It is caused by bunyavirus, called SFTS virus (SFTSV). The main vector of SFTS is Haemaphysalis longicornis tick. We investigated the distribution and detection of SFTSV in ticks collected from the environment using the dragging method and dry ice fogging method from May to November 2014 in Jeollanam-do, Korea. Sampling was taken from the province Suncheon, Gokseong, Boseong, Goheung where patients have occurred in 2013 and Gurye as control. Among the total 3,048 ticks collected, 3,030 ticks were H. longicornis (99.4%) and 18 were Amblyomma testudinarium. H. longicornis was collected 1,330 ticks in Gokseong, 1,188 ticks in Boseong, 240 ticks in Suncheon, 150 ticks in Goheung and 140 ticks in Gurye. Developmental stages by month of H. longicornis were revealed that nymph (92%) was collected from May to June, adult (30%) and nymph (70%) in July, and 93% of larvae from September to October. These results showed the different dominant stage of ticks according to seasons. However, no SFTSV-specific gene was detected in 3,030 ticks of H. longicornis. Key Words: Severe fever with thrombocytopenia syndrome, Haemaphysalis longicornis, Jellanamdo INTRODUCTION 우리나라산림과들에는매우다양한참진드기, 털진드기, 좀진드기등이서식하고있다. 참진드기가매개하는병원체중에는 Severe fever thrombocytopenia syndrome virus (SFTSV) 같은바이러스와 Anaplasma, Ehrlichia, Borrelia 등이있고, 참진드기종에따라서는진드기마비증을일으키는것도있다. 중증열성혈소판감소증후군 (Severe fever thrombocytopenia syndrome; SFTS) 은주로산이나들판의야외에서서식하는 SFTSV 매개체인작은소피참진드기 (Haemaphysalis longicornis) 에물려감염되는것으로알려 졌다. 주요증상으로는 38 이상의발열과소화기증상인구토, 설사, 식욕부진및림프절비대, 두통, 근육통, 자반증, 다발성장기부전등이나타나며, 잠복기는약 1주에서 2주이다. 치료제는지금까지없고, 치사율은 12% 에서 30% 에달하는것으로보고된바있다 (1). SFTS 환자발생은중국중부및동북부지역중심으로 2011 년 ~12년사이에 2,047명의환자중 129명이사망하여 6.3% 의사망률을보였고 (2), 일본은 2013년첫감염자사망확인후 43명이발병, 18명이사망하여 41.9% 의사망률을나타냈다 (3). 우리나라의첫환자발생은 2012년 8월에고열과심각한혈소판감소증을보인의사환자가 4일후다발성장기부전으로진행하여사망하는사례가 Received: March 9, 2016/ Revised: April 25, 2016/ Accepted: May 27, 2016 * Corresponding author: Hyeon Je Song. 2 94 BJ Song, et al. 보고된후 2013년에 36명의환자중 17명이사망하여 48.6% 의사망률을보였고, 그중전남지역에서는환자 5명이발생되어 1명이사망하는사례가있었다 (4). 우리나라에서는 2013년부터제4군법정감염병으로지정하여관리하고있다. SFTSV를매개하는진드기로는작은소피참진드기, 뭉뚝참진드기 (Amblyomma testudinarium), 일본참진드기 (Ixodes nipponensis) 가있으며, 진드기는주로 4월 ~9월에채집되고, 작은소피참진드기가 92% 에서 96% 를차지하는것으로보고되었다 (5). 