Objective: Intrauterine growth retardation of lung has been suggested as being one of the risk factors in the development of bronchopulmonary dysplasia (BPD). The aim of this study was to clarify birth weight percentile among Korean infants using the contemporary Korean reference curve for birth weight by gestational age published in 2014, and on the basis of that clarification, identify the association between birth weight percentile and BPD development. Methods: All newborn infants born at <32 weeks of gestation admitted to the neonatal intensive care unit of CHA Bundang Medical Center from January 2010 through December 2015 were review ed. Infants were divided into BPD group and non-bpd group. BPD was defined as a need of oxygen or positive airway pressure at 36 weeks postmenstrual age. All prenatal and neonatal data were collected via retrospective chart review and analyzed by multivariate logistic regression analysis. Results: During the study period, 198 singleton infants were born at <32 weeks gestation. Among these, 179 infants were included, 54 infants in BPD group and 125 infants in non-bpd group. According to logistic regression analysis, lower gestational age, lower birth weight percentile, occurrence of late onset sepsis showed increased risk of BPD (odds ratio [OR] 0.77, P=0.027; OR 0.98, P=0.021; OR 7.98, P<0.0001). Conclusion: In this single center study, lower birth weight percentile in preterm infants seems to increase the risk of BPD. In these infants, anticipate the high risk of BPD and cautious treatment strategies are needed. Key Words: Intrauterine growth retardation, Bronchopulmonary dysplasia, Preterm birth 서론 산전스테로이드치료와출생후인공폐표면활성제의사용으로신생아호흡곤란증후군 (respiratory distress syndrome of newborn, RDS) 의유병률은크게감소하였지만폐포화 와혈관발달의억제로인한기관지폐형성이상 (bronchopulmonary dysplasia, BPD) 의발생 을막지는못하고있다. 1 따라서신생아집중치료의비약적발전이이루어진현재에도 BPD 의유병률은미숙아생존의증가, 특히초극소저출생체중아의생존율증가와함께오히려 증가한양상을보인다. 1-4 자궁내성장지연을유발하는인자들이여러신체기관의발달과 출생후기능에영향을미치고, 이러한영향으로폐또한발달단계의초기에영향을받고그 구조와기능에있어출생후에도비가역적인영향을받을것으로생각된다. 5 인간의폐발달 속도는일정하지않아서, 기도의발달은태아초기에시작되어재태연령 주에완성되 고, 폐포의발달은재태연령 20 주경에시작되어출생후 2 3 년에완성된다. 그후에는이미 존재하는폐포의크기성장만이일어나는것으로알려져있다. 6 이러한연구결과는태내또 는출생초기의유해자극이호흡기계의구조와기능에장기적인영향을미칠것을뒷받침한 다. 그러나인간을대상으로한임상연구에서는, 주로부당경량아 (small for gestational

