survival rate and 1-year event-free survival rate were 78.4% and 69.6%, respectively. Conclusions : The short-term use of arsenic trioxide for salvage treatment of acute promyelocytic leukemia seems to be efficacious and could be a bridge to intensive chemotherapy or stem cell transplantation in terms of minimizing the risk of severe disseminated intravascular coagulation. (Korean J Med 74:596-64, 28) Key Words : Acute promyelocytic leukemia; All-trans retinoic acid; Arsenic trioxide 서 급성전골수성백혈병 (acute promyelocytic leukemia, APL) 은다른백혈병과뚜렷이구분되는세포학적모양과특이적인 15 번, 17 번염색체전좌 [t(15; 17) translocation] 의유전적형질, 그리고이병의높은사망률에공헌하는파종성혈 론 관내응고(disseminated intravascular coagulation, DIC) 같은임상증상등으로특징지어진다 1, 2). 2 Yoon Young Cho, et al : ATRA or ATO based treatment for AML M3 존율을보여주고있다 3, 4). 그래서새로진단된 APL 환자에게는 ATRA와 anthracyclin을바탕으로한항암화학요법을병용하여관해유도와공고치료를시행하고유지요법으로 5-8) 주기적으로 ATRA를사용한다. 하지만 2 3% 의환자들은재발하게되고이러한환자들에게 ATRA를또다시사용하여재관해를이루기는어렵기때문에공격적인구제항암화학요법을시행하지만독성이심하고완치율도기대보다낮다 7). ATRA 치료에불응성이거나재발한경우이차치료제로서비소 [Arsenic Trioxide (As 2O 3 ), ATO] 사용이많이보고되고있으며이경우비교적높은완전관해율을보여주어 APL 의이차치료로써효과가있을것으로생각된다. 199 년대중국에서재발성또는불응성인 APL 환자에게서직접조제한 ATO를사용하여완전관해를유도했다는보고가있었으며이후미국에서이루어진재발또는불응성 APL 을대상으로한여러파일럿연구와다기관연구에서도 87% 정도의완전관해율을보여주었다. 최근의몇몇연구들에서처음진단된 APL 환자들에게 ATO 를단독사용하여 83 95% 의완전관해율과 8% 의무병생존율을그리고 ATRA와병용사용시에는 88 1% 의완전관해율을보여주고있다 1-13). ATO는대부분의환자에게서경증또는중등도의부작용만을일으키는비교적안전하고효과적인 APL 치료제로 14, 15) 보고되고있다.ATO의정확한약리기전은아직명확하게밝혀지지않았지만 ATO에장기적으로노출될때 wild type PML 과 PML/RARα의키메라단백에분해를일으켜급성전골수성백혈병의세포들의부분적인분화를유도한다고한다 16). 뿐만아니라간접적으로 H 2O 2 의이화작용을방해하고미토콘드리아막의기능을감소시키고또한 cytochrome C 분비를통해세포의자연사멸을유도한다 3). 이번연구는본기관에서진단된 APL 환자를분석해 ATRA와 ATO 를이용한치료성적을살펴보고자하였다. 1. 대상 대상및방법 1999년 2월부터 26년 9월까지경북대학교병원에서 APL로진단받고치료받은 37 치료증례를대상으로후향적분석을시행하였다. 이 37 치료증례에는 APL로첫진단받은 26 증례와재발한 9 증례그리고두번째재발인 2 증례가포함되었다. 각각은모두처음진단받은당시APL의형 9) 태학적진단기준을만족하였으며 15 번, 17번염색체전좌 [t(15; 17) translocation] 와PML/RARα 유전자전위를확인하였다. 2. 방법 APL로처음진단된환자들은첫치료로 ATRA 45 mg/m 2 /day 를두차례로나누어매일투여받았다. 이후혈액검사상 DIC의증거가없고백혈모세포나전골수세포가감소하면 idarubicin 12 mg/m 2 /day를 3 일동안투여받았다. 