The diagnosis of plantar fasciitis is primarily based on the presentation of symptoms and physical examination. Patients usually complain of heel pain at the medial calcaneal tubercle when taking their first step in the morning or when walking after resting. Diagnostic imaging is rarely required for the initial diagnosis of plantar fasciitis; however, it can be used for differential diagnosis. Conservative treatments, such as stretching, rest, ice massage, oral analgesics, foot orthotics, use of night splint, and corticosteroid injection, may be effective. The majority of patients report improvement with conservative treatments, and those who show no response from conservative treatments for a duration of six months or longer can consider extracorporeal shock wave therapy or surgery. Key Words: Heel pain, Plantar fasciitis, Extracorporeal shock wave therapy, Steroid injection 서론 발뒤꿈치통증증후군 (heel pain syndrome) 은크게발바닥발뒤꿈치통증 (plantar heel pain) 과발뒤꿈치후방부통증 (posterior heel pain) 으로나눌수있으며, 발바닥발뒤꿈치통증의원인으로는족저근막염, 족저지방패드의위축, 신경압박, 족근관증후군, 종골잠재골절등이있다. 이중족저근막염은후족부동통의가장흔한원인으로, 미국에서는매년백만명이상이치료를받는흔한질환이다. 1) 최근의연구들에서이질환은족저근막의반복적인미세외상 (microtrauma) 에의해발생하는만성적인퇴행성변화에의한것이며조직학적으로도퇴행성병변이확인되는점등을근거로족저근막염 (plantar fasciitis) 보다족저근막병증 (plantar fasciosis) 으로불러야한다고주장하기도한다. 2,3) 족저근막염 은비만, 족관절의배굴운동제한이있는환자, 편평족, 요족, 아킬레스건의단축환자에서좀더많이발생하는것으로알려져있다 (Table 1). 족저근막염은일반적으로저절로낫는자가회복질환 (self-limiting disease) 이며대부분의환자에서별다른치료없이도 1년내에회복된다. 그러나약 10% 의환자에서는통증이나일상생활에지장을줄정도의증상이지속되어적극적인치료를받아야한다. 이러한족저근막염의치료는아직논란이많고종종이질환의기전에대한이해가부족해치료에어려움을겪는경우가있어족저근막염의진단및치료에대해기술하고자한다. A 비만인 환자와 오랜 시간 서 있는 환자에서 더 많이 발생하는 것 으로 알려져 있는데, 이는 정상 체중이거나 오랜 시간 서 있지 않 는 이들에 비해 족저 근막의 부착부에 과도하게 가해지는 반복적 인 인장력이 염증 및 퇴행성 변화를 유발시켜 증상이 나타나는 것 으로 설명되고 있다.

