Materials and Methods: We prospectively followed 108 patients (121 hips) who underwent cementless THA using an alumina-onalumina articulation. The mean age of the patients was 50.7 years (range, years) and mean the follow up period was 48 months (range, months). Preoperative diagnoses included osteonecrosis of the femoral head in 90 hips; secondary osteoarthritis due to hip dysplasia in 24 hips; secondary osteoarthritis after acetabular fracture in four hips; acute femoral neck fracture in three hips. Clinical evaluation was performed using Harris hip score (HHS) and radiographic evaluation was performed in terms of the fixation of components and the prevalence of osteolysis. In addition, we observed complications, including the occurrence of sound or fracture of the alumina head or liner. Results: Mean HHS improved from 62.4 points to 94 points at final follow-up. Thigh discomfort was found in six hips and mild inguinal discomfort was found in eight hips. Both had disappeared within one year after operation. A clicking sound was detected in two patients. One hip had become dislocated and an alumina liner fracture had occurred in one hip. Loosening of component or osteolysis was not observed in any hip. The radiographic wear was not measurable. Conclusion: Early results of cementless THA using an alumina-on-alumina articulation were favorable, with osseointegration of the components and absence of periprosthetic osteolysis. However, longer-term follow up for a hip clicking sound and fracture of an alumina fracture are necessary. 2 191 한파손등이초기실패의원인이었다. 4) 이후세라믹의재질향상, 디자인의발전, 강도가뛰어난세라믹의개발로관절면의마모가거의없어마모편으로인한합병증을줄일수있었다. 5) 하지만세라믹관절면을사용했을때발생하는관절내소리 (sound) 및세라믹골두나라이너의파손등은여전히중요한합병증으로제기되고있다. 이에저자들은제3세대알루미나 (Biolox Forte, CeramTec, Plochingen, Germany) 관절면을이용한무시멘트인공관절치환술을시행한환자를대상으로임상적및방사선학적조기결과를분석하고자한다. 대상및방법 1. 연구대상및방법 2007 년 6월부터 2009 년 7월까지 COREN Hip System (Corentec, Seoul, Korea) 알루미나세라믹 -세라믹관절면을이용해인공고관절전치환술을시행받은환자 130 명 146 예중, 인공고관절전치환술과관계없이사망한 6명 7예와추시가불가능하였던 16명 18예를제외하고, 최소 3년이상추시가가능했던 108 명 121 예를연구대상으로하였다. 수술은모든환자에서외회전근건과관절낭봉합술을이용한후외측접근법으로한명의술자에의하여시행되었다. 사용한대퇴부품은티타늄합금 (Ti 6 Al 4 V Titanium alloy) 으로만들어진무시멘트스템으로, 이중쐐기형 (double tapered wedge) 에단면은납작한사각형 (rectangular shape) 을가지는직선형스템으로디자인되었다. 