Materials and Methods: Retrospective evaluation was performed for 20 patients with femoral head fracture who received surgical treatments between December 1997 and May According to ipkin s classification, there were five type, six type, one type, and eight type fractures. Results: The average Merle d Aubigne ostel score was 12.8 (12.80±3.53). According to surgical method, the score for the bony fragment excision group was 9.8 (9.83±2.79), and that for the open reduction and internal fixation group was 13.9 (13.92±3.07). Depending on ThompsonEpstein criteria, two patients were good, two were fair, and two were poor in the bony fragment excision group. Four patients were excellent, six were good, and three were poor in the open reduction and internal fixation group. Conclusion: Bony fragment excision should be performed with caution in patients with femoral head fracture. Considering fragment size, location, and presence of acetabular fracture, better outcome can be expected using the open reduction and internal fixation method in comparison with excision. 2 Result of Surgical Treatment for the Femoral Head Fracture 199 탈구와함께발생하나간혹전방탈구와함께동반되기도한다. 4) 대퇴골두골절의치료방법으로보존적치료, 골편제거술, 관혈적정복술및내고정술또는관절치환술등이보편적인방법이다. 57) 대퇴골두골절이발생하였을경우, 대퇴골두무혈성괴사, 외상후관절염, 이소성골화증, 신경손상등의후유증으로인하여추후다양한장애가발생할수있다. 8,9) 그동안고관절주변골절중비구골절, 대퇴경부및전자간골절에대한많은논문이출판되었으나대퇴골두골절에대한논문은흔하지않았다. 간혹있다하더라도소수의증례들로구성된논문이었다. 1014) 이연구는인하대병원에서수술적치료를받은총 20예의대퇴골두골절환자들의임상적, 방사선학적지표에대하여후향적인검토를통해임상결과를분석하였다. 대상및방법 1997 년 12 월부터 2010 년 5 월까지인하대병원에내원하여대퇴골두골절로수술적치료를받은환자 20 명에대하여후향적연구를진행하였다. 환자들의수상당시평균 나이는 38.8 세 (38.8±13.1 세 ) 였고, 13 명은남성, 7 명은여성이었다. 20 명의환자중자동차사고로인한경우가 명, 낙상이 1 명, 추락손상이 3 명, 오토바이사고가 1 명이었다 (Table 1). 모든환자에게수술전전후상, 축상엑스선방사선사진 (ray) 촬영과전산화단층촬영 (computed tomography) 이시행되었다. ipkin 분류에따른골절분류결과 type 5 명, type 6 명, type 1 명, type 8 명이었다. 14 명의환자에서후방고관절탈구가동반되었고, 1 명은전방고관절탈구가동반되었으며, 5 명의환자는고관절탈구가없었다. 20 명의환자모두응급실을통해내원하였으며응급실에서환자평가중고관절탈구가발견된경우가능한빨리도수정복을시행하여해부학적정복을시도하였다. Type 환자 5 명중 2 명은골편제거술, 3 명은관혈적정복술을시행하였다. 5 명중 3 명은고관절후방탈구, 1 명은전방탈구가동반되었다. 5 명중 2 명은자동차탑승객교통사고, 2 명은보행자교통사고, 1 명은오토바이사고환자였다. Type 환자 6 명중 2 명은골편제거술, 4 명은관혈적정복술을시행하였다. 6 명중 5 명은고관절후방탈구가동반되었다. 6 명모두자동차탑승객교통사고환자 Table 1. 3 200 Joon Soon Kang, et al. 였다. Type 환자 1 명은고관절탈구가동반되지않은낙상환자였으며대퇴경부골절의전위가상당하여고관절전치환술을시행하였다. Type 환자 8 명중 6 명은관혈적정복술을, 2 명이골편제거술을시행하였다. 8 명중 6 명은고관절후방탈구가동반되었다. 8 명중 4 명은자동차탑승객교통사고, 1 명은보행자교통사고, 3 명은추락손상환자였다. 