1 강직성척추염에서천골장골관절염에대한다중검출기 CT 와단순촬영의비교 1 유지연 주경빈 최병규 류정아 김태환 2 최우정 목적 : 본연구의목적은강직성척추염이의심되는환자를대상으로천골장골관절염의진단에있어다중검출기 CT의유용성에대해단순촬영과비교분석하는데있다. 대상과방법 : 임상적으로강직성척추염이의심되는 192명의환자를대상으로천골장골관절염의진단을위하여단순촬영과다중검출기 CT를함께시행하였다. 이검사들은두명의근골격계영상의학과의사에의해 Modified New York Criteria에준하여후향적으로분석하였다. 결과 : 단순촬영과다중검출기 CT에서천골장골관절염진단에대한민감도는각각 59.9%, 66.7%, 83.3%, 74.5% 였고, 다중검출기 CT를이용하여천골장골관절염으로진단된환자의수가단순촬영을이용했을때보다통계적으로유의하게증가함이관찰되었다 (p < 0.05). 등급별분류에따른분석에서는 384개의천골장골관절중 114개, 127개 (29.7%, 33.1%) 의천골장골관절이다중검출기 CT에서단순촬영보다높은등급으로평가되었다. 결론 : 강직성척추염과동반된천골장골관절염평가에있어다중검출기 CT는단순촬영만으로는진단이어려운조기천골장골관절염의진단을가능하게하여치료방법결정및예후에도움을줄수유용한방법이라하겠다. 강직성척추염 (Ankylosing spondylitis) 은병인이밝혀지지않은염증성요통을특징으로하는진행성, 쇠약성질환으로천골장골관절염 (Sacroiliitis) 을특징으로한다 (1, 2). 강직성척추염의진단은특징적인임상증상과동반된천골장골관절염의단순촬영을이용한영상의학적진단이포함된 Modified New York criteria가사용되고있다 (3). 단순촬영을이용한천골장골관절염의평가는특히조기일경우천골장골관절 (Sacroiliac joint) 의복잡한해부학적구조로말미암은판독의어려움으로진단이여의치않은경우가많다 (4-6). 이러한단순촬영의단점을보완하여천골장골관절염을평가하려는방법으로신티그래피 (Scintigraphy), 전산화단층촬영 (Computed Tomography, CT), 자기공명영상 (Magnetic Resonance Imaging, MRI) 등이있지만이러한방법들도각각의한계점을가지고있다 (7-10). 신티그래피는천골장골관절염의평가에있어높은민감도를 위골수의부종을평가할수있어전산화단층촬영에서형태학적변화가나타나기전좀더조기에천골장골관절염을진단할수있고, 치료에대한반응을관찰, 감시할수있는장점이있다. 또한방사선노출의위험이없는장점도있으나상대적으로검사비용이비싸고, 검사시간이길다는한계점을보인다 (13-17). CT의경우복잡한해부학적구조를잘보여주고다른방법보다강직성척추염의진단과예후에가장중요한천골장골관절의골변화평가에있어우수함을보이지만, 방사선노출의위험이있다. 따라서이러한천골장골관절염의평가에있어 CT의유용성은항상논쟁의대상이되어왔다. 천골장골관절염평가에서 CT와단순촬영이유사한진단적능력을갖춘다고보고하고있는논문 (18-20) 들이있지만 CT가단순촬영보다좀더높은정확도와민감도를보인다고보고하는연구도있다 (6, 9, 21-26). 이러한연구들은대부분적은수의환자를대상으로고식적 CT를이용하였다 (9, 19-25). 보이지만, 낮은특이도로인해보조적인진단방법으로여겨지 최근 사용이 늘어나는 다중 검출기 CT는 고식적 고있다 (11, 12). MRI는천골장골관절염의진단에있어전산화단층촬영과유사한능력을보이며, 관절내염증과관절주 CT(Conventional CT) 에비해매우우수한영상을제공하여여러부분에서영상의학적진단수준을높이는데기여하고있 다. 이에따라최근다중검출기 CT를이용한천골장골관절염 1 한양대학교병원영상의학과 의진단적유용성에대해일부보고가있지만, 이또한적은수 2 한양대학교병원류마티스내과 이논문은 2008년 7월 28일접수하여 2008년 10 월 24일에채택되었음. 의환자를대상으로하고있다 (26). 따라서본연구에서는비 181

