2 2 1. 서론 Cytomedix는조직, 특히피부조직, 복구분야에전문화된바이오텍기업이다. 주요제품은자가혈소판분비물질을이용한치료제 (Autologus Platelet Releasate Therapy) 인데, 피부조직에재상과복구에사용된다. 이기술 ( 브랜드 : AutoloGel) 은각종성장호르몬과다른분자들을포함하는데, 단기및만성상처치유의치료목적으로개발되었다. Cytomedix는현재임상3상시험중이며, 곧 FDA의최종승인을획득할것으로예상된다. AutoloGel 시스템은환자자신의혈장, 혈소판, 기타혈액단백질을사용하는데, 직접환자에게서추출되어몇가지처리를거친후바로젤형태로사용된다. 따라서, 인체의상처치유과정과비슷하다. 또한, 상업적인관점에있어다른치료법보다경쟁우위를가진것으로평가되고있다. 본글에서는특수한기술과시장을가진 Cytomedix사의기술, 특허, 사업전략등에대해알아보고자한다. < 핵심사항 > *AutoloGel 시스템은세계적으로 11조원으로추정되는만성상처치유분야를타겟으로한다. 즉, Diabetic foot ulcer, venous stasis ulcer, pressure ulcer를치료할수있다. * 이기술은환자자신으로부터유래된물질을사용한다. 따라서, 감염 / 오염이없고, 혈액을환자로부터언제든지뽑아바로젤형태로만들어적용할수있다. 결과적으로타제품대비보존기간과제품운송의문제가없다. * 현재임상시험중인당뇨병성궤양의결과는우수하며, 2005년하반기에공개되고, 곧 US FDA의승인을받을것으로예상된다. *Cytomedix는견고한특허및지적재산을보유하고있으며, 몇차례의소송에서승리하였다. 2. 피부및상처 1. 피부구조 A. 피부는인간의몸에서가장큰기관이며, 외부로부터의보호, 온도조절, 감각기관을가지고있다. B. 피부는아래와같이 3개층구조를가진다. i. Epidermis: 가장외부층이며, Keratinocyte라는일종의 Epithelial cell을포함한다. 항상새로운케라티노사이트가생겨외부에서떨어져나간세포를대체한다.

3 3 ii. Dermis: 콜라겐을합성하는 Fibroblast로주로이루어져있다. 이콜라겐이상처치유에중요한역할을한다. 또한, 혈관, 모발세포, 땀샘이분포한다. iii. Subcutaneous: 지방세포와혈관을보유한다. < 피부구조 > 출처 : MSN.com 2. 상처 (Wound) A. 피부혹은다른외부표면이찢기거나, 구멍이나거나, 잘린상태를말하며, 상처치유 (Wound Healing) 과정에대해많은연구가진척되었다. B. 상처에는 2가지종류가있는데아래와같다. i. Acute: surgical incisions, trauma, and burns, and are characterized by the ability of the body to follow a normal, physiological healing process. ii. Chronic: a wound that has failed to proceed through an orderly and timely series of events to produce a durable, structural, functional, and cosmetic closure. (ex. diabetic foot ulcers, pressure ulcers, and venous ulcers) 3. 상처치유 (Wound Healing) A. 상처치유과정에는세포종류와순서, 그리고성장호르몬들의기능이많이알려져있지만아직도연구해야할분야가많이남아있는상황이다. B. 상처치유과정은아래와같이 3가지단계로구성된다. i. Inflammation: 피부에상처가발생하자마자혈소판이혈관으로부터분비되며혈액을응고시킨다. 곧 blood clot이형성되고출혈이멈춘다. Clot 밑에서는상처치유과정이시작되며, 백혈구가모여들어박테리아와세

4 포조각들을제거한다. ii. Proliferation & migration: Fibrobalst들이모여들어분열하면서연결조직을생성시키고상처를봉합한다. 이후 Keratinocyte가자극되어분열하면서상처를감싼다. Epithelial cell들이굳은딱지밑에서자라며, 근처피부로부터새로운혈관이생성된다. 이단계에서수많은성장호르몬이분비된다. iii.