수술적도달법은후방관절탈구의정복이되지않았거나비구후벽골절의수술이필요했던 7 예에는후방도달법을사용하였고그외관절탈구가성공적으로정복된경우에는전방도달법을시행하였다. 임상적지표는 Merle d Aubigne ostel 평가척도와 ThompsonEpstein 평가척도를사용하였다. Merle d Aubigne ostel 평가척도는 18 점만점의숫자척도로서통증, 움직임, 걷기능력등의기능적결과를반영하며 ThompsonEpstein 평가척도는통증, 기능에다가방사선학적지표까지포함하여우수, 양호, 보통및불량으로구분하는평가척도이다. 방사선학적지표로이소성골화증에대하여서는 Brooker 분류법을사용하였으며, 이는방사선전후사진을이용하여경도, 중등도, 고도로구분하는분류법이다. 17) 비구골절및골반환골절이동반된환자는술후 선촬영을통해추시관찰하며 810 주간부동을유지하였다. 기타동반골절이없는환자군에서골편제거술을시행한환자는통증호전후 1 주이내에부분적체중부하를허용하는범위내에서조기거동을허락하였고관혈적정복술을시행한환자는 24 주이후부터부분적체중부하를허용하였다. 부동기간중 continous passive motion 운동을통하여관절유착방지운동을동반하였다. 임상결과분석은 MannWhitney U test 통계방법으로검증하였다. 통계적유의수준은 p<0.05 로하였고, 통계적분석은 SSS 13.0 (SSS nc., Chicago, L, USA) 버전을이용하였다. 결 평균추시관찰기간은 56 개월 (55.9±37.8 개월 ) 로최소 24 개월부터 163 개월이었다. 평균 Merle d Aubigne ostel 점수는 12.8 점 (12.80±3.53 점 ) 이었다. 치료방법에따라분석한결과골편제거술시행군은 9.8 점 (9.83±2.79 점 ), 관혈적정복술시행군은 13.9 점 (13.92±3.07 점 ) 이었다. ThompsonEpstein 척도로평가결과우수 4 명, 양호 9 명, 보통 2 명, 불량이 5 명이었다. 골편제거술을시행한 6 명은양호 2 명, 보통 2 명, 불량이 2 명이었으며관혈적정 과 복술을시행한 13 명은우수 4 명, 양호 6 명, 불량 3 명이었다. 골편제거술을시행한환자군보다관혈적정복술을시행한환자군에서 Merle d Aubigne ostel 점수및 ThompsonEpstein 평가척도에따른임상적및방사선학적결과가우수하였다 (p=0.011). 이소성골화증은 20 명의환자중 8 명 (40%) 의환자에서발견되었으며 Brooker 분류법을이용한결과 4 단계 1 명, 3 단계 1 명, 2 단계 3 명, 1 단계 3 명이었다. Type 환자 5 명중 1 명 (20%), type 환자 6 명중 3 명 (50%), type 환자 8 명중 4 명 (50%) 의환자에서이소성골화증이관찰되었고 ipkin 분류에따라이소성골화증의발생에차이가있다는것을확인할수있었다. 20 명중 12 명 (60%) 의환자들에게서외상후관절염소견이관찰되었다. 수상당시고관절전치환술을시행한 type 1 명의환자를제외하면 19 명중 12 명, 즉 63% 에서외상후관절염소견이관찰된것이며경도 6 명, 중도 4 명, 중등도 2 명의외상후관절염소견이관찰되었다. 선상대퇴골두의경화소견외대퇴골과비골사이의간격이좁아진경우중도의외상후관절염으로판단하였으며고관절의간격이현저히좁아지며골극및골낭종을동반하며대퇴골두의변형이동반되었을경우중등도의외상후관절염으로판단하였다. 2 명의중등도외상후관절염소견환자들은재수술을시행하였다. 중도의외상후관절염소견환자들분석결과 type 2 명, type 2 명이었다. 2 명의 type 환자모두골편제거술을시행한환자였으며 2 명의 type 환자는 1 명은골편제거술, 1 명은관혈적정복술을시행한환자였다. 경도의외상후관절염소견환자들을분석한결과 type 3 명, type 1 명, type 2 명이었다. Type 환자중 2 명은관혈적정복술, 1 명은골편제거술을시행한환자였고, type 환자는관혈적정복술을시행했던환자였으며, type 환자 2 명중 1 명은골편제거술, 1 명은관혈적정복술을시행했던환자였다. 7 명의환자에서는외상후관절염을호소하지않았고, 이는대부분추적관찰기간이상대적으로짧았던환자들이었다. 예후가불량한 2 명은인공고관절수술을시행하였다. 한명은 type 환자로골편제거술시행술후 93 개월째이소성골화증및대퇴골두괴사로인해고관절치환술을시행하였다 (Fig. 1). 다른한명은 type 환자로 acutrak 나사못을이용하여고정 36 개월후대퇴골두괴사로인한고관절통증과운동제한으로인하여고관절치환술을시행하였다. 자동차탑승객사고로수상후 acutrak 나사못을이용하

(D) Thirtyone months after open reduction and internal fixation, showing no radiologic complications. 여관혈적정복술을시행한지 34 개월째추적관찰중인 type 환자는현재특이합병증을보이지않는상태로좋은예후를보이고있다 (Fig. 2). 위결과들을종합한결과, 관혈적정복술을시행한환자군에서더좋은임상적결과및방사선학적결과가관찰되었다.

