1 대흉외지 2009;42: 임상연구 Panda Pneumothorax Set with Heimlich Valve 에의한외래에서의흉관관리 최순호 * ㆍ이미경 * ㆍ류대웅 * Outpatient Chest Tube Management with Using a Panda Pneumothorax Set with a Heimlich Valve Soon Ho Choi, M.D.*, Mi-Kyung Lee, M.D.*, Dae Woong Ryu, M.D.* Background: Prolonged air leakage and pleural fluid drainage from a chest tube may delay removing the chest tube after a patient undergoes video-assisted thoracoscopic wedge resection and the patient is otherwise ready for discharge. We reviewed 37 outpatients patients who were being managed with a postoperative chest tube (a Panda Pneumothorax set with a Heimlich valve). Material and Method: From January 2005 to December 2007, 294 patients underwent video-assisted thoracoscopic wedge resections & pleurodesis. Of them, 37 patients met the criteria for outpatient chest drainage management with using a Panda Pneumothorax set with a Heimlich valve. The patients received written instructions, and they demonstrated competence with using the Panda system. The patients returned for chest tube removal after satisfactory resolution of their air leak and pleural fluid drainage. Result: The patients discharged with a Panda pneumothorax set had a longer duration of hospital stay (mean: 10.3±1.7 days, range: 11 to 17 days) as compared with the patients without a Panda pneumothorax set (mean: 6.2±1.5 days, range: 4 to 7 days). The chest tube was removed successfully from the patients with a Panda pneumothorax set at an average of 9.8±1.6 days (range: 9 18 days) after discharge. There were no major complications. Four patients experienced minor complications. Thirty six patients (97.3%) experienced uneventful and successful outpatient chest tube management. Conclusion: Successful postoperative outpatient chest tube management with using the Panda set was accomplished in 36 selected patients. This program resulted in a substantially reduced hospital cost and enhanced patient satisfaction by allowing earlier discharge. Key words: 1. Pneumothorax 2. Videothoracoscopy 3. Tube drainage (Korean J Thorac Cardiovasc Surg 2009;42: ) 서론폐절제수술후지속적인공기유출은가장흔한합병증이며입원기간을증가시키는것으로알려져왔다 [1]. 일반 적으로폐절제수술후흉강에내재하는흉관을통한과도한흉수의배액과공기누출은때때로퇴원을지연시켜서입원기간및의료비의증가로인한환자의불만을야기한다. The Panda set can be attached to the body by either the shoulder strap or belt that are provided, which facilitates ambulation. 