The Seventh Report of the Joint National Committee on Detection, Evaluation, and Treatment of High Blood Pressure (JNC-7), published in 2003, provides a streamlined management approach to hypertension for the primary care physician. The JNC-7 is the gold standard also in Korea. According to the JNC-7, special therapeutic considerations are recommended for high-risk individuals with compelling indications. The presence of compelling indications in any given patient should be considered when selecting specific pharmacotherapy to treat hypertension. However, in patients with compelling indications, it is unknown that hepatotoxicity is caused by Calcium Channel Blocker (CCB), one of 1st anti-hypertensive drugs. Now, the CCB is the most used 1st anti-hypertensive drug in Korea Therefore, we evaluated the changes in blood liver function parameters (ALT, AST, Total bilirubin, serum albumin) for the study group. Methods : We randomly collected and retrospectively analyzed Electronic Medical Record data (n=28,788) of patients, and who took calcium channel blockers(non-dihydropyridines; diltiazem, verapamil, dihydropyridines; amlodipine, barnidipine, benidipine, clinidipine, efonidipine, felodipine, isradipine, lacidipine, lercanidipine, nicardipine, nifedipine, nimodipine), with having liver function tests (LFTs) from July 1st 2009 to June 30th 2010 at the Seoul National University Hospital in Korea. Control groups are two antihypertensive agents: RAS blockade (ARB; candesartan, irbesartan, losartan, olmesartan, telmisartan, valsartan, ACE-I; cilazapril, enalapril, fosinopril, imidapril, perindopril, ramipril) and, Diuretics (loop; furosemide, torsemide, thiazide; hydrochlorothiazide[hctz], indapamide). Patients not having LFT results at these three standard points of time(baseline, during, medication, and after finishing medication) were excluded. The collected data were analyzed by using the SPSS (Version12.0) and Microsoft Excel (Version2007). Results : 711 patients who were treated CCB (297), RAS blockade (232) or Diuretics (182) monotherapy were selected for the study. In selected patients, liver damage degree(changes of each LFTs value) was higher in diuretics group than other groups, followed by RAS blockade and CCB. In diuretics group s was loop-diuretics group was higher than thiazide-diuretics group. In CCB group, Nondihydropyridine-CCB s damage degree was higher than Dihydropyrine-CCB s that. Conclusions : Despite the limitations due to the retrospective study, among patients with abnormal LFTs, the use of CCBs led to a less liver damage than other 1st anti-hypertensive agents. It can be recommended CCBs as one of the initial treatments of hypertension in patients with liver disease. 2 칼슘채널차단제, 레닌 - 안지오텐신시스템차단제, 이뇨제의잠재적간독성평가 331 보여왔다. 1) 미국의고혈압유병률 (NHANES , 만 20 세이상 ) 도 32.1% 로우리나라보다약간높은수준이다. 2) 고혈압은생활습관조절과약물요법으로치료된다. 고혈압의약물에는작용기전에따라칼슘채널차단제 (CCB), 레닌안지오텐신시스템 (RAS) 차단제, 이뇨제, 베타차단제, Adrenergic antagonist, 알도스테론길항제, 알파차단제, 알파II수용체효능제, 직접적혈관확장제등이있다. 3),4) 2007년미국심장학회 (AHA) 지침에서는입증된증거에서고혈압 1차치료제로 RAS 차단제인 ACE-I, ARB, CCB, thiazide계이뇨제또는 2가지약의병용사용이추천되고있다. 5) 2011년영국고혈압학회 (BHS), 영국국립보건임상연구원 (NICE) 2011년지침에서는고혈압치료 1, 2차단계로 RAS 차단제와 CCB를고혈압치료약제로추천하고있다. 6) 우리나라건강보험심사청구데이터를이용하여 2007년원외처방전을대상으로혈압강하제처방양상을분석한연구에의하면성분군별처방행태를살펴보면, CCB가포함된원외처방전이 64%, 그중대부분은 DHP계 CCB였지만다음으로이뇨제 44.6%, ARB와 ACE-I가각각 33.3%, 11.7%, 베타차단제 21.6% 였다. 이뇨제는종합병원, 종합전문병원에서의원이나병원에비해 loop계이뇨제를상대적으로더많이처방하였다. 7) 2003년발표된미국고혈압합동위원회제7차보고서 (JNC-7 지침 ) 는진료의들에게고혈압관리의간소화된접근법을제공한다. JNC-7 지침에의하면, 1차고혈압치료이외에 필수적응증 ( 당뇨병, 만성신장질환, 심부전, 관상동맥질환 / 고관상동맥위험, 재발성뇌졸중 ) 을가지고있는고위험군환자들에게특별한치료적고려사항이추천된다 (Table 1). 환자가 필수적응증 이있다는것은고혈압치료를위해특정약물요법을선택할때고려되어야함을의미한다. 8) 그러나필수적응증을가진환자에서, 항고혈압약물 ( 칼슘채널차단제, RAS 차단제, 이뇨제 ) 에의한간독성의비교연구는알려져있지않다. 각고혈압계열별이나개별적인성분별에대한간독성평가는자료가있지만, 9) 필수적응증에추가적인도움이될수있는연구는부족한상태이다. 특히고혈압환자들은질병의특성상혈압의조절유지를위하여장기간의약물복용이필요하고이에따라간독성에대한모니터링및약물의선택이중요하다. 따라서이번연구는 3차병원에서항고 혈압약물중복용빈도가높은 CCB, RAS 차단제, Diuretics 약물을복용한환자의간기능검사 (LFTs) 수치 (ALT, AST, TB, Alb) 의변화량을평가하여고혈압치료약물을복용하는환자중간독성을분석하여고혈압관리및필수적응증에부가적으로간독성변화에따른적절한약물선택의방법을제시하고자하였다. 