F or this reason, centered on its significance in determining treatment plan and predicting prognosis, we retrospectively reviewed 41 arthrog rams in 36 patients; 16 arthrograms of congen ital di slocation of the hip in 13 patients and 25 arthrograms in Legg-Calve-Perthes disease in 23 patients, whic h we re performed at Department of Diagnostic Radiology, Coll ege of Medi cin e, Chungnam Nati onal University, from January 1985 to May 1989 fhe results were as follows 1. Congenital dislocation of the hip 1) Under the fluoroscopic control we evaluated the possibility of closed reduction after injection of contrast media into the joint capsule and if closed reduction was not successful, obstacles to closed reduction were observed. As a result, failure of closed reduction was appreciated in 12 cases of 16 patients and inhibiting factors of closed reduction were as follows; hypertrophied, inturned acetabular limbus: 12 cases, t~ansverse acetabular ligament thickening: 10 cases, ligament teres thickening: 8 cases, iliopsoas muscle thickening: 8 cases, capsular adhesion: 7 cases, other space occupying lesions such as fatty and/or fi brous tisssue: 9 cases 2) Hip arthrogram was an excellent method to evaluate the possiility of success of closed 이 논문은 년 1 0 월 31 일접수하여 년 11 월 11 일에채택되었음

Index Words: Hip. dislocation Hip necros 1S Hip. arthrography 서 고판절조영술은영아와소아에서단순촬영으로는 보이지않는연골성대되골두와고관절주위의연부조 직을관찰하기위하여고판절탈구냐 Legg-Cal ve-perthes ( 이하 LCP로약함 ) 뱅에대해서종종시 행되고있으냐칭습적이고벤거로운검사망엽이여그 의외에대해많은논란이되고있는바, 이에저자들은 고판절조영술이고판절달구냐 LCP 뱅에있어서어떤 의의가있으며과연치료에도웅이펼수있는가에대 해 13 영의고판절달구환자에서 16 예와 23 영의 L CP 병환자에서 25 예의고판절조영술소견을분석고찰하 였으며유의한결과를얻었기에문현고찰과함께보 고하는바이마. 흐료 대상및방법 년 3 월부터 1989 년 5 월까지, 만 3 년 2 개월간충 남대학교영원에서 선천성고판결탈구및 L CP 명의 진단하에고판절조영솔을시행했던고판결탈구환자 1 3 명에서 16 예와 LCP 영환자 23 명에서 25 예를대상 으로하였다. 고판젤조영술은 12 명의환자에서는수술방에서천 신마취후시행하였고 24 영의환자에서는방사선과촬 영실에서국소마취후시행하였마. 