2 유지연외 : 강직성척추염에서천골장골관절염에대한다중검출기 CT 와단순촬영의비교 교적많은수의강직성척추염이의심되는환자를대상으로천골장골관절염의진단에있어다중검출기 CT의유용성에대해단순촬영과비교분석해보고자하였다. 대상과방법환자 2004년 12월부터 2007년 12월까지한양대병원류마티스내과에내원하여천골장골관절다중검출기 CT를시행한 367명의환자중임상적으로 Modified New York Criteria에따라강직성척추염과동반된천골장골관절염으로진단되고단순촬영도함께시행한 192명의환자를대상으로하였다. 이중남자가 150명, 여자는 42명이었고, 나이범위는 13-64세이었으며평균연령은 26.7세였다. 사용장비와영상획득방법본연구의기간에단순촬영과다중검출기 CT의촬영방법은같게시행하였다. 단순촬영은천골장골관절의전후상 (anteroposterior view) 과좌우비스듬영상 (oblique view) 을각각시행하였다. 전후상은앙와위에서평균적으로남자는 여자는 caudal tube angle로, 비스듬영상은전후상보다평균적으로 10 가량작은 caudal tube angle을사용하여영상을얻었다. 다중검출기 CT는 Sensation 16 (Simens medical System, Erlangen, Germany) 과 Brilliance CT 64 Slice (Philips Medical System, Best, The Netherland) 기기를이용하여횡단스캔 (transverse image) 을얻고, 워크스테이션 (workstation) 에서 3차원소프트웨어를이용하여관상다평면재구성 (coronal Multiplanar reformation, MPR) 영상을얻었다. 3 mm 영상두께 (slice thickness) 와골재구성알고리듬 (bone reconstruction algorithm) 을사용하였다. 다중검출기 CT의파라미터로는 Sensation 16기기는 160 ma, 120 kvp, 0.5sec tube rotation speed와 0.75 beam pitch 를, Brilliance CT 64 Slice기기는 350 ma, 120 kvp, 0.75sec tube rotation speed와 beam pitch를사용하였다. 모든환자에서단순촬영을다중검출기 CT보다먼저시행하였고, 두촬영방법사이의기간차이는 94명의환자가같은날검사를시행하였고, 78명의환자가 3개월이내, 12명이 6 개월이내, 6개월이상의차이를보인경우가 8명이었다. 천골장골관절염의등급-등급 0: 정상, 등급 1: 천골장골관절염이의심되는변화, 등급 2: 천골장골관절강의너비에변화없이천골장골관절의국소적미란이나경화를보이는극소의변화, 등급 3: 천골장골관절의미란, 경화, 관절강이넓어지거나좁아짐혹은국소적인관절강직을보이는명확한변화, 등급 4: 천골장골관절의완전강직성을평가하였다. 퇴행성관절염 (Degenerative joint disease), 경화성장골염 (osteitis condensans ilii), 종양 (tumor), 골절등과같은천골장골관절염과직접적으로연관되지않은소견들은정상군으로분류하였다. 이에따라평가된등급을바탕으로양쪽에서천골장골관절에서의등급이 2 이상인경우와한쪽에서만등급 3이나 4 인경우를천골장골관절염으로정하였다. 이러한결과를바탕으로천골장골관절염이있는군과없는군으로나누어단순촬영과비교하여다중검출기 CT의민감도에대해분석하였고, 천골장골관절염의등급별분류에따른결과를분석하였다. 또한, 천골장골관절염의진단여부에따라단순촬영과다중검출기 CT를나누어판독자사이의일치도를구하고, 단순촬영과다중검출기 CT 사이의일치도를판독자별로분석하였다. 통계처리방법천골장골관절염진단에따른다중검출기 CT와단순촬영의분석에는카이제곱검정 (Chi-sequre test) 을시행하였고, 촬영방법에따른두판독자사이의일치도와촬영방법사이의판독자별일치도는코헨의카파 (kappa) 검사를이용하였다. 카파값은 의범위를정하고, 일치도는 : 약간일치 (poor), : 어느정도일치 (fair), : 중간정도일치함 (moderate), : 상당히일치함 (good), : 거의완벽함 (excellent) 으로정의하였다. 통계적수치의계산과분석을위해통계용소프트웨어 Table1. 다중검출기 CT를이용하여천골장골관절염으로진단된환자의수가단순촬영을이용했을때보다통계적으로유의하게많았다 (p < 0.05) (Table 1, 2)

