1 경희의학 : 제 29 권제 1 호 특 집 J Kyung Hee Univ Med Cent : Vol. 29, No. 1, 2014 일반내시경으로위점막패턴을근접관찰하면 Helicobacter pylori 감염유무를예측할수있다 Close Observation of Gastric Mucosal Pattern by Standard Endoscopy Can Predict Helicobacter Pylori Infection Status 장영운 1 조준형 2 경희대학교의과대학내과학교실 1, 순천향대학교서울병원내과학교실 2 서 급속요소분해효소검사, 조직검사및배양등내시경적생검에기초한검사들은 Helicobacter pylori 감염을진단하는데널리사용되고있다. 많은연구자들이 H. pylori에감염된위의내시경적소견의특징들을분류하는시도를해오고있다. 2002년일본내시경전문의들은일반및확대내시경검사에서 H. pylori 감염이없는정상위소견의특징으로위체부의수많은작은붉은점들로보이는집합정맥들을관찰하였다. 1 이러한소견은 집합정맥들의규칙적배열 (regular arrangement of collecting venules, RAC) 이라고명명하였다. 비록확대내시경이집합정맥망에관하여좀더정확한정보를제공할수있지만 2 모든내시경검사실에서가능한것이아니며, 경험있는감독관아래에서교육이필요하다. H. pylori에감염된위의내시경적소견은홍반, 미란, 전정부결절, 두꺼워진위주름및가시적인 (visible) 점막하혈관등이다. 그러나이러한소견들은낮은민감도와낮은특이도때문에진단방법으 론 책임저자 : 장영운, 경희대학교의과대학내과학교실 Tel: , Fax: cywgi@chollian.net 로신뢰할수없다. 3-6 최근타이완내시경전문의들은내시경끝과위점막사이를근접관찰하는연구를시행하였으며, 체부점막에서 모자이크패턴 을발견하였다. 이방법은 H. pylori 감염상태를결정하기에좀더민감도와특이도가높은방법이다. 7 그들은두가지범주로 ( 정상 RAC 패턴과비정상모자이크패턴 ) 위점막패턴을분류하였다. 그러나그분류는모든 H. pylori 감염들을예측하기에불충분했다. 이에저자등은정상 RAC 패턴과비정상모자이크패턴 (type A) 에추가로미만성균질발적 (type B) 과혼재성패턴 (type C) 의 4가지범주로분류했다. 저자등은비확대일반내시경을사용하여저자들만의새로운내시경적분류로 H. pylori 감염상태의진단적정확도를평가하기위해대규모연구를수행하였다. 본연구의목표는첫째, 4가지범주 ( 정상 RAC, type A, B, C) 를사용하여 H. pylori에감염된위의새로운분류를확립하는것, 둘째, 위점막패턴과위염의중증도와의연관성을조사하는것, 셋째, H. pylori 감염과연관된임상적특징및다른내시경적모양을밝히는것, 마지막으로내시경적패턴의평가에서관찰자간및관찰자내동의 (agreement) 를입증하는것이다

2 경희의학제 29 권제 1 호 2014 대상및방법 1. 환자및연구계획 2011년 8월부터 2012년 1월까지상부위장관내시경을시행받은환자들이전향적으로경희대학교병원에서연구에등록되었다. 내시경적검사전, 모든참가자들은말초혈액검사, 혈청생화학검사, 소변검사, 흉부 X선, 심전도검사등을포함한기본적인검사실검사를받았다. 배제기준은연령이 20세미만이거나 70세이상인대상자와빈혈 ( 혈청헤모글로빈농도 < 10 g/dl), 심한전신질환또는진행된만성간질환, 양성자펌프억제제, H 2 수용체길항제, 또는항생제등의복용력, H. pylori 제균병력, 위수술력, 최근상부위장관출혈병력, 내시경에서범위염 (pangastritis) 의관찰등이다. 범위염이란체부의대만곡에서위주름이보이지않는위축성병변이전체위에퍼져있는것을의미한다. 연구프로토콜은병원의생명윤리위원회 (KMC IRB ) 의승인을받았으며, Clinical-Trials.gov (NCT ) 에등록되었다. 2. 내시경검사한명의경험있는내시경전문의가확대내시경이아닌일반내시경 (GIF-H260; Olympus, Tokyo, Japan) 및 CV260 비디오프로세서를가진 Olympus-EVIS Lucera 시스템을사용하여모든내시경검사를수행하였다. 비디오내시경영상들은고해상도 19인치평면 LCD 모니터 (OEV191H; Olympus) 를통해관찰하였다. 검사에앞서참가자들은시메티콘 0.2 g이섞인 10 ml 용액을먹고, 브롬화시메트로피움 5 mg 을근육내또는정맥내주사하였다. 처음에전체위를관례적인방법으로관찰하였다. 그후위의중간체부혹은저체부의대만곡부분에서내시경끝과점막표면의거리가 10 mm 이하가되도록유지하면서위점막패턴을주의하여근접관찰하였다. 3. H. pylori 감염의평가위전정부 (1 조각 ) 및체부 (1 조각 ) 에서급속요소 분해효소검사 (CLO test; 녹십자, Seoul, Korea) 를위해 2개의위생검을하였다. 추가로위유문부 (1 조각 ) 및체부 (1 조각 ) 에서조직학적검사를위한 2개의생검검체도얻었다. 