Association between birth weight and bronchopulmonary dysplasia age, SGA) 를대상으로하였으며, SGA가폐발달의위험인자가되는지는일치된결과를보이지않았다. 5,7-9 본연구는 2014년도에국내단태아를대상으로하여새롭게발표된재태연령별, 성별체중곡선을활용하여정확한체중의백분위수를반영하고이를바탕으로출생체중의백분위수로대표되는자궁내성장과 BPD 발생과의연관성을알아보고자하였다. 10 대상및방법 1. 대상 2010년 1월 1일부터 2015년 12월 31일까지 6년간분당차병원에서출생하여신생아집중치료실에입원한재태연령 32주미만의단태아를대상으로하였다. 교정연령 36주이전에전원되었거나사망한경우는연구에서제외하였다. 연구대상자들에서산모의임상적특성및주산기합병증과신생아기임상양상을의무기록을통해후향적으로조사하였다. 본연구는분당차병원의료연구윤리심의위원회의승인이후에진행하였다 (CHAMC ). 2. 방법산모의특성과주산기합병증으로는, 출산시산모의나이와체외수정시술을통한임신 (in vitro fertilization, IVF), 임신성당뇨 (gestational diabetes mellitus, GDM), 임신성고혈압 (pre gnancy induced hypertension, PIH), 18시간이상의조기양막파수 (premature rupture of membrane, PROM), 융모양막염, 산전스테로이드완료, 제왕절개출산여부를조사하였다. 신생아의임상양상으로는재태연령, 출생체중, 분만장에서의소생술, 신생아호흡곤란증후군, 동맥관개존 (patent ductus arte riosus, PDA), 후기패혈증 (late onset sepsis), 괴사성장염 (ne crotizing enterocolitis, NEC), 미숙아망막증 (retinopathy of prematurity, ROP), 뇌실내출혈 (intraventri cular hemorrhage, IVH), 뇌실주위백질연화증 (periventri cular leukomalacia, PVL), 인공폐표면활성제투여횟수를조사하였으며, BPD군과 non-bpd군으로나누어비교분석하였다. 체중은신생아집중치료실에서생체징후가안정화된후, 완전탈의상태에서 10 g 단위까지측정가능한표준화된신생아측정용체중계를이용하여측정하였으며 2014년발표된우리나라단태아를대상으로한재태연령별, 성별성장곡선을활용하여백분위수를구하였다. 10 백분위수는참조한성장곡선내에서선형적관계를가정하고주수차이의증가분을고려하여반영하였다. 또한 3백분위수미만에해당하는 7예는 1백분위수로지정하고, 97백분위수초과에해당하는 5예는 99백분위수로지정하였다. 3. 정의 BPD는교정연령 36주에산소또는양압환기치료가필요한경우로정의하였으며 National Institutes of Health consen sus 의중증도기준에따라중등도이상의기관지폐형성이상을분석하였다. 11,12 융모양막염은산모가 38 이상의발열이있으며자궁압통, 산모나태아의빈맥, 또는악취가나거나화농성양수를보이는경우로하였다. 13 RDS는호흡곤란의증상과그에합당한흉부 X-ray 소견을보이고인공폐표면활성제가투여된경우로하였고, PDA는소아심장전문의가시행한심초음파결과로확인하였고, IVH는 Papile의 4단계분류기준을따랐으며, 14 3단계이상인경우를중증 IVH로정의하였다. NEC는임상증상, 신체검사소견, 방사선검사및혈액검사소견을고려하여 modified Bell s criteria 15 에따라서 2단계이상으로진단받은환아를포함시켰으며, ROP는국제위원회진단기준을사용하였고, 16 stage 3 이상을미숙아망막병증으로정의하였다. 