고판절에대한접 근방법은환자를투시대위에앙요 L 위로둡힌후투시 하에서대퇴동액을피하여천자부위를정한후천자 하여, 22 G 척추천자침을대되경부의내측이나대퇴 경부의중앙을향하여진챙시켜서판절낭에도달하는 전방도달법을이용하였다. 조영제는 60 % Diatrizoate Meglumine (Hypaque<lll Meglumine) 을사용하였 m - 으며생리식영수에 50 % 로희석하여 4 내지 7 ml 를 주입하였다. 조영제주입후환자를둡핸상태에서 anterior-posterior view 와 frog- leg view 를촬영하였으여 필요한경우에대퇴골두를내회전파외회천그러고내 전이냐외전시킨후촬영하였다. 고판절달구에대한고판절조영숭은, 조영제주입 후도수정복을시행하여동싱원석정복이가능한가를 보았으며도수정복이되지않는고판절에대해서는도 수정복을땅해하는판절내요인들의존재유우와그종 류를판찰하였다. LCP 뱅에있어서는 23 영의 25 예에서발영연령이 7 세이상이거나나이가어리더라도 Catterall 분류상 1 ) 3 군이나 4 군에속해있는환아에서고판결조영소견상 대퇴골두의변형이심하지않아서보전석치료를했던 9 예중 6 개월이후에추석검사가가능했던 4 예를대상 으로치료선과치료후의고판절조영소견을비교하였 으며또한치료방첨의결정과예후를예측하는지침 A 로서이용되고있는 Cattera ll 의분류와발영연령에대 하여고판절조영술로판찰이가능한대퇴골두의변형 과고판절의 lncongrulty 그러고매되골두의아탈구등 이 유의한상판판계가있는지판찰하였다 잔단시영의진챙정도를 Salter 의방법에 2 ) 의하여 Avascular stage, Fragmentation s tage, Regenerative stage, Residual s t age 와같이 4 시기로분류하여고판 절조영술을시행하는시기가명의진행시기중어느해 에해당되는지관찰하였마. 또단순촬영소견과고판절조영소견과의상호판계 를바교하기위해 Catte r all 의분류 1) 를참고로하여단 순촬영소견을골두의첨벙정도에따라 4 군으로냐누 었는데제 1 군은대퇴골두의전연부만침범펀군으로 골두으 l 붕괴, 골간단변화, 성장판밴화를판찰할수

3 - 大韓放射線醫學會註 : 第 26 卷第 l 號 없는경우, 제 2 군은대퇴골단의전연부가광범위하게 침엄되어전후방 X 선상에서부골이양쪽내외측의 생골현 ( viable fragme nt ) 파함께나타나고측땅 X 선 상에선부골이후방으로분리되어 V" 형을나타낼 때, 제 3 군은대퇴골단의 3/4 정도가침범펀것무로 골두의붕괴가 2 군보다더욱심하고골간단의변화도 더욱광범위하고경부의확장을보일혜, 제 4 군은천 대회골단이첨병되어있고전후땅 X 선상대퇴골단의 완전붕괴가조밀한선상을나타내여골두의펀평화가 버섯모양을이루게되는경우로하였다. 고관절조영상에서대퇴골두의 s pheri c it y 는 Mose 형판 (te mpl ate) 을사용하여 spheric al, round, flatttene d 형으로대벨하였다 (Fig. 2 )4 ). 7~ <= 1. 선천성고관절탈구 성 t 셜은 16 예선예에서여자였고, 연령분포는 2 개월 에서 1 5 세까지였 A 며평균연령은 3 년 107~ 월이었다. 이환된부위는좌측과우측이각각 56 %(9/16 ), 과 44% (7/16 ) 이었 ξl 며양측성으로발생한경우는 38 %(6/16) 를차지하였마 (T able 1). 도수정복의성공여부는고판절내에조영제주입후 도수정복을시챙한상태에서판정하였으여평가기준 은비구내에내퇴골두의장경이들어가게되어동싱원 석정복이된것을완선정복으로판정하였 며이는 16 예중 4 예를차지하였고. 12 예에서는대퇴골두의장 경이바구내에있지않고비구의업쿠로부터 6mm 이 상떨어져있어서도수정복이살패한것 A 로판정하 였다. 도수정복을망해하는판절내안자로는비구순의 "1 Table 1. 