Hicks4)는 입각기 후반 즉, 중족지 관절이 과신전될 여 중족족지 관절의 족저판(plantar plate)과 족지의 근위지골 기저 때 족저 근막이 긴장되고 이것이 종골 기시부에서 당김 작용을 하 여 종아치를 유지시켜준다고 하였으며 이 기전을 감아올림 기전 (windlass mechanism)이라 하였다(Fig. 1). A 비만인 환자와 오랜 시간 서 있는 환자에서 더 많이 발생하는 것 으로 알려져 있는데, 이는 정상 체중이거나 오랜 시간 서 있지 않 는 이들에 비해 족저 근막의 부착부에 과도하게 가해지는 반복적 인 인장력이 염증 및 퇴행성 변화를 유발시켜 증상이 나타나는 것 으로 설명되고 있다.5) 특히 아킬레스건 단축 등의 원인으로 족관 절의 배굴 운동이 제한된 경우 제 1중족 족지 관절의 배굴이 증가 함으로 감아올림 기전이 과도하게 작용하여 염증 및 퇴행성 변화 가 더 잘 유발될 수 있다.5,6) 최근에는 이 질환이 염증성이라기보 Flexor hallucis longus Plantar aponeurosis 다는 퇴행성에 의한 것으로 연구되면서 족저 근막병증(plantar fasciosis)으로 불리기도 한다. 실제 조직학적 검사상 염증 소견보다는 B 퇴행성 건병증과 유사한 소견이 관찰된다.2) 족저 근막염의 증상은 주로 아침에 처음 걸을 때나 휴식 후 걷기 시작할 때 발생한다. 이 원인은 수면 시 또는 휴식 시에 발이 보통 족저 굴곡된 상태를 유지하다 일어나서 걸을 때에는 배굴되는데, 이때 족저 근막은 수축된 상태에서 보행 시 늘어나게 되어 통증을 유발하는 것으로 생각할 수 있다. 진 Figure 1. The Diagnosis and Treatment of Plantar Fasciitis 95 이발생하며이통증은수면후일어나서걸을때나휴식후일어나걸을때증세가심하고계속보행을하면통증이사라지며, 과도한활동을하면다시통증이심해지는양상을보인다. 이학적검사로는압통을확인해볼수있으며후족부의압통위치에따라족저지방패드위축, 족저근막염, 신경포착, 종골잠재골절, 족근관증후군등을감별할수있는데, 종골내측결절부위에압통이있을경우족저근막염을의심할수있다 (Fig. 2). 이외에감별해야할진단에는강직성척추염, Reiter 증후군, 골관절염, 류마티스성관절염, 장무지굴곡건염등이있다. 증상이양측성일때, 여자의경우류마티스성관절염을감별하여야하며남자는강직성척추염이나 Reiter 증후군을고려하여야한다. 당뇨환자인경우연부조직농양을생각해야하며체중감소, 야간통증등을동반할경우종양이나감염을의심할수있다. 제 1천추신경근병증등도감별해야한다 (Table 2). 7) 단순방사선검사는진단에특이적이지않다. 편평족및요족등변형이동반되어있는지확인하여치료선택시참고할수있으며골절이나관절염등감별해야할질환을확인해야한다. 약 50% 의환자에서종골골극을관찰할수있으나이골극은족저근막의기시부가아닌단족지굴곡건의기시부에부착되어있어족저근막과는관계가없는것으로여겨지고있다. 실제로족저근막염이없는환자의 20% 에서도골극이발견되고있어많은저자들은증세의원인이아니라고주장한다. 8) 초음파검사는족저근막염진단시자기공명영상이나골스캔만큼효과적이다. 족저근막염환자에서는족저근막의두께가두꺼워지고저에코소견을보여진단에도움을준다. 9) Tsai 등 10) 이나 Steinbach 11) 의연구에서는정상적인족저근막의두께를 4 mm 이하로보고하는등일반적으로족저근막의두께가 4 mm 이상일때족저근막염으로여겨지고있다. 그러나서양에비해체구가작은동양인에게동일한기준을적용하기는어려울것으로생각되며근막의두께가증상정도와반드시일치하지않을수도있다. Kim 등 12) 은건측에비해 1 mm 이상비후된경우의미있는소견이라하였으며, 양측성일경우 3 mm 이상의두께를의미있는소견으로구 분하였다. Yoon 등 13) 은건측에비해 1 mm 이상비후되거나두께가 3.8 mm 이상인경우족저근막염을의심할수있다고하였다. 골스캔시족저근막염환자에서는후족부의압통부위에동위원소의흡수가증가되는것을관찰할수있어진단에도움이되며, 잠재골절이있는경우에도동위원소의흡수가증가될수있다. 근전도검사는천추신경포착이나족근관증후군, 외측족저신경포착등과같은신경원성원인이의심될때시도해볼수있다. 자기공명영상에서근위족저근막이두꺼워진것과 T2 강조영상에서증가된신호를보일때족저근막염을진단할수있으며, 또한연부조직종양등다른연부조직질환이나잠재골절등을감별할수있다. 