후기패혈증은생후 72시간이후부터교정연령 36주이전시기에임상증상, 이학적소견, 혈액검사결과로의심되며혈액배양검사에서균이동정되어 5일이상항생제치료를받은경우로정의하였다. 임상증상및이학적소견으로는빈맥또는서맥, 빈호흡또는무호흡, 저긴장증, 기면, 보챔, 피부색의변화등을고려하였으며, 혈액검사결과로는 C-reactive protein (CRP) >4 mg/dl, 총백혈구수 < /L, 혈소판수 < /L 중적어도 1가지이상을포함하는경우로하였다. 혈액배양검사는 1 쌍이라도균이동정된경우를배양검사양성으로판단하였다. PVL은소아영상의학과전문의의판독을참고하였다 통계처리방법통계분석은 SPSS statistics ver (SPSS Inc., Chica go, IL, USA) 을이용하였다. 명목형변수의분석에는카이제곱검정또는 Fischer s exact test를적용하여 n (%) 로나타내었고, 연속형변수의분석에는 Mann-Whitney U검정을적용하여 median [IQR] 또는 mean±sd로나타내었다. BPD 발생의위험요인을밝히기위해다변량로지스틱회귀분석을시행하여 odds ratio (OR) 와 95% 신뢰구간 (confidence interval, CI) 을제시하였으며, 단순분석에서유의한차이를보인변수들중에서단계적방법을통해가장적합한최종변수를선정하여분석하고자하였다. 모든분석결과에대해 P값이 0.05 미만인경우를통계적으로유의한차이가있는것으로판정하였다

3 2017 June;28(2):35-40 결과 2010 년 1 월 1 일부터 2015 년 12 월 31 일까지 6 년간분당차 병원에서출생하여신생아집중치료실에입원한재태연령 32 주미만의단태아수는모두 198 예였다. 이들중심각한선천성 기형이나염색체이상을가진경우는없었다. 이중교정연령 36 주이전에전원된 2 예와사망한 17 예를제외한 179 예가연 구에포함되었다. BPD 군에는 54 예 (30.2%), non-bpd 군에는 125 예 (69.8%) 가포함되었다. 연구에포함된 179 예환아들의 재태연령분포는 주 11 명 (6.1%), 주 26 명 (14.5%), 주 45 명 (25.1%), 주 97 명 (54.2%) 이었으며 23 주 이하의미숙아출생은없었다. 남녀의비율은남아 90 명, 여아 89 명으로각각 50.3%, 49.7% 였다. 평균재태연령은 ±2 +0 주, 남아의평균출생체중은 1,370±370 g, 여아의평균출생 체중은 1,350±370 g 이었다. 중등도, 중증 BPD 의빈도는각각 37 명 (20.7%), 17 명 (9.5%) 이었다. 1. 산모의특성과주산기합병증 연구대상자의주산기특성은 Table 1 에제시하였다. Abbreviations: IVF, in vitro fertilization; GDM, gestational diabetes mellitus; PIH, pregnancy-induced hypertension; PROM, premature rupture of membrane. 65) 분위에비해 60 (35-80) 분위로현저히낮았다 (P= 0.007). 또한 BPD 군에서 IVF 임신의빈도 (P=0.013) 및제왕절개출산 의빈도가높고 (P=0.001), 5 분아프가점수가낮으며 (P=0.022), 분만장에서기관내삽관의빈도가더높았다 (P<0.001). 연구대 상산모의출산시연령, GDM, PIH 의유병률과산전스테로이 드완료, 융모양막염, 18 시간이상 PROM 의빈도는두군간에 유의한차이를보이지않았다. 2. 신생아의임상적특성과예후 신생아질환의빈도와특성은 Table 2 에제시하였다. BPD 군 에서 1 회이상인공폐표면활성제를투여하게되는경우가더 많았고 (P=0.001), PDA 결찰술, 중증 IVH, 후기패혈증의빈도 가더높게나타났다. RDS, ROP, 낭종성 PVL 의빈도는두군간 에유의한차이를보이지않았다. 단순분석결과재태연령, 출생체중, 출생체중의백분위수, IVF, 제왕절개술, 5 분아프가점수, 분만장에서기관내삽관, 인 공폐표면활성제투여횟수, PDA 결찰술, 중증 IVH, NEC, 후 기패혈증의빈도에서두군간에통계적으로유의한차이를보 였다. 