Methods : Acu-points in the right forepaws, HT 7 and PC 7 were stimulated with 3~4mA, 2ms, and 10Hz after 5% formalin (50ul) s.c. injection to the left hind paw. In addition, it was investigated whether the dorsolateral funiculus (DLF), known to be related the descending inhibition, mediates analgesic effects of the contralateral heterotropic EA or whether administration of naltrexone, an opioid antagonist, blocks the effect of EA. Results : The results showed that contralateral heterotropic electroacupuncture (EA) inhibited late phase ( %) of pain induced by formalin in the behavioral test, but sham-ea had little effect on pain behavior ( %) and no analgesic effects after transection of the dorsolateral funiculus ( %). The pretreatment of naltrexone (10mg/kg, i.p.) could not inhibit the analgesic effects of EA on formalin-induced pain behavior ( %). Also,EA suppressed formalin injection induced expression of cfos like protein (cfl) in the dorsal horn, but not sham-ea. Suppressed expressions of cfl in the spinal cord were eliminated after transection of the ipsilateral dorsolateral funiculus at T10-11 levels. However, pretreatment of naltrexone could not prevent the suppressive expressions of cfl at the spinal cord. Conclusions : These results suggest that the analgesic effect of contralateral heterotropic electroacupuncture may be modulated through the dorsolateral funiculus constituting the descending inhibition. 2 (406) 대한한의학회지제 24 권제 2 호 (2003 년 6 월 ) 經 > 1), 巨, 이라하여거자법을제시하였으며이는임상에서각종통증이나뇌혈관질환으로인한신경학적장애등에많이활용되고있다 2). 거자법은통증유발부위와해부학적으로상관없는 부위에대한침구자극이통증이나마비등을억제할수있는근거로, 거자법과관련하여최근 spinothalamic track와 dorsal horn neurons of spinal cord들이그들의 peripheral receptive field과상관없는다른영역 (heterotropic area) 의다양한자극에의하여흥분성이억제될수있다는결과들이보고되는등많은연구 3) 들이진행되고있다. formalin test 는유해성통증에대한진통약물의효과여부를측정하는데광범위하게사용되고있는방법이다 4). 아울러 formalin반응은 peripheral sensory c- fibers 와 dorsal horn level에서 spontaneous activity를증가시키며 glutamate, substance P 및 NO와같은 neurotransmitters 분비를증가시키기도하는것으로알려져있다 5-7). c-fos는신경세포에서발현되는 immediate early gene 중의하나로다양한유해성자극에대한 정도를확인하는데활용되고있다 c-fos 단백질은자극에대한반응으로 30분이내에 c-fos mrna 발현과 1~3시간사이에단백질의관찰로신경세포의활동성및흥분성을확인할수있는표식자로이용되고있다 8). 