5 202 Joon Soon Kang, et al. 고 대퇴골두골절의치료목표는해부학적으로정복된대퇴골두및관절내골편이없는안정적인고관절을만들어주는것이다. 위의목표는수술적인치료또는비수술적인치료를통해도달할수있다. 하지만아직고관절탈구가동반되었을때 6시간이내에빠른정복이필요하다는것외에대퇴골두골절의정확한치료방법에대하여는논란이있다. 4,18,19) 비수술적치료방법을택할수있는환자군은 2 mm 이내의해부학적정복이이루어졌으며골편이관절내에끼어있지않고고관절이안정적인 type 또는 type 환자군이다. 1997년 12월부터 2010년 5월까지인하대병원에내원한대퇴골두골절환자중 5명이보존적인치료를진행하였다. 그중 2예는상기기준에부합하여보존적인치료를진행하였으나그외 3예는뇌출혈및복강내출혈등타과적인문제로인해본래수술적인치료가필요하나수술적인치료를시행하지못했던환자들로본연구에서제외하였다. 이에본연구에서는수술적치료를결정한 20예의증례를바탕으로연구를진행하였다. 20명의환자중 1예는일차수술로고관절전치환술을시행하였다. 이환자는 type 56세여자환자로대퇴골두골절과대퇴경부골절이동반된환자였다. 환자의나이가젊다거나활동이많은직업을가진사람이라면치환술대신관혈적정복술도고려할수있으나, 본증례에서는 56 세의활동이많지않은주부로관혈적정복술보다는고관절전치환술을시행하였고만족스러운임상적및방사선학적결과를얻었다. 과거에는골편이대퇴골두의 1/3 이하일경우에는골편제거술시행을권유하였다. 2,20) 반면최근에는골편제거술과관혈적정복술사이에비슷한결과가나타난다고보고하고있다. 11,21) 수술방법을결정하는요인으로는골편의크기, 분쇄의정도, 골편과체중부하면사이의관계등을들수있다. 골편의크기가단단하게고정할수있을정도의크기가된다면관혈적정복술이원칙이며, 골편의크기가작거나분쇄가심할경우, 골편이체중부하면과연관이없을경우에는골편제거술을시행할수있다. 본연구에서는위기준에따라골편의크기가작거나크더라도분쇄되어있어고정술이어려웠던환자및골편이크기가상당하더라도상대적으로골두의하부에위치하였던 6명은골편제거술을시행하였고그외 13명은관혈적정복술및내고정술을시행하였으며골편제거술을시행한환자군보다관혈적정복술을시행한환자군에서 Merle d Aubigne ostel 점수및 ThompsonEpstein 평가척도에따른임상적및방사선학적결과가더좋은결과가관찰되었다. 찰 ipkin 분류에따라 Merle d Aubigne ostel 점수를구분하였을때, type 은 13.4점 (13.40±3.72점), type 는 12 점 (12.00±3.42점), type 는한증례로 16점, type 는 12.6점 (12.63±3.43점) 이었다. 앞서치료방법에따라점수차이가있었던것과는달리골절분류에따라서는 Merle d Aubigne ostel 점수의차이를확인할수없었다 (p=0.374). 이는 ipkin 단계가높아짐에따라골편제거술보다관혈적정복술을더많이시행한결과라고생각된다. 관혈적정복술을시행한환자 13명중 3명은상대적으로다른관혈적정복술시행환자군과비교하였을때임상적및방사선학적결과가나쁘게관찰되었다. 상기 3명모두 ipkin 분류상 type 환자로고관절의후방탈구및비구골절이동반되었던환자들이다. 이와같이고관절탈구및비구골절이동반되었던환자중고관절이불안정하여한번에탈구가정복이되지않고 2차례이상반복적으로응급실에서폐쇄적정복술을시행한환자및비구골절의크기가크며여러조각의파편으로나뉘어졌던환자들은관혈적정복술이후에도상대적으로예후가좋지않았다. 대퇴골두골절후발생가능한합병증으로대퇴골두무혈성괴사, 외상후관절염, 이소성골화증, 신경손상등이있다. 대퇴골두무혈성괴사는 6%23% 에서발생한다고보고되고있으며대부분젊고활동이많은사람들에서발생하므로발생시치료가어렵다. 