5 jl t 끼- 大韓放射線훌훌學會誌 : 第 26 卷第 1 號 예 (32 %) 를차지하였으며양측성으로발생한경우는 2예 (9 %) 이었다 (Table 1). 진단시병의진행정도는 Salter의방엽에의한 4시기중 fagmentation stage 가 18예로천체의 72 % 를차지하였고그마음이 regeneratlon stage 로 20 % 를차지하였다 (T able 3)2). 발병연령이 7세이상인경우나이가어리더라도 Catterall 분류상 3군이나 4군에속했던환아에서판절조영상대퇴골두의밴형이심하지않아서보전석치료를했던 9예중 6개월후에추석검사가가능했던 4예에서연골성대퇴골두의변형이심하게진행된것을판찰할수있었마 (Fig.5) 이환된나이와대퇴골두의 sphericity와의판계는 5 세이전에발생했을경우에 spherical, round, flattened 형이각각 1 예, 1 예, 3예이었으며 5세에서 7세에발생했을경우엔각각 3예, 5예, 5예이였고 7세이후이 1 발생했을경우엔각각 2예, 3예, 2예이었다. 또위와같은연령군에있어서대퇴골둔의외측방아탈구 Oateral extrusion of femoral epiphysis) 는평균 25. 7, 16.8, 19.2의 acetabulum head index 4 ) 을보여주었다 (T able 4 ) Sphericity와 Catterall 군과의관계는 Catterall 3군과 4군에서 round 흑은 flattened 형을보였던예가 7 예와 10예로바교적않았으냐 spherical 형도 2예와 1 예를차지하였다 (Ta ble 5) Catterall 군과고판절의 lllcongrul ty 와의판계는 3 군과 4 군에서 1 군과 2 군보마심한 lllcongr Ul ty 를보 였으나 3 군파 4 군에서도정미한변화를보였던예가 각각 2 예씩있였마 (T able 6), (Fig. 6). Spherical shaped, slightly enlarged capital femoral epiphysis without incongruity of hip )olnt a b 파행이나무릎의통증등증상이나타난이후고판결조영술을하기까지의시간을근거로맹의이환기간파대퇴골두의 sphericity, 대퇴골두의외측방아달구와의판계를보았으며그결과는 Table 7과같다. 즉, 이들사이에서유의한상관관계를관찰할수없었마. 고찰선천성고판결탈구에있어서남녀성벨비는 L o renz 5 ) 는 6 : 1, 정등 15 ) 은 3 : 1 이었고온저자의경우에서는 13예천예가여자이었다. 좌우딸생빈도는 Lorenz는좌측이호말하며양측성은 34 % 로보고하였 냐본저자의경우좌측이 56 % 로좌측이다소않았 며양측성은 37 % 이었다. 고판절조영술은파거에고판결탈구의치료를위하 Table 7. 8 - 송창준외 : 선천성고관절딸구와 Legg-Calv e-pe rth es 벙어 l 있이서고판절조영술의의으 1 - 도수정복을시행하였을경우에대퇴골두의괴사를초 래하게된다는사실이알려침에짜라공기나조영제를 이용하여시행되기시작하였마. 고관절조영숭은연골 성 대퇴골두와고판절내의연부조직을판찰하는것이 외에도도수정복을시도하는경우에는도수정복의성 공여부를정확하게파악할수있으여정복후대되골두 의위치를정하는데있어서신속하게시행할수있는 유영한땅엽이라하겠다. 도수정복의성공은일반적으로나이가어릴수록좋 은것으로되어있으며 Ponseti 7 ) 는 3 세이하의경우 에보존석치료효서양호한결과를얻을수있으나냐 이가않아질수록어렵게되는데이는도수정복을방 해하는안자들이더욱많아지게되고더욱심한장애 를초래하기혜문이라하였다, makenzie 등 8 ) 에의하 연 18개월 l 상된아이에있어서도수정복이잘되지 않았는데그이유로는비구순의함몰, 판절섬유지땅 조직의비후, 대퇴골두의변형, 천땅판절낭악의단 축, 장요은의단축, 원형인대의비후, 판절낭악의모 래시계모양으로의변형등을열거하였마. 