Materials and Methods: Pre-operative, post-operative, technical, radiological factors of patients who diagnosed osteoporotic compression fracture and treated with vertebroplasty or kypholasty during from January 2008 to December 2010, were compared by outcomes that acute and chronic back pain, refracture rate and maintenance of the reduction of body height. Results: The period of acute pain after the operation was less than 3 days in groups of trauma( p=0.0262) and hospital treatment(p=0.0113) before the operation as pre-operative factors. When it comes to technical factors, the cases of vertebroplasty(p<0.0001), injection of cement(p=0.0053), the case without leakage of cement(p=0.007) and the case being under anesthesia (p<0.005)achieved statistical significance. In the groups who got conservative treatment less than 1 week (p=0.0119) and local anesthetics(p=0.0246), refractures occurred many times. The groups of non-trauma(p=0.0105), having had more than 30% compression(p=0.0244) and the group that prescribed bisphosphonate(p=0.0286) statistical significantly maintained restoration. Conclusion: Conservative treatment in the hospital for more than 1 week before the operation, operation being under anesthesia and intake of bisphosphonate after the operation put positive effects on the outcome of vertebroplasty and kyphoplasty. Key Words: Osteoporotic compression fracture, Vertebroplasty, Kyphoplasty, Outcome, Factor 서론 척추체의 골다공증성 압박골절은 골감소증에 따른 가 장 흔한 합병증으로 1,2) 65세 이상의 여성 인구중 1/3이 한 번 이상 압박골절을 겪게 되고, 6) 70대의 인구 중 20%가, 75세 이상의 인구 중 90%가 압박 골절의 발생을 겪는다 고 알려져 있다. 7) 미국의 통계를 따르면 이로 인해 연간 4400만명의 환자가 발생하고 그로 인해 약 4억 4000만 달 러의 비용이 드는 것으로 알려져 있으며 3,4) 요통과 보행기 능의 저하 및 침상안정에 따른 폐기능 저하, 및 자립의존 도의 증가를 불러일으켜 노인인구에서의 삶의 질을 저하 시킨다. 4,5) 이의 치료를 위하여 침상 안정과 보조기 착용, 그리고 진통제를 이용한 2~3개월 간의 보존적 치료를 통 하여 전반적인 증상의 감소 및 골절의 치유를 얻을 수 있 다고 알려져 왔으나 8) 노인 환자군에서 이러한 보존적인 치료가 실패할 가능성이 보고되어 왔다. 그 이유로는 압 박골절의 진행에 따른 골절 치유의 실패나 압박골절 이 후의 척추변형에 따른 역동학적 변화, 또는 골절된 척추 체의 불유합 등이 거론되어 왔다. 9) 이를 보완하기 위해 Corresponding author: Seok Woo Kim, M.D. Spine Center, Hallym University Sacred Heart Hospital,College of Medicine, Hallym University, 896 Pyeongchon-dong, Dongan-gu, Anyang-si, Gyeonggido , South Korea TEL: , FAX : seokwookim@yahoo.com Copyright 2011 Korean Society for the Advancement of Spine Surgery 23

