Although there have been extensive studies on screening tools for depression used in domestic mental health settings, there is little consideration and lack of a thorough review of the diagnostic validity of screening tools. In the current review, we selected 13 representative screening tools for depression which were evaluated in a total of 19 validation studies conducted in Korea. We summarized DSM-5 target domains, diagnostic indices, sensitivity, specificity, cut-off scores, and diagnostic validity information for each tool. Finally, the depression measurement expert group was constituted to evaluate the current status of screening tools for depression, and their recommendations for a new screening tool were summarized. This study was conducted as part of the Mental Health Technology Development project to develop the Korean screening tool for depression (K-DEP). 2 국내우울증평가도구타당화연구의체계적고찰 는 정확한 진단 효과적 치료 추적이 가능한 체계를 갖춘 임상 실제에서는 일차 의료 장면에서 우울증의 선별은 우울증 환자를 정확히 확인하고 이들을 위한 치료를 제공하는 데 도움이 되므로 모든 성인들에 대해 우울증 선별 검사를 실시하도록 권고한 바 있다 우울증의 선별에 사용되는 검사 도구를 개발 및 선택하는 데 있어 고려해야 하는 첫 번째 요인은 평가도구가 우울증 환자를 감별해낼 수 있을 만큼의 양호한 진단적 정확성을 확보하였는지 여부이다 선별 검사의 정확도는 검사 개발 단계에서 중점적으로 고려하는 요인으로 민감도와 특이도를 지표로 판단할 수 있다 민감도 는 실제 질병이 있는 환자를 임상군으로 분류할 수 있는 능력 정도를 수치화 한 것이다 그러나 우울증 평가도구의 민감도가 라고 할 때 이는 대상자가 우울증이 있거나 없거나 상관없이 모든 사람에게 양성 진단을 내린다는 것을 의미하기 때문에 특이도 를 진단을 위해서 민감도와 함께 고려된다 특이도는 실제로 질환이 없는 일반인을 비임상군으로 분류할 수 있는 능력을 수치화 한 것으로 민감도와 특이도가 모두 에 가까울수록 진단적 능력이 높은 평가도구로 판단이 된다 우수한 민감도와 특이도가 확보된 선별검사는 잠재적 임상군의 정확하게 파악하여 일반군을 환자군으로 오진할 경우 발생할 수 있는 의료진 연구진의 비용 낭비를 절감할 수 있다 또한 잠재적 환자군을 일반군으로 오진할 경우 적절한 개입 시기를 놓쳐 우울증의 악화로 이어질 수 있는데 진단적 타당성이 확보된 선별 도구는 조기 개입의 기회를 증진시킬 수 있다는 이점이 있다 한편 국내 의료 및 지역사회 등 다양한 임장 장면에서 사용되는 우울증 평가도구를 통합적으로 분석하려는 시도가 있어왔다 등 은 국내에서 사용되는 대표적인 우울증 평가도구를 소개하고 국내 임상 현장에서 우울증 환자의 진료 진단 심각도 평가 감별 선별 경과 관찰의 목적에 따라 전문가들이 선호하는 평가도구를 조사하였다는 의의가 있다 또한 등 은 국내에서 우울증 치료 장면에서 주로 사용되는 소수의 우울증 평가도구에 대해 척도의 구성과 국내 연구 동향 점수의 해석과 평가도구 사용 장면에서의 이점을 보다 자세하게 보고하였다는 의의가 있다 선행연구의 발견에서 나아가 본 연구는 한국형 우울 선별도구를 개발하기 위한 첫 단계로 진행된 연구이므로 우울증 평가도구 개발의 관점에서 우울증 평가도구가 측정하는 구성 개념을 정리하고 국내에서 시행된 표준화 연구에서 진단적 타당성을 보고한 연구를 중점으로 고찰하고자 한다 즉 본 연구는 국내에서 사용중인 우울증의 전문가 면담 평가도구와 자기보고식 평가도구의 속성을 고찰하고 의 주요 우울장애 진단 영역을 기준으로 하여 단일한 분석 틀을 구성 각 평가도구가 측정하는 각기 다른 조작적 정의의 영역을 동일한 기준으로 분류하고자 한다 더불어 각 평가도 구의 국내 대상 진단적 효용성을 분석한 선행 연구를 대상으 로 민감도 및 특이도를 산출한 과정의 타당성을 분석하였다 이를 통해 임상 현장에서 주로 사용되는 우울 평가도구의 효 용성과 정확도를 가늠하는 양질의 근거를 제공하고자 한다 마지막으로 국내 정신건강 의료 현장에서 일하고 있는 전문 가 집단을 대상으로 국내 우울증 평가도구의 한계점을 보완 하고 효과적으로 한국인의 우울증을 선별해 낼 수 있는 한국 형 우울증 선별도구 개발을 위한 전문가 의견을 수집하였다 본 예비연구는 한국형 우울증 선별도구 이하 개발의 년차 성과이며 추후 진행될 본 검사 개발의 초석이 될 것으로 기대한다 단계 1 : 국내우울증평가도구의측정영역및진단적타당성고찰 첫번째로 국내에서 주로 사용되는 우울증 평가도구를 선 정하였다 도구의 선정은 우울증을 측정하는 대표적인 임상 가 면담 우울증 평가도구인 해밀턴 우울 평가 척도 이하 몽고메리 아스버그 