究는흰쥐의 formalin pain을유발한후, 통증이유발된말초수용야와거리가먼영역에대한 戟이진통효과를유발하는지를관찰하고, 아울러이러한진통작용의경로가무엇인지를밝히고자하였다. 이를위해요척수후각에서의 cfos 발현을중심으로진통의경로로일반적으로알려진척수의배측외속 (dorsolateral funiculus) 과 central nervous system의 opioid receptor의작용을억제하였을때거자법을근거로한전침자극의효과가어떻게변화하는지를실험적으로규명하여다음과같은결과를얻게되었다. 방법 1. 실험동물약 g의 78 male albino sprague-dawley rats 를 cage마다 4-5마리로그룹을지정하였으며, 12시간간격으로빛과어둠을교환해주면서음식과물을자유롭게주었고실험 1주일전부터환경에적응할수있게하였다. 아울러 research protocols 은 the institutional animal care and use committees에의해서승인되었고통증에관한국제윤리위원회의윤리지침을준수하여동물실험이수행되었다. 2. 마취외과적인절차에있어동물들은 100% 산소상태 (at 2L/min) 에서 3% halothane (2-Bromo-2-chloro-1,1,1- trifluoroethane, Sigma-Aldrich Inc, U.S.A.) 으로깊이마취되었고실험이진행되는동안에는 0.5% halothane 상태를유지하였다. 3. Formalin induced pain Formalin induced pain model 은 27-G 바늘로 5% Formalin 용액 50μl를 sural nerve territory( the plantar aspect of left hind paws) 에피하주사하였다. 4. Electro-acupuncture Two stainless steel needle (0.25mm diameter) 을우측신문혈 (HT 7, On the transverse crease of the wrist, radial to the tendon of m. flexor carpi ulnaris) 과대릉혈 (PC 7, On the transverse crease of the wrist, between the tendon of m. palmaris longus and m. flexor carpi radialis) 에 3-5 mm 깊이로 transcutaneously 하게삽입되었다. 이어서 Electro-acupuncture (3-4 ma, 2 ms duration and 10 Hz, lasting 30 min) 를 30분동안계속해서일정한자극으로전침을시행하였다 (stimulator, PulseMaster A300, WPI, U.S.A). 그리고신문혈및대릉혈의전침효과와 sham EA를비교하기위해 ipsilateral forelimb의 elbow area( 소해혈, HT 3) 에 30 분정도의전침자극을시행하였다. 194

Behavioral Test Behavioral Test 은 Formalin 주사후에시작되었고 60분동안계속되어졌다. 동물들은깨끗한 plexigal cylinder(25x25x25 cm) 안에서관찰되었고동시에 video tape으로기록되었다. 관찰하는동안 shaking, lifting, rearing, biting, grooming 과같은비정상적인행동들이 5분간격으로기록되었다. composite pain score(cps) 는먼저category weights (category1, 2 and 3, respectively) 에서부여된 behavioral category 에소비된시간의양 (sec) 을적용하여총합을구함으로써산출하였다 (WST). 이중통증반응양상이심한두번째와세번째범주의두단계에대하여각각 category2와 3의가산점을부과하여각각소비한시간과횟수를합산함으로써유도하였다. 