판독자 2의경우에서는단순촬영으로진단되지못한 61명의환자중 28명이다중검출기 CT를통해천골장골관절염으로분류될수있었다. 단순촬영에서진단되지못한환자의 57.8%, 45.9% 가다중검출기 CT을통해천골장골관절염의진단기준을충족시켰다.

반면에 5명, 16명의환자가단순촬영에서는천골장골관절염의진단기준을충족하였으나다중검출기 CT에서는진단기준을충족시키지못했다 (3.9%, 12.2%).

3 Statistical package for the social science(spss) 12.0 KO for windows release을사용하였고, p < 0.05를통계학적으로의미가있음으로정의하였다. 결과임상적으로천골장골관절염이의심되는 192명의환자중단순촬영에서는각각판독자에따라 128명, 131명과다중검출기 CT에서는각각 160명, 143명의환자가천골장골관절염으로진단되어그비율이각각 59.9%, 66.7% 와 83.3%, 74.5% 로나타났다. 다중검출기 CT를이용하여천골장골관절염으로진단된환자의수가단순촬영을이용했을때보다통계적으로유의하게많았다 (p < 0.05) (Table 1, 2) (Fig. 1, 2). 판독자 1에서는단순촬영에서천골장골관절염으로진단되지못한64명의환자중37명이다중검출기CT을통하여천골장골관절염으로진단되었다. 판독자 2의경우에서는단순촬영으로진단되지못한 61명의환자중 28명이다중검출기 CT를통해천골장골관절염으로분류될수있었다. 단순촬영에서진단되지못한환자의 57.8%, 45.9% 가다중검출기 CT을통해천골장골관절염의진단기준을충족시켰다. 단순촬영에서 64명, 61명의천골장골관절염으로진단되지못한환자중다중검출기 CT에게서도천골장골관절염으로진단되지못한환자가 27명, 33명으로 42.2%, 54.1% 로나타났다. 반면에 5명, 16명의환자가단순촬영에서는천골장골관절염의진단기준을충족하였으나다중검출기 CT에서는진단기준을충족시키지못했다 (3.9%, 12.2%). 판독자에따른단순촬영과다중검출기 CT에게서도등급별분류에의한분석에서는판독자 1에서총 384개의천골장골관절중 114개 (29.7%) 가단순촬영보다다중검출기 CT에서높은등급으로평가되었다. 36개 (9.4%) 의천골장골관절은다중검출기 CT에서단순촬영보다낮은등급을보였다. 또한, 136 개의천골장골관절이단순촬영에서등급 0, 1로평가되었는데, 이중다중검출기 CT에서 62개가등급 2 이상으로진단되었다 (Table 3, 4). 판독자 2에서는다중검출기 CT에서 A B C Fig. 1. Comparison of the Radiographic Grade as Detected by Plain Radiography and in Observer 1. 고 찰 단순촬영은현재까지천골장골관절염을진단하는데있어널리인정되고, 쉽게사용할수있는영상의학적방법이지만치함을보였다 (6, 9, 20-26). 고식적 CT를이용해 70% 에서 (15) 천골장골관절의복잡한해부학적구조때문에판독의어 75% 민감도를보고한이전의논문보다 (16, 24, 25) 다소높려움이있고증상의시작과단순촬영에서천골장골관절염의은민감도를보였다. 이는 81.1% 의민감도를보고한다중검소견이나타나기까지의시간이빈번하게지연되어, 단순촬영출기 CT를이용한논문과는유사한결과를보였다 (26). 을이용한강직성척추염과동반된조기천골장골관절염의경단순촬영과다중검출기 CT에게서의등급별분류에따른분우진단이어려운경우가많고판독자간일치도도상대적으로석에서는 384개의천골장골관절중다중검출기 CT에서 114 낮다 (4-6). 