We investigated the diagnostic utility of PCT for the patients with suspicious sepsis, and compared it with that of C-reactive protein (CRP) and WBC counts. Subject & Methods:The serum PCT levels, as well as CRP levels and WBC counts, were simultaneously measured in 595 blood samples from 491 patients with clinically suspicious sepsis and 17 healthy controls. Their diagnostic accuracies were compared with ROC curves. Results:In patients with positive and negative blood culture, the mean levels of PCT were ± ng/ml and 3.79 ± ng/ml, and the mean levels of CRP were 14.6 ± 9.9 mg/ml and 11.4 ± 10.0 mg/ml, respectively. The PCT and CRP value of the positive blood culture group were significantly higher than those of the negative blood culture group (P=0.001 and P=0.012, respectively). Furthermore, the PCT level of gram-negative bacteria group was significantly higher than that of gram-positive bacteria groups (P 0.001). The areas under the ROC curves (AUC) for PCT and CRP were (95% CI, ) and (95% CI, ), respectively, and showed a significant difference (P=0.022). Conclusions:The diagnostic and follow-up accuracy of PCT is higher than those of CRP as a marker for sepsis and bacteremia in the patients with clinically suspicious sepsis. Key words : Procalcitonin (PCT), C-reactive protein (CRP), bacteremia, sepsis 서론 감염성병원체가인체에들어오면인체는방어기전으로국소적인염증반응을일으킨다. 이런국소적인염증반응에도불구하고감염성병원체나병원체에의해생성되는내독소와같은물질이혈관내로도달하면전신적인염증반응이일어나게된다. 선천성면역계의자극에의한전신적인염증반응의결과로혈류내로 interleukin-6 (IL-6) 와 tumor necrosis factor-α (TNFα) 등의전염증성사이토카인이분비되고, 호중구와단핵구등의면역세포들이활성화된다 [1-3]. 전신성염증반응증후군 (systemic infla- mmatory response syndrome, SIRS) 은임상적으로체온, 심박수, 호흡수, 백혈구등의변화를동반하며, 여기에감염이입증된경우패혈증으로진단할수있다 [1, 3]. 패혈증은각종장기부전을동반한중증패혈증으로진행할수있으며, 심한경우불응성저혈압을나타내는패혈증쇼크 (septic shock) 에이르게되므로치명적인결과를초래할수있다. 그러므로패혈증의진단, 중증도판정및경과관찰에빠르고예민하고특이적인표지자가요구된다. 현재까지알려진염증이나감염표지자로는백혈구수 (WBC count), C-반응성단백 (C-reactive protein, CRP), procalcitonin (PCT), IL-6과 IL-8과같은사이토카인등

