1 대한임상병리사회지 : 제 20 권제 1 호 1988 혈액응고검사에있어서혈소판이미치는영향에대한고찰 고대의료원혜화병원임상병리과 이부형 전현주 나종석 1. 서론 ( 序論 ) 지혈기전은한세기이상동안끊임없는연구의주제가되어왔다. 이복합적인체계의필수적인요소들은관련분야의많은연구자들에의해끊임없이밝혀지고또연구되어오고있다. 이러한연구과정속에서, 혈소판은지혈체계에서가장먼저작용하는요소들중의하나이고지혈기전에중요한역할을하는것으로알려져 역할이분명히알려진것은아니다. 특별히임상검사실에서보통으로시행되는혈액웅고검사들에있어서의혈소판의영향은아직도불분명한상태이다. 혈소판은내인성 prothrombinase 또는혈장트롬보프라스틴의형성에필요한모든인자들을포함하고있으며전자현미경구조적으로, 혈소판의기능과구조의관계를한정하기위한시도로혈소판내에 4 개의기능적이고해부학적인부분으로규정하고있다. 이것들은소위 있다. 또한혈소판생리학에대한지식이증가함에따 Peripheralzone, Solgelzone, Orgamell zone, 그리고 라서그구조와기능사이에밀접한관계가있다는것 이분명해지고있으나지혈기전에있어서의혈소판의 Membradne system 이다. 2 white는혈소판에있어서외부지역바로안에있는부분을하나의전형적인단위막으로서술하고있는데여기에혈장내의응고인자들과혈소판의상호작용에필수적이고중요한역할을하는인지질이들어있다. 그러므로혈장내의인지질의양은혈액내의혈소판의숫자와직접적으로배례한다. 실제로혈소판내의인지질은 Fig 2 에서보듯이혈액응고과정의내인계와외인계경로에모두작용한다. BI d coagulation pathway. 이런관점에서, 우리는혈소판의숫자와관련된검 사들을시행하고 와 PPP 를 사용한결과사이의 차이점을조사하여혈액웅고검사에있어서혈소판이 미치는영향에관하여고찰해보았고, 그검사들은 다음과같다. 1) Actívated Partíal Thromboplastín Tíme Fest 2) Activated Recalcífícatím Tíme Test 3) Recalcífícation Tíme Test ll. 방법과재료 (Method and Materials) (1) METHOD CD BLOOD COLLECTION(IN SODIUM. CENTRIFUGE AT RECENTRIFUGE A T 3000rpm x 30 mínute=ppp CULATE MEAN MEASURE THE AMOUNT OF PHOSPHOLIPID WITHIN SERUM.. PPP (2) MATERIALS CD REAGENT:ACTINR(FROM DADE COMPANY) 0.5 % KAOLIN SUSPENSION(AS A ACTIVATOR) M CaCl JAPAN) 정상인 50 명으로부터 citrate 용액에 혈액을채취하여 3.8% Sod, 넣고잘섞은후에 1000 rpm 에 5 분동안 원심분리하여 를얻는다. 그리고남은검체를 3000 rpm 에서 30 분간재원심분리하여 PPP 를얻어낸다. 먼 저위에서술한세가지검사를이들 와 PPP 를 가지고학각세번씩검사하여얻어진결과를 3 으로 나눠평균치를구한다. 그런다음동일한사람의혈청 속에와, PPP 속에있는 phospholípíd 의 양을측정 한다. 시약은 Dade 사제품인 ThrQmbopastín. C 와 Actín R, 0.025M CaCI 을 Activater 로, 0.5 %Kaolín Sus 야 nsíon 2 을사용했으며분석기는 CoagStat DO EACH TEST THREE TIMES AND CAL 26

3 ill. 본론위와같은방법으로먼저 Activated partial Thromboplastin Time을측정하여평균치를구하였더니다음과같았다. 표 1 에서보면 의평균시간은 25.8초이고 PPP 의평균시간은 25.1초였다. 두결과의평균차이시간은 0.7 초호나타났다. 그러므로 PPP 의시간이 의시간보다아주약간짧은것으로보이나두결과사이의줍요한차이점은없었다. 두번째로 Activated Recalci pication Time 을측정해보았다. The Result of Activated Partial Thromboplastin Time Test 표 2 에서보면 로검사한평균시간은 45.9 초이 고 PPP 로검사한평균시간은 50.4 초를나타내며둘 사이의평균차이시간은 4.5 초를보여준다. 그러므로 로 검사한시간이 PPP 로한시간보다약간더 Mcan Value : Platelet Rich Plasma PPP: Platclet Poor Plasma PPP Difference 짧깨나타났으며둘사이의차이는그다지중요한점블보이지않았다. 세번째로는갇은방볍으로 Recalcification Time 검사틀시행해보았다. 표 3 에서보면 의평균시간은 122.7초이고 PPP 의평균시간은 157.2초를나타내며두결과의차이시 Table 3. Activated patial Thronboplastin Time 은중요한변화가없어서이표 상에는생략했다. 또한혈청속에 있는 phospholipid 의 양이가장많은것을볼수있는데아마도혈액응고과 정이끝나면서혈소판의막속에있던 phospholipid 가 혈병퇴축이일어나면서유리되어나왔기때문이아닌 가싶다. 어떻튼 속에있는 phospholipid 의양은 156 mg / dl 이고 PPP 의 phospholipid 의양은 151 mg / 며이어서두결과의차이는 5mg/dl 이다. 