이중재태연령, 출생체중의백분위수, 제왕절개술, 분만 장에서의기관내삽관, 인공폐표면활성제투여횟수, PDA 결 찰술, 후기패혈증을고려한다변량로지스틱회귀분석결과재 태연령이낮을수록 (OR=0.77, 95% CI= , P=0.027), 출생체중의백분위수가낮을수록 (OR=0.98, 95% CI=0.97 Table 2. Multivariate Logistic Regression Analysis for Bronchopulmonary Dysplasia (n=179) Risk factor 0.99, P=0.021), 후기패혈증에노출된경우 (OR= 7.98, 95% CI= , P<0.001) BPD 발생의빈도가높은것으로나 타났다 (Table 3). 고찰 본연구결과, 재태연령 32 주미만의단태아를대상으로한 단일기관연구에서, 출생체중의백분위수가작을수록 BPD 발 생의위험이높아지는것을확인하였다. 또한재태연령이낮거 나, 출생후감염에노출된경우 BPD 발생의빈도가높은것으 로나타났다. 이는태아성장을방해하는인자가폐성장에도영 향을미치고, 출생후의염증반응이폐발달에유해한영향을미 치는것을반영한다고볼수있다. 본연구의중등도 BPD 빈도 와중증 BPD 빈도는각각 20.7%, 9.5% 로출생체중 1,500 g 미 만환아를대상으로한 Korean Neonatal Network (KNN) 자료 를활용한선행연구결과에비하여낮게나타났다. 1 이는본연 구에포함된환아들이좀더높은재태연령과출생체중에위치 하는것과일치한다. Odds ratio 95% Confidence interval P-value Gestational age Birth weight percentile Cesarean section Intubation at delivery room Number of surfactant treatment PDA ligation Late onset sepsis <0.001 Abbreviation: PDA, patent ductus arteriosus. 낮은출생시체중의백분위수는 BPD 유병률과유의한상관 관계를보이는것으로나타났다. 충분한영양은정상적인태아 의발달및폐의발달과성숙에필수적인요소로알려져있 다. 18,19 불충분한영양공급을비롯한태아의정상적인성장에 불리한조건들이체중으로대표되는양적성장을저해하였을 것이고이러한성장저해요소들은폐의실질발달을지연시키 고새로운폐포의발달을저해할것으로예상된다. 성장지연이 BPD 에미치는영향에대한선행연구들은대부분대상환아들 을 SGA 와 AGA 로구분하여연구하였다. 그러나 SGA 를대상으 로한연구결과 BPD 발생과의연관성은일관되지않은결과를 보이고있다. Lal 등 20 은재태연령 32 주이하의신생아를대상 으로한연구에서 SGA가 BPD의위험을 1.87배올리고 LGA의경우는 BPD의위험을낮춘다고보고하였다. Zeitlin 등 21 은 10 개유럽지역에서재태연령 24 31주의단태아와다태아 4,525 명을대상으로한연구에서체중의백분위수와 BPD 발생의연관성을보고하였다. 또한, Egreteau 등 22 은재태연령 32주미만신생아를대상으로한연구에서 IUGR 환아들의산소의존도가출생후 28일, 교정연령 36주에각각 2.4배, 4.7배로보고하였다. 반면, 초기의연구에서는 BPD의위험이오히려 SGA 환아에서더낮다는보고도있었다. 9 이러한다양한연구결과는연구집단이상이하고치료방법이다르고 BPD의정의가각기다른것에서기인하기도한다. 본연구는단일기관연구로균일한치료전략 ( 수액요법, 인공환기기적용, 출생후스테로이드사용 ) 을따르고위험군을 SGA로한정하지않고출생체중의백분위수를활용하여이러한문제를없앴다. 미숙아의재태연령에따른신체계측치는미숙아의여러가지예후를예측할수있는중요한지표이다. 이전연구에서가장많이인용된기준치는 Lubchenco 등 23 에의해 1960년대에발표된것으로그간의비약적인경제적, 의학적발전으로인해현재의상황을제대로반영한다고보기어렵다. 또한인종과민족적으로도우리나라사람에게적용하기에는맞지않는부분이있을것으로생각된다. 