스템의표면은알루미나입자를이용한 grit blasting 방법으로처리하였으며, 표면의거칠기는평균 5.5 μm 를유지하여표면골성장 (bone on growth) 을극대화할수있도록하였다. 비구컵은반구형태의티타늄합금으로만 들어져있으며, 비구컵의표면은다공성티타늄을플라즈마분사방식으로피복하여 μm 두께를가지도록하였으며, 관절면은알루미나골두와알루미나라이너를사용하였다 (Fig. 1). 남자가 70명, 여자가 38명이었으며수술시평균연령은 50.7 세 ( 범위, 세 ) 였고, 평균추시기간은 48개월 ( 범위, 개월 ) 이었다. 수술전진단으로는대퇴골두골괴사증 90예, 고관절이형성증에의한이차성고관절염 24예, 비구골절후이차성고관절염 4예, 대퇴경부골절 3예였으며양측으로시행한경우는 13명이었다. 31 예에서 28 mm, 90예에서 32 mm 크기의대퇴골두를사용하였고, 경부는 short neck 46예, medium neck 73예, long neck 은 2예를사용하였다. 수술후 2-3 일째에배액관을제거하였고, 일주일을전후해서보행기등을이용한기립운동및부분체중부하운동을시작하였다. 혈전예방을위하여보행을시작하기전까지하지공기압박장치를이용하여하지에간헐적인압박을시행하였다. 2. 임상적평가추시는수술직후, 3개월, 6개월, 12개월, 그후 1년간격으로평가를하였다. 평가는 Harris Hip Score (HHS) 6) 를이용하였고, 대퇴부와서혜부의통증, 관절내소리, 삽입물주위골절, 치환물의이완, 감염, 탈구, 이소성골화형성등합병증의유무를조사하였다. HHS 가 90점이상인경우를우수, 90점미만 80점이상인경우를양호, 80점미만 70점이상인경우를보통, 70점미만인경우를불량으로평가하였다. 7) 관절내소리는들리는양상에따라 squeaking, clicking, grinding, popping 으로나누었으며, 관절내소리가주 1회미만의경우 grade I, 주1-4 회의경우 grade II, 주 4회이상의경우 grade III, 매일발생하는경우 grade Ⅳ로분류하였다. 소리의강도에따라환자에게만들리는경우 grade A, 가끔타인에게도들리는경우 grade B, 항상타인에게들리는경우 grade C, 관절내소리로인하여사회적인문제가발생할경우 grade D로분류하였다. 8) Figure 1. Photograph showing the COREN Hip System with aluminaon-alumina bearing. 3. 방사선학적평가매번추시때촬영한골반전후면및측면사진을술후방사선사진과비교분석하였다. 비구측에서는고정도 (fixation), 비구컵의경사각및전염각, 컵의위치가 2 mm 이상이동이나 5 o 이상의각변형의해리, 9) DeLee 와 Charnley 10) 의영역별골용해와방사선투과성선을관찰하였다. 대퇴삽입물주위의 spot welds, 삽입물의위치변화나수직침강, 피질골비후, 삽입물주변의방사선투과성선, 경화선, 골용해등을 Gruen 등 11) 이기술한영역에따라관찰하였다. Spot welds 는내막골과삽입물사이의골내막신생골형성으로정의하였으며, 12) 대퇴삽입물의수직침강은 5 mm 이상일때의미있는것으로판단하였다. 13) Pedestal 은대퇴삽입물의말단부와그말단부주위의골수내강을채우는신생골이직접접촉되어있는경우를안정형으로, 직접접촉이되어있지않

3 192 이종철 백승길 김신윤 은경우를불안정으로분류하였다. 12) 대퇴스템의위치는삽입물이 5 o 이상내측으로기울어져있을때내반으로, 반대로기울어져있을때외반으로보았으며, 14) 이소성골화형성은 Brooker 등 15) 의기준에의해판정하였다. 결과 1. 임상적결과 Harris 고관절점수는술전평균 62.4 점 ( 범위, 점 ) 에서최종추시에는평균 94 점 ( 범위, 점 ) 으로향상되어, 우수가 110 예 (91%), 양호가 11예 (9%) 로전예에서양호이상의결과를보였다. 대퇴부통증을호소하는환자는 6예 (4.9%) 에서있었으며경도가 5예, 중등도가 1예있었다. 이들은보행시주로통증을호소하였으며휴식시에는호전되었다. 서혜부통증을호소하는환자는 8 예 (6.6%) 에서관찰되었으며, 앉아있는자세에서일어날때경도 의서혜부통증을호소하였다. 대퇴부통증과서혜부통증은술후 1년내에통증이사라졌다. 2. 방사선학적결과비구경사각은평균 41 o (36 o -53 o ), 비구전염각은평균 27 o (12 o - 38 o ) 였으며비구전염각이 30 o 이상인경우는 3예에서관찰되었다. 최종추시당시비구컵의회전이동이나수직이동은전예에서관찰할수없었고, 2 mm 이상의방사선투과성선이나경화선은관찰되지않았다. 