The available gold standard methods for diagnosis, Tc-99m dimercaptosuccinic acid (DMSA) scan and computed tomography (CT) are invasive and expensive. This study was performed to assess the role of power Doppler ultrasound (PDU) for diagnosis of acute pyelonephritis (APN). Materials and Methods:A prospective study was conducted in 25 children with aged 2 weeks to 5 years who were hospitalized with the first episode of febrile UTI suggesting acute pyelonephritis. All children were examined in the first 3-5 days of admission by PDU and Tc-99m DMSA scan. The comparison between PDU and DMSA scan was performed on the basis of patients. 1) Results:The sensitivity and specificity of PDU for the detection of affected kidneys were 38.1% and 50.0%, and the positive predictive value and negative predictive value were 61.9% and 50.0%, respectively. Vesicoureteral refluxes (VUR) were identified in 11 patients (44.4%) and 18 kidneys (36%). The PDU and DMSA scan showed a matching perfusion defect in 23.8% and 50.0% respectively. Key Words: Acute pyelonephritis, Dimercaptosuccinic acid scintigraphy, Power Doppler ultra- sonography. 서 요로감염증은소아들에서흔한감염질환중의하나이다. 그러나영유아및학동전기소아에서의상부요로감염은비특이적인임상양상을나타내는특징때문에하부요로감염과의감별이어려운경우가많다. 하부요로감염은항생제의단기간의경구복용치료만으로후유증이거의없으나어린소아에서의상부요로감염특히요관역류가있는경우에는신손상이심하여고혈압과역류성신증으로인한만성신부 1, 2) 전으로진행할수있다. 따라서조기진단하여적절한치료를함으로써신손상을효과적으로예방할수있기때문에하부요로감염과의감별은매우중요하다. 이들을감별하는것은임상및실험실소견만으로는쉽지않다. 특히이학적소견이비특이적인매우어린환자들에서는더욱어렵다. 3) 신장초음파검사는어린소아들의요로감염의진단을위한첫영상진단법으로흔히이용되고있으나신실질의염증소견이나반흔을진단하는데는한계점이있다. 상부요로감염의확인을위하여여러가지영상기술이사용될수있겠지만이중 99mTc-dimercaptosuccinic acid (DMSA) 스캔이가장유용하게이용되고있다. 그러나침습적검사를하여야하고, 동위원소방사선에노출되며정맥약물투여를하여야하고대부분에서진정제투여의필요가 론 있다. 한편전산화단층촬영술과자기공명영상은급성신우신염의진단에매우좋은수단이지만고비용, 오랫동안진정을유지해야하고접근성이떨어진다는단점이있어실제임상에적용하기에는적합하지못하다. 근래에는보다접근성이뛰어난출력도플러신초음파 (color power Doppler sonography) 가급성신우신염의진단에보다유용한방법이라는보고들이있었다. 4-6) 우리나라에서는아직소아들의요로감염증에서보편적으로사용되고있지않은출력도플러신초음파가상부요로감염의감별진단을위하여어느정도유용할지를알고자본연구를시행하였다. 대상및방법 2002년 1월부터 2007년 8월까지영남대학교병원소아과에급성신우신염으로입원한만 7 세이전의소아중발열, 농뇨, 백혈구증가와급성염증반응의증가를보였지만빈뇨, 배뇨곤란, 요실금및하복부동통등의하부요로증세는없었던 25명을대상으로하였다. 요로감염증의진단은발열과농뇨를보이며치골상흡입이나도뇨를하여집락수가 10 3 이상인경우로하였다. 모든환아에서항생제를투여하기전혈액을채취하여백혈구수, 적혈구침강속도, C-반응성단백질, 혈액배양을실시 180