이들에대 해단순허수숭정복만을시행하변정복후불안정성을 주는요소가상존해있으으호하지를기능석위치로 했을얘불완전탈구나완전달쿠를유발시킬위험이 였다하였마. 강등 9 ) 은탈구된상태로시간이경과하여대뢰골두 및 "1 구의발육부천으로인한변행, 또는판절주위 연부조직즉장요은, 내천군, 관섣낭, 인대들의우축 이 였마. 얼어냐연도수정복및정복유지가어렵게된다하 정복에방해되는요소는판절외석인요인과판절내 석인요인으로대별되는데판절외석안요인은골반대 퇴픈 ( Pelvifemoral muscle ) 의쿠축, 비쿠및대퇴골의 변형등이있을수있고, 관절내석안요안으로는판결 순의 "1 후및내벤, 판절막의협착등이있 A 며, 이외 에도원안내와횡비구안대의비후및섬유지방조직의 증식이있을수있다 9 ) 이런경우에고판절조영술은 단순고판절촬영으로는얄수없는관절내정복저해 인자를보다정확하게파악할수있으며정복후동싱 원석정복여부를확실하게알수있다하였마. 이 저자들의경우에있어서 16 예의고관절중도수정복 가능하였띤 4 예를제외한, 도수정복이불가능했던 12 예에서도수정복을땅해한원인들로는바구순의비 후및내벤을보인경우가 12 예, 원인대의비후가 8 예, m M 횡바구인대의 "1 후가 10 여 1. 장요근의바후가 8 예, 관 절낭의유착이 7 예, 기타섬유성조직이나지방조직과 같은공간점유뱅소가 9 예를차지하였마. 도수정복을 실패했던건예에서판혈척정복을시행하였으며위와 같은도수정복땅해인자들은수술시확안되었다. 도 수정복이가능했던 4여 l 에서는고판절조영술상도수정 복을방해할만한요인들을발견할수없였마. 치료는일반적으로 3 세 이하에서는판절내적요인 이나판절외석원인에판계없이도수정복이가능하연 도수정복을시행하여, 3 세이후에있어서는고판절조 영술을시행하여판절내적요인이있는지확인한후 도수정복을땅해하는판절내석요인이없으연고판걸 의 안정성을높이기위한수숨을시행하고판결내석 요인이있으연판헬석정복을시챙하여야하며이해고 판절이불안정한것이영려되면고판절의얀정성을높 이는수술을뱅챙할수있을것이다 9 ) 우, 이와같이고판결조영숭은수숭적치료를요하는경 판절외석요인의제거만으로정복가능한지의 여부를평가할수있 A 며 판결내석요인이있는경우 각각의저해인자의정확한부위및밴형정도를솔전에 파악함으로써치료방첨을정할수있을뿐만아니라 도수정복을하였다하더라도단순촬영으로는보이지 않는대퇴골누의동성원석정복여부를정확하게파악 할수있어유용한검사방법으로사료된다. LCP 뱅은대퇴골두의우혈성괴사로서 1897 년 Mandle 이처음보고하였으며많은학자들에의해연 쿠와논란의매상이되고있는질환중의하나이마. 발생 반도는일반석으로여자보다남자에서 4 배로 않이발생하고 3, 13 ) 대부분일측성이여양측성으로침 범하는경우는 Aegerter, Duthie, Salter 에의하연 % 정도이다. 본저자들의경우남자에서약 7 배 로않았고양측성은 9 %(2/23) 였다. 호말연령은 Aegerter 등은 13 ) 3 세에서 12 세, Salter 3 ) 는 3세에서 10세라하였고저자의증례분석에서 는 4 세에서 8 세가 16 예로 70 % 를차지하였마. 엄상증상으로서가장초기에흔히볼수있는것은 파행이며, 그외에칭벙된대퇴부및슬판절부의내측 에서반사통을호소하기도하고, 또고판절의운동장 애특히내회전과외선운동의제한을올수있다. 저자들의경우에있어서도진찬할당시의주증상으로 서파행이 16 예로 70 % 를차지해가장않았다. 9 大韓放射線흥홉學會註 : 第 26 卷第 l 號 예측하는지침으로서발병연령과단순촬영상대퇴골 두의갱범정도를근거로한 Catterall 의분류가널리 이용되고있다는것은주지의사실이다. 그러나영에 이환되는연령은대퇴골두의골화가완성되지않은시 기이며 l 때단순촬영으로논대부분연골로되어있는 대퇴골두의모양을정확하게관찰할수없마. 그러나 이러한경우에고판절조영술을시행하면연골성대퇴 골두의모양을관찰할수있을뿐만아니라고판절의 mcongr Ul ty 와연골성대퇴골두의외측방아탈구를정 확하게판단할수있을것이마. 