CRP는가장널리이용되는급성기반응물질중의하나로민감도가높고자동화검사법으로빠르게측정할할수있어서임상경과및추적관찰에이용되고있지만세균감염외에도종양이나골절, 또는조직괴사상태등에서도비특이적으로증가한다 [4]. 최근 PCT은새로운염증및감염의표지자로주목받는물질로 calcitonin의전구체로주로갑상샘의 C세포에서생성, 분비된다. 분비조절기전이정상적인상태에서는 calcitonin 형태로혈중에존재하지만세균에의한감염이발생하면염증반응의정도에비례하여실질장기에서조절기전없이 PCT로서혈중으로분비된다 [5]. CRP는염증자극후 4-6시간부터증가하여 36시간후최고치에도달하지만, PCT는감염에의한염증자극 4시간후부터분비되기시작하여약 8시간경최고치에도달하여 24시간정도지속된다. 따라서혈중 PCT 농도의측정은감염환자를진단하는데있어 CRP보다특이적이며빠른검출이가능하다 [6, 7]. 패혈증의진단및임상경과와관련하여 PCT에대한많은연구들이있었지만실제임상검사로서의 PCT 검사는측정시간및검사방법에있어서제한점을가지고있었다 [8-10]. 최근자동화기기를사용하여검사시간을단축하고민감도가더욱향상된새로운 PCT 검사법이개발되어임상검사로서활용되고있다. 본연구에서는패혈증이의심되는환자의선별진단을위한혈중표지자로서 PCT의 유용성을평가하고자응급실및집중치료실에서패혈증이의심되어혈액배양이의뢰된환자들을대상으로 CRP와함께 PCT를동시에측정하고혈액배양결과와비교분석하였다. 대상및방법 대상 :2008년 10월부터 2009년 12월까지부산대학교병원응급실및집중치료실에서패혈증이의심되어혈액배양을의뢰하였던 491명환자의 595 혈액검체를대상으로하였다. 남자 303명, 여자 188명이었고연령범위는 16세 ~91세 ( 중앙값 63세 ) 이었다. 이들환자에서혈청 CRP와 PCT를동시에측정하였다. 건강대조군으로서건강인성인 17명을대상으로혈청 PCT 및 CRP를측정하였다. 혈청 Procalcitonin(PCP) 농도 : 효소연관형광법 (enzyme linked fluorescence assay) 을이용한 VIDAS BRAHMS PCT 시약 (biomerieux Co., Lyon, France) 을사용하여제조사의지시에따라 VIDAS 자동면역장비 (biomerieux Co., Lyon, France) 로측정하였다. 최소측정한계치는 0.05 ng/ml 이며, 결과는제조사에서제시한구간에따라건강한상태에서는 0.05 ng/ml 미만, 국소감염은 0.05~0.49 ng/ml, 패혈증의전신감염은 0.5~1.99 ng/ml, 심한패혈증은 2~9.99 ng/ml, 그리고 10 ng/ml 이상이면패혈증쇼크를시사하는결과로해석하였다. 혈청 CRP 농도 : 자동화장비 (Roche DP

4 이은엽, 박순원, 조선아 Hitachi, Basel, Switzerland) 를사용하여, Particle-enhanced immunoturbidimetric assay 법으로측정하였고, 최소측정한계치는 0.5 mg/ml 이다. 통계 : 결과분석은 SPSS 14.0 (SPSS Inc., Chicago, Illinois, USA) 프로그램을이용하여평균및표준편차를계산하였고, 혈액배양양성군과혈액배양음성군으로나누어두그룹간의 PCT와 CRP치및 WBC 수의평균값을 Mann-Whitney U-test를이용하여비교하였다. PCT와 CRP치의상관성은 Spearman 법으로분석하였고, 진단적유용성은상대수행능 (ROC) 곡선으로민감도와특이도를비교하였다. 통계적유의수준은 P <0.05를기준으로유의성을판단하였다. 