연구방법 대상및기간이연구는 2009년 7월 1일부터 2010년 6월 30일까지의기간중서울대학교병원 (1650병상, 진료과 24개, 종합병원 ) 의 EMR(Electronic Medical Record) 를통해수집된자료를가지고후향적으로실시되었다. 자료수집과이용은병원내의 IRB(# H ) 의승인을받아진행되었다. (1) 포함기준 (Inclusion criteria) 1 연구기간내 LFTs 값 (ALT, AST, TB, Alb) 이기록된환자 2 1차고혈압치료제인 CCB, RAS 차단제, 이뇨제를복용한환자. 3 CCBs : Dihydropyridine계 (amlodipine, barnidipine, beramlodipine, barnidipine, bernidipine, clinidipine, efonidipine, felodipine, isradipine, lacidipine, lercanidipine, nicardipine, nifedipine, nimodipine), Non-dihydropyridine계 (diltiazem, verapamil) RAS 차단제 : ACE-I(captopril, cilazapril, enalapril, fosinopril, imidapril, perindopril, ramipril), ARB(candesartan, irbesartan, losartan, olmesartan, telmisartan, valsartan 이뇨제 : Loop계 (furosemide, torsemide), Thiazide계 (hydrochlorothiazide, indapamide, metolazone) 4 약물복용전, 중그리고약물복용종료후 30일이내의기준시점 (Point A, B, C) 에서 LFTs 결과가 3가지시점모두가지고있는환자. 5 복용환자는 1차고혈압치료제 CCB, RAS 차단제, 이뇨제각각을단독으로복용한환자 (2) 제외기준 (Exclusion criteria) 1 연구기간동안 LFTs 수치 (ALT, AST, TB, Alb) 4개중하나라도결과가기록되지않아간기능상태를확인할수업는환자 Table 1. Pharm., Vol. 22, No. 4, 서로다른 1차고혈압치료제를연구기간내중복하여복용한환자 분석방법약물복용전 LFTs 표준수치 (A값) 의기준은약물복용전가장최근시점의값들을선택하였고, 약물복용중의 LFTs 표준수치 (B값) 는복용중검사수치내 ALT, AST, TB는최대값, Alb은최소값을선택하였다. 약물복용후의 LFTs 표준값 (C값) 은해당약물복용종료후 30일이내에서의 ALT, AST, TB는최소값, Alb는최대값을선택하였다. 약물복용전, 중, 후 LFTs값을표준화한다음에는 3시점 (A, B, C값 ) 모두존재하는환자만을최종추출하였다. 3시점중 1개라도측정값이없는환자는분석에서제외시켰다. 복용 3시점의간수치가표준화된환자들은간손상의정도를복용전표준화한값과복용중표준화한값의차이 (B-A) 로수치화하였고간회복의정도는복용중과복용후의값차이 (B-C) 로수치화하였다. 간기능검사평가기준 LFTs 수치의 Normal 범위와 Abnormal범위는서울대학교병원의기준을적용하였다. 각수치별정상한계치는 ALT( 40 IU), AST( 40 IU), TB( 1.2 mg/dl), Alb( 3.3 g/dl) 로하였다. 추가적으로 Severe abnormal 상태에대한기준은 US- FDA에서적용하고있는 ALT( 120 IU/L), AST( 120 IU/L), TB( 2.4 mg/dl) 로하였다. Alb에있어서 Severe abnormal 상태에대한기준은서울대학교병원과 US-FDA 기준이없기때문에 Severe hypoalbuminemia의기준인 Alb(< 2 g/dl) 로하였다 (Table 2). 통계방법연구의모든자료는통계분석프로그램인 SPSS(Version 12.0) 과 Microsoft Excel(Version 2007) 을통해분석하였다. 약물별, 표준화간기능수치의평균차이는일원배치분산분석 (one-way ANOVA test) 를사용하였다. 