또는 ATRA와항암화학치료를동시에시행하기도했는데이런경우에는 idarubicin 12 mg/m 2 /day를 3일동안투여하거나 cytarabine을 3 mg/m 2 /day 의저용량으로투여받았다. 만약 ATRA의부작용으로투약을지속하기어렵거나 ATRA 에저항성이있는경우에는매일 ATO mg/kg 또는 anthracyclin 을포함한다른항암제를투여받았다. ATO는경북대학교병원에서직접조제하여사용하였고하루용량으로 1 mg ( mg/kg) 를 5 ml의생리식염수에희석하여두시간이상천천히주입하였다. 치료기간은부작용이없는경우에는골수검사상전골수세포가 1% 이하가될때까지또는 DIC가완화될때까지비교적단기간으로사용하였다.ATO사용중에는매일이학적검사, 전혈검사와심전도검사를시행했으며적어도일주일에한번이상간및신장기능검사와전해질검사를시행했다. 결과에따라필요시에는충분한수액과전해질을보충했으며적절한약물치료를하였다.ATO사용중부작용으로의심되는임상증상, 또는혈액검사나심전도상이상소견이발견되면 ATO 투여를중지하였고, 이상소견이호전되면재투여했다. 또한질병의악화나 ATO 증후군이나타나면투여를중단하였다. 환자의상태에따라 ATO 투여중이나중지시에 ATRA 투여또는항암화학치료를시행하였다. 완전관해가된환자들은 3주기의공고항암화학요법을시행하였다. 1주기는 idarubicin 7 mg/m 2 를 4일동안투여하고 2주기는 mitoxantrone 1 mg/m 2 을 3 일동안, 3주기는 idarubicin 12 mg/m 2 을하루또는이틀투여하였다. 3. 치료반응 골수검사상백혈모세포와전골수세포가 5% 미만이면서말초혈액검사상백혈모세포와전골수세포가보이지않으면서모든세포들이성숙한상태를완전관해로정의하였다. 또한역전사효소중합연쇄반응 (reverse transcriptase polymerase chain reaction, RT-PCR) 을이용해PML-RARα 검사

ATRA를수일사용후순차적으로항암화학치료를시행한경우는 6 예였다. 첫관해유도시ATRA 사용후반응이없거나부작용으로 ATO를사용한경우는 5 예였다. ATO 사용후반응정도에따라항암화학치료를추가하기도하였다. 대부분특히진단당시 DIC가있었던환자들은 ATRA 또는 ATO 사용후말초혈액또는골수에서전골수세포가감소하는것을확인한후항암화학치료를시행했다( 표 2). 분석한예중총 29 예의(78.4%) 치료에서관해유도치료

(B) 1-year event-free survival of patients treated with ATO. 후완전관해를이루었으며 3 예에서관해유도에실패했다. 총 5 예(13.5%) 에서반응평가를할수없었는데 2예는 ATRA 치료도중DIC로인해사망했으며 3예는검사및더이상의치료를거부해평가가불가능하였다.ATRA와병용또는순차적으로시행한항암화학치료도중에 DIC가악화되거나새로생긴경우는 2 예(5.4%) 에서관찰되었다( 표 2). APL 치료증례의총생존일의중간값은 637 일( 범위: 일) 이었으며일년생존률은 74.3% 였다. 무사고생존일의중간값은 491 일( 범위: 일) 이었고, 일년무사고생존율은 66.3% 였다( 그림 1). 2. ATO 를사용한치료결과 서른일곱치료증례중총 16예에서 ATO 로치료받았고 ATO의중간치료기간은 1 일( 범위: 3-15 일) 이었다. 3예는첫치료시 ATRA 에저항성을보였던경우였고, 2예는 ATRA 부작용으로인해 ATRA를더이상사용할수없어치료를중단하고사용한경우였다.11예는 ATRA를사용한첫치료에완전관해를획득한후재발한경우였다( 표 3). 재발시 ATO를우선적으로사용한경우는 8 예였다. 7 예 (43.8%) 에서 ATO 만으로치료후완전관해에도달했으며총 14예 (87.5%) 에서ATO 및ATRA 또는다른항암화학치료후완