이러한계산방법을 Weighted scores technique (WST) 라고부르며, 본실험에서는 CPS를 WST로대체하였다. WST = {(2 X time in category 2) + (3 X time in category 3)}/ 300 (category1= the injected paw has little or no weight on it. category2 = the injected paw is elevated and is not in contact with any surface. category3 = the injected paw is licked, bitten or shaken).cps = {(2 X time in category 1) + (3 X time in category 2)} / Lesion of the dorsolateral funiculus T10-11 levels 왼쪽에서의 Spinal lesion 는 halothane-anesthetised 된실험동물을이용하였으며, laminectomy 는척수를드러내기위해서 T10-11 levels 왼쪽에서행했다. dorsolateral funiculus(dlfx) 는정교한가위로해당부분을절개하였으며척수주요혈관에미치는손상을피하기위한치료를병행했다. dura mater 은 Gelfoam으로상처는봉합용실로봉합되었다. 모든동물들은테스트에앞서 5일동안회복시켰다. 실험의끝부분에서척수의 20μl정도를잘라서제거했고 hematoxylin and eosin (H&E) 로염색시켜조직학적으로입증했다. 7. Naltrexone 전처치 mu opiate 수용체를차단시키기위해 naltrexone hydrochloride(sigma-aldrich Inc., U.S.A.) 와같은 opioid antagonist(10mg/kg, i.p.) 를 saline에희석하여사용하였으며 test 15분전에주입되었다. 8. immunohistochemistry cfos-like protein(cfl) 발현을보기위한 immunohistochemistry는기존방법을약간변형시켜시행하였다. 모든동물들은 chloral hydrate(400mg/kg, i.p.) 로깊이마취시켰고 0.9% 식염수를고루뿌렸으며 0.05M Na2HPO4와 0.137M NaCl을포함한 phosphate buffered saline solution(pbs) 에녹인 4% paraformaldehyd 로고정되었다. 그리고척수는 spinal L1-5 level 에서떼어서 PBS에서후고정하고 rinse 하였으며 4 에서 1~2일동안30% sucrose solution에담궈두었다. 조직은저온유지장치에서 40μm 의두께로박편했고 30분동안6% H2O2에서 incubating 했으며 5% dry milk를포함한 0.1M PBS로 10분동안 2번헹군다음0.5% bovine serum albumin (PBS BSA) 를포함한 0.1M PBS에녹인0.8% triton X-100 과함께 incubating 했다. 간단한워싱후조직을 primary cfos polyclonal antibody(ab-2; Oncogene; diluted 1:250) 와함께4 에서 overnight incubating 했다. 다음날조직의절단부분을실온에서한시간동안이차항체 (goat anti-rabbit; DAKO) 와 ABC Elite Kit(Vector Lab.) 를처리하여 incubating 하고나서 avidin-biotin complex를처리하고한시간동안실온에방치했다. cfl 단백질을가진 neurons은 0.05% diaminobenzidine HCl과 0.003% H2O2에서 incubating 한조직에서가시화되었다. DAB 반응후조직은 0.1M PB로헹군후겔로코팅된슬라이드위에얹어놓고공기로말리고탈수, permount로덮는다. 정량을위해서오직 cfl neurons 만이 spinal dorsal horn에서 digital image analysis system을사용하여카운트되었다. 195

4 (408) 대한한의학회지제 24 권제 2 호 (2003 년 6 월 ) 9. 통계적인분석모든자료는 mean±s.e.m으로표현되었다. 유의성있는통계차이는 statyiew 4.0을사용하여평가되었다. 평균값사이의비교는 unpaired student s t-test 와 one-way ANOVA에의해수행되었으며 p < 0.05 를유의수준으로삼았다. 결과 1. formalin 통증모델실험동물에서 Electroacupuncture inhibits the pain behavior of formalin induced pain 먼저정량화된통증모델의안정성을확인하기위하여, formalin(5%, 50μl ) 을정상흰쥐 (n=10) 의하지족저부위피하에주입하고모든실험동물에서 flinching, licking와 bitting 등과같은 행동을관찰하여, 통증수치 (CPS) 로환산하였다. Formalin 주입직후부터 5분까지 수치 (composite pain score) 가 1.56±0.20인제1차 반응 (first phase) 이출현하였고, 이후 15분까지는통증반응이급격히감소하는양상을보였다. 