One segment Multiple segment Patients no Sex M F Treated segment Thoracic Thoraco-lmbar junction 135 Lumbar Anesthesia General anesthesia IV anesthesia 44 8 Local anesthesia Method Vertebroplasty Kyphoplasty Mixed 13 척추체 성형술과 후만 성형술이 시행되었고 단기 추시상 에서 만족스러운 결과를 보였으며 10~17) 최근의 무작위 연 구에 따르면 보존적인 치료보다 더 나은 치료효과를 가 지는 것으로 알려져 있다. 18~20) 하지만 현재까지 시술 후의 합병증 발생의 감소를 위한 연구에 비하여 술 후 결과에 영향을 미치는 인자에 대한 연구는 부족한 실정이다. 따라서 본 연구는 술 전 환자 인 자나 수술 술기, 수술 후 인자 또는 방사선학적인 인자에 따른 결과를 비교하여 술 후 결과에 영향을 미치는 인자 를 알아보고 나아가 결과의 개선을 위해 조절 가능한 인 자를 찾고자 하였다. 연구 대상 및 방법 2008년 1월 부터 2010년 12월 까지 본원에서 골다공증 성 압박골절을 진단받고 척추체성형술 또는 후만성형술 을 시행한 356명의 환자를 대상으로 하였다. 이중 추시 기간 중 사망하거나 추시 기간이 6개월 미만인 환자, 또 는 기구 고정술을 함께 시행한 118명의 환자를 제외한 238명의 환자를 조사하였다. 시술은 각각 6명의 정형외 과 및 신경외과 전문의에 의해 시행되었으며 이중 한 분 절을 시술 받은 환자는 191명, 두 분절 이상을 시술 받은 환자는 47명 이었다. 이 두 군을 각각 술 전, 술 후, 방사 선학적 및 기술적인 인자에 따른 임상적, 방사선학적 추 시 결과를 비교하였다(Table 1). 술 전 임상적인 인자로는 골절발생시의 원인, 1주일 이 상의 보존적 치료 여부, 골절 부위 및 보존적 치료시의 입 원치료유무에 따라 술 후 결과를 비교 하였으며 술 후 인 자로는 bisphosponate의 복용과 보조기 착용기간과 종류 를 비교하였고, 방사선학적 인자로 일반 방사선 사진에서 의 압박률 및 Cobb s angle과 자기공명영상 소견상의 척 추체 및 척추경 골절 범위에 따라 결과를 비교하였다. 또 Fig. 1. Within 3 days Duration of pain More than 3 days Pre-operative factors Mode Traumatic miscellaneous P-value Conservative treatment period More than 1 week Within 1 week Conservative treatment method Admission miscellaneous Post-operative factors Type of brace Hard miscellaneous 2 2 Brace applying period More than 3mo Within 3mo Radiological factors Cobb s angle -8.8 (±13.8) -8.9 (±14.2) Pedicle involvement Involved Not involved Compression rate Technical factors Treatement method Vertebroplasty 32 7 < Kyphoplsaty Shape of injected cement Solid Interdigitate Leakage Occurred Anesthesia Not occurred General / IV anesthesia Local anesthesia < Amount of injected cement 13.2± ± 한 기술적인 인자로 척추체 성형술과 후만 성형술의 결 과를 비교하였고, 시멘트 주입양(Fig. 1)과 누출여부 (Fig. 2), 시멘트 주입형태에 (Fig. 3) 따라 결과를 비교하였다. 환자의 임상적 결과는 VAS 및 술 후 통증 호소 기간, 재골절 여부, 시술 후 만성적인 통증의 발생여부로 비교 하였으며 방사선학적으로 술 전 및 최종 추시 상의 척추 체 후면의 높이에 대한 척추체 전면의 높이의 비율을 구 하여 척추체 높이의 복원율을 비교하였다(Fig. 4). 통계분석은 SPSS를 사용하였으며 결과의 비교는 paired T-test 를 이용하여 비교 분석 하였다. 결과 1. 