5 5 4. Growth Factors & Cytokines A. 상처치유과정에는다양한단백질호로몬이작용한다. i. Platelet-derived Growth Factor(PDGF), Transforming Growth Factor(TGF), Tumor Necrosis Factor-alpha(TNFa), IL-1, IL-4, IL-8, Fibroblast Growth Factor(FGF), Epidermal Growth Factor(EGF) 등 B. Platelet-derived Growth Factor (PDGF) i. Platelet, macrophage, endothelial cell, keratinocyte에의해분비되면, PDGF-a, PDGF-b로구성된 dimer들로이루어진 3 isoform family이다. ii. 2가지수용체와의상호작용에따라겹치거나또는다른양상을보인다. iii. Fibroblast, neutrophil, macrophage의 chemotaxis와분열을자극한다. iv. 또한, extracellular matrix 구성성분인 Fibronectin와 hyaluronan의생 산을촉진한다. C. Transforming Growth Factor-beta(TGFb) i. 3 isoforms: b1, b2, b3 ii. 그중 b1이모든조직에서가장풍부하며, 혈소판에함유된형태이다. 또 한, 상처치유과정에연관된모든세포들이 TGFb1에반응한다. iii. 상처의 성숙과 강도에 가장 중요한 역할을 하는데, collagen, proteoglycan, fibronectin, matrix degrading protease 등을조절한다. iv. Topical application의경우, 상처의강도를증가시킨다. 3. 상처치유시장 1. 특이성 A. 상처치유시장은다른시장과달리넓고다양한환경의시장을포함하고있다. B. 즉환자가치유과정동안반드시동일한병원등의시설에있는것이아니며, 다양한시점과환경에놓일수있다. C. 상처치유제품은사용이간단하면서도다양한상처에적용이가능해야비용을줄이고효능을높일수있다. 2. 목표시장 A. Cytomedix의목표시장은 Diabetic/Venous/Pressure Ulcer이다. B. 세계적으로는 18백만명의환자가, 그리고미국에는 4.5백만명의환자가존재하는것으로추정되고있다.

Pressure Ulcer A. 활동이적은노인에게서주로나타나며, 특히뼈의압력이있는팔꿈치, 힙, 발뒷꿈치등의부위에서주로발생한다. B. 이는뼈의압력이혈관을막아결과적으로조직손상을가져오는것이다.

Clinical Trial A. 약력 i. 2003년초, 원심분리기에대한 Pre-Market Notification(510K) 및 Investigational Device Exemption(IDE) 신청 : 최초단계 ii. 2003년 2분기에원심분리기에대한 510K 인증획득 : 그러나, 특정한질병치료에대한안전성과효능을 FDA로부터인정받은것이아니라, 현재판매중인원심분리기들과의동등성인정받은것임. iii. 2004년 3월, FDA로부터 IDE에대한허가획득하여, 비로소당뇨병성족부궤양환자들에대한임상시험이가능해짐. iv. 진행중인임상시험은 14개센터에서 72명의환자들을대상으로진행중이며, 2005년 3분기에마무리될것으로예상됨. B. 요약프로토콜 i. 1단계 : Inclusion & exclusion criteria를만족시키는환자들은스크리닝 ii. 2단계 : 영입된환자들을 prospective, randomized, controlled, and double-blinded system으로분리하여치료시작

12 12 iii. 3단계 : 12주간관찰하여완전히치료가되었는지확인 (Primary endpoint) 하고, 12주간 follow-up iv. 2005년 5월 6일현재, 최소요구환자수인 72명을완료하였음. C. A Retrospective Case Study: 102 Wound Case Review Analysis: All Wound Etiologies, Diabetic and Pressure Ulcers: Wound Volume Change During Initial 28 Days Treatment with the AutoloGel System i. 2002년에, Cytomedix는 AutoloGel 시스템을사용했던병원들에게상처치유에관련된데이터들을요구하여분석하였다. ii. 이를통해총 102명의사례가통계적으로분석되었으며, 제품사용후 28 일을기준으로, 모든 Ulcer 환자에서상처부위가 50% 이상현저히감소하였다. iii. 즉, 모든환자들의경우, 상처부위가 50% 이상감소한환자가 70% 였다. iv. 또한, Diabetic ulcer 환자의경우, 50% 이상상처부위가감소한환자가 81% 였다. v. 요약결과는아래와같다. 출처 : Cytomedix D. 19 19 Provant applicator adjacent to the wound and activating the automatically dosed and timed system 7. 결론 Cytomedix는아직확실한치료법이개발되지않은 Wound Management 분야의리더이다. 특히환자유래혈소판을이용하여피부조직을재생시킬수있는기술을개발하였다. 이기술은 2005년하반기에 US FDA의허가를받을것으로예상된다. 위에서언급된다양한기술과함께상보적인기술로개발되고있는데, 언급되지않은새로운기술은유전자치료가있다. 즉, 각성장호르몬의유전자를이용한치료법이새로운경쟁기술로개발되고있다.