그검체들은 10% 포르말린에고정되었으며, 파라핀왁스에포매하였다. 5 µm 절편들은헤마톡실린-에오신 (hematoxylin-eosin) 및변형김사 (Giemsa) 방법으로염색하였으며한명의병리과전문의가 H. pylori 양성여부를평가하였다. 만약두가지검사 ( 급속요소분해효소및조직학적결과 ) 중하나에서라도결과가양성소견이면, H. pylori 감염을확진하였다. 4. 위위축및장상피화생의등급결정위축의조직학적등급은최신시드니체계 (updated Sydney System)8 에따라 4점스케일 ( 없음 =0, 약간 =1, 중등도 =2, 뚜렷한 =3) 로평가하였다. 위축의정도는전정부 ( 점수 0-3) 와체부 ( 점수 0-3) 의각점수를합하여, 약한위축 (range 1-2), 중등도위축 (range 3-4), 심한위축 (range 5-6) 으로분류하였다. 장상피화생의범위는약간의유백색을띄거나융모모양을가진회백색 patch로정의하였다. 9 이들은겸자생검으로조직학적으로확진하였다. 5. 위점막패턴들의분류 2011년 5월부터 7월까지 H. pylori 감염된위의내시경적소견을밝히기위해예비연구를수행하였다. 저자등은 H. pylori 관련만성위염 150 증례분석을통해정상표면점막과비정상패턴을정의하였다. 본연구에서는 H. pylori 감염을발견하기위해점막패턴의 4가지범주즉정상 RAC 패턴, 모자이크패턴 (type A), 미만성균질발적 (type B), 혼재성패턴 (type C) 를사용하였다 (Fig. 1). 일반내시경검사에서정상 RAC 패턴은많은작은붉은점들로보였다. 정상위소구 (areae gastricae) 는미묘한고랑 (groove) 으로나누어지는위점막의작은다각형의형태로정의하였다. 대조적으로비정상모자이크패턴인 type A는깊은고랑을가진융기되고부어오른위소구의형태이며뱀가죽 (snake skin) 과비슷하다. 미만성 - 2 -

Type A가관찰되면모자이크패턴의크기를표준컵겸자 (standard cup forcep) 로측정하였으며, 모자이크패턴을 3 mm 미만혹은이상으로세부분류하였다. 병변위의점성홍반들의존재도평가하였다. 6. 관찰자간및관찰자내동의내시경시행중근접정지관찰은위체부의대만곡에서시행하였다. 깨끗한점막패턴을보여주는고화질영상들중, 관찰자간동의연구를위해 80장을선택하였고, H. pylori 감염상태를모르는다른내시경전문의가영상을검토하였다. 1주일뒤에동일한내시경전문의가다른순서로제공된같은영상을재평가하였다

H. pylori 양성예측의정확도와관련있는임상병리학적인요인들은로지스틱회귀분석으로평가하였다. P값은 0.05 미만인경우를유의한것으로고려하였고, 모든통계분석은 PASW Statistics 18판을사용하였다 (IBM Co., Armonk, NY,, USA). 그들중 4명은내시경에서범위염의존재로제외되었다. 최종적으로 617명의대상자 ( 남자 296명, 여자 321명 ; 평균나이 50±10.0세 ) 가포함되었다. 연구참가자들중 254명이정상 RAC 패턴을보였으며 363 명은비정상패턴을보였다. 양군의기본적인임상특징은 Table 1에정리하였다. 정상군과비정상패턴군의평균나이는유의하게달랐다 (47.9±10.9 vs 51.4±9.1세, P<0.001). 남성참가자의비율이정상군보다비정상패턴군에서더높았다 (52.6% vs 41.3%, P=0.006). 정상군의 9.4% (n=24/254) 에서 H. pylori 감염양성이었으나, 비정상군의 92.3% (n=335/363) 가 H. pylori 감염양성이었다 (P<0.001). 비정상점막패턴은위위축의존재와유의한상관관계를보였다. 결 과 2. 점막패턴과 H. pylori 감염상태의연관성 위체부에서비정상점막패턴을보인 363 명의참 1. 연구대상자및기본임상특징 총 621 명의참가자가연구를위해등록하였으며, 가자들은 type A가 187명 (51.5%), type B가 154명 (42.4%), type C가 22명 (6.1%) 으로분류되었다. In gastric atrophy, there were significant differences between normal pattern and three abnormal patterns (P<0.001). In marked atrophy, type B pattern was frequently observed, compared with types A and C (P<0.001, P=0.001, respectively). In the presence of intestinal metaplasia, type B pattern was frequently seen, compared with type A (P=0.008). type B에서 98.1% (n=151/154), type C에서는 90.9% (n=20/22) 이었다. 반면에, 정상패턴대상자들중 90.6% (n=230/254) 는 H. pylori 음성으로진단되었다. 민감도, 특이도, H. pylori 양성예측에대한모든비정상패턴의양성예측치, 음성예측치를구하였다 (Table 2, Table 3). 전반적인진단정확도는 91.6% (n=565/617) 이었다. 3. 점막패턴과위위축및장상피화생의관련성위위축의정도는정상 RAC 패턴과비교하여비 - 5 -