작은소피참진드기는자연계에존재하는 SFTSV의주된매개체이며, 한국을포함하여중국, 몽골, 일본등동남아시아전역에오랫동안서식해온참진드기로알려져있다 (6). 본연구에서는 2014년 5월부터 11월까지질병관리본부의 2013년감염병감시연보 (4) 에따라전라남도에서 2013년에 SFTS 환자가발생한지역을중심으로진드기를채집하였다. 채집한진드기종을분류동정하여시기별및장소별분포특성을조사하고 SFTSV 보유여부를 Reverse transcription polymerase chain reaction (RT-PCR) 로확인하여 SFTS 질병발생에대비한예방기초자료를제공하고자하였다. MATERIALS AND METHODS 참진드기채집본연구에사용한중증열성혈소판감소증후군매개진드기는 2014년 5월부터 11월까지채집하였다. 2013년전라남도의 SFTS 환자발생대상지역인순천 (34 53'36.6"N '01.7"E), 곡성 (35 16'24.5"N '40.5"E), 고흥 (34 32'57.0"N '40.4"E), 보성 (34 50'15.0"N '20.2"E) 지역을선정하였으며, 환자발생은없었으나산악지대인구례 (35 12'08.2"N '56.8"E) 지역을대조군으로선정하였다. 진드기채집방법으로는융으로조사지역의수풀을쓸어내는방법 (Flag method) 과 Dry ice fogging을이용하여한조사지역에서 60분동안채집하였다. 채집한진드기는습도를유지하기위하여특수제작한 Conical tube (50 ml) 를이용하여물에적신솜을넣고실험에사용할때까지냉장보관 (4 ) 하였다 (5, 6). 참진드기의형태학적분류와생활사구분참진드기의형태학적분류는실체현미경 (Zeiss, Germany) 을이용하여 Yamagutsi et al. (7) 이제시한방법에따라다음과같은특징들을참고하여분류동정하였다. 즉, 참진드기는대부분색반을가지고있지않으며, 눈은없으나화채는가지고있다. 촉지는짧으며고깔형태인데촉지의 2절은뚜렷하게양쪽으로돌출되어있다. 특히작은소피참진드기의특징으로는수컷의촉지제 3절 (palpal segment III) 은등쪽으로거슬러뻗은가시돌기 (dorsal retrograde spur) 를가지고있으며, 촉지제 2절 (palpal segment II) 은위와같은돌기가없고복후방의가장자리가약간볼록하다. 촉지제 3절 (palpal segment III) 은중앙에등쪽으로거슬러뻗은가시돌기 (dorsal retrograde spur) 를가지고있다. 암컷의경우촉지제 3절 (palpal segment III) 은수컷과마찬가지로등쪽으로거슬러뻗은가시돌기 (dorsal retrograde spur) 를가지고있고촉지제 2절 (palpal segment II) 은외측양옆으로어느정도돌출되어있다. 촉지제 3절은중간정도의복측가시돌기 (ventral spur) 를가지고있다. 흡혈하기전의암컷에비해서흡혈후의암컷의크기는 100배이상으로커지는반면수컷은 3~4배밖에커지지않는특징이있다는 Soulsby (8) 의방법에따라성장발육별분류를하였다. 참진드기유제액제조및 RNA 추출유제액은 2.8 mm stainless-steel bead가들어있는분쇄용튜브에 600 μl의 buffer를넣고, 참진드기의유충은최대 50개체, 약충은최대 30개체, 성충의경우최대 5개체씩성숙단계별, 종별로구분한후 pooling하였다. Buffer 의조성은멸균된 phosphate-buffered saline (ph 7.0) + 10% fetal bovine serum (GIBCO BRL, Grand Island, NY, USA), penicillin (500 IU/ml, GIBCO BRL) 과 streptomycin (500 μg/ml, GIBCO BRL) 를이용하였고, 분쇄용 tube는 Hard Tissue Griding MK28R (Bertin Technology, Bretonneux, France) 를사용하였다. 분쇄기는 Precellys 24 tissue homogenizer (Bertin Technologies, Bretonneux, France) 를이용하여 6,000 rpm으로 25초씩 2회분쇄하고, 분쇄된진드기유제액은 10,000 rpm에서 5분간원심분리하여시험액을제조하였다. RNA 추출방법은 Gene-spin Viral DNA/RNA Extraction kit (intron Biotechnology, Seongnam, South Korea) 를이용하여제조사의방법으로분리하였다. 즉, SFTSV의특정유전자인 Medium Segment 유전자를확인하기위하여전처리한시료 200 μl를 1.5 ml tube에옮겨 15초간 vortex하고상온에서 10분간방치후 RNA를분리하였다.