제 2차 반응 (late phase) 은 fomalin 주입 20분이후부터나타나기시작하여 30분에 1.25±0.15, 35분에 1.54±0.22, 40분에 1.44±0.1, 그리고 45분에 1.20±0.12로증가하였고, 이후점차통증반응이감소했다. 이같은실험결과를통하여전침자극의진통효과를규명하고자하는통증반응검사와분자생물학적실험을수행하는근거로삼을수있었다 (Fig. 1). 진통효과의비교에서, 전침자극군 (EA) 의 63.3±11.7 % (p<0.01) 로대조군 (Con) 에비하여유의한진통효과가관찰되었다. 그러나임의혈전침자극군 (Sham- EA) 에서는 85.6±16.8 % 로전침군 (EA) 와비교하여전침자극의효과가뚜렷하지않아, 전침자극의효과를명확히확인할수있었다. 이러한전침자극의진통경로를추적하기위하여, 배외측속 (dorsolateral funiculus) 손상군 (DLFx, n=6) 과 naltrexone 전처치군 (Nalt, n=6) 에서 formalin 주입후전침자극을가하여행동검사를각각실시하였다. 동측의배외측속손상군 (DLFx) 에서전침자극은통증반응이 95.7±18.7 % 로관찰되어거의진통효과가없는것으로관찰되었으며, Naltrexone 전처리군에서는 70.7±13.1% (p<0.05) 로대조군에비하여통증반응이억제된것을확인하였다 (Fig. 2). 3. Effects of electroacupuncture on the expression of cfos like protein in the spinal dorsal horn Formalin 주입후시간의경과에따른요척수후각회백질에서 c-fos 단백질의발현양상과전침자극에따른효과를관찰하기위하여 formalin 주입후 1시간과 2시간에각각제 3-5번요척수에서관찰하였다. Fig. 3에서보는것처럼, 정상상태에서는척수후각에 2. 거자법전침자극에의한 formalin 유발통증억제효과거자법에근거한전침자극이 formalin 유발통증반응을억제할수있는지를확인하기위하여, 전침자극군 (EA, n=6) 과 전침자극군 (Sham-EA, n=6) 에서통증반응을관찰하였다. 특히, 대조군 (Con) 에서가장통증반응이심하게출현하는제 2차통증반응 (late phase) 기간중 30~45분사이를관찰비교하고자하였다. 전침자극직후통증반응을중심으로한 Fig. 1. Con, formalin injection to the left hindpaw; EA, electroacupuncture on the contralateral acupoints of the forepaw to formalin injection; Sham-EA, electroacupuncture on the ipsilateral non-acupoints of the forelimb to formalin injection; SDH, superficial layer of dorsal horn; NP, nucleus proprius; DDH, deep layer of dorsal horn. *p< 0.05, **p<0.01 and ***p<0.001 compared with the same time of control, respectively. 서 cfos 단백질의발현이거의나타나지않는다. formalin 주입후 1h 에서는 superficial layer에서 ±4.19개, nucleus proprius에서 22.64±2.52개, 그리고 deep layer에서 45.27±5.41개가발현되었다. 그러나전침자극군에서는각각, 27.6±2.89개 (p<0.05), 8.6 ±1.24개 (p<0.001), 23±2.93개 (p<0.01) 로발현이유의하게감소되었다. formalin 주입후 2시간에서는척수후각표층부에서 34.85±3.75개, nucleus proprius 에서는 15.69±2.68개, 척수후각심층부에서 ±6.43개로 1시간에비하여심층부에서큰증가를나타내고있다. 반면같은시간대의전침자극군에서는각각27.63±2.83개, 7.63±1.35개 (p<0.05), ±3.47개 (p<0.001) 로심층부에서가장큰감소를보여주고있다. 