대퇴스템의내반고정이 8예있었으나, 추시중스템의위치가변한예는없었으며, 치환물이완은관찰되지않았다. 대퇴스템주위는전예에서골내막골형성과함께골성안정소견을보였고, spot weld 는 102 예의경우에관찰되었으며, 주로 Gruen zone 2, 3, 5, 6영역에서나타났다. 이밖에대퇴삽입물주위경화선은 20예에서관찰되었으며, 주로 Gruen zone 1영역에생겼다. 불안정 pedestal 이형성된예는없었으며, 골용해및 Figure 2. Photograph shows a fractured alumina liner (black arrows) and black discoloration of the alumina head (white arrow) that was retrieved form a 51-year-old female patient at 51 months postoperatively. 수직침강은보이지않았다. 방사선학적분석이가능한관절면 의마모는관찰되지않았다. 3. 합병증 관절내소리는술후 2 년째 1 예와, 술후 4 년째다른 1 예에서발 생하였다. 술후 2 년째관절내소리가발생한환자의비구경사 각과비구전염각은각각 46 o 와 24 o 였으며, 술후 4 년째관절내소 리가발생한환자의비구경사각과비구전염각은각각 35 o 와 28 o 였다 (Fig. 2). 소리의정도는 2 예모두 clicking 으로, 본인만들을수 있는 grade A 에속하였고, 앉아있는자세에서서는동작을할때 발생하였으며, 소리의빈도는간헐적으로일어나는 grade I 에속 하였다. 평소쪼그려앉는자세를취하는경우가많았던여자환 자에서특이한외상병력이없이알루미나라이너의골절이 1 예 발생하였다. Medium neck 을사용하였으며, 재치환술은새로운 알루미나골두와라이너를사용하였다 (Fig. 3, 4). 관절의탈구가 술후 1 개월째 1 예에서발생하였으나, 비수술적방법으로정복하 였고재탈구소견은없었다. 이소성골화형성은 Brooker 분류에 따라 class I 이 2 예에서있었다. 삽입물주위골절이나감염은발생 하지않았다. 70 세이상의무증상환자군에서심부정맥혈전증검 사를위한컴퓨터단층촬영을실시하여심부정맥혈전증 5 예, 폐 색전증 2 예를진단하였고, 항응고제투여후치료가되었다. 고찰 폴리에틸렌을이용한인공고관절전치환술에서폴리에틸렌마 모입자에의한골용해와무균성이완의방지를위해고도교차결 합폴리에틸렌, 금속 - 금속관절면및세라믹 - 세라믹관절면이소 개되었다. 16,17) 고도교차결합폴리에틸렌은감마선이나전자기선 을폴리에틸렌제품에쬠에따라폴리에틸렌의관절면의단점인 마모에대한저항성을높인제품으로현재에도많이사용되고있 다. 금속- 금속관절면은 1960 년대초부터사용되어왔으며, 초기시행된성공적인예들에서금속- 폴리에틸렌관절면보다마모가적고 18) 금속입자에의한국소적및전신적부작용도없었다. 그러나아직금속입자가높은농도에서세포에독성이있는것으로알려져있고, 방사선검사상 pseudotumor 형성과고관절의활액막염, 금속농도의상승으로인한발암의우려로현재는사용이줄어들고있는추세이다. 19) 세라믹 -세라믹관절면은금속- 폴리에틸렌관절면에비해마찰계수가낮고친수성이높아관절면마모율이매우낮으며, 마모입자에의한골용해와삽입물의해리가적은장점으로인해점차세라믹관절면의사용이증가하고있다. 또한마모가있더라도생물학적반응성이낮기때문에마모입자에의한전신적인부작용의염려는거의없는것으로알려져있다. 20) 한편, 세라믹은폴리에틸렌에비해마모가현저히적지만, 깨지기쉬운특성때문에세라믹골두나비구라이너의골절로재치환술을요하는합병증이발생할수있다. 21) 최근제3 세대세라믹은제1, 2세대와는달리, 고온균등압박 (hot isostatic pressurization) 과정을통해고밀도의보다매끄러운세라믹표면을제작함으로써기계적강도가더욱향상되었고, 세라믹인공골두의생산신뢰성을위해각제품에는인식번호를새겨야하는데이때생기는흠집효과에의한파손을최소화하기위해레이저를이용한제품인식표시방법 (laser marking) 도도입되었다. 22) 제3세대세라믹및테이퍼디자인의개발등에힘입어 1990 년대부터는골두파손의빈도가크게줄었으며 3세대세라믹회사에서보고된자료에의하면 500,000 개이상삽입된제3세대세라믹골두의파손빈도는 0.004% 정도로알려져있다. 23) 그럼에도 Yoo 등 24) 은알루미나 -알루미나관절면을사용한고관절전치환술에서 5.8 년추시에 2.6% 의실패율을보고하였는데, 이는한국인이서양인에비해좌식생활을많이하면서양반다리, 쪼그려앉기등의고관절의운동범위가넓은자세적특성과도연관이있는것으로추정하고있다. 세라믹 -세라믹관절면을이용할경우정확한수술적수기로관절삽입물의정렬을바르게하여변연부접촉을없애는것이세라믹골절과충돌현상을줄이는데중요하다. 