Developmental stages of Haemaphysalis longicornis collected from the five regions of Jellanam-do, 2014 Regions Ticks number Larvae Nymph Adult Suncheon Gokseong 1,320 1, Gurye Goheung Boseong 1, Total 3,030 1,850 1, PCR 반응액제조및반응조건 M절편유전자검출방법으로 RT-PCR은 primer set (5'- GATGAGATGGTCCATGCTGATTCTAA-3', 5'-CTCATGGG- GTGGAATGTCCTCAC-3') 을이용하여 DiaStar 2X OneStep RT-PCR Pre-Mix Kit (SolGent, Daejeon, South Korea) 의방법에따라수행하였다. 즉 2X OneStep RT-PCR Pre-Mix 15 μl, Primer Forward (10 pmol/μl) 1 μl, Primer Reverse (10 pmol/μl) 1 μl, Template RNA 5 μl, DW 8 μl, Final volume 30 μl로 PCR 반응액을제조하였다. PCR 조건은 95 (15 min) 에서 Predenaturation 하고, Amplification을 35 cycle (95 /20 sec, 58 / 40 sec, 72 /30 sec) 한후, 72 (5 min) 에서 Final extension 을하였다. 증폭산물전기영동증폭된 PCR 산물의확인은 2% agarose 겔 (1X TAE buffer) 에 PCR 반응액 3 μl씩 6X loading buffer와섞어 100 V로 40분간전기영동한후밴드의유무를 gel image analyzer인 MiniBis-Pro (Shimadzu, Kyoto, Japan) 로확인하였다. 산물의크기는 1 kb DNA Ladder (Invitrogen, Carlsbad CA, USA) 5 μl (500 ng) 를사용하였고, 음성대조는반응물에 template DNA를넣지않은것을사용하였다. RESULTS 참진드기의지리적분포 2014년 5월부터 11월까지전라남도에서 2013년에 SFTS 환자가발생한 4개지역과환자발생이없었던 1개지역에서총 3,048마리의진드기를채집하였다. 작은소피참진드기가 3,030마리로대부분이었고 (99.4%), 뭉뚝참진드기가 18마리채집되었다. 채집지역에따라곡성에서 1,330마리, 보성지역에서 1,188마리채집되었고, 순천과고흥에서각각 240마리, 150마리채집되었다. 환자발생이없었던구례가 140마리채집되었다 (Table 1). 채집된 3,030마리의작은소피참진드기는성충 160마리, 약충 1,020마리, 유충 1,850마리로동정되었다. 지역별로는곡성에서는 1,320마리중성충 40마리, 약충 280마리, 특히유충이 1,000마리채집되었고, 보성에서채집한 1,180마리중성충 30마리, 약충 450마리, 유충 700마리가채집되었다. 순천에서채집한 240마리에서는성충 40마리, 약충 150마리유충 50마리였으며, 고흥에서채집한 150마리중성충 30마리, 약충 20마리유충 100마리가채집되었다. 구례에서채집한 140마리를분류한결과는성충 20마리, 약충 120마리였으며유충은채집되지않았다 (Table 2). 월별진드기분포채집시기별작은소피참진드기의채집현황과주변환경온도를비교한결과, 채집한평균기온은 5월에는 26~ 27, 6월에는 30~33, 7월에는 32~34, 8월에는잦은우천으로인하여채집을못하였으며, 9월에는 26~33, 10월에는 30~31, 11월에는 24 를나타냈다 (Table 3). 계절별과발육단계별로채집유형을보면, 유충은봄부터여름까지나타나지않다가 9월에나타나기시작하여 10월까지짧은기간동안많은개체수 (1,800마리) 가집중적으로채집되었다. 약충은 5월부터 9월까지채집 (998마리 ) 되었으며, 특히 5월에많은개체수 (650마리) 를나타냈다. 성충도 5월부터 9월까지꾸준히채집되었다 (Table 3).