또한동측 (ipsilateral) 상지의임의경혈부위에가한전침자극 (Sham EA) 에의하여진통효과가있는지를관찰한결과에서는 cfos 단백질의발현이표층에서 41±5.84개, nucleus proprius에서 12.63± 2.34개, 심층에서 44.13±5.10개 (p<0.05) 로나타나, 심층부를제외하고는 cfos 단백질발현이억제되지않았다. 이로써동측상지의임의혈전침자극의진 통효과는행동검사와함께고려할때미약하였다 (Fig. 3). 4. The pathway of contralateral heterotropic EA analgesic effect on formalin induced pain 거자법에근거한전침자극으로족저에유발된 formalin 통증의억제효과와진통경로에관련한행동반응검사에서배외측속 (dorsolateral funiculus) 의역할이관여함을관찰할수있었다. 이러한결과를확인하기위하여통증유발전에 T10-11 수준에서동측의배외측속을손상시킨흰쥐에서 cfos 단백질발현의변화를관찰하였으며 T10-11 수준에서통증유발부위와동측인배외측속의손상을확인할수있었다. 더욱이, 배외측속손상군에서 formalin 유발통증에대한전침자극의효과를관찰하였을때, 표층에서는 40.57±5.49개, nucleus proprius에서 14.14±3.29개, 심층에서 50.43±5.88개의 cfos 단백질이발현되어, 정상동물에서전침자극에의한 cfos 단백질의발현억제효과가배외측속손상군에서는관찰되지않았다. 197

EA, electroacupuncture 2hrs after formalin injection; DLFx-EA, electroacupuncture 2hrs after formalin injection following transection of the ipsilateral dorsolateral funiculus; Nalt-EA, electroacupuncture 2hrs after formalin injection following pretreatment of naltrexone (10 mg/kg, i.p.); SDH, superficial layer of dorsal horn; NP, nucleus proprius; DDH, deep layer of dorsal horn. *p< 0.05 and **p<0.01 compared with EA, respectively. 또한 Formalin 유발 의 검사에서 naltrexone 전처리군에서전침자극을통한진통효과가계속유지되는것을관찰된점을바탕으로, 요척수후각의 cfos 면역단백발현양상을관찰하였을때, 표층에서는 21.5±2.35개, nucleus proprius에서는 9.9±0.94개로감소된경향을나타내었으며, 심층에서는 28.7±3.55개로전침자극군과같은수준의발현억제를나타내었다 (Fig. 4). 고찰 전통적인침구치료는氣 및經 을바탕으로 經, 經, 根結, 經 과巨 등을이용하고있으며, 이러한 은經 系를기초로 을치료하는방법으로제시되고있다. 巨 은왼쪽에병이있을때오른쪽혈위를취하고, 오른쪽에병이있을때왼쪽혈위를취하여치료하는자침법이다. 그이론적근거는 < >1), 客 經 巨 라하여, 질병부위와 로대칭되는부위에대한치료작용을이론적으로제시하고있다. 巨 의원리는근대에와서는 << 灸經 >> 9) 에서 이나 奇 을통해효과를검증하였으며, 오늘날에도여전히실용적가치가인정받아뇌혈관장애로인한반신불수, 안면신경마비, 요부염좌, 좌골신경통등에巨 을이용하여모두좋은임상효과를보고있다 10). 최근들어巨 과유관하여침에의한진통경로가몇가지로제시되고있다. 첫째로는뇌간의 Periaqueductal grey (PAG) 가 STT의일부가연결되어 Nucleus raphe magnus (NRM) 와 Nucleus reticularis paraginantocellularis (Rpc) 등을거쳐, dorsolateral funiculus (DLF) 를통해척수후각으로연결되어진통역할을담당한다는 supraspinal descending pathways이며 11), 두번째로는 Le Bars 등 12) 이마취된흰쥐에서척수후각과삼차신경핵의폭주성신경세포 (convergent neurons) 의신경활동성이흥분된말초수용영역과는거리가먼신체다른영역에가한유해성전기자극에의하여억제된다는 diffuse noxious inhibitory controls (DNIC) 이다. 