If the disease is not treated, stroke risk will increase more than five times, and about one in three people will experience a stroke during their life time. Therefore, AF needs active management, and anticoagulation therapy is effective in the prevention of stroke. Warfarin and aspirin, the medicines for the prevention and treatment of thromboembolism, are effective in the prevention of stroke in AF. Medication is determined by stroke risk, generally classified by CHADS2 score. Warfarin is recommended primarily for highrisk patients, and aspirin is recommended for lowrisk patients and those unable to take warfarin. Despite the combined warfarinaspirin therapy is not recommended in general, complementary effects are expected due to different mechanisms of the drug. However, some cases have been reported that the combination of these medications just administered to increase bleeding risks and did not represent a remarkable therapeutic effect. Therefore, this study is in 2, Jan. to Oct. in outpatients with AF who underwent the anticoagulation therapy, and patients were classified according to medication uses. This study was analyzed cardiovascular diseases, medical treatments, and hematologic data. In addition, this 4

2 JKSHP, VOL., NO. (2) study retrospectively analyze the status of anticoagulation therapy in patients with AF. The warfarin group and warfarin+aspirin group were 54 and 5 patients and average duration of drug administration were 4.7 and.6 months, respectively. Bleeding events occurred in 5 and 6 patients in warfarin group and warfarin+aspirn group, respectively. Retrospective analysis of the two drugs showed no significant differences between the bleeding risk and stroke prevention, additional research is needed for the clinical evaluation of the combination therapy of warfarin and aspirin. CHADS2 score and stroke risk 연구방법 CHADS2 score Stroke risk(%) 연구기간및연구대상 2년 월 일부터 2년 월 일까지세브란스병원에서 warfarin 처방으로복약상담의뢰된외래환자들중심방세동환자를대상으로 Warfarin 단독복용환자군, Warfarin+Aspirin 병용복용환자군의두군으로분류하여각각 54명, 5명으로총 5명을분석하였다. 나뉘며, 저위험군 (score=) 일경우약물요법이시행되지않거나 aspirin 단독요법이권고된다. 중등도위험군 (score=) 일경우 warfarin 또는 aspirin 단독요법이권고되며고위험군 (score 2) 일경우 warfarin 단독요법이권고되지만 warfarin과 aspirin 병용요법에 5, 6) 대한명확한치료지침은제시되어있지않다. Warfarin과 aspirin은서로다른기전을가지고있다. Warfarin은 vitamin K epoxide의재생을차단하여 vitamin K dependent clotting factor (factor Ⅱ Ⅶ Ⅸ Ⅹ, anticoagulant proteins C S) 의합성을저해함으로써항응고효과를나타낸다. Aspirin은 acetylation을통해 cyclooxygenase를비가역적으로저해하여 prostaglandin의합성과혈소판의응집을저해한다. 따라서 warfarin과 aspirin의병용요법은두약제의서로다른기전으로인한상호보완적인효과가기대되는약물요법이며, 실제로본병원에서도많은환자들에게사용되고있다. 9) 하지만유의한효과가없고출혈경향을높일뿐이라는연구결과가보고된바있으며, 두약제병용요법에대한명확한치료지침이제시되어있지않다. 9) 본연구는 warfarin과 aspirin 병용요법에대한임상적평가를위해진행되었다. () 연구기간 Warfarin 시작또는 warfarin+aspirin 시작일로부터최대 6개월까지 복용중단 : 약제중단 주이상일경우 Clopidogrel, triflusal 등다른항혈전제복용을추가로시작한경우는그전날을종료일로한다. (2) 연구대상 Warfarin을처음복용하는환자를대상으로한다. Aspirin 복용이력이있는환자는환자군에포함한다. Clopidogrel, triflusal 등다른항혈전제복용이력이있는환자는환자군에포함한다. 2 약물복용기간 개월이상인환자를대상으로한다. 출혈증상으로 개월이전에복용을중단한경우는포함한다. 연구기간중측정된평균 INR(international normalized ratio) 이.5 이상, 2.5 미만인환자를대상으로한다. 2. 환자분석대상환자들의전자의무기록 (EMR: eletronic medical record), 복약상담기록지, 통합검사결과등을통해자료를수집하여후향적연구방법으로분석하였다. 45