개, 127개 (29.7%, 33.1%) 의천골장골관절이단순촬영보다천골장골관절평가에있어 CT의유용성은논쟁의여지가있높은등급으로평가되었고, 단순촬영에서등급 0, 1로평가된었지만최근에보고된연구들에서는 CT가천골장골관절염을 119개, 136개의천골장골관절중다중검출기 CT에서 53개, 평가하는데있어높은정확도와민감도를보여신뢰할만한검 62개가등급 2 이상으로높게평가되었다 (p < 0.05). 또한, 단사로보고되고있다 (6, 9, 21-27). 일부연구에서는 CT가천순촬영에서등급 2 이상으로평가된경우에도다중검출기 CT 골장골관절염을평가하는데있어진단적효용성이단순촬영를이용한경우에서 39개, 51개의천골장골관절이더높은등과비교해서통계학적으로차이를보이지는않지만 (18-20), 급으로진단되었다. 천골장골관절의복잡한해부학적구조를보여주고이상소견다중검출기 CT는고식적 CT에보다검사시간이짧고, 시을발견하는데있어서단순촬영보다민감도가높다고보고하간적 (temporal) 공간적(spatial), 대조해상도 (contrast 고있다 (18, 25). 이러한연구들대부분은적은수의환자를 resolution) 가뛰어나다는장점이있다. 특히다중검출기 CT 대상으로고식적 CT를이용하였다 (9, 19-25). 일부많은수의에서사용되는다평면재구성영상은매우우수한영상을제공환자를대상으로한연구가있지만, 다중검출기 CT를이용하하고있어여러부분에서영상의학적진단수준을높이는데기지않았고 (27), 가장최근보고된다중검출기 CT를이용한여하고있다 (28). 연구는적은수의환자를대상으로하고있다 (26). 따라서복잡한해부학적구조를가지는천골장골관절을평본연구의결과는천골장골관절염의평가에있어 CT가단순가하는데있어다중검출기 CT는단면영상과관상다평면재촬영에비해높은민감도를보인다는이전연구들의결과와일구성영상을통해천골장골관절을둘러싸는연부조직이나골 184

We wanted to determine the role of multidetector row CT for the evaluation of the sacroilitis in patients with ankylosing spondylitis. Materials and Methods: One hundred ninety two patients with clinically suspected ankylosing spondylitis were evaluated by conventional radiography and multidetector row CT. Two musculoskeletal radiologists retrospectively analyzed the images, and they graded the sacroiliitis using the modified New York Criteria. Results: Multidetector row CT demonstrated a significantly higher sensitivity (74.5%, 83.3%) than did plain radiography (59.9%, 66.7%) for detecting early sacroiliitis (p<0.05). Multidetector row CT showed a higher grade of sacroiliitis in 114 and 127 of 384 sacroiliac joints. Conclusion: Performing multidetector row CT rather than plain radiography for making the diagnoses of accompanying ankylosing spondylitis allows an early start of treatment with a subsequently improved prognosis.