개발되었고, 1968년이후임상에서이용해왔는데 [2], Heimlich valve의사용은기흉치료의성공률을증가시킬뿐아니라낮은합병증으로외래에서이동성치료를활성화시킨다고주장된이후 [3], 기흉의외래치료에 Heimlich valve가사용돼왔으나그시기에는흉수를처리하는데는불완전하였다. 그이후로이런단점을보강한여러 Pneumothorax set가개발되었는데, 그중국내개발상품인 Panda pneumothorax set [ 同和판다 ( 주 )] 를이용해비디오흉강경하에서다수의폐쐐기절제술과흉막유착술후에지속적인공기유출과흉수를보이는 37명의환자에서조기퇴원을활성화시키고이차재수술을피하기위해서사용하고그효용성을평가하였다. 대상및방법 2005년 1월부터 2007년 12월까지원발성과이차성기흉으로비디오흉강경하에서다수의폐쐐기절제수술을받았던 294명의환자중수술후 10일이상지속적인공기유출과흉수배액을보여 Panda pneumothorax set with Heimlich valve (Fig. 1) 를착용시키고퇴원시켰던 37명의환자를대상으로하였다 환자의평균나이는 43.4±5.7세 (19 73세) 이었고남녀비는 34:3으로대부분이남자였다. 좌우의비율은 17:20 이었다 (Table 1). 퇴원후외래치료대상의평가는지시사항을수행할수 Age 43.4±5.7 years (range years) Hospital length 10.3±1.7 days (range days) Gender Male 34 patients Female 3 patients Right/Left ratio 20/17 Chest tube removal 9.8±1.6 days (range 9 18 days) after discharge Surgical procedure Multi-wedge resections +pleurodesis Age given in years±standard deviations. Length given in days± standard deviations. 있는가, Panda set에관한기록된지시사항을이해할수있는능력, 필요시적당한거리안에의료지원이가능한가의환자의거주지환경이었고, 그중에서도가장중요한것은 Panda set를착용하고퇴원하려고하는환자의의지였다. 대상환자들은임상적으로는안정된환자로, 즉단지병원에머무르는유일한이유가지속적인공기유출과흉수의배액이고퇴원이준비된환자로이들대상이만족할때는 Panda set로바꾸었다. 선별평가후대상환자는 Water-seal system에서 Panda pneumothorax set로바꾸고이들환자들은이기구에친근하도록하루더입원하였고그동안에집에서관리에관하여교육하였고환자들이잘견뎌내고안전하게관리할수있도록확신을주도록교육하였다. 그리고흉관과 Panda set의연결부위는빠지지않도록반창고를이용하여견고하게고정을하였다. 모든환자는필요할때는주위병원이나흉부외과외래에접촉하도록하였고비상시흉부외과의사와연락이가능한전화번호를알려주었다. 퇴원후흉관이삽입되어있는동안은구강용항생제와진통제를복용하도록하였고일주일에 1 2회정도이학적인검사와드레싱을위해서외래를방문하도록하였다. 외래방문시더이상의공기누출과흉부배액이없을때는흉관제거전에방사선사진을찍어완전한폐의재팽창을확인하였고, 흉관제거후다시방사선사진을촬영해재팽창이유지되는가를다시확인후귀가하도록하였다. 498
3 최순호외외래에서의흉관관리 결과 Panda pneumothorax set를거치하지않은 257명의평균입원일은 6.2±1.5일 (4 9일) 이었고수술일로부터평균 4.7± 1.4일 (3 6일) 에흉관을제거하였다. 그러나공기누출이지속되어서 Panda set를착용한채퇴원한환자의평균입원일은 10.3±1.7일 (12 18) 이었고퇴원후외래에서평균 9.8±1.6일 (9 18일) 만에흉관과 Panda set를제거할수있었다. 전체 36/37 (97.3%) 의환자는순조롭게성공적으로치료하였으며주요한또는생명을위협하는합병증은없었고또한 Panda set의작동불량에의한교환도없었다. 4 명 (10.8%) 에서합병증을보였는데 1명은퇴원후경도의피하기종을보인환자로써재입원하여 Water seal system 으로 3일간치료후다시 Panda set를착용하고퇴원후성공적으로외래치료를하였고, 다른 3명에서흉관주위의경도의염증을보였으나흉관제거후별문제없이해결되었다. 결론적으로 36/37명 (97.3%) 의환자에서 Panda set를착용하고조기퇴원한후외래로다니면서성공적으로치료를마칠수있어서, 수술후지속적으로공기누출과흉수를보이는환자에서 Panda Pneumothorax set를이용함으로써조기퇴원과병원비의절감을이룰수있어환자의만족도를증진시킬수있을것으로생각된다. 