The term C3 glomerulopathy is often used to include C3GN and dense deposit disease (DDD), CFHR5 nephropathy, those of which result from dysregulation of the alternative pathway of complement. C3GN shares some aspects of atypical hemolytic uremic syndrome,, late stage of post infectious glomerulonephritis and other glomerulonephrtis. When C3GN is considered, measurement of serum complement proteins including C3, CFH, CFI, CFB and testing for the presence of C3 nephritic factor, anti-factor H autoantibodies are necessary. To screening for mutations, genes that encode complement regulators should be evaluated. This disorder equally affected all ages, both genders, and typically presented with hematuria and proteinuria. In both the short and long term, renal function remained stable in the majority of patients. Key words: C3 glomerulonephritis, C3 glomerulonephropathy, alternative complement pathway, pathology 서론 This is an open-access article distributed under the terms of the Creative Commons Attribution Non-Commercial License ( creativecom mons.org/licenses/bync/3.0/) which permits unrestricted non-commercial use, distribution, and reproduction in any medium, provided the original work is properly cited. 2007년 Servais 등[1]은, 용혈성 요독 증후군(hemolytic uremic syndrome, HUS)과 같은 유전적 소인을 갖는 사구체 신염 중, 일반적인 HUS와는 달리, 대체보체경로(alternative complement pathway)가 활성화되고, 사구체에 보 체3 (C3)이 침착되는 새로운 사구체 신염을 수집 정리하였다. 광학현미경적 변화보다는, 면역형광현미경 상 C3의 침착이 특징임으로, C3 사구체신염(C3 glomerulonephritis, C3GN)으로 명명하였다. 그러나 기존 사구체 질환에서 도, C3만 침착되는 비전형적인 예들이 있어서, 이들을 C3GN과 감별하여 재 분류하고 있다. 본 논문에서는 지금까지 발표된 문헌들을 정리하여, C3GN의 Copyright 2013 The Korean Society of Pediatric Nephrology

C3GN의 정의 Servais 등[1]은 면역형광 현미경 검사에서 C3만 강하 게 염색되는 것을 특징으로 분류하기를 제안하였다. 따라 서 다른 면역 글로불린이나 보체1q 혹은 보체4 등의 침착 은 관찰되지 않아야 한다. 광학 현미경 관찰에서는 사구 체 세포의 증식이 있는 경우도 있고 없는 경우도 있으며, 전자현미경으로는 전자밀도가 높은 물질(electron dense deposit, EDD)의 침착이 메산지움에 혹은 내피세포 밑으 로 관찰되며, 두 군데 모두에서 관찰되기도 한다. C3GN의 형태학적 특징과 감별점 사구체 세포의 증식이 있는 경우와 없는 경우로 나눌 수 있다. 증식이 있는 형에서는 기존 막증식성 사구체신 염(membranoproliferative glomerulonephritis, ) 과 유사한 소견을 보이며(Fig. 1), 반면 증식이 없는 C3GN 는 HUS, 병리학적으로는 혈전미세혈관병증(thrombotic microangiopathy, TMA)와 유사하다(Fig. 2). 즉 C3GN는 형태적으로 HUS와 제 2형 (dense deposit disease, DDD)의 양극 사이에 존재한다고 말 할 수 있다(Fig. 3). 또한 증식이 있는 C3GN의 경우는, DDD의 특징인 C3 nephritic factor의 발견 빈도가 높았으며, 증식이 없는 경 우는 HUS에서처럼, factor H, factor I, MCP(membrane cofactor protein) 돌연변이 등이 발견되는 확률이 높았다 [1, 2]. 이와 같이 광학현미경적으로 전형적인 의 모 습을 보이더라도 면역형광 현미경 관찰에서 면역글로불린 의 침착은 없고, C3만 양성인 경우, C3GN를 의심해야 하 며, 임상적으로 정형 혹은 비정형 HUS와 유사하면서 신장 조직검사에서 사구체에 세포의 증식은 없으나, 면역형광현 미경에서 C3가 강하게 침착된다면 역시 C3GN를 의심해 보 아야겠다. Servais 등[1]이 HUS와 유사한 기전의 사구체 질환에서 C3GN를 구분한 경우라면, Sethi 등[3-7]은 의 비 특이적 병리소견에서 C3GN를 분리하는 연구들을 발표하 였다(Fig. 4). 은 사구체 세포의 증식이 있고, EDD 가 메산지움 및 내피세포 아래로 침착하는 경우를 1형, 사 구체 기저막 내부에 리본 형태로 침착하는 경우를 2형으 로 분류한다[8]. 1형을 좀 더 자세히 보면, 면역글로불린 G 의 침착을 특징으로 하며, C3의 침착도 보인다. 사구체 세 Fig. 1. Those are in between in C3GN, Modified from fig 3 in reference [1]. 포의 증식이 심하여 사구체 소엽이 강조되어 있고(lobular accentuation), 기저막이 두층으로 갈라지면서 (doubling) 메산지움이 밀려들어가는 현상들이 보인다. 혈청 C3와 C4 의 감소가 있어서 전형적 보체경로(classic pathway)의 활성 화에 의한 병변이다. 반면 2형 는 1형에 비해 세포의 증식이 적으며, 따라서 소엽의 강조도 뚜렷하지 않다. EDD 가 기저막을 따라서만 침착되지만 기저막이 두꺼워 지지는 않기에, 막증식성 이라고 부르기에는 다소 미흡하여, 주로 dense deposit disease로 통용되고 있다. 또한 1형과 결정적 으로 다른 점은 면역 글로불린의 침착이 없고 C3 의 침착만 관찰된다. 혈청 C3의 감소만 있어서 대체보체경로의 활성화 에 의한 것으로 생각한다. Sethi 등[3,4]은 의 분류 기 준을 보체 활성화 측면 으로 접근하여 1형 은 전형적 보체경로 활성화 질환으로, 2형은 대체보체경로 활성화 질 환으로 분류하였다. 대체보체경로 활성화 질환은 혈청 C3의 지속적인 감소를 특징으로 하며, 대표적인 질환이 DDD 이 지만 기저막이 아닌 메산지움 혹은 내피세포 아래로만 EDD 가 있는 변형도 있음을 강조하고 이를 C3GN로 분류하였다. 즉 형태와 상관없이 C3의 사구체 침착을 특징으로 하는 모 든 사구체 질환은 C3GN에 속한다는 주장이다. 이런 관점에 서 이들 모두를 대체보체경로 신질환(alterantive pathway glomerular disease)라고 부르는 것이 더 좋을 지도 모른다. 이상의 설명으로, C3GN의 개념이 없던 시기에는 제 1형 형태를 취하면서 면역 글로불린의 침착이 없는 경우 에도 1형 으로 진단되었고[5], HUS에 의한 신염은 면 역글로불린은 물론 보체의 침착도 없어야 하는데도 C3의 침 착이 있는 경우도 있었지만 이들 대부분이 비정형 HUS 신염 으로 분류되었을 것이다. 또한 감염후 사구체신염은 면역글 로불린 G와 C3의 침착이 특징이나, 말기에는 세포의 증식도 점차 줄어지면서, 면역글로불린의 침착 정도도 점차 적어지

