1 The Korean Journal of Gastrointestinal Endoscopy Room A 소화성궤양환자에서헬리코박터제균치료의최신이슈 김나영 서울대학교의과대학분당서울대학교병원내과학교실 Issues in the Treatment of Helicobacter pylori for Peptic Ulcers Nayoung Kim, M.D. Department of Internal Medicine, Seoul National University College of Medicine, Seoul National University Bundang Hospital, Seongnam, Korea 서론 Helicobacter pylori ( 이하 H. pylori로약함.) 감염은만성전정부위염, 소화성궤양, 점막연관림프조직형위림프종 (MALT- OMA) 등의가장중요한원인으로알려져 H. pylori 제균이이들치료에필수요건이되었다. 1 최근상부위장관내시경시행및 H. pylori 제균치료가보편화되고, H. pylori 감염률이 4 세미만에서급격히감소하면서소화성궤양도줄것으로생각되었지만전반적인고령인구층의증가및소화성궤양을유발하는것으로알려져있는비스테로이드성진통소염제나아스피린과같은약제의사용이급격하게증가하여중증의합병증을동반하지않는소화성궤양의경우그발생률이감소하지않고있다. 2-5 특히우리나라소화성궤양유발위험인자에대한다변량분석을보면남자, H. pylori 감염, 4주내비스테로이드성진통소염제사용, 흡연이유의한위험인자로나타났고소화성궤양재발에있어서, 유일하게 H. pylori 제균실패나제균하지않은상태가다변량분석결과유의한위험인자로나타남으로써우리나라에서의소화성궤양치료에있어 H. pylori 제균은아직매우중요한요인임을알수있다. 6 H. pylori 제균치료법으로적합하기위해서는 per protocol analysis (PP 분석 ) 에서 9% 이상, intention-to-treat analysis (ITT 분석 ) 에서 8% 이상치료성공률이있어야하며부작용이 5% 이하, 가급적 1주일간의치료기간이바람직하다고권유되어왔다. 7-9 이에맞추어현재가장널리쓰이는 H. pylori 제균요법은강력한위산분비억제제인프로톤펌프억제제 (proton pump inhibitor, PPI) 에 2개의항생제 (amoxicillin, clarithromycin) 를추가하는 PPI 삼제병합요법이다. 이요법의제균율은초창기에는효과적인것으로인정받았지만항생제내성률의증가와함께그제균율이감소하여국내외보고에서 7 8% 정도로그리만족스러운편이아니어서 1,11 1차에서 PPI 3제요법대신 tetracycline을포함한 4제요법 12 이나 fluroquinolone 을포함한 3제요법등이대안으로제시되고있다. 13,14 이처럼제균율이낮아진것은항생제사용의증가에따른항생제내성의증가때문이라생각는데 차제균에실패한상당수의환자들에게 2 차치료가필요한경우가빈번해졌다. 최근 Maastricht 2-2 Consensus Report에서는 2차치료로서 PPI와 bismuth가포함된사제요법 7일이상이추천되고있으나 15 일주간의치료제균율이 7 8% 이며 16,17 1주간의국내보고또한 ITT 분석 61.3%, PP 분석 7.4% 에불과하여 18 2차요법을무엇으로할지또한 2차에서도실패하면 3차구제로는어떤약제로하는것이바람직할지매우해결하기어려운문제라하겠다. 또한효과적인 H. pylori 제균방법을강구하기위해서는결국항생제내성에대한파악이필요하다고생각된다. 본고에서는소화성궤양치료에있어제균치료의최근이슈로제균요법시점을궤양치료를위한 PPI 치료를궤양치료과정중어디에위치할것인지, 그리고 23년이후의 H. pylori의항생제내성변화및 H. pylori 내성의지역적분포, PPI 삼제요법대신일차제균요법으로서의사제요법, 연속요법, quinolone 포함삼제요법에대해간단히정리해보고자한다. 본론 1. 소화성궤양치료과정에서의 H. pylori 제균치료 소화성궤양을유발하는원인중가장큰원인은여전히 H. pylori 감염이지만 PPI 삼제요법은 1주만보험이되므로 1주외의소화성궤양치료를위해서는 PPI 내지 H2 blocker로좀더치료를해야한다. 통상위궤양은십이지장궤양보다궤양 healing 기간이길어서 PPI 삼제요법을포함하여 PPI를사용하는기간을 6 8주로하고있고십이지장궤양의경우 4 6주로권장하고있다. 물론이기간은궤양의크기나깊이에의해달라질수있다. 한데 PPI 삼제요법을소화성궤양치료과정처음에넣을것인지아니면후반에넣을것인지는통일된가이드라인 Vol. 42 (Suppl 2), 211 (37-42) 37