결과 혈액배양결과에따른 Procalcitonin (PCT) 와 CRP, 백혈구수의비교혈액배양이의뢰된 595 검체중 70 검체에서혈액배양이양성이었으며양성률은 11.8% 이었다. 혈액배양양성군과음성군에서의 PCT의평균치와표준편차는각각 ± ng/ml과 3.79 ± ng/ml으로혈액배양양성군의 PCT 평균치가유의하게높았다 (P=0.001) (Fig. 1). CRP의평균치와 표준편차는혈액배양양성군과음성군에서각각 14.6 ± 9.9 mg/ml과 11.4 ± 10.0 mg/ml이었고, 백혈구수의평균치와표준편차는각각 ± 9.09 x 10 3 /ul와, ± 8.67 x 10 3 /ul로통계적으로유의한차이를나타냈다 (P=0.012, P=0.039) (Fig. 2, 3)(Table 1). 건강인 17명의 PCT 및 CRP는각각측정한계치 0.05 ng/ml 이하, 0.5 mg/ml 이하였으며, 백혈구수는 x 10 3 /ul로참고범위이내였다. 혈액배양음성군중에서차트분석이 135 검체에서가능하였으며이중에서임상적진단기준에따라전신성염증반응증후군 ( 이하 SIRS 군 ) 으로추정되는군이 56 검체이었고, 진단기준에일치하지않는군이 79 검체 ( 이하 non-sirs 군 ) 이었다. 두군간에 PCT와 CRP 및백혈구수의평균치 ( 표준편차 ) 를비교하였을때, SIRS군과 non- SIRS군에서 PCP의평균치는각각 6.82 ± ng/ml과 1.60 ± 4.30 ng/ml, CRP 의평균치는각각 ± mg/ml과 ± mg/ml, 그리고백혈구수의평균치는 ± 7.16 x10 3 /ul과 ± 4.31 x10 3 /ul로, SIRS군에서 non-sirs군보다 PCT, CRP, 백혈구수가유의하게높았다 (P=0.010, P=0.006, P=0.001) (Table2)

PCT; procalcitonin, CRP; C-reactive protein, WBC, white blood cells, 혈액배양의동정균종에따른 PCP와 CRP, 백혈구수의비교혈액배양결과양성인 70 검체에서총 20 종의균이동정되었고그중 54 검체는그람양성균, 16 검체는그람음성균이동정되었다. 그람양성균중에서가장흔한균은 32 검체의 coagulase-negative staphylococci (CNS) 이었고다음은 Staphylococcus aureus (N=16) 순이었다. 그람음성균중에서는 Acinetobactor baumannii (N=4) 와 Escherichia coli (N=4) 가가장많이동정되었다 (Table 3). 그람양성균과음성균에서 PCT의평균과표준편차는각각 3.64 ± 6.43 ng/ml과 ± ng/ml이었다. PCT의평균은그람양성균보다그람음성균에서높았고통계적으로유의한차이를나타내었다 (P<0.001). 그람양성균과음성균에서의 CRP치의평균과표준편차는각각 ± 9.74 mg/ml과 ± 9.91 mg/ml이었고, 백혈구수는각각 4.25 ± 9.09 x 10 3 /ul와 ± 8.67 x 10 3 /ul로서그람양성균과음성균간에유의한차이는없었다 (Table 4)

9 패혈증의증환자에서 Procalcitonin 정량검사의임상적유용성 패혈증의심환자에서 PCT와 CRP치의진단적유용성혈액배양양성환자에서 PCT과 CRP 측정의진단적유용성을알아보고자 ROC 곡선분석을시행하였다. 각표지자의임계값으로 PCT 는 0.55 ng/ml, CRP는 8.07 mg/ml를사용하였다. PCT는 0.55 ng/ml 에서민감도 (95% confidence interval, CI) 와특이도 (95% confidence interval, CI) 는각각 72.8% ( %) 와 58.7% ( %) 이었고, CRP 8.07 mg/ml에서는각각 71.4% ( %) 와 47.4% ( %) 이었다. 해당기준치에서각각의 area under the curve (AUC) (95% CI) 는 ( ) 및 ( ) 이었으며두 AUC 간의차이는 0.102로통계적으로유의한차이를나타내었다 (P=0.022) (Fig. 4). 