ATO를사용한 16예의총생존일의중간값은 386 일( 범위: 일) 이었고일년생존율은 78.4% 였다. 일년무사고생존율은 69.6% 였으며중간값은 343 일( 범위: 일) 이었다그림 ( 2). 3. 약물이상반응 ATRA 를사용한반응평가가가능했던 35 치료증례( 재발포함) 중 24 (68.6%) 예에서이상반응이관찰되었다. 이상반응으로 ATRA 를완전히중단한경우는 2예에불과했고 3 예에서는이상반응으로인해 ATRA 일시중단하거나스테로이드사용후증상호전되어 ATRA 를다시사용하였다. 5 예는이상반응의증상이경미해별다른치료없이 ATRA 만계속사용했으며 4예에서는 ATRA를계속사용하면서스테로이드를사용했다. 가장흔히나타난이상반응은두통과흉막또는심막삼출액이었으며각각 9예에서관찰되었으나 3 도이상의중중증상은관찰되지않았다. 근염이나근육통의증상과발열도각각 7 예에서관찰되었다. 백혈구증다증, 간효소수치증가, 중성지방증가같은실험실수치이상도각각 4 예, 2 예, 1예에서보였으나대부분경미했다.ATRA로인한이상반응중3도이상의중증이상반응 Table 3. 6 Yoon Young Cho, et al : ATRA or ATO based treatment for AML M3 도가약해서 ATO 투약을중단하거나스테로이드를추가하면회복되었다.ATO사용에서흔히나타나는간효소수치증가는모두 7예에서관찰되었는데본연구에서는 3도이상의고도독성은관찰되지않았다. 간독성이발견되면대부분 ATO 투약을중단하였고, 1 2 주후에는정상간수치로회복되었다. 위장관계독성은 8예에서나타났는데가장흔한이상반응이었다. 대부분오심의증상으로나타났으며 3 도이상의독성은나타나지않았다. ATO의중요한부작용중하나인 QT 간격연장은총 3 예에서나타났다. 연장된 QT 간격의평균은.467이었으며부정맥이발견된경우는없었다.QT간격연장이관찰되면 ATO투약을중단하였다. 분석된증례들중에서 7 (37%) 예에서폐침윤, 수분저류, 발열같은분화증후군(differentiation syndrome) 또는레티노이드증후군과비슷한증상을보였다. 이중 3예에서는 ATO와 ATRA 를같이복용중이었다. ATO 복용중발생된이상반응은거의 2도이하의경미한증상을나타내었으며한예에서정신이상, 우울증의중증정신신경학적증상을보였다. 이증상은 ATO 투약중단후호전되었다표 ( 4). 고 European APL study group의보고에의하면 ATRA 와항암화학요법을동시에사용했을때와순차적으로사용했을 17) 때각각84% 와77% 의높은무병생존율을보여주었다. 하지만여전히 2 3% 의환자들은완전관해후에도재발하게되며이런재발한환자들에게서의치료가문제로남아있다. 이번본기관의분석에서 APL로처음진단받은환자의완전관해율은 76.9% 였고, 재발된경우를포함한전체 APL의완전관해율은 78.4% 였다. 첫진단받는당시의완전관해율이더낮은것은첫치료중 DIC로인해사망하거나치료또는반응평가를위한검사를거부해반응평가를하지못했던경우가 13.5% 로포함되어있어서그럴것으로생각된다. GIMEMA 나 PETHEMA 연구에서는 APL의첫치료로 ATRA와 anthracyclin 계의항암화학치료를병행하여 18, 19) 완전관해율및생존율의유의한증가를보여주었으나본기관에서는 4 예(1%) 에서만 ATRA 와idarubicin의병합요법을사용하였다. 대부분의 APL 환자의첫치료로 ATRA 또는 ATO를사용한후말초혈액이나골수검사상골수모세포또는전골수세포가감소하는것이확인되면순차적으로항암화학치료를시작하였고, 항암화학치료를병행하는경우에는저용량 cytarabine도주로사용하였는데주목적은 찰 ATRA 증후군으로인한백혈구증가증의예방및 DIC 악화를최소화하고자하는이유였다. 이러한치료방법으로 APL 환자에게서사망의주요원인이되는 DIC로인한사망은항암화학치료후또는도중에 2 예(5.4%) 에서만관찰되었다. 첫치료후재발율은 34.5% 로일반적인보고보다는높은재발율을보여주었다. 재발시, 예전에 ATRA 사용경험이있는환자에게는 ATRA 단독사용만으로는관해가잘되지않거나관해가된다하더라도단기간만지속될가능성이있고특히 ATRA 사용중또는사용후짧은기간내에재발된환자들은 ATRA에고도내성을가진다 2-24). ATRA 외에도구제항암화학요법을통해재발시관해유도를시도할수있지만치료의독성이문제가된다. 분자생물학적재발만있는경우에는적극적인구제항암화학요법으로심각한독성없이완전관해가될수있지만명백한혈액학적재발시에는이러한치료가권장되지않으며 8) 실상이런환자들은재발시에 DIC의임상양상을띄는경우가자주있기때문에재관해유도시에사망하기도한다. 더구나완전관해가된다하더라도치료의독성때문에조혈모세포이식이용이하지않을수도있다 25). 그래서이식을시행할경우라도이식전에상대적으로낮은독성을가진치료제로관해유도하는시도가필요하겠다. Thomas 등의보고 26) 에의하면재발한 5명의 APL 환자에서완전관해는 9% 였으며그중동종조혈모세포이식을시행한 11명의환자중 8명이완전관해상태는지속되었지만중중증감염이나조직편대숙주반응으로사망했다. 