분석의유의성은 P 값이 0.05 미만일경우통계적으로유의하다고판정하였다. P값의범위에따라 0.05미만일때는별표 (*) 의개수를 3가지로구분하여표시하였다 (p 0.05: *, p 0.01: **, p 0.001: *** ). Table 2. CCBs, RAS 차단제, 이뇨제각각의환자는 5,698명, 6,183명, 2,907 명이었다. 그리고연구에서정한 3가지시점의 LFTs 값을가지고있으며, 1차고혈압치료제를단독복용한환자들을최종선출한환자수는 711명이었다. 약물군별로최종환자수는 CCBs군 297명, RAS 차단제군 232명, 이뇨제군 182명이었다 (Figure 1). 연구대상의나이는 18세이상성인환자가 CCBs군 92.3%, RAS 차단제군이 90.3%, 이뇨제군은 97.8% 이었다. 3가지연구약물군을세부적으로나눈결과, DHP계 CCB군은 79.6%, Non-DHP계 CCB군은 95.1%, ACE-I군은 70.6%, ARB군은 98.2%, Loop계이뇨제군은 97.5%, Thiazide계이뇨제군은 100% 의 18세이상성인환자의비율을보였다. 연구대상의기저질환은환자별로중복인정되었다. 간질환환자는 CCB군은 92명, RAS 차단제군은 50명, 이뇨제군은 111명이었다. 비간질환환자는 CCB군은암환자 177명, 순환기계질환 169명, 신장질환 108명이었다. RAS 차단제군은순환기계질환이 124명, 신장질환 114명, 암이 80명이었다. 이뇨제군은암환자 111명, 순환기계질환 71명, 신장질환 46명이었다 (Table 3). 약물복용중 (Point B) 약물별간기능상태약물복용중의시점에서간기능상태가가장안좋을때의값으로 Normal, Abnormal, Severe abnormal의 3가지상태로나눈비율을분석하였다. AST 수치에서는 CCB군에서는 69.7%, 24.6%, 5.7% 였다. RAS 차단제군은 82.3%, 12.5%, 5.2% 였다. 이뇨제군은 45.6%, 36.8%, 17.6% 였다. ALT 수치에서는 CCB군이 68.4%, 22.9%, 8.8% 의비율분포를보였다. RAS 차단제군은 78.9%, 15.9%, 5.2% 였다. 이뇨제군은 60.4%, 26.4%, 13.2% 였다. TB 수치는 CCB군이 82.5%, 12.1% 5.4% 였고, RAS 차단제군은 88.4%, 10.3%, 1.3% 였다. 이뇨제군은 37.9% 23.6% 38.5% 였다. Alb 수치에서는 CCB군에서 Normal 81.1%, Abnormal 18.9% 의비율을보였고 Severe abnormal은없었다. RAS 차단제군은 86.2%, 13.4%, 0.4% 를보였다. 이뇨제군은 37.4%, 58.8%, 3.8% 의비율을보였다 (Figure 2). 3가지약물군 (CCB, RAS 차단제, 이뇨제 ) 을세부적으로나누어약물복용중 Normal, Abnormal, Severe abnormal 환자비율을나누면, ALT 수치에서 DHP계 CCB군은 68.3% 23.4% 8.3% 를보였고, Non-DHP 계 CCB군은 68.8%, 18.8%, 12.5%, ACE-I군은 73.5%, 19.1%, 7.4% 를보였다. ACE-I 군은 80.9%, 13.2%, 5.9% 이고 ARB군은 82.9%, 12.2%, 4.9였다. Loop계이뇨제군은 41.6%, 38.5%, 19.9% 이고 Thiazide계이뇨제군은 Normal 76.2%, Abnormal 23.8%, Severe abnormal은없었다. TB 수치에서는, DHP계 CCB군이 81.9%, 12.1%, 6.0%, Non-DHP 계 CCB군은 87.5%, 12.5% 이고 Severe abnormal은없었다. ACE-I군은 89.7%, 8.8%, 1.5% 였고 ARB군은 87.8%, 11.0%, 1.2% 였다. Loop계이뇨제군은 34.8%, 24.2%, 41.0% 이고 Thiazide계이뇨제군은 61.9%, 19.0%, 19.0% 였다. Alb 수치로는 DHP계 CCB군은 81.1%, 18.9% 였고 Severe abnormal은없었다. Non-DHP계 CCB군은 81.3%, 18.8% 였고여기서도 Severe abnormal은없었다. ACE-I군은 86.8%, 11.8%, 1.5% 였고, ARB군이 86.0%, 14.0% 였고 Severe abnormal 환자는없었다. Loop계이뇨제군은 32.3%, 63.4%, 4.3% 이고 Thiazide 계이뇨제군은 76.2% 23.8%, Severe abnormal 환자는없었다 (Figure 3). 