단분절에서 척추체 성형술 또는 후만 성형술을 시행 한 환자군의 결과 한 분절의 시술을 시행한 환자군에서 술 전 및 술 후 의 VAS는 술 전 7.52 에서 술 후 1.18로 변화하였으며 술 후 급성통증의 지속 기간은 3일 이내의 통증을 호소 한 환자가 75명, 4일 이상의 통증을 호소한 환자가 116명 으로 술 전 요소중 외상성으로 골절이 발생한 환자군과 (p=0.0262) 시술전 보존적 치료를 위해 입원치료를 선행 하였던 환자군 (p=0.0113)에서 유의하게 술 후 통증의 지 속기간이 3일 이내로 짧았고, 기술적 인자 중 척추체 성 형술을 시행한 환자와 (p<0.0001) 시멘트의 주입양상이 상호교대감입 (interdigitation) 형태인 경우 (p=0.0053), 술 중 시멘트의 누출이 없었던 경우 (p=0.007), 정맥마 취를 통해 환자를 안정시키고 시술한 경우에서 통계 학적으로 유의하게 지속기간이 짧은 것으로 나타났다 (p<0.0001). 하지만 골시멘트의 주입양과는 유의한 차이 가 없었다. 또한 방사선학적인 인자 중 척추경의 골절여 부나 술 전 압박률과는 통계학적으로 유의한 차이를 찾 을 수 없었다(Table 2). 술 후 만성적인 통증을 호소한 환자는 61명, 통증을 호 소하지 않은 환자는 130명으로, 환자 간에서 술 전 모든 요소들간에서의 유의한 차이를 관찰 할 수는 없었으며 재골절 발생의 경우 17.8%인 34례에서 재골절의 발생이 25

4 있었으며 그 중 절반인 17례가 시술을 시행한 인접분절 에서의 골절 양상을 보였다. 술 전 인자중 보존적 치료를 1주일 미만으로 시행한 군에서 1주일 이상 시행한 군보 다 재골절 발생이 높았고 (p=0.0119) 기술적 요소 중 국 소마취를 통한 시술을 시행한 환자군에서 재골절 발생이 통계학적으로 유의하게 높았다(p=0.0246). 방사선학적인 결과로 척추체 높이의 복원 및 유지를 보 면 시술전 비외상성으로 증상 발생한 환자군 (p=0.0105) 및 압박률이 30% 이상이었던 환자군 (p=0.0244), 시 술 후 압박골절의 복원이 있었던 환자군 (p=0.0048) 및 bisphosponate를 처방받아 복용하였던 환자군에서 (p=0.0286) 통계학적으로 유의한 술 후 복원률의 유지를 Fig. 2. (A) Pre-operative ratio of anterior vertebral height to posterior vertebral height, (B) Measuring repeated with last follow up X-ray. α/β= 0.62, α` /β= 0.9, (ὰ/β`) (α/β) / (α/β) = 46% of reduction rate. 본 연구의 목적은 술 후 결과에 영향을 미치는 찾기 위 한 연구로 결과에 따르면 술 후 급성 통증이 3일 이내의 빠른 회복을 보인 환자군으로는 술 전 인자중 외상성으 로 발생한 골절환자군과 입원을 하여 보존적 치료를 시 행한 환자군, 기술적 인자중 척추체 성형술을 시행한 환 자군 및 시술 중 마취를 시행하였던 환자군과 시멘트가 누출 없이 척추체 안에 골고루 분포된 환자군에서 좋은 결과를 보였으며 재골절의 발생이 적었던 군은 술 전 인 자중 1주일 이상의 보존적 치료를 시행 후 시술을 받은 환자군과 기술적 인자중 시술시에 마취를 통해 환자를 안정시킨 뒤 시술을 시행한 군에서 좋은 결과를 보였다. 또한 마지막으로 술 후 인자중 시술 시행후의 압박골절 의 복원의 유지를 위해 bisphosponate의 처방이 동반되었 던 환자 군에서 복원율의 유지를 보였다. 이는 외상성으로 인해 골절이 발생한 환자와 비교하여 뚜렷한 외상 없이 골절이 발생한 환자에서 노화로 인한 시상면 정렬의 변화나 심각한 골다공증의 합병증으로 골 절이 발생하여 골절 부위의 안정성의 유지가 어려워 통 증이 더욱 길게 지속된 것으로 보이며 시술 전 입원치료 를 하여 침상 안정 및 약물 투여 등의 적극적인 보존 치료 가 가능하였던 환자군에서 통증의 회복이 빨랐던 것으로 사료된다. 또한 마취를 통해 환자를 안정시킨 뒤 시술을 시행하였던 군에서 보인 술 후 급성 통증의 유의한 감소 는 시술중의 환자의 움직임을 최소화 하고 충분한 시간 을 가지고 시술 하여 정확한 needling 을 통해 척추경의 손상이 주는 등의 복합적인 요인으로 인한 결과로 보이 며 누출은 만성적인 통증과는 연관이 없으나 시술 직후 의 급성통증에는 관련이 있는 것으로 보이며 시멘트 주 입양은 결과와는 직접적인 관련이 없는 것으로 사료된다. 본 연구의 제한점으로는 척추체 성형술 및 후궁성형술 에서의 복원률의 유지를 비교하기에 평균 8개월의 비교 적 짧은 추시기간을 가졌다는 점과 재 골절과 관련 있는 인자의 확인이 안된 점을 들 수 있으며 후향적인 연구로 환자의 임상결과를 비교한 연구로 이의 보완을 위해 더 긴 추시기간을 가지는 전향적인 연구가 필요할 것으로 사료된다. 결론 본 연구의 결과는 골다공증성 압박골절 환자에서 척추 체 성형술 또는 후만 성형술을 시행하는 경우 술 전 1주 일 이상의 입원 치료를 선행하고 시술 중에는 마취 하에 시술을 하며 시멘트 누출에 주의하여 시술 하고 시술 뒤 에는 bisphosphonate의 처방을 할 것을 권장하는 바이다.
