Methods : We have studied the in vitro activities of cefditoren and other oral antibiotics against 12 S. pneumoniae isolates, including 8 penicillin non-susceptible isolates and 8 H. influenzae isolates from clinical specimens of patients at the Asan Medical Center. Minimal inhibitory concentrations (MICs) were determined by the agar dilution method. Results : All S. pneumoniae strains tested were inhibited by 1 g/ml of cefditoren (MIC 5/MIC 9.25/1 μg/ml; range.15~1 μg/ml). The MICs were lower for penicillin-susceptible S. pneumoniae (MIC 9.15 g/ml) as compared to penicillin-intermediate resistant (MIC 9.5 g/ml) or penicillinresistant strains (MIC 9 1 μg/ml). Cefditoren was active against all tested H. influenzae strains (MIC 5/MIC 9.15/.3 μg/ml; range <.8~.3 g/ml) and its activity was comparable to levofloxacin and cefixime. Conclusions : Cefditoren had an excellent activity against S. pneumoniae and H. influenzae irrespective of penicillin or ampicillin resistance, respectively. 2 -Yee Gyung Kwak, et al : In vitro activity of cefditoren - 서론 Streptococcus pneumoniae와 Haemophilus influenzae는원외호흡기감염증의가장흔한원인균으로전통적인치료제로사용해오던항균제에대한내성이최근크게증가함에따라이들균주에의한감염증의치료제선택에어려움을겪고있다. 특히우리나라의경우 S. pneumoniae에서의 penicillin 내성률은 58~8%, H. influenzae 의경우 ampicillin에대한내성률이 55~68% 정도로전세계적으로도가장내성률이높은지역으로알려져있다 1, 2). Cefditoren 은비교적최근에국내에소개되기시작한경구용 3세대 aminothiazolyl cephalosporin 으로 S. pneumoniae, H. influenzae 를포함한호흡기감염증의원인균과 Enterobacteriaceae 를비롯한그람음성균및 methicillin-susceptible Staphylococcus aureus 등에대해광범위한항균력을가지는것으로알려져있고, 일부연구에서는 penicillin 내성 S. pneumoniae에대해서도항균력을나타내는것으로보고하고있다. 국내에서도 cefditoren 의시험관내항균력에대한보고가있기는하지만비교적제한적이어서본연구에서는국내에서분리된 S. pneumoniae와 H. influenzae 임상분리주에대해 cefditoren 과다른경구항균제의시험관내항균력을비교함으로써임상적사용에의유용성을재평가하고자하였다 3, 4). 대상및방법 2년 1월부터 24년 3월까지서울아산병원에서분리된임상균주중 S. pneumoniae 12균주와 H. influenzae 8균주를대상으로 cefditoren을비롯한항균제들의시험관내항균력검사를실시하였다. 시험대상이된 S. pneumoniae는 penicillin 감수성 4균주 (MIC.3~.6 μg/ml), penicillin 중간내성 4균주 (MIC.125~1 μg/ml), penicillin 내성 4균주 (MIC 2~8 μ g/ml) 였고, H. influenzae 는 ampicillin 감수성 4균주, ampicillin 내성이 4균주였다. 