약물복용후평균간손상정도 (B-A, Mean Damage Degree) 약물복용직전 LFTs수치 (Point A, Baseline) 와약물복용중 LFTs(Point B) 의변화량을전체환자, 간질환환자, 비간질환환자에따라약물별로분석하였다. ALT 수치에서는전체환자의경우 CCB군은 IU, RAS 차단제군은 IU, 이뇨제군은 IU였고, 간질환환자의경우 CCB군은 IU, RAS 차단제군은 IU, 이뇨제군은 IU였다. 비간질환환자는 CCB군은 IU, RAS 차단제군은 IU, 이뇨제군은 IU였다. ALT에서의약물별평균간손상정도의차이는통계적인유의성은없었다. AST 수치에서는전체환자에서 CCB군은 9.97 IU, RAS 차단제군은 IU, 이뇨제군에서는 IU였고통계적인유의성을보였다 (P=0.001). 간질환환자에서도 CCB군은 8.05 IU, RAS 차단제군은 IU, 이뇨제군은 IU로약물별간손상정도에통계적유의성을보였다 (P=0.007). 비간질환자에서는 CCB군 IU, RAS 차단제군 IU, 이뇨제군 IU였고통계적유의성은없었다. TB 수치에서는전체환자에서 CCB군이 0.24 mg/ dl, RAS 차단제군이 0.13 mg/dl, 이뇨제군이 1.17 mg/dl로

Normal, Abnormal, Severe state rate(%) during medication(point B) by subclasses of drugs -. 통계적유의성이있었다 (P<0.001). 간질환환자에서도 CCB 군이 0.33 mg/dl, RAS 차단제군 0.19 mg/dl, 이뇨제군이 1.77 mg/dl로통계적유의성을보였다 (P<0.001). 비간질환자에서는 CCB군 0.20 mg/dl, RAS 차단제군 0.12 mg/dl, 이뇨제군 0.24 mg/dl였고통계적유의성은없었다. Alb 수치에서는전체환자에서 CCB군 g/dl, RAS 차단제군 g/dl, 이뇨제군 g/dl, 간질환환자에서 CCB군 g/ dl, RAS 차단제군 g/dl, 이뇨제군 g/dl, 비간질환자에서 CCB군 g/dl, RAS 차단제군 g/dl, 이뇨제군 g/dl였고통계적유의성은없었다 (Figure 4). 약물종료후평균간회복정도 (C-B, Mean Recovery Degree) 약물종료후 30일이내 LFTs (Point C) 와약물복용중 LFTs (Point B) 의변화량을전체환자, 간질환환자, 비간질환환자에따라약물별로분석하여약물복용중단후간회복정도를분석하였다. ALT 수치에서는전체환자의경우 CCB군은 IU, RAS 차단제군은 IU, 이뇨제군은 IU, 간질환환자의경우 CCB군 IU, RAS 차단제군은 IU, 이뇨제군은 IU, 비간질환자에서는 CCB군 IU, RAS 차단제군은 IU, 이뇨제군은 IU였다. ALT 수치에서는통계적유의성은없었다. AST 수치에서는전체환자의경우 CCB군이 IU, RAS 차단제군이 IU, 이뇨제군에서 IU, 간질환환자의경우 CCB군은 IU, RAS 차단제군은 IU, 이뇨제군은 IU였고, 비간질환환자의경우 CCB군 IU, RAS 차단제군은 IU, 이뇨제군은 IU였고통계적유의성은없었다. TB 수치에서는전체환자의경우 CCB군 mg/ dl, RAS 차단제군 mg/dl, 이뇨제군은 0.59 mg/dl로통계적유의성이있었다 (P<0.001). 간질환환자에서도 CCB 군이 mg/dl, RAS 차단제군이 mg/dl, 이뇨제군이 1.08 mg/dl로통계적유의성이관찰되었다 (P=0.029). Alb 수치에서는전체환자에서 CCB군 0.21 g/dl, RAS 차단제군은 0.17 g/dl, 이뇨제군은 0.29 g/dl로통계적유의성이있었다 (P=0.021). 간질환환자에서도 CCB군 0.21 g/dl, RAS 차단제군 0.14 g/dl, 이뇨제군 0.28 g/dl로통계적유의성이있었다 (P=0.043). 비간질환환자에서는 CCB군 0.22 g/dl, RAS 차단제군 0.17 g/dl, 이뇨제군 0.30 g/dl로통계적인유의성은없었다 (Figure 5).