3 - 소아상부요로감염의진단을위한출력도풀러초음파조영술 - 하였고요검사및소변배양도실시하였다. 모든환아들은입원 3~5일이내에 DMSA 신스캔및출력도플러신초음파를시행하였다. DMSA 스캔을하기위하여서는모든환아에게 0.25~0.5 ml의 chloral hydrate로진정을한이후에검사를시행하였으나, 출력도플러신초음파를위하여서는약간의진정이필요한경우는 5명뿐이었다. 요로감염증의치료를한후더이상균배양이되지않은것을확인한후배뇨방광요도조영술을시행하였다. DMSA 신스캔은 20 Kg미만인환아에서는 111 MBq, 20 Kg이상인환아에서는 185 MBq 의 99mTc-DMSA 를정맥주사하고 3시간후에양측신장을 Siemens사의 e.cam 감마카메라를사용하여 matrix format에연속회전영상으로촬영하여획득하였다. 배경을보정한후, 각신장에서좌, 우활동량분획을계산하였으며총신장활동량의 45~55% 섭취율은정상으로판단하였다. 표지한 DMSA가신장에서한개이상의국소적또는미만성섭취감소를보이거나, 확대된신장에서전반적인섭취감소가있을때이상이라고판정하였다. 출력도플러심초음파는 General Electric 사의 LOGIQ7 초음파영상진단기에다중주파수곡선탐침자를사용하여앙와및복와위에서양측신장을검사하였다. 신장실질에서동일신장의동일한깊이의다른부위와비교해혈류가없거나감소한부분이있을때이상이라고판정하였다. 배뇨방광요도조영술은염증치료를한후배양검사에서균배양이되지않고적어도 3주이후에실시하였다. 얻어진결과수치의통계는 SPSS 통계프로그램으로 Chi-square test와 Fisher s exact test를이용하여 p<0.05인경우를통계적으로 유의한것으로판정하였다. 결과상부요로감염의증상을갖고있어서이연구에포함된환아는생후 1개월에서 7세사이로모두 25명이었으나 DMSA 스캔에병변의양성소견을보인경우는 21명으로서이들을상부요로감염으로진단하였다. 상부요로감염환아는남여각각 14명과 7명이었다. 상부요로감염과하부요로감염환아들의말초혈액의백혈구수 (/ μl ) 와적혈구침강속도 (mm/h) 는각각 12,610.0 ± 3,627.9과 38.7 ± 34.5이고 17,081.9 ± 6,750.9와 51.1 ± 3 0.7로차이가없었다. 그러나 C-반응성단백질 (mg/l) 은 2.6 ± 3.9과 9.1 ± 5.9로상부요로감염에서유의하게높았다 (p<0.05). DMSA 스캔에서병변을보인환아중출력도플러신초음파에결손소견을나타낸경우는 8명이었으며, DMSA 스캔에서정상소견을보인 4명의환아중 2명에서는결손소견이없었다. DMSA 스캔을상부요로감염의기준으로하였을때출력도플러신초음파의상부요로감염진단에대한민감도는 38.1%, 특이도는 50% 이고, 위음성율은 61.9%, 위양성율은 50% 이었다. 전체환아에서배뇨방광요도조영술을실시하였는데이중방광요관역류는 11명 (44.4%) 환자의 18개신장 (36%) 에서발견되었고좌우각각 9개씩이었다. 이중 grade 1은 1개, grade 2는 2개, grade 3는 9개이고 grade 4는 6개신장이었다. 방광요관역류는 DMSA 스캔과 23.8% 그리고출력도플러신초음파의소견과 50% 에서일치하였다. 6 - 최정연 조재호 박용훈 - 호흡운동, 늑골허상에의하여나타날수있으며, 위음성은급성신우신염의초기에염증부위의부종으로인한부분적정맥패쇄로혈류가완만하기때문이라고하며, 더구나간음향창 (hepatic acoustic window) 이없으면좌측신장의상부말단의혈류량투시에영향을받을수있다고한다. 15, 27) 특히다초점병변의급성신우신염에서는 DMSA 스캔에비해민감하지못하다고한다. 컴퓨터단층촬영이나 DMSA 스캔에비해여러가지이점이있으나좋은영상을얻기위하여서는검사자의상당한노력이필요하며검사시간이많이소요될수있으며시술중에환자의협조가매우중요하다. 15) 요약결론적으로출력도플러신초음파는급성신우신염의병변을찾는데에특이도나민감도가그리높지않았으나하부요로감염과급성신우신염을감별하는데에어느정도유용하였으며, 특히 DMSA 스캔을같이실시하는경우에는신손상을찾는데더도움이될수있었다. 소아들의상부요로감염의진단을위하여출력도플러신초음파가 DMSA 스캔을대체하는영상검사로서는한계가있어향후보다많은시술로진단을위한검사의질을높이는방법을찾아보아야할것이다. 참고문헌 1.