아울러셋째는뒤늦게밝혀진것으로상위경추 (upper cervical) 의 C1-C2의 nucleus proprius에존재하는고유감각성신경세포들 (propriospinal neurons) 이요척수까지하행성투사되어요척수 (L5-S1) 신경세포의구심성흥분을억제할수있다는 propriospinal neurones connecting different spinal cord segments 13) 이다. 이외에도, 관문조절설 (Gate Control theory) 14) 의 segmental neuronal circuits와 Basbaum과 Field 15) 가신경조직에 opiate에대한수용체들이진통효과를일으킨다는내재진통계 (Endogenous analgesic system) 가존재한다. 그러나내재진통계의경우는이후계속된실험결과들에서, 상기에서언급한 DNIC, suprapinal descending pathway와 propriospinal projection과같은경로와다양하고복잡하게연결되고있다. 최근연구에서 lidocaine 또는 DLF의손상으로뇌간에서척수로하행하는경로를제거할경우, 하지의 198

7 류영수 : 巨 에의한 戟이 의 formalin 에 한 機 (411) 염증으로유발되는통증반응이증가하는것을보여주고있다 16). 아울러흉추수준에서 DLF를손상시켰을때, formalin 유발통증으로인한요추후각신경세포에서의 cfos 발현이증가되는것을보고하며, DLF 에존재하는하행성신경섬유가유해자극에의하여발현되는 cfos를억제할수있다는것을주장하였다 17). 이러한결과는동측또는반대측에행한자극이 DLF를통해하행성억제경로를활성화할수있다는것을보여주는것이다. Le Bars 등 18) 은마취된흰쥐에서척수후각과삼차신경핵의폭주성신경세포 (convergent neurons) 의신경활동성이흥분된말초수용영역과는거리가먼신체다른영역에가한유해성전기자극에의하여억제된다는것을발견하였으며, 이를 diffuse noxious inhibitory controls (DNIC) 라고칭하였다. DNIC 기전에대하여, Le Bars 등은척수후각의신경세포를흥분시키는말초수용영역으로부터멀리떨어진영역에가한유해성자극이복외측속 (ventrolateral quadrant) 를통해뇌간을포함한상위중추로신호를보내어 DNIC 시스템을흥분시키게되고, 통증의하행성억제노선인동측의배외측속 (dorsolateral funiculus) 을통해척수또는삼차신경의폭주성신경세포의활동성을억제한다고제안하였다 19). 또한 acupuncture와 DNIC를비교연구한실험에서, 족삼리에가한전기자극이삼차신경폭주신경원 (trigeminal convegent neurons) 활성을억제하는것을관찰하여, 침자극이 DNIC와관련된신경기전을촉발한다고주장하였다 20). 그리하여, DNIC가침구치료의巨 원리와유사해보이는측면을지니고있지만, 문제는진통에대한위치 (localization) 의특이성 (specificity) 결여와 DNIC 를유발하는자극이유해성이어야한다는데있다. Propriospnal projection에의한진통효과에관련하여, 건측하지또는상지에가한유해성자극, cardiopulmonary sympathetic, splanchnic 과 hypogastric afferents의전기자극이 lumbosacral 척수시상로의신경활동성을억제하는것이보고되었다 21). 이러한효과에대하여, 경추의 C1-C2의 nucleus proprius에존재하는고유감각성신경세포들 (propriospinal neurons) 이요척수까지하행성투사되는것을확인하였고 22), 아울러요척수 (L5-S1) 신경세포의구심성흥분을억제를중개할수있다는것을밝혀내었으며, 이를토대로 C1-C2의고유감각성척수신경세포들이하지의다양한말초자극에대한반응성을조절하는데중요할수있다고가설을내놓았다 23). 더욱이 DNIC가관련된 brain stem 영역의구조들이관련되지않고, 단순히 propriospinal projection 하행성경로를통하며, 진통관련물질이 opioid 이라는점이현재까지알려져있다. 본연구의통증모델인 formalin induced pain은흰쥐의하지족저부위에희석된 formalin을피하에주입하여약물주입직후부터약 5분간발생하는제 1 차 과이후 반응이소실된후 20분 ~60분이상까지지속되는제 2차 반응을유발하는것이다. formalin induced pain의특징을살펴보면, 탈수초신경인 c-fiber의흥분성을증가시키며 24), wide dynamic range (WDR) 신경세포를포함한감각신경세포들의자발활동성을증가시키는것으로보고되었다 25). 