Compelling indication 속에포함된당뇨병, 신장질환, 순환기계질환등에대해서는그동안많은연구를통해재확인되고입증되었다 ) 고혈압환자들은대부분성인에서발생하며다수가중년에서노령환자가많다. 그환자들은다양한만성질환을동반하고있으며그에따라많은약물복용을하고있다. 또한고혈압치료제를장기간복용하는환자들이많다. 이러한결과를볼때이러한경우를볼때간해독능력등에영향을주어간기능파라미터변동을가져올수있다고할수있다. 그동안개별적인고혈압치료약물들의간독성에대한보고는있었다 ) 이러한사항을고려하여볼때이번연구는간질환환자의고혈압치료제간독성비교평가연구로의미를가지고있다고볼수있다. 고혈압치료제에대한간기능비교평가를시도하기위해서 여러계열의고혈압치료제중에서도 2007년 AHA Guideline 에서정립된 1차고혈압치료제 (CCB, RAS 차단제, 이뇨제 ) 들을이번연구에적용하였다. 1차고혈압치료제를복용하는간질환또는비간질환고혈압환자들의간기능을비교적평가를해보았다. 약물에따른간기능변화를확인해보고자약물복용중간손상이가장심할때 (Ponit B) 의간기능상태 (Normal, Abnormal, Severe abnormal) 비율, 간손상정도 (B- A), 간회복정도 (C-B) 3가지의표준화한측정값을적용하였다. 약물복용중간손상이가장심할때의비율은 ALT, AST, TB, Alb 모두의경우 RAS 차단제군, CCB군, 이뇨제군순서로 Abnormal의비율이적었다 (Figure 2, 3). 이뇨제군은 Severe 군를포함한전체 Abnormal 상태환자비율이 AST 기준 39.6%, ALT 기준 54.4%, TB 기준 62.1%, Alb 기준 62.6% 로이뇨제군의경우, Abnormal, Severe abnormal 비율이타군에비해압도적으로높았다. CCB군과 RAS 차단제군은비슷한 Abnormal 환자의비율을보였다 (Figure 2, 3). 평균간손상정도값 (B-A) 과평균간회복정도값 (C-B) 은 1 차고혈압약물의복용전, 후와약물의복용종료후간기능의변화추이를비교가능하다. Alb에서의평균간회복정도

Mean Recovery Degree(C-B) of LFTs in Total, Liver, Non-liver disease patients. 에서이뇨제가타군에비해우수한결과가나오긴했지만 ALT, AST, TB의평균간손상, 간회복정도및 Alb의간손상정도에서는이뇨제가타군에비해유의성있는결과를얻지못했다. 따라서이뇨제가 CCB와 RAS 차단제에비해간기능저하등바람직하지못한영향이있을것으로보인다. 최근 2011년영국심장학회 (BHS) 의 NICE guideline에서 2006년 Version 의고혈압 1차치료제지침, ACD Rule(A:RAS 차단제, C:CCB, D:Diuretics) 에서이뇨제를 1차에서제외한것에이번논문에서이뇨제의결과도연관성이있을것이라는가능성이제기되어진다. 6) 평균간손상및간회복정도에서있어서 CCB군과 RAS 차단제군의비교는 TB의평균간손상정도에서만 RAS 차단제군이 CCB군보다우수한결과를보였지만, 다른모든경우에서는 CCB군이 RAS 차단제군보다우수하거나비슷하였다 (Figure 4, 5). 따라서간질환을동반한고혈압환자의초기치료에있어서 CCB를우선적으로고려되어야할것으로보인다. 본연구에서 CCB가간독성및간회복에있어서타약물에비해우수한결과를얻은것은기존의동물실험에서밝혀진 CCB의항산화효과와세포보호가간세포에 기여한것과관련이있을것이라사료되나이는추후연구가필요할것이다 ) 이번연구의핵심이되는장기인간은복용약물, 식품, 혹은환자상태등다양한 Confounding factor에의해영향을받는다. 따라서한, 두가지요인을조절하여규명하기어렵다. 그럼에도불구하고이번연구는 3가지 1차고혈압약물모두동일한조건에서, 즉약물복용전-중-후간수치가있어약물복용에대한간기능변화추이를볼수있는환자그리고고혈압 1차약물끼리서로중복되어복용하지않고단독요법으로치료받는환자, 2가지동일조건만충족되는환자를선택하여본것이라는점에서의의가있다. 이러한연구는복잡한요소의혼재속에서드러나지않지만임상적으로진행되고있는하나의의미있는 signal을발견하므로써, 앞으로임상연구가필요한주제를제기하는것으로의미를가질수있다. 2009년부터 2010년까지전세적으로대유행하였던신종플루의주요사망원인인급성심근염도과거에이번연구와비슷한맥락에서연구가이루어져밝혀진것이었다 ) 일반적으로계절적인독감으로여겼던환자들을추적관찰조사하던중결과가상당히임상적예