25,26) 세라믹인공관절전치환술후관절내에서소리가나는 squeaking 의경우약 % 에서발생하는것으로보고되고있다. 8,27) 본연구에서는 squeaking 없이 clicking 만 2예, 1.6% 에서발생하였는데비구경사각이나전염각이허용범위내에이루어진것이낮은발생률과관계가있다고생각된다. Walter 등 28) 은비구컵의전염각이허용범위를벗어나면세라믹관절면사이의정상적인수분윤활막이부분적으로파괴되고그접촉면사이에미세한마찰력의증가가일어나며이렇게증가된마찰력이정상적으로수용할수있는범위를넘는경우마찰에의한진동이발생하고이로인해관절내잡음이발생한다고하였고, Chevillotte 등 27) 은세라믹골두표면에금속입자가끼어들어가며관절면사이의마찰력에영향을주기때문이라고하였다. Park 등 29) 은 COREN Hip

5 194 이종철 백승길 김신윤 System 을이용한무시멘트고관절전치환술의 3년이상추시에서감염, 탈구, 세라믹파손등과같은합병증없이우수한임상적, 방사선학적결과를보고하였다. Baek 과 Kim 30) 은수술한환자중 13예에서 clicking, 1예에서 squeaking, 총 20% 에서관절내소리가발생하였으나이는동통이나관절내운동의제한과는무관하였으며환자군이나삽입물과는연관성이없으며높은발생률에대해서는어릴적부터쪼그려앉는아시아인의특성으로인한것으로보고하였다. 저자들의경우관절내소리가 2예에서발생하였는데타인에게들리지않는본인만느끼는소리였으며오랫동안앉았다가일어설때간헐적으로발생하였다. 본연구의소리가발생한 2예에서삽입물의정렬은정상범위에있었다. 평소쪼그려앉는등의동작등을많이취하는환자에서특별한외상병력없이알루미나라이너의파손이발생하였고이예에서도삽입물의정렬은정상범위이어서과도한고관절운동과관련이있을것으로생각한다. 결론 제3세대알루미나 -알루미나관절면을이용한무시멘트인공고관절전치환술은최소 3년이상의조기결과에서양호한임상적결과, 골용해발생이없는안정된고정을나타내었다. 7 196 pissn : , eissn : Original Article J Korean Orthop Assoc 2013; 48: 이종철 백승길 김신윤 이종철 백승길 김신윤 경북대학교의학전문대학원정형외과학교실 목적 : 알루미나 -알루미나세라믹관절면을이용한무시멘트인공고관절전치환술의최소 3년추시의조기결과를평가하고문제점을알아보고자하였다. 대상및방법 : 알루미나 -알루미나세라믹관절면을이용하여인공고관절전치환술을시행받은환자중최소 3년이상추시가가능했던 108 명 121 예를대상으로하였다. 수술시평균연령은 50.7 세 ( 범위, 세 ) 이었고, 평균추시기간은 48개월 ( 범위, 개월 ) 이었다. 수술전진단은대퇴골두골괴사증 90예, 고관절이형성증에의한이차성고관절염 24예, 비구골절후이차성고관절염 4 예, 대퇴경부골절 3예였다. 임상적결과는 Harris 고관절점수로, 방사선학적결과는삽입물의고정도, 골용해등을평가하였고합병증으로소리발생이나세라믹골절발생등을관찰하였다. 결과 : 최종추시시평균 Harris 고관절점수는 94점이었다. 대퇴부의통증으로호소하는예가 6예, 경도의서혜부통증을호소하는예가 8예있었으나술후 1년내에사라졌다. 관절내소리 2예, 세라믹라이너골절 1예, 고관절탈구가 1예에서관찰되었다. 전예에서골성고정을얻어비구컵과대퇴스템의이동, 위치변형, 침강, 이완은관찰되지않았고골용해소견도관찰되지않았다. 세라믹관절면의마모는관찰되지않았다. 결론 : 알루미나 -알루미나세라믹관절면을이용한무시멘트인공고관절전치환술의조기추시결과, 안정된골내성장고정과골용해가없이양호한결과를얻을수있었다. 하지만관절내소리의발생및세라믹의골절에대해서는좀더장기적인연구가필요할것으로생각한다. 색인단어 : 고관절, 인공고관절전치환술, 세라믹 - 세라믹관절면 접수일 2012 년 11 월 23 일수정일 2013 년 1 월 12 일게재확정일 2013 년 3 월 11 일교신저자김신윤대구시중구국채보상로 680, 경북대학교의학전문대학원정형외과학교실 TEL , FAX , syukim@knu.ac.kr 대한정형외과학회지 : 제 48 권제 3 호 2013 Copyright 2013 by The Korean Orthopaedic Association This is an Open Access article distributed under the terms of the Creative Commons Attribution Non-Commercial License ( which permits unrestricted non-commercial use, distribution, and reproduction in any medium, provided the original work is properly cited.