양측성으로병발한환자에서는류마티스성관절염이나혈청음성척추관절병등을감별하기위해백혈구수, human leucocyte antigen B27 (HLA B27), antinuclear antibodies (ANA), uric acid 등을검사하여야한다. 치료 족저근막염환자에서우선적으로보존적치료가시행되며많은환자가이러한치료만으로도좋은결과를나타낸다. 14) Thomas 등 3) 이 2010년발표한후족부통증의치료가이드라인에따르면단계별치료가있으며수주또는수개월간치료에반응이없을때다음단계의치료를시행하여야한다 (Fig. 3). 족저근막염환자가처음방문하였을때는스트레칭, 휴식, 냉찜질, 진통제, 보조기등을사용할수있으며 6주이상치료에도효과가없을경우스테로이드주사, 맞춤형안창, 야간부목등으로치료한다. 이러한치료로 2 3개월내에약 85% 90% 의환자에서호전을보이며, 이때에는증상이없어질때까지치료를지속해야한다. 하지만 6개월이상치료에반응이없거나호전이더딘환자에대해서는체외충격파요법이나수술적치료등을고려하여야한다. 스트레칭은쉽게할수있고비용이적게드는방법으로, 스트레칭을이용한치료로족저근막염환자의 83% 까지치료되었다는보고가있을정도로효과적인치료법이라할수있다. 15) 족저근막스 Table 2. Treatment algorithm for patients with plantar fasciitis. 트레칭이 아킬레스건만 스트레칭하는 것보다 결과가 더 좋다고 보 고되고 있다.16) 족저 근막 스트레칭은 한 손으로 족지를 배굴시키 면서 다른 한 손으로 족저 근막이 긴장되는 것을 확인하는 방법으 로 한다(Fig. 4). 1회에 30초 동안 유지하고 최소 3회 이상 매일 시 행하여야 하며, 특히 아침에 일어나서 걷기 전이나 오랜 시간 앉아 있다가 서기 전에 시행하면 도움이 된다. 또 다른 방법으로는 잠들 기 전 얼린 깡통을 발 아래 두고 5 10분간 압력을 주어 굴리는 방 법이 있다(Fig. 5). 아킬레스건 단축이 있으면 아킬레스건의 스트 Figure 5. Ice massage method, which involves rolling a frozen can under the foot. 레칭도 병행하는 것이 좋다. 휴식 및 냉찜질, 아세트아미노펜이나 비스테로이드성 소염 진 춤형 안창(custom-made insoles)은 편평족이나 요족, 후족부 내반, 통제(nonsteroidal antiinflammatory drugs)의 사용 또한 족저 근막 하지 부동 등 생역학적인 위험인자를 가진 환자에서 주로 사용된 염의 통증 완화에 도움이 된다. 족저 근막염 환자를 대상으로 비스 다. 제작된 안창과 맞춤형 안창은 편평족이나 요족과 같은 변형이 테로이드성 소염 진통제를 사용한 군과 대조군을 비교한 연구에서 없다면 결과가 큰 차이가 없는 것으로 보고되고 있다.18,19) 비스테로이드성 소염 진통제를 사용한 군의 통증 완화 정도 및 기 17) 능 회복이 더 우수하다는 연구가 있다. 야간 부목은 보통 밤 사이 발을 중립위나 경하게 배굴되도록 만 들어 준다. 일반적으로 발은 자는 동안 족저 굴곡되어 족저 근막이 족저 근막염 환자에서 보조기는 근막에 인장력을 덜어주는 방법 수축이 되는데, 야간 부목을 사용함으로써 족저 근막이 신장되게 이다. 뒤꿈치 컵(heel cup)과 안창은 족저 근막염 환자의 통증을 효 하여 아침에 첫 보행 시 긴장을 줄여주는 이론이다. 그리고 수면하 과적으로 완화시켜준다. 뒤꿈치 컵은 종골의 직접적인 충격을 줄 는 동안 배굴된 상태로 종아리 및 족저 근막에 수동적으로 스트레 여주고 이론적으로 뒤꿈치를 들어줌으로써 족저 근막의 긴장도를 칭을 유지시켜주는 효과도 있다. 여러 연구에서 야간 부목을 사용 줄여주는 효과가 있다. 제작된 안창(prefabricated insoles)은 경한 하는 환자의 약 80%가 호전을 보였다고 보고하고 있다.20,21) 야간 편평족이 동반된 족저 근막염 환자에게 도움이 되며 특히 발의 모 부목을 시행하면 수면하는 동안 불편감으로 수면장애가 발생할 수 양이 빠르게 변하는 성장기의 청소년에게 적절한 보조기이다. 맞 있다는 단점이 있다.