각균주에대한최소억제농도 (Minimal Inhibitory Concentration; MIC) 는한천희석법 (agar dilution method) 으로시험하였다. 감수성시험용배지로 S. pneumoniae 에대해서는 5% 면양혈액 (sheep blood) 을넣은혈액한천배지 (blood agar plate) 를사용하였고, H. influenzae 에대 해서는 Haemophilus test medium 을사용하였다. 시험항균제는 cefaclor (Sigma Chemical CO., St. Louis, MO, USA), cefixime (Fujisawa Pharmaceutical CO., Osaka, Japan), cefpodoxime ( 한일약품, 서울, 한국 ), cefditoren ( 보령제약, 서울, 한국 ), clarithromycin (Pharmacia, New Jersey, USA), levofloxacin (Bayer AG, Leverkusen, Germany) 이었고, S. pneumoniae에대해서는 amoxicillin (Sigma Chemical CO., St. Louis, MO, USA) 을추가로시험하였다. 대상균주를한천배지에계대배양하고, 35 에서 16~ 2시간배양후성장한균집락으로.5 McFarland 혼탁도의균부유액을만들어 1배희석한후 2 L를취하여각종항균제가 2배수의연속적인농도로함유된배지에접종하였고, 이것을 35, 5% CO 2 항온기에서 2~24시간배양후결과를판독하였다. 각항균제의 MIC는세균의증식이없는최소한의항균제농도로하였으며각균주에대한 MIC는 2회측정하였다. 감수성시험의정도관리는 S. pneumoniae ATCC 49619, H. influenzae ATCC 49247, 균주를이용하여이표준균주들에대한각항균제의 MIC가허용범위내에있는지를확인하였다. 감수성여부는미국임상검사표준화협회 (Clinical and Laboratory Standards Institute; CLSI) 의기준을이용하여해석하였고, CLSI의기준이정해지지않은 cefditoren 의경우에는기존의보고들을참고하여 S. pneumoniae에대해서는감수성 MIC 기준을.25 μg/ml,.5 μg/ml, 1 μg/ml 이하, H. influenzae 에대해서는.5 μg/ml, 1 μg/ml 이하로하여각각판독하였다 5-11). 결과대상균주가분리된임상검체의종류는표 1에제시된바와같다. S. pneumoniae는객담과혈액, H. influenzae 의경우는객담에서분리된균주가가장많았다. 1. S. pneumoniae Penicillin 감수성, 중간내성및내성 S. pneumoniae 균주에대한 cefditoren 을비롯한 7종의항균제들의 MIC 범위및 MIC 5, MIC 9 은표 2와같다. Penicillin 감수성여부에따른 cefditoren 과다른항균제의항균력을 MIC 9 을기준으로비교하였을때 penicillin 감수성 S. pneumoniae에대해서는 amoxicillin (MIC 9.8 μ

Penicillin 감수성균주에서 cefditoren의 MIC (.15~.3 μg/ml) 는 penicillin 중간내성 (.3~.5 μg/ml) 및내성균주 (.5~1 μg/ ml) 에비해낮았으며, 이러한경향은다른경구용 cephalosporin 제제에서도비슷하게나타났으나이에비해 levofloxacin의항균력은 penicillin 에대한감수성여부에영향을받지않았다. Cefditoren의 MIC 5/MIC 9 은.25/1 μg/ml, MIC 범위는.15~1 μg/ml 이었고모든 S. pneumoniae 균주는 cefditoren 농도 1 μg/ml 이하에서억제되었다. 다른 cephalosporin과비교하였을때에도항균력이우수하였고, 이것은임상적으로문제가되고있는 penicillin 내성균주에서더욱뚜렷하게나타났다. Cefditoren 에대한감수성기준을 MIC.25 μg/ml, 내성기준은 MIC 2 μg/ml 으로하였을때 penicillin 감수성균주의 1%, 중간내성균주의 89.