뿐만아니라재태연령을추정하는방법에서도정확한방법이사용되지못하였으므로이러한점들을보완한동시대우리나라아기들을대상으로한재태연령별, 성별자료의필요성은충분하다. Lim 등 10 은 년기간 Korean Statistical Information Service 자료를활용하여재태연령 23 43주, 단태아 2,249,804명 ( 남아 1,159,070명 ) 을대상으로한재태연령별, 성별성장곡선을발표하였다. 본연구에서는이자료를활용하여재태연령별, 성별체중의백분위수를명확히하여분석의정확을기하였다. 재태연령은선행연구를통해가장잘알려진 BPD의연관인자이다. 본연구결과에서도재태연령은 BPD 발생의빈도와유의한상관관계를보이는것으로나타났다. 재태연령이낮을수록폐의구조적기능적미성숙으로인해 BPD의위험을높일것으로예측된다. Lenoir 등 24 의연구에의하면재태연령, 24 25주, 26 27주, 28 29주, 30 31주에서각각 72%, 50%, 28%, 13% 의빈도로 BPD가발생한다고보고하였다. 또한 Jo 등 1 은출생체중 1,500 g 미만환아를대상으로한연구결과 BPD 발생빈도가재태연령 23주에서 42.2%, 24주에서 48.9%, 25주에서 47.5%, 26주에서 43.1%, 27주에서 42%, 28주에서 34.3%, 29주에서 22.8%, 30주에서 20.5%, 31주에서 13.0%, 32주에서 10.3%, 33주이상에서 7.1% 를보인다고하였다. BPD 발생에관여하는인자와경로는매우다양하고복잡하

5 2017 June;28(2):35-40 지만폐의염증반응이폐포화와혈관화에손상을입히는마지막단계임은잘밝혀져있다. 25 융모양막염의경우는 BPD 발생과연관되어있다는연구들도있지만한편관련성이높지않다는연구결과도있다. Mittendorf 등 26 과 Yoon 등 27 은 cord blood 내에 proinflammatory cytokine인 IL-6의상승과 BPD 연관성을보고하였지만 Bose 등 8 의연구에서는 funnisitis가 BPD 발생과연관성이없는것으로나타났다. Ballard 등 28 의연구에서는재태연령 32주이하의극소처출생체중아를대상으로한연구결과임상적융모양막염은 BPD의독립적위험인자가되지못하고, 재태연령과패혈증은독립적위험인자가되는것으로나타났다. 반면, 일부연구에서는조직학적융모양막염이 BPD 발생을막는다는연구결과도보고한바있다. 29,30 최근의메타분석을고려해보면조직학적융모양막염은명확한 BPD의위험인자가되는듯하다. 31 또한융모양막염자체가 BPD 위험을증가시키기보다는융모양막염으로인하여발생한조산의영향으로합병증이증가한다는가설도제기되고있다. 32,33 본연구결과로는융모양막염과 BPD 발생위험의통계적유의성을확인할수없었다. 그러나본연구에포함된대상자중조직생검을통해확진된예는오직 1예에불과하였으며임상적융모양막염을진단기준으로삼았다. 임상적으로의심되는사례의경우조직검사를통해명확한진단을내리려는노력이더필요할것으로생각된다. 반면, 출생후발생한패혈증에대한연구결과대부분의연구에서 BPD 발생과의연관성을보고하고있다. 본연구에서도후기패혈증의경우 BPD의 OR가 7.98로매우강력한위험인자로나타났다. 여러연구에서 Staphylococcus epidermidis로인한패혈증이 BPD의위험을높인다고하였으나 본연구에서는원인균에따른 BPD 발생률의차이를조사하지는못하였다. 태내에서의손상에더불어출생후패혈증이나인공환기의경우이러한염증반응을가속화하거나, 손상복구의시간을주지못하고악화를반복하게하는것으로생각된다. 한편으로는이미손상된폐가감염에취약하고, 또한이러한손상된폐가장기간침습적인공환기의보조를요구하는것일수있겠다. 본연구에서는산모의키와몸무게를대비하여 constitutionally small baby와 growth retardation 환아를구분하지못한한계점이있다. 향후이런변수를고려한연구를통해통계적유의성을확인해보아야할필요가있다. 결론적으로, 낮은체중백분위의미숙아에서기관지폐형성이상의빈도가높은것으로나타났다. 이는태아시기에성장을제한하는인자들이폐의구조와기능에손상을유발하고출생후감염및자극에의한염증반응의손상을가속화하고회복의잠재력을저하시켜기관지폐형성이상발생에영향을미치는 것이라고생각된다. 따라서출생시재태연령별체중의백분위 수가낮은미숙아에서는 BPD 발생위험이높아지므로, 예후를 향상시키기위해감염예방및인공환기기사용등에서더주의 깊은관리가필요할것으로생각된다.