The results of amplification of SFTSV RNAs by RT- PCR from the pooling ticks collected from Jellanam-do, Lane 1 show molecular weight marker, lane 2~15: PCR products from the pooling ticks, lane 16: positive control (410 bp), lane 17: negative control. 채집된작은소피참진드기의 SFTSV 검사 적은수가채집된뭉뚝참진드기를제외하고채집된작은소피참진드기의 RNA를추출하여 SFTSV를확인하기위하여진드기를 103회 pooling하여 2X OneStep RT-PCR 방법으로 SFTSV 유전자를탐지한결과양성은확인되지않았다 (Fig. 1). 우리나라에는 8개속 32개종의참진드기가보고되었다 (9). 경기도의동물과수풀에서 5종의진드기가동정되었고그중작은소피참진드기가 91.8% 였고, 강원도에서총 516마리의진드기중 95.7% 가작은소피참진드기로동정되었다 (10). 충주지역야산에서는 2002년과 2003년에 85% 가작은소피참진드기로보고되었다 (11). 동물에서참진드기를채집하여분류한결과 54.4% 가작은소피참진드기로동정되었으며 (12), 국내에서주로관찰되고있는진드기는특히방목장을중심으로작은소피참진드기가우리나라거의전지역에서서식하는것으로보고하였다 (13~15). 본연구에서도작은소피참진드기가 99.4% 로대부분이었다. 지역적으로곡성과보성지역에서 1,000마리이상채집되었고, 순천과고흥에서 150~250마리채집되었다. 환자발생이없었던구례가 140마리로가장적은수가채집되었으며 (Table 1), 또한구례에서채집한 140마리에서유충은채집되지않았다 (Table 2). 이결과는구례지역에 2013년환자발생이없었던사실을고려해볼때서

5 Distribution of Ticks Collected in Jeollanam-do 97 식하는작은소피참진드기가적어서환자발생이없었는지에대한추가연구가필요하다고생각된다. 참진드기는알, 유충, 약충, 성충등의 4가지발육단계를가지고있으며, 유충은흡혈한후에탈피하여약충 (nymph) 으로발육하고다시숙주에게달라붙어흡혈한후탈피하여성충 (adult) 으로발육한다. 계절에따라채집되는진드기의발육단계가차이가있는데, 유충은 9월과 10월에집중적으로채집되고약충과성충은 5월부터 9월까지채집되다가 10월이후에는채집되지않는뚜렷한계절적차이를보였다 (Table 3). 다른보고 (11, 12) 에서성충은 4월부터 8월까지채집되었지만, 자충은전조사기간에걸쳐다른발육단계의개체보다훨씬높은채집비율을보이면서 5월에최고치를나타냈고, 유충은 8월에나타나기시작하여 10월까지채집되었다는결과와비교하여볼때본조사에서도같은계절적분포를보였다. 작은소피참진드기가자연계에존재하는 SFTSV의주된매개체라고알려져있고 (16), SFTSV를진단하는다양한방법중 SFTSV의 L, M 및 S segment 모두의 highly conserved region을이용한 OneStep RT-PCR 이사용되어지고있고, 민감도와특이도는각각 98.6% 와 99.0% 라고보고되었다 (17). Park et al. (14) 은우리나라 9개지역에서 11,856 마리의작은소피참진드기를채집하여 SFTSV를확인한결과 0.46% (55 pools) 의최소감염률을보고하였다. 한편 Yun et al. (16) 은 261마리의참진드기를채집하여 SFTSV 양성률이평균 6.9%, 특히작은소피참진드기는 5.7% 를나타냈다고하였다. 그러나본연구에서는채집된진드기를 pooling하여 2X OneStep RT-PCR 방법으로유전자를분석하였으나 SFTSV 유전자검출이되지않았다 (Fig. 1). 이와같이진드기에서바이러스의검출률이다른보고와차이를보였는데, 앞으로조사지역에따라진드기의바이러스감염률이차이가있는지를알아볼필요가있어지속적인조사가요구된다.