고찰일반적으로 Heimlich valve를사용하기전까지는폐실질부절제수술후지속되는흉수배액과공기누출이완전히해결될때까지는전통적으로입원하여고식적인방법, 즉 Water-seal 배액과흡인으로치료하였는데, 저절로폐쇄될때까지는장기간의시간이필요해결과적으로입원시간을증가시키게되었다. 이런경우 Heimlich valve의사용은입원일을단축시키고외래치료를가능하도록한다고하였다 [4]. Rice 등 [5] 은폐수술을받은환자의거의 15% 는단지폐기포공기누출때문에입원하고있다고하였다. 이런문제들을해결하기위해서 Heimlich[6] 는초기에소변수집자루에단지 Heimlich valve만을연결하여외래로다니면서치료를하도록하였다. McKenna 등 [7] 은폐용적감소수술후불완전한폐재팽창때에도 Heimlich valve의사용으로이차수술의필요성을배제할수있었을뿐아니라, 평균입원일을약 46% 정도감소시킨다고하였다. Ponn 등 [8] 은폐절제수술후지속적으로공기누출을보이는환자에서 Heimlich valve를장착후퇴원시켰고, 98% 의성공률을보였다고보고를하였지만, 동시에섬유성흉수배액이 Heimlich port를폐쇄할수있는문제점과동시에상행성흉막강감염이문제가될수도있다고하였다. 또한, Choi 등 [9] 은 Heimlich valve의사용은외래로다니면서도치료를활성화할수있으면서의료비를감소하기때문에입원에의한폐쇄성흉관삽관술에의한 Water-seal bottle 사용과흡인보다더이점이있다고하였다. 외래에서의흉관및 Pneumothorax set의관리는남의도움없이도휴대, 보행할수있도록도와줄뿐아니라비용면에서도절감되기때문에정신적으로도환자에게안정감을줄수있다. 그러나초기에 Heimlich valve의이용은개방된말단부가항상젖어있어거즈의교환을필요로하며, 동시에상행성감염의위험성때문에 Heimlich valve 의유용성을제한하는요소이었다 [10,11]. 그이후에도지속적인공기누출의외래치료를위한여러 Pneumothorax set들이보고되었으나흉수처리의미흡때문에광범위하게이용되지않았으나 [12,13], 2000년이후이문제를해결한기구들이개발되었고 [14-16], Panda peumothorax set도그중의하나이다. Panda Pneumothorax set의이점은공기누출의배기가수월하고, 배액된흉수를수집할수있으며, 투명한자루의눈금을통한계측도가능하며, 또한자유롭게휴대하면서도이동성이있다는것이다. 이것은환자들이더편하고쉽게 Set를관리하도록할뿐아니라흉관주위를항상마른상태로깨끗하게유지할수있도록해준다. 또한 Pneumothorax set system이이중밸브체계로밀폐는되지않았지만훨씬무균적이어서상행성감염의경우도예방할수있다는것이다. 더욱중요한것은흉관제거후의추적기간동안에도폐염이나농흉의예는없었다. 결론적으로 Panda Pneumothorax set는비디오흉강경하에다수의폐쐐기절제수술후지속적으로공기유출과흉수배액이있는환자들을조기에퇴원시킨후외래에다니면서도치료를촉진시킬수있어서입원치료비의감소와더불어서환자의만족감을증진시킬수있을것으로생각된다. 결론비디오흉강경하폐실질부쐐기절제수술과흉막유착술후지속적인공기누출이있는환자 36명에서 Panda set 499

5 최순호외외래에서의흉관관리 = 국문초록 = 배경 : 비디오흉강경하폐쐐기절제수술후흉관을통한지속적인공기유출과흉수배액은퇴원할준비가되어있는환자의흉관제거를지연시킬수있다. 저자는 Heimlich valve 가부착된 Panda Pneumothorax set 를장착하여외래에서흉관관리를시행한 37 명의환자를관찰하였다. 대상및방법 : 2005 년 1 월부터 2007 년까지 294 명에서비디오흉강경하폐쐐기절제술과흉막유착술을시행하였다. 이중 37 명의환자가 Panda Pneumothorax set with Heimlich valve 를이용한외래에서의흉관관리를위한대상에적합하였다. 환자들은쓰여진주의사항을받았고 Panda set 사용에대한믿음을보여주었다. 환자들은공기누출과흉수배액의만족할만한소실후흉관제거를위해서병원에복귀하였다. 결과 : Panda Pneumothorax set 를착용하고퇴원한환자 [ 평균 4.7±1.4 일 (3 6 일 )] 는착용하지않았던환자 [ 평균입원일은 6.2±1.5 일 (4 9 일 )] 와비교할때보다장기간의입원을하였다. 그리고또한 Panda Pneumothorax set 환자의경우퇴원후외래에서평균 9.8±1.6 일 (8 15 일 ) 만에성공적으로흉관을제거할수있었다. 주요한합병증은없었으며 4 명의환자에서사소한합병증을보였다. 36/37 명 (97.3%) 에서순탄하고성공적인외래에서의흉관관리를경험하였다. 결론 : Panda set 를이용한성공적인외래에서의흉관관리를선택된 36 명의환자에서성실히수행할수있었다. 이런프로그램은결과적으로명백한병원비절약을이룰수있었고조기퇴원을가능케하여환자의만족감을증가시켰다. 중심단어 :1. 기흉 2. 비디오흉강경수술 3. 흉관배액 501