우 울 평가 척도 이하 와 자기보고식 우울증 평가도구인 벡 우울 검 사 판 이하 역학연 구센터 우울척도 - 이하 환자건강질문지 이하 자가평가 우울 척도 이하 병원 불안 우울 척도 이하 기분 및 불안 증상 질문지 - 이하 에딘버러 산후 우울 척도 이하 노인 우울 척도 이하 를 대상으로 하였다 한편 국내 임상 및 지역사회 연구 현장에 서 사용되는 평가도구는 주로 국외에서 개발된 도구이므로 저작권 및 문화적 차이를 이유로 국내에서도 자체적인 개발 을 시도한 선행 연구가 있다 이에 국내에서 개발된 자기보고 식 우울증 평가도구인 한국 우울증 척도 이하 신 우울불안 척도 - 이하 한국형 노인 우울 검사 이하 를 선정하였다 두번째로 선정된 우울증 평가도구가 측정하는 구성개념을 에서 명시한 주요우울장애 진단기준의 주요 증상으 54 Anxiety and Mood Volume 13, No 2 October, 2017

3 정수연등 로 분류하여 각각 어떤 하위 차원을 측정하거나 측정하지 못 하는지 여부를 확인하여 정리하였다 그리고 선정된 우울증 평가도구의 진단적 타당성 고찰을 위해 국내 논문 검 색을 통해 선정된 평가도구의 표준화 연구 중 진단적 타당성 정보를 명시한 연구를 선정하여 표로 정리하였다 각 평가도 구의 타당화 논문에서 밝혀진 연구 대상 일반인과 임상군 과 준거타당도를 검증하는데 사용된 준거 검사 검사 도구의 판 별력 정보 민감도 특이도 절단점 진단적 타당성 를 정리하 였다 그 결과 총 개의 평가도구에 대한 개의 연구 논문 이 우울증 평가도구의 판별력 정보를 제공하여 이를 정리하 였다 단계 2 : 국내우울증평가도구의평가영역및진단적타당성고찰 기존 평가도구들이 측정한 하위 영역 범위 타당화 과정의 엄정성 그리고 진단적 타당성 검증 여부를 중점으로 논의한 뒤 이를 바탕으로 한국형 우울증 선별도구를 새롭게 개발하 는 단계에서 얻을 수 있던 함의를 제언하고자 한다 기존평가도구측정영역범위 의 주요우울장애는 진단을 위해 우울 기분 흥미 저하 체중 변화 수면 변화 정신운동성 초조 혹은 지연 에너 지 감소 무가치감 혹은 죄책감 주의집중력 감소 자살 사고 및 시도 중 가지 이상의 진단기준을 충족해야 하며 충족된 기준 중 하나는 반드시 우울기분과 흥미저하 중 하나여야 한다 그리고 이러한 증상이 개인의 사회 직업 일상 생활의 기능을 현저하게 손상하여 고통을 초래해야 한다 더불어 주요우울장애의 진단을 위해서는 조증 혹은 경조증 삽화의 이력이 없어야 한다는 조건이 있다 이러한 진단 기준을 각각 하위 차원으로 명명하여 기존 우울증 평가도구의 측정 영역과 얼마나 겹쳐지는지 확인하였다 그 결과 우울증 평가 도구 중 가장 많은 하위차원을 측정할 수 있는 평가도구는 이었다 그러나 은 조증 이력 여부를 평가하는 문항이 없다는 점이 한계로 판단되었다 등 의 국내 우울증 평가도구 분석 연구에서 다양한 정신건강 장면에서 선별을 목적으로 하였을 때 가장 선호하던 평가도구인 의 경우 주요 증상기준 개를 측정할 수 있지만 사회 직업 일상생활 기능손상 영역과 조증 이력 영역을 평가하는 문항이 부재한 점이 눈에 띄었다 연구방법의엄정성및진단적타당성검증기존 우울증 평가도구의 타당화 연구는 내적 일치도 분석 다른 척도와의 상관 분석 요인분석 진단적 타당성 혹은 판별분석을 주요 분석 방법으로 채택하였다 대부분의 연구에서 환자군 규명 기준을 진단 체계 등 를 기준으로 한 정신건강의학과 전문의의 진단으로 하였는데 이 과정에서 타당화하고자 하는 척도의 점수를 알지 못하는 전문의가 진단을 내리는 이중맹 검사 처치를 한 연구는 조사 대상이 된 표준화 연구 중 건에 그쳤다 이중 Table 1. 4 국내우울증평가도구타당화연구의체계적고찰 맹 검사는 척도 타당화 연구에서 엄정한 연구 설계를 위해 사용되는 기법이다 또한 본 연구진이 개발하고자 하는 한국형 우울증 선별도구는 일차 선별을 주 목적으로 하며 측정 도구의 진단적 타당성 민감도 및 특이도의 개선을 주안점으로 두고 있다 고찰의 대상이 되었던 개 평가도구 중 국내 타당화 연구에서 진 단적 타당성 혹은 민감도 특이도가 보고된 평가도구는 개 이며 관련 연구 논문은 건이었다 각 논문에서 제시한 민감도 및 특이도는 수준으로 다양하였다 이 중 민감도 및 특이도가 모두 이상인 척도는 와 이다 의 경우 임상군의 샘플 사이즈가 명으로 Table 2. 5 정수연등 매우 작으며 지표 검사 또한 정신건강의학과 전문의가 를 기준으로 진단하였으나 이중맹검이 아니라는 한계점이 있었다 의 경우 척도 시행 결과를 모르는 전문의의 진 단으로 지표 검사를 시행하였지만 주요우울장애군 환자 수 가 명으로 매우 적다는 한계점이 있었다 단계 3 : 국내우울증평가도구사용실태에대한전문가견해수집 국내에서 다양한 장면에서 사용되는 우울증 평가도구의 사용 실태에 대한 전문가 견해를 수집하기 위해 전문가 집단 을 구성하여 회의를 개최하였다 전문가 집단은 임상심리전문 가 명 정신과 전문의 명 측정심리 전문가 명으로 구성된 본 연구진과 외부 자문단 정신과 전문의 명 임상심리 전문 가 명 심리측정 전문가 명 으로 구성되었다 기존 평가도구 의 한계점 및 보완점에 대해 고찰하고 이를 바탕으로 본 연 구진이 제작하고자 하는 한국형 우울증 선별도구 개발의 의 미와 지향점 최종 평가도구의 구체적 결정 사항을 중점으로 토의가 이루어졌고 자세한 내용은 에 정리하였다 전문가 자문단의 의견을 토대로 본 연구진은 한국형 우울 선별도구의 용도를 임상 장면에서의 자세한 진단보다는 일차 의료현장 등에서의 간편하고도 정교한 선별에 방점을 두어 개발하는 것에 대해서 의견을 모았다 이를 위해 주요우울장애의 선별을 