Values represent percent numbers (patients infected with resistant H. pylori/total subjects). 은없다. 다만궤양출혈과같은응급상황이아닐경우는제균치료를궤양치료처음에시작하는것이궤양치유율을높일수있는것으로알려져있다. PPI 주사약을주입하고 24시간위 ph검사를시행했을때 H. pylori 양성인경우 ph 6 이상이 6% 이상인경우가 H. pylori 감염의증거가없는경우보다의미있게높기때문에 19 궤양출혈과같은응급상황에서는 H. pylori 제균치료를출혈한직후시작하는것보다는 3 14일이상소화성궤양치료를충분히하여궤양출혈재발가능성이매우적다고판단될때제균치료를시행하는것을권유하고있다. 이처럼 H. pylori 양성인경우오히려 PPI에의해위산도가높게유지되는현상에대한관찰은역류성식도염치료환자에서도관찰되는데그이유는 H. pylori의 urease가암모니아를생성하여위점막 ph를올리기때문이다. 2. 소화성궤양의 H. pylori 양성률저하와 H. pylori 진단검사에서의주의점 H. pylori 감염률이낮아지고 H. pylori의제균율이높아지면서 H. pylori의양성률은과거보다많이떨어지고있어소화성궤양환자에서의 H. pylori의양성률또한낮아지고있다. 하지만 H. pylori의검사를적게할수록, 장상피화생이있는위궤양의경우체부가아닌위전정부에서 CLO test만할경우그가음성률이높아 2 29년대한소화기학회지에발표된헬리코박터파일로리감염의진단및치료가이드라인에서는다음과같이권유하고있다. 21 즉 조직학적검사로헬리코박터감염을진단하는경우, H&E 염색만으로는불충분하며 Giemsa 염색법이나 Warthin-Starry 은염색법등다른염색방법의병용이추천되고있다. 2 조직염색검사결과에영향을미치는요소로는생검위치및조직의개수, 검사전의항생제복용여부, 조직병리판독의사의숙련도등이있다. 다행스럽게도위축성위염이나장상피화생이있는경우라도 Giemsa 염색법이나 Warthin-Starry 은염색법으로가음성가능성이적어 2,22 위축성위 Table 2. The Effects of Resistance of Amoxicillin and Clarithromycin on the Eradication Rate of H. pylori after Proton Pump Inhibitor Triple Therapy 23 Amoxicillin Clarithromycin No Eradication rate (%) Sensitive Sensitive (95.3) Sensitive Resistance 17 (65.4) Resistance Sensitive 2 2 (1) Resistance Resistance 2 2 (1) Total (87.2) Breakpoints of resistance were defined as.5 μg/ml for amoxicillin and 1. μg/ml for clarithromycin. 염이나장상피화생이있는경우는조직요소분해효소검사보다는 Giemsa 염색내지 Warthin-Starry 은염색법에의한조직검사가유리하다. 보통의경우전정부에서 2표본이상, 체부에서 2표본이상조직을채취하는것이바람직하나조직검사를가능한적게해야하는상황에서는전정부보다는체부에서 2표본이상조직을채취하는것이권장된다. 22 특히헬리코박터의제균치료후성공여부를판정하는경우에는전정부에서 2표본이상, 체부에서 2표본이상조직을채취하는것이바람직하다. 21 로제안하고있는것이다. 3. H. pylori 의항생제내성동향 23년부터 29년도사이에경기도에위치한분당서울대학교병원에서 agar dilution 방법에의해검사결과를 년과 27 29년으로나누어비교해보았을때항생제내성의가장큰특징은 clarithormycin 내성률로그내성률이 16.7% 에서 38.5% 로의미있게증가하였고, tetracycline은 4.5% 에서 34.6% 로, ciprofloxacine 과 levofloxacine 의내성도크게증가하였다 (Table 1). 