이에저자플은치료땅 법의결정및예후를예측하는지침으로서발병연령, Catterall 분류와더불어고판절조영숭이사용될수있 을젓이라는기대를가지고 23 영에서 25 예를대상우로 하여고판섣조영소견을분석하였다. 이를위해저자들은발영연령이 7 세이상이거나 Catterall 분류상 3 군이냐 4 군에속해있어도고판절조 영숭상대퇴골두의변형이심하지않아서보선석치료 를했던 9 예중 6 개월이후에추석검사가가능했띤 4 예 를대상으로치료전에시행했던고판섣조영소견과추 석검사로서시챙한고판절조영소견에서대퇴골두의 변형이진행되는지를비교하였으역또한치료방챔의 결정과예후를예측하는지첨우로서이용되고있는 Catterall 의분류와말병연령에대해서고판설조영솔 로관찰이 가능한대퇴골두의변형과고판절의 m congrulty 그러고대되골두의아탈구등이유의한상판 관계가있는지판찰하였마. 그러냐고판절조영술로 판찰된대퇴골두의모양을근거로보건석치료를하였 던전예의환아에서추석검사상대퇴골두의변형이 진행된것을볼수있었으여영에이환된냐이와대퇴 골두의 sphericity 와의판계는 5 세 이천에발생했을 경우에 spherical, round, flattened 행이각각 1 예썩있 였으며 5 세에서 7 세에발생했을경우엔각각 3 예, 4 예, 3 예이었고, 7 세이후에발생했을경우에 3 예, 2 예, 2 예이었마. 또위와같은연령군에있어서대퇴골 두의외측방아탈쿠는펑균 25.65, 16.84, 의 acetabulum head index 4 ) 를보였다. Sphericity 와 Catterall 군파관계는 Catterall 3 군파 4 군에서 round 혹은 flattened 형을보였던 6 예와 7 예로비교석많았 으냐 spherical 형도 2 예와 3 예를차지하였다. 파행이나무릎의통증등중상이나타난이후고판절 조영술을하기까지의이환기간은거의대부분 2 년이 내였으며이러한이환기간과대퇴골두의변형이나외 1, i 측방아달구사이에는어떤연판성도갖고있지 았다. Catterall 군과내퇴골두의외측방아탈쿠와의 판계는에교적 3 군과 4 군에서 1 군과 2 군에서보다섬 한정도의외측땅아탈구를보였 나 3 군과 4 군에서도 경미한변화를보였던예가각각 1 예씩있었다. 결국, 대퇴골두연골의밴형과고판절의 mcongrul ty, 그리 고내퇴골두의아탈구는치료방청을정하거나예후를 예측하는지침으로서이용되고있는말병연령이나어 % 떤 Catterall 군에속하는가에판계없이관절조영술을 시행했년당시에마양한도습을하고있었으며이들사 이에유의한혹은일판펀연판판계를판찰할수없 었다. 이와같은이유로는 2 년에서 3 년사이의장기간의치 유기간을갖는 LCP 영에있어서환자가내원해서고 판철조영솔을시행하는시기는이미대퇴골두의변형 l 끝난상태의대되골두를판찰하는것이아니라앞 으로도더욱심한변형이일어날가능성이있는뱅의 진행과정중의대퇴골두모양을보기때문이아닌가 사료된다. 즉발병연령이 7 세이상이거나 Catterall 3 군이나 4 군에속하는 LCP 뱅에있어서도고판철조영 술이대부분명의잔행과정중활동기에시행이되므로 연골성대퇴골두의변형이심하지 이다. 않 않을수있을것 그러냐연골성대퇴골듀 - 의모양은추석검사가 가능하였던 4 예에서볼수있듯이냐이가많거나 Cat. terall 3 군이냐 4 군에속하는환아에서시간이지남에 따라섬한변형이일어나게되며결국더심한매회골 두의변형을 - 막기위해서수숭을시행해야만할것 이다. 이와같이고판절조영술이 8] 록연골성대퇴골두의 모양이내고판절의 mcongrutty 그러고대되골두의외 측땅아탈쿠등을정확하게판찰할수있는검사방엽이 냐고판절조영소견들을근거로해서치료방칩을정 하거나예후를예측하는데는크게도움이되지뭇하며 굳이사용하게된다연오류를벙할가능성이있을것 이다. 명 결 저자들은고판절조영술을고판절탈구 16 예와 LCP 25 예에서시행하여그소견을분석고찰하고마음 파같은결롱을얻을수있었다. 고관절탈구에있어서는, 고판절에조영제를주입 트료