% 가 cefditoren 에감수성을나타냈고, penicillin 내성균주는모두 cefditoren에내성이었다. Cefditoren 에대한감수성기준을 MIC.5 μg/ml 이하로한경우에는 penicillin 감수성및중간내성균주는모두 cefditoren 에 감수성이었고, penicillin 내성균주의 42.5% 가 cefditoren 에감수성이었다. Cefditoren 에대한감수성기준을 MIC 1 μg/ml 이하로하였을때는 penicillin 에대한감수성여부에상관없이모든균주가 cefditoren 에감수성을보였다. 2. H. influenzae Ampicillin 감수성및내성균주에대한 cefditoren을비롯한 6종의항균제들의 MIC 범위, MIC 5, MIC 9 은표 3과같다. Cefditoren (MIC 9.15~.3 μg/ml) 과 levofloxacin (MIC 9.15~.3 μg/ml), cefixime (MIC 9.125~.6 μg/ml) 이 ampicillin 감수성및내성 H. influenzae 에대해가장강한항균력을보였고, 모든 H. influenzae 균주는 ampicillin 내성여부에관계없이 cefditoren.3 μg/ml 이하의농도에서억제되었다. Cefditoren 의감수성기준을 MIC.5 μg/ml 이하, 혹은 MIC 1 μg/ml 이하로하였을때 ampicillin에대한감수성여부에상관없이모든균주가 cefditoren 에감수성을보였고, 다른경구용 cephalosporin 항균제와 clarithromycin에비해서우수한항균력을보였다. 고찰 Cefditoren은경구용 3세대 cephalosporin 제제로경구흡수율을높이기위해 pivoxil ester의형태로제조되며이것이장내 esterase 에의해가수분해되어항균력을가지는 cefditoren 이된다. 그람양성균및음성균에대하여광범위한항균력을나타내며특히원외호흡기감 - 7 -

Cefditoren 의각균종에대한 CLSI의감수성기준은아직확립되지않은상태로 Fuchs 등은 penicillin 내성 S. pneumoniae 감염증에서의 cefditoren의치료효과에대한자료가부족한것을근거로하여 S. pneumoniae에대한 cefditoren 의감수성판단기준을 MIC.25 μg/ml 이하,.5~1 μg/ml를중간내성, MIC 2 μg/ml 이상을내성의기준으로제시하면서 penicillin 내성 S. pneumoniae 감염증에서 cefditoren 이효과가있다는임상자료가보완되면감수성기준은 MIC 1 μg/ml 이하로상향조정할수있을것이라고하였으며 H. influenzae 에대해서는 MIC.5 μg/ml 혹은 1 μg/ml 이하를기준으로제시하였다 9). Jones 등은 cefditoren의감수성기준으로 MIC 1 μg/ml 혹은.5 μg/ml 이하를제시하였고일본화학요법협회에서도폐렴에대해 MIC 1 μg/ml 이하를감수성기준으로적용하고있다 5, 7-9). Cefditoren 의시험관내항균력에대해국내에서는이등이주요임상분리세균에대한 cefditoren의 MIC를측정하여항균제감수성양상을보고한바있으며그자료에의하면 cefditoren 은 S. pneumoniae에대하여 penicillin 감수성균주와 penicillin 내성균주에서 MIC 범위가각각.15~1 μg/ml,.15~2 μg/ml, MIC 9 은모두 1 μg/ml로감수성기준을 MIC 1 μg/ml으로하였을때 penicillin 에내성인 42균주중 MIC가 2 μg/ml인한균 주를제외하고는모두 penicillin 에감수성을보였다. 또한 H. influenzae 의 MIC 범위는.15~.3 μg/ml, MIC 9 은.3 μg/ml로역시감수성기준을 MIC 1 μ g/ml 이하로하였을때모든균주가 cefditoren 에감수성을보인다고하였다 4). 본연구는 S. pneumoniae와 H. influenzae 를대상으로 penicillin 혹은 ampicillin에대한감수성여부에따른 cefditoren 의항균력을평가하고이를다른경구항균제들의항균력과비교하고자한것으로 S. pneumoniae와 H. influenzae 에대한 cefditoren의 MIC 중최고농도는각각 1 μg/ml,.3 μg/ml이었고, 이전의보고들에서처럼 penicillin 감수성 S. pneumoniae에대한 cefditoren MIC가 penicillin 내성균주에비해낮은경향을보였다 1, 12, 13). 