이식전집중적인항암화학요법으로발생된독성이높은사망률과관련이있음을보여주는연구결과였다. 이러한연구결과들로인해독성은적으면서효과는높은재관해유도를위한또다른약물의필요성이대두되었다. 그러던중중국의한연구에서 ATO가 ATRA와일반적인항암화학치료에저항적이거나재발한경우의 APL 환자에게서유용한구제치료라고보고하였고 13) 더욱이최근에는 ATO 화합물에대한효과와안정성이확립되었다 9). 이번연구에서는첫관해유도시 ATRA 치료에저항적이었던 3예와부작용으로사용이어려웠던 2 예, 그리고첫번째완전관해후재발한 11 예의경우에서(9 예는첫번째재발,2 예는두번째재발)ATO 를사용했다.ATO를사용한 16증례중다른약제사용전 ATO 만으로완전관해에도달한경우는 7예로 43.8% 였으며 ATO의부작용으로또는불응성으로인해다른항암제를병합하거나바꾸어사용한경우를포함했을때전체적인이차치료후완전관해율은
7 대한내과학회지 : 제 74권제6호통권제574 호 % 로비교적높은관해율을보여주었다. 치료초기 ATO 단독사용으로관해에이른경우가 43.8% 로비교적낮은이유로는본연구에서본원자체에서제조한 ATO를사용함으로써부작용등의안전성측면을걱정하여사용기간을최소화하였기때문으로추정된다[ 중간사용기간: 1 일, ( 범위: 3-15 일)]. 환자들의일년생존율은 78.4% 였으며일년무사고생존율은 69.6% 로비교적좋은결과를보여주었다. ATO 와관련된부작용은수분저류, 말초신경병증, QT 간격연장, 고혈당증, 저칼륨혈증, 피부반응등이보고된다 27). 대부분의부작용은생명을위협할정도는아니지만심장부정맥, 돌연사또는설명되지않는사망같은치명적인부작용도보고된다 28, 29). 그래서 ATO 사용시에는적극적인전해질교정과심전도검사가필요하다. 효과가유지되는범위내에서의용량을경감해심장독성을줄일수있긴하지만이전에심장이상이있던사람에게 ATO 사용시에는특별한주의가필요하다 3). APL 분화증후군은 ATO로치료받는 APL 환자들이치료를중단하게되는가장주요한원인이며일부에서는이로인한치명적인부작용을보고하기도한다. 본연구에서는 APL 분화증후군이의심되었던환자의대부분은다른증상이없는백혈구증다증이었고, 이경우에는저절로호전이되었다. 또증상이있었던환자들대부분은 ATO 일시중단또는스테로이드사용으로증상이조절되어 ATO를계속사용할수있었다. 우리연구에서비록수가적기는하지만재발한 APL에서직접조제한 ATO의사용이가능한선택중의하나라는것이확인되었다. 완전관해율이높을뿐아니라골수억제작용이나독성은적어서,ATO치료이후동종또는자가조혈모세포이식이용이하며, 이식시에도이식관련합병증이나사망률을낮출수있을것으로기대된다. 요 목적 : 본기관에서진단된 APL 증례를분석하여치료방법, 결과, 관해율과생존율을알아보고아울러이차적치료약제로서 ATO 의효용성에대해알아보고자하였다. 방법 : 1999년 2월부터 26년 9월까지경북대학교병원에서 APL로진단된 26명의환자들에서 37차례의항암치료를시행한결과를후향적으로분석했다. 새로진단된환자는첫치료로 ATRA (45 mg/m 2 daily) 를복용했으며만약 ATRA 에반응하지않거나부작용으로복용할수없는경우 ATO [1 mg ( mg/kg) daily] 를투여받거나또는 약 anthracyclin 을기초로한항암화학요법을시행받았다. 결과 : 첫진단된또는재발된 APL 환자를대상으로총 37 치료증례를분석하였다. ATRA 및항암화학치료또는 ATO 치료후총 29 예(78.4%) 가완전관해에도달했다. 일년생존율은 74.3% 였으며일년무사고생존율은 66.3% 였다. 그중총 16예가 ATO로치료받았으며그중 3예는 ATRA에불응성이었고,2예는부작용으로인해 ATRA사용을할수없는환자였으며 11 예는재발된환자였다. ATO 만을사용했을때 16예중 7 예(43.8%) 가완전관해에도달했고이후약제를바꾸거나다른약제와병합했을때총 14 예(78.4%) 가재관해에성공했다. 일년생존율은 78.4% 였고, 일년무사고생존율을 69.6% 였다. 결론 : ATRA를이용한일차치료에불응성이거나재발한 APL 환자에서 ATO 사용은완전관해율도높고부작용도심하지않아구제요법으로적절할것으로생각된다. 중심단어 : 급성전골수성백혈병; 비소 REFERENCES 1) Stone RM, Mayer RJ.