또한 Activated Recalcification Time 의 결과차이시간은 4.5 초이고 Recalcification Time 의결과의차이는 35.5 초이다. 이상 의결과들을좀더알기쉽게도표로나타내보았다. ιιι{ 디떠ιν {}{ ιqrν φ}}경0 m second the differcnce of and PPP ACTIV A TED RECALCIFICA TION TIME Figure 3. The Relationship Between The Amount of Phospholipid and The Results of Recalcification Time Test 이상의결과를종합해볼때혈소판의수즉 phospholipid 의양은 Activated Recalcification Time 과 Recalcification Time Test 에는영향을미친다는것이 나타났고반면 에 Activated partial thrombo plastin Time Test 에는 아무과련이 없는것으로나타났다. 물론혈소판속에 있는 phospholipid 외에도다른여러가지요소들이이 러한혈액응고검사들에영향을미칠수있을것으로 사료되며이 된다. N. 결 것들에관하여는좀더많은연구가요청 이와같은결과를토대로임상에서위와같은 혈액응고검사를시행할때정상혈소판수를가진환 자에있어서는별주의를요하지않지만혈소판감소증 론 을나타내는환자에있어서는검사결과를보정해 거나또는검사결과에대한주의를첨부해서 보고할필요가있을것으로사료된다. 주 임상에 28

5 The Study Concerning The Effect Of Platelet On Some Blood Coagulation Test Boo Hyoung Lee Hyun Joo )eon Chong Suk Ra Department of Clinical Pathology, Hae Wha Hospital, Korea University Medical Center ABSTRACT The hemostatic mechanism has been the subject of continual investigation for more than a century. The essential elements of this complex system have been defined and studied constantly by many investigators of the related area. In the course of this investigation, it has been known that platelet is one of the earliest components system to be discovered and played a great rolp of hemostatic mechanism. As knowledge of platelet physiology has grown, the close relationship between structure and function has become evident but the knowledge of its role in the hemostatic mechanism has not been achieved completely. Especially, the study concerning the effect by platelet is not totally comprehensive in some blood coagulation test which is routinely is performed by our medical laboratory. Platelet contains all the coagulation factors necessary for the formation of intrinsic prothrombinase or plasma thromboplastin. And the number of platelet in the plasma have marked effect on some blo:xi coagulation test. At this point of view, we performed the test associated with the numbers of platelet and investigated the difference between the result of PLATELET RICH PLASMA and that PLATELET POOR PLASMA. The tests are followings; 1) Activated partial thromboplastin time with and PPP 2) Activated recalcification time with and PPP 3) Recalcification time with and PPP. Conclusively, in cases that platelets are normal, the clotting time of Platelet Rich Plasma is shorter than that of Platelet Poor Plasma but in case of abnormal platelet counts the results are fluctuating. 1) Frances.