특히 2차 반응 (late phase) 은 formalin이피하에위치한 감각수용체들의직접적인자극에의하여 Aδ, C 감각신경섬유들이흥분이척수표층부에존재하는 WDR 신경세포들의자발활동성을직접적으로증가시킴으로써나타난다고제시하며 26), 척수에서 glutamate 27), substance P 28), NO 29) 등신경전달물질이증가되는것으로알려져있다. c-fos 단백질은신경세포에서발현되는 immediate early gene 중의하나로다양한자극에의하여 30분이내에 cfos mrna 발현과 1~3시간사이에단백질의관찰로신경세포의활동성및흥분성을확인할수있는표식자로이용되고있다 30). 특히 Hunt 등 31) 은처음으로마취된흰쥐의말초부위에유해성자극을가하여척수에존재하는신경세포에서 cfos 단백질을관찰을시작하였고, 그뒤다양한유해성자극에대한 정도를확인하는데활용되었으며, 흰쥐에서하지의피하에 formalin 주입시척수및후근신경절에서시간경과에따른 cfos 단백질의발현양상및척수부위별분포양상이관찰되었다 32). 특히 cfos 단 199

8 (412) 대한한의학회지제 24 권제 2 호 (2003 년 6 월 ) 백질과 formalin 유발통증의상관관계를확인한실험에서는, fos mrna antisense oligonucleiotide를척수강주입한후흰쥐척수후근신경세포들에서 formalin에의한preprodynorphin mrna의전사를억제하고제 2차 반응을억제한다고보고 33) 를통하여, 통증정도와 cfos 발현량의비례하고있음을알수있다. 본연구에서는 formalin 유발통증에서거자법에근거한전침자극의진통효과여부를확인하였다. 그결과 formalin 주입직후가하여진전침자극이 pain behavioral response를약 37 % 까지억제하였는데, 반면 sham-ea에서는진통효과가약 14,4% 밖에없는것으로나타나, 거자법에의한전침자극의특이성 (specificity) 이명확해졌다. 또한이러한통증반응억제한진통효과는척수수준에서관찰한 cfos 단백질발현량도유의한수준으로억제하는것이관찰되었다. 현재까지 formalin induced pain에거자법에근거한전침자극의진통효과를관찰한예는아직까지보고된바가없다. 다만, 족삼리에가한전침자극 34) 이나봉독요법 (bee venom) 35) 이 formalin induced pain을억제할수있다는연구가일부보고되어있는데, 이는통증인근부위의경혈인족삼리가내재진통계와밀접한연관성을가지고있음을보여주고있다. 결국, 이러한연구보고들의바탕에는이전의침자극에의한진통효과연구의방향이주로 Electro-acupuncture 가내재진통계 (endogenous opioid system) 를자극한다는주장이가장일반적으로받아들여지고있기때문이다. EA에의한진통효과가 naloxone에의하여차단된다는 Mayer의주장과는일치하지않는의견으로 Watkins 등과 Polmeranz 등 36) 은 EA를시술하기전에 opiate antagonist인 naltrexone을투여하면진통효과가억제되는반면 EA를시술한후에 naltrexone을투여하면진통효과가차단되지않는다는연구결과를발표하였다. 이러한결과에대해 Pomeranz 등은 endogenous opiate의일차적인진통효과와 non-opioid 에의하여매개되는이차적인진통효과가있음을언 급하였으며, Chapman 등 37) 은합곡자침에의한치통의침진통의효과가 naloxone에의한억제되지않는것을관찰하였다. 본연구결과에서도 naltrexone의전처리동물에서행동검사에서는대조군에비하여약 29.3% 의진통효과가관찰되었으며, 요척수의 cfos 단백질의발현에서도전침자극군과마찬가지로발현이억제됨으로써 formalin 주입후전침자극을가하는상태에서발생되는진통작용에 opioid 수용체가직접적으로관여하지않는것으로사료된다. 기존의연구결과와비교하여볼때, opioid 수용체가진통작용을매개하지못하는결과는본실험에서사용되는통증모델과전침자극조건의차이에서기인한것인지를고려해야할것이다. 즉, 침진통이내재진통계와연계된경우의대개는정상동물에서의침자극과그에따른유해성자극에대한반응성을관찰한경우가가장많으며, 본연구에서처럼, 이미통증이유발된경우의진통문제는침자극이진통경로를활성화시키기이전에생체자체가생리적으로통증에대한억제기전을활성화시킨상태라고보아야한다. 이경우침자극이진통경로에작용하는문제가정상상태와는매우다를것이며, 결국본연구결과에서 naltrexone 전처리된동물에서 formalin 유발통증에대한전침자극의진통효과가유의하게억제되지않는이유는병리적상태에서진통경로에 opioid 이외에많은인자들이복합되어있을것이라는추측으로해석하여야할것이다. 