또한 PCT치와 WBC 수의 AUC 간의차이는 0.125로서통계적으로유의한차이를나타내었다 (P=0.005). 혈액배양음성환자에서임상적으로 SIRS 의심군에서의 PCT 측정의유용성을알아보고자 ROC 분석으로 CRP와백혈구수를비교하였을때 PCT의 AUC가 CRP의 AUC 보다는높았으나통계적으로유의한차이는보이지않았다. 10 이은엽, 박순원, 조선아 패혈증환자의치료경과관찰중 PCT 및 CRP 치의비교혈액배양결과양성인환자중 4명의환자에서항생제치료후 PCT 및 CRP를추적관찰하였다. 3~4일간격으로총 4~6회혈액배양과함께 PCT 및 CRP의측정을동시에시행하였다. 4명의환자모두에서항생제치료후시간이지날수록 PCT과 CRP 는모두감소하는경향을나타내었다 (Fig. 5). 특히항생제치료시작후추적관찰에서혈액배양결과가양성인경우 PCT는증가하는경향을나타내었다 (Fig. 5A, B and C). 백혈구수는 4명의환자모두에서항생제치료후시간이지날수록특정한양상을보이지는않았지만전반적으로감소하는경향을나타내었다. 11 패혈증의증환자에서 Procalcitonin 정량검사의임상적유용성 고찰 염증반응은시간경과에따른일련의과정으로서국소적으로시작되어혈액내의각종매개물질들에의해전신적인영향을나타낸다. 염증반응이일어나면수분이내에호중구와단핵구가동원되어 IL-6와 TNF -α 같은전염증성사이토카인 (cytokine) 들이분비되고 4~6시간이내에 procalcitonin과 CRP가분비된다 [11, 12]. 염증과관련되어나타나는발적, 부종, 동통, 열감및기능장애와같은증상및징후들에대한판단은주관적이며이상상황에따라서약화되거나나타나지않을수도있다 [13]. 그러므로민감도와특이도가높은객관적인염증및감염의표지자들의중요성이대두되고있다. Procalcitonin (PCT) 는 1993년 Assicot 등에의해혈중 PCT 농도와세균감염과의연관성이보고된후부터 [5] 여러연구에서전신성염증반응및패혈증의검출, 감별및추적에이용될수있는특이적인염증및감염의표지자로서제시되었다 [14-18]. 하지만여러질환들의진단및예후에대한 PCT의유용성을평가한기존의연구들은연구대상군, 임상상황, PCT 측정시점이나측정방법들의차이로인해다양한결과가보고되었다 [13]. 즉, PCT의유용성에대해서는연구결과마다차이가있으며보다예민한 PCT 측정법에대한필요성이제기되어왔다. 최근의 PCT와관련된연구에서는기능적민감도가향상된새로운측정법인 VIDAS PCT 시약이이용되고있다. [19, 20]. 이 PCT 측정법의기능적민감도는 0.09 ng/ml로제시되고있으며이전의여러연구들에서주로이용되었던면역발광측정법 (LUMItest, BRAHMS, Henningsdorf, Germany) 의기능적민감도 (0.05 ng/ml) 보다훨씬개선되어경한 PCT수치의상승도검출할수있을것으로알려져있다. 본연구에서는 PCT의진단적유용성을확인하고자패혈증이의심되어혈액배양이의뢰된환자에서새로이개발된효소연관형광검사법으로 PCT를측정하였다. 패혈증은전신성염증반응증후군 (systemic inflammatory response syndrome, SIRS) 이있으면서감염이의심되거나확인되는경우에진단할수있는데 [3, 21], 실제검사실에서미생물학적으로입증되는패혈증은전체패혈증의 60~70% 정도이고, 나머지 30~40% 의패혈증은임상적판단에따라진단이이루어진다 [3]. 본연구에서는혈액배양양성군과음성군에서 PCT과 CRP 의진단적유용성을비교해보았다. 혈액배양양성군과음성군을비교했을때 PCT와 CRP치및백혈구수는혈액배양양성군에서높았으며두군간에통계적으로유의한차이를나타냈다 (Table 1, Fig. 1, 2 and 3). 이러한결과로부터 PCT와 CRP치및백혈구수는모두염증및감염의표지자로서유용할수있을것으로생각된다. 패혈증이의심되는환자에서 PCT의진단적유용성을알아보기위하여 ROC 곡선분석으로 CRP와백혈구수와비교한결과 PCT 의 AUC가 CRP와백혈구수의 AUC보다높았으며각각표지자의 AUC 간에통계적으