23 반면 metronidazole 의경우통계적차이는아직없었으나내성률이 34.8% 에서 29.7% 로감소하였고 (Table 1) metronidazole의이러한경향은한국다른보고에서도이미확인되고있다. 24- PPI가포함된삼제요법의제균율에가장큰영향을미치는항생제는 clarithromycin 인데 (Table 2) 그기전으로는 23S rrna 의 A2143G 변이 (mutation) 로보고된바있다 (Table 3) H. pylori 항생제내성의지역적분포 이처럼항생제내성의증가, 특히 clarithromycin 내성의증가가 H. pylori 제균율에영향을미친다고생각되고있어지역별항생제내성에대해서파악하는것은 H. pylori를효과적제균방법을강구하기위한필요한요소로생각된다 년서울과경기지역에위치하는두기관 H. pylori에대한일차항생제내성을비교한결과두지역간항생제내성에차이가있었으며특히 clarithromycin에대한내성이제균율결 38 The Korean Journal of Gastrointestinal Endoscopy

PPI 삼제요법외시도되고있는 1차제균요법 1) RBC 사제요법 : 사제요법과함께시도되고있는방법으로는 PPI 대신 ranitidine bismuth citrate (RBC)+amoxicillin (or tetracycline)+nitroimidazole (or tinidazole) 즉 RBC 삼제요법이있다. PPI+clarithromycin+amoxicillin 삼제요법으로의 1 차치료실패후 RBC 삼제요법을 14일사용하여 81, 82% 의치료율을보인바있는데 29,3 이들약의갯수가적고복용법이간 편하여외국에서는좋은반응을보이고있는듯하다. 12 이들 RBC 삼제요법이보이는좋은효과에대한분석으로는 H. pylori 제균에있어서 metronidazole이나 clarithromycin 내성을최소화하는기전을가지고있을것이라고추측되고있으나 31 좀더검증을거쳐야할것으로보인다. 이러한 RBC 사제요법제균율은 metronidazole 내성이높은지역에서는그효과가적을수있어사용기간을 7일에서 1일내지 14일로연장하는방법이효과적일수있다는주장도있다. 32 2) Sequential therapy: Clarithromycin 내성을극복하기위해연속요법이고안되었다. 1일연속요법 (sequential therapy) 은 5일간 PPI와 amoxicillin 으로 5일간치료한후, PPI, clarithromycin, tinidazole 로 5일간투여하는치료법으로서서구에서는이요법의제균율이기존삼제요법보다우월하다는보고가있었으며 33,34 특히 clarithromycin, metronidazole 내성환자에게서제균율을높인다는보고가있다. 35 하지만이탈리아이외지역의제균율을보면태국에서 96% 36 대만에서 92% 37 를제외하면프랑스에서 85%, 스페인에서 84% 등으로실망스러운편이다. 우리나라에서의연속요법에관한보고를보면연속요법에서의제균율이 3제요법보다높았다는후향적연구도있었으나 38 기존 3제요법과유의한차이를보이지않았다는무작위연구가있어 39 좀더관심을가지고추시를기다려봐야할것같다. 3) Quinolone based 삼제요법 : 다음으로 clarithromycin 대신 quinolone 제제를포함한삼제요법이있는데세계적으로 Vol. 42 (Suppl 2), 211 (37-42) 39

4 많이시도된 quinolone 제제로는 levofloxacin이있다. 즉 levofloxacin 5 mg 1일 1회내지 25 mg 1일 2회를 PPI와 amoxicillin과함께복용하는삼제병합요법이다. 서양에서는 7 일간 levofloxacin 삼제요법 PP 분석상이탈리아 66 91%, 스페인 71%, 독일 93% 로좋은경우가많지만동양에서는일본 7 72%, Hong Kong 6% 로많은차이를보이고있다. 국내보고를보면서울에서시행한사제요법성적 (ITT 48.9%, PP 62.9%) 과비교한 7일간의 levofloxacin 포함삼제요법효과를보면 ITT 51.6%, PP 53.3% 로매우낮아실망스러운데부작용의발생률은 1% 로사제요법의 31.4% 보다유의하게낮았다. 4 이처럼제균율이낮은이유는 levofloxacin 에대한높은내성률관계있는것으로보인다. 한편 moxifloxacin은비교적최근개발된광범위 fluoroquinolone 제제로다른 fluoroquinolone 제제보다중추신경계독성이나광독성이적고복용방법이경구투여로 1일 1회로환자의유순도가높고다른약제와상호작용이적은장점을가지고있다. 