다음으로본실험결과와관련하여침진통이앞에서언급한하행성진통경로와또는기타다른진통경로와의관계에대한기존연구를살펴볼필요가있다. Takeshige 등 38) 은저주파전침자극이 serotonin mediated pathway와 noradrenergic mediated pathway 로나누어지는 descending inhibitory system에작용하는진통경로를제안하여침에의한진통작용은내재진통계보다는다른진통경로와의밀접하다는것을예상해볼수있다. 본연구결과에서는배외측속 (DLF) 손상동물에서전침자극의진통경로를관찰하고자하였는데, 200

9 류영수 : 巨 에의한 戟이 의 formalin 에 한 機 (413) formalin test전에 T10-11 수준에서동측의배외측속을손상시킨흰쥐에서 formalin 유발통증에대한전침자극의진통효과가행동검사에서관찰되지않았다. 이러한경향은 cfos like protein 발현량을관찰한실험에서도같은반응을보였다. Takeshige 등 39) 과 Fields 등 40) 이 nucleus raph magnus 신경세포들의일부 axon이배외측속 (DLF) 을통해동측및반대측척수후각에종지한다는것을보고함을근거로본실험에서건측 (contalateral) 에가한전침자극의효과를이해할수있다. 이상의결과를통해, 거자법에근거한전침자극의진통효과의우선순위는배외측속 (dorsolateral funiculus) 라는것이확인되었다. 그러나향후 DLF를통한진통경로가능성과함께상위중추에관련한보다다양한연구를보강하여 propriospinal, DNIC 또는전통적인 supraspina descending inhibition 인지를확인하여야할것이다. 더욱이, 본연구결과배외측속 (DLF) 을통한해부학적경로가가장크게진통경로로작용하였고, 반면 opioid성내재진통계는비중이약한것으로관찰되었다다는점은기존연구에서 DLF와 opioid간의밀접성과는상충되는부분이있다. 물론, 침진통에관련하여내재진통계의역할을배제할수없으나, 최근에다양한침진통경로가계속하여언급되고있는점과침진통의경로가단순히하나에의존하기보다는다양한진통경로를복합적으로활용하고있는점을고려할수있을것이다. 이에본연구결과는침진통의다양성을추구할수있는점에서그의의를찾을수있을것이라생각되어진다. 물론이러한진통경로에직접적이고, 강력한매개물질을확인하기위한연구가보강되어야할것이다. 결론 究는흰쥐에게 formalin을주입하여 을유발시킨뒤, 거자법을근거로한 戟이진통효과가있는지를관찰하고아울러이러한진통작용의경로가무엇인지를밝히고자하였다. 1. 전침자극은 formalin induced pain behavioral test 에서관찰된 반응 (late phase) 을유의하게하였다. 또한거자법에근거한전침자극의진통경로를확인하기위한통증행동검사에서, 전침자극군 (Sham-EA, 85.6±16.8 %) 은전침자극군 (EA, 63.3±11.7 %) 의비하여진통효과가감소하였다. 동측배외측속손상군 (DLFx) 에서전침자극은통증반응이 95.7±18.7 % 로관찰되어진통효과가관찰되지않은반면, naltrexone 전처리군에서는 70.7±13.1% 로진통효과가관찰되었다. 2. 요척수의 cfos like protein 발현량을관찰한결과에서 formalin 주입후 1시간과 2시간에관찰된 cfos 면역단백질발현이전침자극에의하여척수후각의 superficial laminae, nucleus proprius와 deep laminae에서모두유의하게억제되는것이관찰하였다. 3. 전침자극의하행성통증억제경로와의관계에서 DLF 손상군의경우, formalin 주입후 1시간과 2 시간에관찰된 cfos like protein 발현이전침자극군에서처럼억제되지않았으며, 이는앞서관찰된통증행동반응검사에서와같은결과를나타내었다. 이상의실험결과를종합하여보면, 거자법에근거한전침자극은 formalin 유발통증을억제하는진통효과를나타내었고, 이러한진통경로에는 opioid 수용체의역할보다는배외측속 (dorsolateral funiculus) 의역할이더욱중요하게작용하는것을확인할수있었다. 그러나향후진통경로에대한구체적인실험연구및임상에서거자법에입각한각종통증이나신경학적장애에대한연구가계속진행되어야할것으로사료된다. 참고문헌 1. 홍원식 : 內經 ( 校勘 ), 서울전통문화연구회, 1994;356: 문상관외 4인 : 중풍환자에있어서거자법에대한유용성검토, 전국한의학학술대회논문집, 1999;