6 경희의학제 29 권제 1 호 2014 정상점막패턴에서유의한차이가관찰되었다 (P<0.001). Type B 대상자들이 type A와 C 대상자들과비교하여위축의정도가각각유의하게더심하였다 (P<0.001, P=0.001). 그러나 type A와 C 대상자들사이에서위축의정도는유의한차이가없었다 (P=0.851). Type A와 B에서는정상 RAC 패턴보다장상피화생이유의하게많이관찰되었다 ( 각각 P= 0.011, P<0.001). 그리고 Type B는 A와비교하여장상피화생이더많이존재하였다 (P=0.008). 반면에, type C는 type A와 B 및정상 RAC 패턴과비교해서장상피화생존재의차이가없었다 ( 각각 P=0.066, P=0.562, P=0.490) (Fig. 2). 4. H. pylori 감염상태를예측하기위해사용되는임상적특징과내시경적소견 H. pylori 감염상태의예측의정확도에영향을미치는다양한임상적특징과내시경적소견을조사하였다 (Table 3). 진단정확도와성별, 나이, 혈색소수치, 위위축의정도사이에는유의한상관관계가관찰되지않았다. 장상피화생이있는경우에 H. pylori 진단정확도가낮았으나, 통계학적으로유의하지는않았다 (P=0.051). 또한저자등은모자이크패턴 (type A, n=187) 이관찰되는경우에는그크기및병변위의점성홍반의존재를평가하였다. 모자이크패턴의크기에따른정확도는유의한차이가없었다 (P=0.747). 반면에, 병변위의점성홍반은 H. pylori 감염상태의더신뢰할만한예측변수였다 (P=0.036). 5. 관찰자간및관찰자내동의평가내시경점막패턴들의관찰자간및관찰자내동의 Table 4. H. pylori 감염상태예측에 관한관찰자간및관찰자내동의에대한 κ-값도유의하였다 (Table 4). 예측할수있다는것을보여주었다. 이전의연구들은전정부의결절형성, 두꺼워진위주름, 점막하혈 관의가시화등이 H. pylori 와관련된내시경적소견 고찰 본전향적연구는비확대일반내시경을사용하여위체부의주의깊은근접관찰이 H. pylori 감염을 임을보고하였다. 특히, 전정부결절형성은 H. pylori 감염예측의가장중요한내시경적소견으로고려되었다. 그러나전정부결절형성은높은특이도 (96%) 를보였으나, 낮은민감도 (32%) 를보였다. 3 비정상 - 6 -

CI, confidence interval. 전정부표면, 유두양돌기, 백색의전정부미란등도 H. pylori 감염의좋은진단기준으로보고되었다. 4 그러나, 그들의민감도와특이도는단지 75% 와 63% 이었다. 홍반성미란과가시적인혈관의존재와같은내시경적소견은 H. pylori 감염진단에 56% 의민감도를보였다. 5 정상 RAC 패턴이밝혀진후, 확대내시경을사용한몇몇의연구들이 H. pylori 감염이있는비정상위의내시경적소견을입증하기위해노력했다. 그들은 H. pylori 감염된점막을 RAC 패턴소실과위막공 (gastric pits) 들을둘러싸고있는모세혈관의연결망 (network) 의소실로정의하였다. 10,11 그러나, 확대내시경은모든기관에서가능하지않으며, 훈련과교육에더많은시간이요구된다. 일반내시경의개선된해상도는확대관찰없이위점막의주의깊은관찰을가능케하고 H. pylori 감염된점막패턴을가까이관찰할수있도록발전하였다. 최근, 타이완의내시경전문의들은내시경끝과위점막사이의근접관찰을사용하는연구를수행하였고, 체부점막에서모자이크패턴 ( 점성홍반의동반혹은없음 ) 을발견하였다. 이방법은 H. pylori 감염여부를결정하는데민감도와특이도가매우높게보고되었다. 7 전정부점막의집합정맥들이체부점막과비교하여상피표면보다더깊은곳에위치에있기때문에, 위전정부점막관찰은 H. pylori 진단에적합하지않다. 타이완연구자들은두가지범주 ( 정상 RAC 패턴, 비정상점막패턴 ) 를사용하여 H. pylori 감염예측에민감도 100% 및특이도 86% 의좋은결과를 얻었다. 그러나대상자숫자가상대적으로작다는단점이있다 (n=112). 또한비정상패턴을모자이크패턴과발적을동반한모자이크패턴으로구분하였는데, 본연구는더많은대상자 (n=617) 에서정상 RAC 패턴과모자이크패턴 (type A), 미만성균질발적 (type B), 고랑을가진불규칙적인발적 (type C) 의 4가지범주로좀더세분화하여사용하였다. 본연구에서모든비정상점막패턴은민감도 93.3% 와특이도 89.1% 로 H. pylori 양성에일치하였다. 전반적인진단적정확도는 91.6% 이었다. 이러한진단적가치는 95.2% 의민감도와 82.2% 의특이도를가진확대내시경보다열등하지않았다. 11 저자등은위점막패턴들이위위축과장상피화생의조직학적인중증도와연관성이있음을관찰하였다. 위위축의정도는비정상적인점막패턴중 type B에서가장높았다. 그리고 type A와비교하여 type B에서더높은장상피화생을관찰할수있었다. 또한저자들은 H. pylori 감염상태예측의정확도에영향을미칠수있는다른내시경적소견들을조사하였다. 점성홍반이없는모자이크모양과비교하여점성홍반이있는모자이크모양이 H. pylori 감염을더예측할수있는소견이었다. 위축성위염은위체부의대만곡보다소만곡에서더흔히관찰되었다. 위축성위염의영역은 H. pylori 검사에서위음성결과를유발하기때문에 H. pylori 의집락화에적대적인것으로인식되었다. 12,13 그러므로본연구에서는위체부의대만곡에서무손상점막이유지될것으로가정하여, 중간체부혹은저 - 7 -