The Diagnosis and Treatment of Plantar Fasciitis 97 스테로이드주사는항염증효과가있어급성및만성족저근막염환자에서효과적이라고보고되고있다. 22,23) 그러나조직학적으로이질환은염증성이아닌퇴행성질환으로스테로이드주사가통증감소에단기적으로만효과가있거나또는효과가없다는주장도있어논란이있다. 24) 최근에이루어진메타분석 (metaanalysis) 연구에의하면스테로이드주사는주사후약 1개월가량은통증감소에효과적이나그이후에는큰효과가없는것으로보고하고있으며, 25) 반복적인스테로이드주사는족저지방패드의위축이나족저근막파열의위험을증가시키므로반복적인주사는가능한한피해야하고다른보존적치료를시행해야한다. 26) 최근에는 botulinum toxin이나본인의혈장을이용한혈소판농축혈장 (platelet rich plasma) 을이용한주사치료도좋은결과를보인다는연구가소개되고있으나연구가더필요한실정이다. 27,28) 이상의보존적치료에도 6개월이상효과가없을때는체외충격파요법 (extracorporeal shock wave therapy, ESWT) 이나수술적치료를고려해볼수있다. 족저근막염에서체외충격파요법을이용한치료의정확한기전은밝혀지지않았으나재혈관화 (revascularization) 를증진시키고손상된조직의치유를돕는다고알려져있다. 29) 체외충격파를이용한시술은비침습적이며회복기간이짧고수술적치료와비슷한결과를보이는장점이있다. 30) 그러나시술중이나시술후통증을야기시키며국소적인부종이나감각이상, 저린증상등이발생할수있는단점이있다. 27) 또한체외충격파의적용방법이저자마다다양하게보고되고있다. 충격파의발생양식에따라전기수압방식 (electro-hydraulic type), 전자기방식 (electromagnetic type) 및압전기방식 (piezoelectric type) 등이있는데 Fuselier 등 31) 은전기수압방식이다른방식에비해효과가월등한것으로보고하였다. 저에너지체외충격파를사용하면환자에게불쾌감을유발한다는보고가있으며고에너지를사용할경우 1회의시술로끝날수있고치료효과도우수하다고보고하고있으나국소마취의사용이필요하다는단점이있다. 충격파의횟수도 1,000 2,000 회로다양하게적용되고있으며진동수역시 MHz로적용되고있다. 치료간격역시논란이많으나보통 1 4주간격으로 3회이상실시한다. 앞으로적절한치료방침에대한연구가필요할것으로생각된다. 수술적치료는모든보존적치료에반응하지않는족저근막염환자에서시행할수있다. 족저근막절개술 (plantar fasciotomy), 관절경적근막유리술 (endoscopic plantar fascia release), 비복근유리술 (gastrocnemius recession) 등의여러가지방법이소개되고있다. 족저근막절개술은흔히사용되는방법으로족저근막의내측근위부절개를통해근막의내측 1/3 가량을절개해준다. 보존적치료에효과가없었던족저근막염환자에게개방성족저근막절개술을시행하여 70% 90% 의성공률을보였다 ) 수술적치료시골극의제거가결과에도움을주지않는다고보고되고있으 며, 35,36) 임상적으로외측족저신경분지포착의동반이의심될경우신경유리술을같이시행해주는것이좋다. 16) 관절경적근막유리술은숙련된기술이필요하여아직은널리사용되지않고있으나, 37) Lundeen 등 38) 은 53명의족저근막염환자에게관절경적근막유리술을시행하여 81% 의환자에서만족할만한결과를보고하는등좋은결과가보고되고있다. 내측복사골끝 (tip) 에서발바닥방향으로연장선을그어발바닥과만나는부분을내측삽입구로한다. 내측삽입구를통해족저근막의내측에닿으면뭉툭한투관침 (blunt trocar) 으로족저근막의배측을따라서외측으로접근한다. 외측피부에서투관침이닿으면그부위를절개하여외측삽입구를만든다. 관절경을이용한수술은개방성수술에비해연부조직손상이적은장점이있으나시야가좁고외측족저신경등의신경손상의가능성이있다. 비복근유리술은비복근이단축또는긴장된환자에서사용할수있는수술법으로, 39,40) 비복근의긴장도는 Silfverskiöld 검사법 41) 으로평가할수있다. 비복근의유리는비복근근위부나근건이행부또는원위부에서이루어질수있는데대부분에서좋은결과를보고하고있다. 39,40,42) Abbassian 등 40) 은만성족저근막염환자에게비복근유리술을시행하여 81% 의환자에서통증이소실되었다고보고하였으며, Maskill 등 42) 의연구에서비복근유리술후 93% 의환자에서만족할만한결과를얻었다고보고하였다. 비복근유리술시행후장딴지근육의힘이소실될수있다는보고가있으나, 43) 장딴지근육의힘에전혀영향을주지않았다는연구도보고되고있다. 44) Monteagudo 등 44) 은만성족저근막염환자중비복근의단축또는긴장된환자에서비복근유리술이족저근막절개술보다결과가더좋다고보고하는등비복근유리술의좋은결과가많이보고되고있어수술적치료의선택시고려할수있는수술법으로생각된다. 결론 족저근막염은후족부통증의가장흔한원인이며과도한사용에의한퇴행성질환으로여겨진다. 진단은전형적인임상증상으로가능하며영상학적검사를통해감별진단을할수있다. 대부분에서보존적치료로호전되는데스트레칭, 얼음찜질, 보조기 (heel cup, prefabricated insoles), 진통제등으로먼저치료하여약 6주간경과관찰후증상호전이없으면야간부목, 맞춤보조기 (custommade insoles), 스테로이드주사등을사용할수있다. 이러한보존적치료에 6개월이상효과가없을시체외충격파요법이나수술적치료를고려해볼수있다.