위해서는 에 명시된 아홉 가지 핵심 진단기준을 모두 포괄할 뿐만 아니라 수검자의 신체적 호소 불안장애 양극성 장애와의 감별을 위해 신체적 증상 불안 증상 조증 증상 등의 구성개념에 대한 질문이 평가도구에 포함되는 방안에 대해서 검토하였다 또한 측정하고자 하는 증상의 기간 단서 문항의 앵커링 채점 방식 문항의 수량 등의 심리측정적 속성을 기존 우울증 평가도구를 참고하여 결정하였고 최종 결과물을 배급하고 활용하는 과정에서 예측하고 고려해야 할 지점을 검토하였다 이를 바탕으로 본 연구진이 개발한 한국형 우울증 선별도구 예비문항의 예시를 에 정리하였다 결론 전통적으로 주요우울장애는 정신질환 평가 및 치료 장면에서 임상 전문가 의료진 보건 정책 담당자 등 다양한 구성원이 주목하는 정신질환이다 우울 환자의 높은 유병률과 질환으로 인한 사회적 비용을 고려하였을 때 주요우울장애의 선별에 대한 임상 현장과 학계의 관심이 높다는 점은 고무적이다 기존 우울증 평가도구의 체계적인 고찰과 전문가 집단의 견해를 바탕으로 본 연구진은 한국형 우울증 선별도구 Table 3. Summary of Recommendations by Expert Committee for the Korean Depression Screening Tool (K-DEP) The main purpose of -0 The K-DEP development team needs to define the main purpose of the rating tool in order to screen development of K-DEP out latent patients with depression related disorders or give diagnostic suggestions. -0 The committee recommends to set the main goal of the K-DEP as a primary screening tool, which gives latent depression patients a chance to get proper psychiatric services. -0 Time/Cost efficiency : The K-DEP needs to be short and cost-effective enough to simply rate the depression in the primary medical setting or local community setting for screening. -0 It would be critical to retain sufficient examinee size to be survived as a useful screening tool in Korean clinical and public health settings within the first several years from the distribution of K-DEP. Distribution of the K-DEP -0 It is needed to consider both the universal criteria to capture diagnostic depression and unique properties of Korean depression. in various settings -0 The item-driven scoring pattern will give us the information of how Korean depression looks like. Define what the Korean -0 Domains : To check whether the examinee has had any manic episodes, it would be recommended screening tool means to add items rating manic symptoms. -0 Anchoring : The united anchoring system would be recommended, as the people with depression tend to show the diminished ability to concentrate. -0 Item numbers : Questions of K-DEP driven by the item response theory will assess the depression with the minimum number of items. -0 Sensitivity/Specificity : The K-DEP is needed to be improved the diagnostic efficiency better than the existing screening tools. The psychometric -0 The K-DEP will be expected to be the successful replacement of the existing depression rating scales properties of K-DEP which have a royalty issue, as the K-DEP will be a free test for public health settings. The Final product of -0 Based on the bi-factor model, the compulsory items, which function as linking items between onlineoffline tests, will be included in both on-line and off-line version of K-DEP K-DEP.