8 경희의학제 29 권제 1 호 2014 체부의대만곡이내시경검사를하기에최적의장소로생각하였다. 본연구는 H. pylori 감염상태의예측에두가지중요한시사점을제안한다. 첫째, 일반내시경을사용하여위점막패턴을관찰하였으며, H. pylori 양성예측의정확도가확대내시경을사용한다른연구의결과에열등하지않았다. 그러므로, 주의깊은근접관찰은 H. pylori 감염상태의예측에충분하다. 둘째, 정상 RAC 패턴과 3가지비정상점막패턴을포함하는새로운내시경적분류체계를세웠다. 반면에, 본연구는몇가지의제한점을갖고있다. 첫째, 일반내시경의진단적정확도와확대내시경의정확도를비교하지않았다. 둘째, 다중기관이아닌단일기관에서시행되었다는점이다. 앞으로다른경험의수준을가진내시경전문의들을포함한다중기관연구가본연구의신뢰도를확증하기위해필요하다. 셋째, H. pylori 제균치료후의점막패턴의변화를평가하지않았다. 결론적으로일반내시경의근접관찰은 H. pylori 감염상태를예측할수있는중요한진단기술이다. 본연구에서제안한기술과비정상점막패턴진단기준은임상진료에적용될수있을것으로생각한다. Magnifying endoscopy can reveal more precisely the abnormal mucosal patterns in an H. pylori-infected stomach; however, it requires more training, expertise, and time. We aimed to establish a new classification for predicting H. pylori-infected stomachs by non-magnifying standard endoscopy alone. Materials and Methods: A total of 617 participants who underwent gastroscopy were prospectively enrolled from August 2011 to January 2012.We performed a careful close-up examination of the corpus at the greater curvature maintaining a distance 10 mm between the endoscope tip and the mucosal surface. We classified gastric mucosal patterns into four categories: normal regular arrangement of collecting venules (numerous minute red dots), mosaic-like appearance (type A; swollen areae gastricae or snakeskin appearance), diffuse homogenous redness (type B), and mixed pattern (type C; irregular redness with groove) to predict H. pylori infection status. Results: The frequencies of H. pylori infection in patients with a normal regular arrangement of collecting venules pattern and types A, B, and C patterns were 9.4%, 87.7%, 98.1%, and 90.9%, respectively. The sensitivity, specificity, and positive and negative predictive values of all abnormal patterns for prediction of H. pylori infection were 93.3%, 89.1%, 92.3%, and 90.6%, respectively. The overall accuracy was 91.6%. Conclusion: Careful close-up observation of the gastric mucosal pattern with standard endoscopy can predict H. pylori infection status.