(2) 환자기본정보성별, 연령, CHADS2 score, 기저질환 2 약제복용기간, 복용중단기간 출혈발생여부와그처치 Major bleeding: 두개내출혈, 경막하출혈, 복막후출혈, 출혈로인한약제중단, 장기내출혈등입원을유발한출혈 Minor bleeding: 코피, 잇몸출혈등 major bleeding을제외한모든출혈 뇌졸중발생여부와그처치 혈액학적수치 (INR). 통계방법 SPSS version 8.을사용하여 chisquare test 또는 Fisher s exact test를시행하였다. 각항목은 pvalue가.5 미만일경우유의성이있는것으로평가하였다. 연구결과. 환자기본정보 () 성별 (Fig. ) 대상환자총 5명중남성이 56.2% (59명), 여성이 4.8%(46명 ) 로남성의비율이높았다. Warfarin+Aspirin 병용군에서남성 6.8%( 명 ), 여성 9.2%(2명 ) 으로남성의비율이높았으며, Warfarin 단독군에서도남성 5.9%(28명 ), 여성 48.%(26명 ) 으로남성의비율이높았다. (2) 연령 (Fig. 2) 대상의평균연령은 Warfarin+Aspirin 병용군이 66.세, Warfarin 단독군이 65.9세로큰차이를보이지않았다. 연령대의경우 65세이상 75세미만인군이 4%(42명 ) 로가장높은비율을차지하였으며, 55세이상 65세미만인군이그다음을차지하였다 (.5%, 2명 ). Fig. Gender 46

5 박지은 : 심방세동환자에서 Warfarin 단일요법과 WarfarinAspirin 병용요법간의출혈경향발생과뇌졸중예방효과고찰 () CHADS2 score 평균점수는 Warfarin+Aspirin 병용군.5, Warfarin 단독군.로큰차이가없었다 (Table 4). 점수가 ( 저위험군으로약물요법시행되지않거나 aspirin 단독요법권고 ) 인경우임에도 warfarin 을복용한환자는 Warfarin+Aspirin 병용군에 2 명, Warfarin 단독군에 명분석되었으며대부분이시술예정이거나시행받은경우 (8명), 처음심방세동진단받아항부정맥제로조절하며항응고요법을같이하는경우 (6명), 75세에근접한경우 (명) 등이었다 (Table 5). (4) 기저질환 (Table 6) CHADS2 score와관련이있는 hypertension, congestive heart failure, diabetes mellitus, cerebral infarction 등의질환이높은비율로나타났으며, 그외 dyslipidemia, coronary artery obstructive disease, tricuspid regurgitation 등의질환이높은비율을차지하였다. 2. 약제복용기간 () 약제복용기간 (Table 7) 평균약제기간은 Warfarin+Aspirin 병용군.6 개월, Warfarin 단독군 4.7개월로단독군이 개월정도더길었다. (2) 약제중단기간 (Table 8) 주미만의약제중단경험이있는환자의경우 Warfarin+Aspirin 병용군에 명, Warfarin 단독군에 5명이분석되었으며, 대부분이각종시술과수술을위해단기간복용중단한경우였다.. 출혈또는뇌졸중증상유무와그처치 () 출혈증상 (Table 9) Major bleeding Warfarin 단독군에서만 명 ( 두개내출혈 ) 발생하였으며, 타병원을내원하여치료받은경우였다. 2 Minor bleeding Warfarin+Aspirin 병용군 6명, Warfarin 단독군에서 4명이발생하였으며대부분눈충혈이나멍이심하게드는증상을호소한경우였다. 8 JKSHP, VOL., NO. (2) (2) 평균측정빈도 Warfarin+Aspirin 병용군과 Warfarin 단독군의평균측정횟수는각각 4.회, 7.회이고, 해당기간중의평균복용기간은각각.6개월, 4.8개월로병용군은월평균약 회, 단독군은약 2회의빈도로측정하였다 (Table ).