2,3,11,21,22) 외상후관절염은 8%75% 까지보고되고있으며이소성골화증은 6%64% 까지보고되고있다. 2,3,11,2226) 본연구에서는 40% 의환자에서이소성골화증이발생하였으며외상후관절염은 60% 의환자에서발생하였다. 이소성골화증은상대적으로나이가젊고 ipkin 분류상높은단계의환자들에서많이발생하였으며외상후관절염은상대적으로 ipkin 분류보다는추적관찰기간이길었던환자들에서많이발생하였다. 이소성골화증의경우골편제거술을시행한군에서는 6 명중 4명 (66%) 에서발생하였고, 관혈적정복술을시행한군에서는 13명중 4명 (%) 에서만발생하였다. 외상후관절염의경우에도골편제거술을시행한군에서 6명중 6명모두, 관혈적정복술을시행한군에서는 13명중 6명 (46%) 에서만발생하였다. 위와같은결과를종합하여볼때, 앞서언급한임상적결과뿐만아니라방사선학적결과에서도골편제거술시행군보다관혈적정복술시행군에서더좋은예후를기대할수있다. 결 총 20 명의환자를대상으로시행한본연구는대퇴골두골절환자에서골편제거술시행한환자군보다관혈적정 론

6 Result of Surgical Treatment for the Femoral Head Fracture 203 복술을시행한환자군에서더좋은방사선학적및임상적결과가관찰되었다. 대퇴골두골절환자들에게보존적인치료를결정할때에는전산화단층촬영을비롯한충분한검사를통해 2 mm 이내의해부학적정복이이루어진골편이관절내에끼어있지않으면서관절의조화정도와안정성이확실히보존되어있는군에서만시행하여야한다. 상기조건을만족하지않아수술을선택한환자군에서는골편제거술은제한적으로시행되어야한다. 술전방사선촬영및전산화단층촬영등을통해정확한골편의크기, 위치, 비구관절면골절등을고려하여야하며, 골편의크기가작아고정술이어려울경우에는골편제거술을시행하여야하나그외크기가상당하여골편의고정이가능하다고판단될경우에는체중부하에영향을주지않는골두하부의골편이라할지라도가능하면골편제거술보다관혈적정복술을시행하는것이더좋은예후를기대할수있을것으로생각된다. 8 SSN (rint) SSN (nline) 대한골절학회지제 27 권, 제 3 호, 2014 년 7 월 J Korean Fract Soc 2014;27(3): riginal Article 대퇴골두골절의수술적치료결과 강준순 문경호 이동주 양종혁 인하대학교의과대학정형외과학교실 목적 : 대퇴골두골절로수술적치료를시행한환자들의임상적, 방사선학적지표에대하여장기추적결과를확인하였다. 대상및방법 : 1997년 12월부터 2010년 5월까지인하대병원에내원하여대퇴골두골절로수술적치료를받은환자 20명에대하여후향적연구를진행하였다. ipkin 분류에따른골절분류결과 type 5명, type 6명, type 1명, type 8명이었다. 결과 : 최종추시에서평균 Merle d Aubigne ostel 점수는 12.8점 (12.80±3.53점) 이었다. 치료방법에따라분석한결과골편제거술시행군은 9.8점 (9.83±2.79점), 관혈적정복술시행군은 13.9점 (13.92±3.07점) 이었다. 골편제거술을시행한 6명의환자는 ThompsonEpstein 평가척도에따라 2명은양호, 2명은보통, 2명은불량으로평가되었다. 관혈적정복술을시행한 13명의환자는 4명은우수, 6명은양호, 3명은불량으로평가되었다. 결론 : 대퇴골두골절에서골편제거술은제한적으로시행되어야하며, 골편의크기, 위치, 비구관절면의골절등을고려하여골편제거술보다관혈적정복술을시행하는것이더좋은예후를기대할수있다. 색인단어 : 고관절, 대퇴골두골절, 골편제거술, 관혈적정복술 접수일 수정일 게재확정 교신저자강준순인천시중구인항로 27, 인하대병원정형외과 Tel , Fax , kangjoon@inha.ac.kr 본논문은인하대학교연구비지원으로수행하였음.