C3신염(C3 glomerulonephritis)과 C3신증 (C3 glomerulopathy)의 구분 기존의 사구체신염들 중에 C3GN에 합당한 질환들은 DDD, 제 3형, CFHR5 신증(complement factor H related protein 5), 감염후 사구체신염의 일부 등이 있다. 이들은 면역글로불린의 침착이 없고 C3만 양성인 경우로 서 C3GN의 조건에 합당한 질환이지만, 원인이 분명하고, 형태적으로도 구분되는 특징이 있다. 따라서 이상의 경우 는 기존의 병명을 유지하도록 하지만 C3GN와 많은 부분에 서 일치함으로, C3신증으로 분류하여 C3GN와 구분하기로 하였다[9]. C3GN의 병리기전 이 질환의 병리기전은 대체보체경로의 조절이상임이 여 러 연구자들에게서 증명되었다[1]. 보체 H 인자(complement factor H, CFH) 결핍, C3 nephritic factor, CFH, CFI, CMP 의 돌연변이, 항 CFH 항체 등이 보체 조절이상의 원인으 로 인정하였다[1]. de Cordoba 등[11]은 CFH의 결핍이 DDD를 유발하며, 비전형 HUS에는 이 요소의 결핍이 없 음을 밝혔으며, CFH 돌연변의를 유도하여 만든 DDD 실험 동물에서 혈청 C3가 정상으로 돌아오면 형태적으로도 비 전형 HUS와 비슷한 모양으로 변한다는 것을 증명되었다. Pickering 등[12]은 Cyprus에서 발견된 CFHR5 신증에서 hybrid CFHR3-1 유전자가 이와 같은 가족형 C3GN를 만 든다는 것을 증명하였다. C3GN의 임상양상 아직 발표된 예들이 많지 않아서 일반화 할 수는 없지만 Sethi 등[13]이 12명의 환자 분석에 의하면 특별한 호발 연 령이 없고, 남녀 성별차도 없었다. 대부분의 환자에서 고혈 압, 단백뇨, 혈뇨를 보였으며 신장 기능이상은 심하지는 않 았다. 그러나 3명은 신기능 부전에 빠졌으며, 이식을 받은 두 환자에서 재발이 생겼다. 이는 Servais 등[1]의 19명 환 자의 경우와도 비슷한 결과였다. 치료는 ACE 길항제 등의 대증적이었으며, C5 항체인 eculizumab으로 치유된 보고 도 있다[14].

5 Kim YJ: Pathology of C3 Glomerulonephritis 5 요약 사구체에 면역글로불린은 음성이면서 C3 만 침착되는 경 우, 다른 원인이 없는 경우 C3GN을 의심하여야 한다. 광 학현미경으로는 세포의 증식이 있는 경우와 없는 경우가 있지만, 전자현미경으로는 EDD가 내피세포 밑으로 혹은 메산지움 등에 침착함을 확인하여야 한다. 대체보체경로 의 조절이상이 원인임으로 혈청 C3 치를 비롯하여 CFH, CFI, CFB 등을 측정하여야 하며, C3 nephritic factor, antifactor H 자가항체 등을 조사하는 것이 필요하다. 보체 조 절이상을 일으키는 유전자 돌연변이를 조사해 보아야 한 다. 예후는 안정적이지만, 치료는 대증적이다. 이론적으로 보체의 활성화를 저지하는 약으로 가능할 것으로 생각되 며, C5 항체인 eculizumab으로 치유된 보고도 있다.