감수성기준을 MIC.25 μg/ml 이하로했을때는전체균주의 61.7% (74/12) 가 MIC.5 μg/ml 이하로했을때는 8.8% (97/12) 가 cefditoren에감수성이었고, MIC 1 μg/ml 이하를기준으로했을때는시험대상이된모든균주가 penicillin 내성여부에상관없이 cefditoren 에감수성이었다 8, 1). 이는 cephalosporin 계열을포함한다른경구용항균제들과비교할때가장우수하였고, levofloxacin과유사한결과였다. H. influenzae 에서 ampicillin 에대한내성은 β-lactamase 생성과 penicillin 결합단백질 (penicillin-binding protein; PBP) 의변화에기인하며일반적으로 PBP 변화로인한내성의수준은 β-lactamase 생성에의한것보다낮은것으로알려져있다. 본연구에서 ampicillin 내성의기전에대해따로분석하지는않았으나 ampicillin 내성 H. influenzae 의경우 ampicillin 감수성균주와비교했을때

Comparative activity of cefditoren and other orally administered antibiotics tested against 8 H. influenzae isolates Antimicrobial agents Cefditoren Cefaclor Cefixime Cefpodoxime Clarithromycin Levofloxacin H. influenzae (n= 8) Ampicillin-susceptible H. influenzae (n= 4) Ampicillin-resistant H. influenzae (n= 4) MIC(μg/mL) MIC(μg/mL) MIC(μg/mL) MIC 5/MIC 9 Range MIC 5/MIC 9 Range MIC 5/MIC 9 Range.15/.3 < /.15 < /.3 < / / / / / / / / / / / / /.15 < /.15 < /.3 < 큰차이없이 cefditoren의 MIC가낮게유지되어 ampicillin에대한내성여부가 H. influenzae 에대한 cefditoren의항균력에영향을미치지않음을알수있었다. 이러한결과는우리나라에서분리되는 H. influenzae 의 ampicillin 내성률이 55~68% 이며 β-lactamase 생성률은 39~58% 정도로 β-lactamase 생성이 ampicillin 내성의주된기전이라는점을고려할때이전의연구에서알려진바와같이 cefditoren이 β-lactamase에안정하여분해되지않기때문일가능성을시사하는것으로생각된다 2, 14-16). 아직까지 cefditoren 의 CLSI 감수성기준이확립되지않아다른항균제와의감수성비율이나항균력비교가제한적일수밖에없는것이본연구가가지는한계점이다. 향후감수성기준이확립될경우재평가가필요하겠고, penicillin 내성 S. pneumoniae 감염증의실제임상에서의치료효과에대해서는추가적인연구가필요하다. 결론적으로 cefditoren 은감수성기준을 MIC 1 μ g/ml 이하로하였을때 penicillin 이나 ampicillin 에대한내성여부에상관없이원내호흡기감염증의흔한원인균인 S. pneumoniae와 H. influenzae 모두에서우수한시험관내항균력을보여이들에의한감염증을치료하기위한경구용항균제로유용할것으로생각된다. 요약목적 : 본연구에서는그람양성및음성세균에대해광범위한항균력을나타내는것으로알려진 cefditoren 의임상적유용성을평가하기위해국내에서분리된 S. pneumoniae와 H. influenzae 임상균주를대상으로 cefditoren의시험관내항균력을흔히임상에서사용되 고있는다른경구용항균제들과비교하고자하였다. 방법 : 2년 1월부터 24년 3월까지서울아산병원에서분리된 S. pneumoniae 12균주와 H. influenzae 8 균주를대상으로미국임상검사표준화협회의방법에따라 cefditoren 과다른경구용항균제에대한최소억제농도를측정하였다. 결과 : 모든 S. pneumoniae 균주는 cefditoren 농도 1 μg/ml 이하에서억제되어 (MIC 5/MIC 9.25/1 μ g/ml; range.15~1 μg/ml) cefditoren의감수성기준을 1 μg/ml로하였을때 penicillin 에대한감수성여부에상관없이모든균주가 cefditoren 에감수성을보였다.