한편 amoxicillin은여러연구에서위점막분비가잘되어 H. pylori 제균에좋은조건을갖고있고, 비교적항생제내성률이낮은것으로알려져왔다. 또한 amoxicillin이 1차치료에사용되어실패한경우라도 2차치료에다시사용하는경우높은제균율을보인다. 23 4년분당지역에서는 moxifloxacin 내성률이 5.% (3/6) 로낮아, 1 차제균치료에실패한환자들대상으로 esomeprazole 2 mg 및 amoxicillin 1 g 1일 2회, moxifloxacin 4 mg 1일 1회로구성되는삼제병합요법을 7일간시행하여사제요법과비교한결과부작용이반영되는 ITT 분석상사제요법보다의미있게높았고환자의유순도가높았다. 41 한데제균율을좀더높이기위해 moxifloxacin 포함삼제요법을 1일로늘렸으나 (25 년 ) 그 PP 분석제균율이 82.6% 로 7일간 (24년) 의 83.8% 와비슷하고다시 14일로늘렸지만 (27 28년) 79.9% 로오히려감소하는경향이있었다. 42 이에세시기에서의 moxifloxacin에대한 H. pylori 세균의내성률을알아본결과 24년도의 5.6% 에서 25 년에 12.%, 27 28년에 28.2% 로급격히증가하여항생제사용기간을늘렸음에도불구하고제균율이증가하지않는이유를설명해주었다. 이는항생제내성이제균율결정에있어매우중요한문제임을말해주는바현실적으로어렵지만이론적으로는항생제내성에맞추어 H. pylori 제균요법을결정하는것이바람직하게보인다. 6. PPI 삼제요법실패후 2 차제균요법 최근 Maastricht 2-2 Consensus Report에서는 2차치료로서 PPI와 bismuth가포함된사제요법이추천되고있으나 15 이치료또한제균율이 7 8% 이며 16,17 1주간의국내보고또한 intention to treat 분석 61.3%, per protocol 분석 7.4% 에불과하여 18 그기간을 2주로늘리는것이바람직하다는보고가나온바있다. 32 즉사제요법 7일사용은 PP 제균율이 77% 였으 나 2주사용했을때 94% 로증가한것이다. 32 한편사제요법부작용이무서운경우 quinolone 포함삼제요법을 2주간사용하는방법이있다. 하지만 quinolone 내성이급속히증가함으로써그기간을늘려서사용하되자주그제균율을확인해볼필요가있겠다. Moxifloxacin 이외에 3차구제요법의약제로해외에서시도되고있는약제로 rifabutin이있는데 rifabutin은 rifamycin S의유도체이고낮은 ph에서도항세균작용이유지되는특징이있으며항비정형결핵약제로사용되고있다. Pantoprazole, amoxicillin과삼제병합요법의형태로 1일간투여한결과 ITT 분석에서는 66 86%, PP 분석에서는 68 86% 의제균율을보여기존의사제요법과유사한성적을보고한바있다. 43 하지만 rifabutin 가격이매우비싸고. 백혈구와혈소판감소증과같은골수에의부작용이보고된바있고결핵균에대한내성출현가능성때문에우리나라처럼결핵의발생률이높은나라에서는 rifabutin 사용에다소주의할필요가있다. 또한 rifabutin 15 mg, amoxicillin 1 g, lansoprazole 15 mg 모두 1일 2회 7일복용한 6명중 3명에서만제균되어 (unpublished data) 외국과는다르게그제균율은높지않을것으로생각된다. 결론 H. pylori 감염률이떨어지고있는추세에있는우리나라에서소화성궤양은아직극복이되지못한질환이며특히소화성궤양환자의 3% 정도에서발생하고있는출혈은의료비용, 재발률, 사망률, 발생빈도가여전하여소화성궤양은매우중요한질환이라하겠다. 특히우리나라에서소화성궤양재발의유일한위험인자로나타난 H. pylori 감염은소화성궤양환자에서양성일때그제균이필수적치료내용이라할수있다. 본고에서는소화성궤양환자에서헬리코박터제균치료의최신이슈를살펴보았는데제균실패의가장큰요인은역시항생제내성임을알수있었고내성이적은항생제사용을위해서는각지역에서의내성률에대한파악이선결요건이라생각된다. 제균력이떨어지고있는 PPI 삼제요법대신사용될수있다고소개된 RBC 사제요법, 연속요법, quinolone 삼제요법을살펴보았으나우리나라에서는 RBC 사용이아직잘안되고있고연속요법, quinolone 삼제요법은그성적이별로좋지않아추후더좋은요법을개발할필요가있는상황이다. 참고문헌 1.