12 이은엽, 박순원, 조선아 로유의한차이를나타내었다 (Fig 4). 그리고본연구의결과는혈액배양양성환자에서 PCT와 CRP의진단적유용성을비교분석했던이전연구들의결과와일치하였다 [19, 20]. 따라서, 패혈증이의심되는환자에서혈청 PCT는 CRP 또는벡혈구수와함께패혈증진단을위한감염표지자로유용할것으로생각된다. 혈액배양음성군에서 PCT치가 50 ng/ml 이상으로매우높게증가하는경우들이있어서임상적진단기준에따라전신성염증반응증후군으로진단된환자 (SIRS) 와아닌환자 (non-sirs) 들을구분하여비교하였는데, SIRS군에서 PCT와 CRP치및백혈구수가 non-sirs 군보다유의하게높았다 (Table 2). 이는 PCT와 CRP치및백혈구수를포함한결과치와임상적으로패혈증이의심되지만혈액배양이음성인경우는혈액내로균이침범하지않고신체일부의염증반응및염증물질의생성에의해서발생한패혈증이포함될수있다고생각된다. 그러므로혈액배양음성 SIRS 환자에서 PCT를패혈증진단의지표로이용하기위해서는민감도와특이도가높은진단적기준치설정을위한연구가더필요할것으로생각된다. 혈액배양양성군에서그람염색에따른 PCT와 CRP치및백혈구수를비교한결과그람음성균의환자군에서그람양성균의환자군보다 PCT치가 3배이상높게나타났으며통계적으로유의한차이를나타내었다 (Mann-Whitney test, P<0.001). 이전연구들에서도그람음성균에의한균혈증에서 PCT가높은결과치를나타내었다 [20, 22]. 따라서혈액배양그람양성균에비해그람음성균환자에의한균혈증에서 PCT 치가더높게증가함을확인할수있었다. 또한임상적으로패혈증이의심되는환자중에서혈액배양결과가나오기전에경험적항생제치료후질환의추적관찰에있어서 PCT의유용성을알아보기위해서 CRP 와비교한결과, 항생제치료후 PCT와 CRP치가모두감소하는경향을나타내었다 (Fig. 5). 특히, 항생제치료시작후추적관찰에서혈액배양결과가양성인경우 CRP는감소되는경우가있었지만 PCT는감소되지않고증가된소견을보였다 (Fig. 5A, B and C). 즉, PCT가 CRP보다혈액배양결과를더민감하게반영하므로 PCT 는패혈증의심환자의치료및임상경과의추적에이용할수있을것이다. 결론적으로 PCT는 CRP에비해민감도와특이도가높은감염표지자의하나이므로패혈증진단에유용할것으로생각된다. 그러나감염성질환의치료및임상경과의추적관찰에있어서 PCT의임상적유용성에대하여는보다더많은연구가필요할것이다. 요약 배경 : 패혈증은발생초기정확한진단과적절한치료를하지않는경우발병후짧은시간내에사망에이를수있다. PCT과 CRP농도는각각 VIDAS BRAHMS PCT kit (biomerieux, Marcy l'etoile, France) 와 Particle enhanced immunoturbidimetric assay (Roche DP-Hitachi, Basel, Switzerland) 를사용하여측정하였다. 그리고상대수행능곡선 (ROC curve) 을구하여 PCT과 CRP의진단적유용성을비교하였다. 결과 :PCT의평균치는혈액배양양성군 (10.51 ± ng/ml) 에서혈액배양음성군 (3.79 ± ng/ml) 보다상당히높게나타났다. CRP의평균치역시혈액배양양성군 (14.62 ± 9.91 mg/ml) 에서혈액배양음성군 (11.41 ± 9.99 mg/ml) 보다높게나타났다. 혈액배양양성군과음성군간 PCT (P=0.001) 과 CRP (P=0.012) 의평균치는각각통계적으로유의한차이가있었다. 혈액배양양성군에서그람염색양성균 (3.64 ±6.43 ng/ml) 과음성균 (10.51 ± ng/ml) 간 PCT의평균치는유의한차이가있었다 (P<0.001). PCT과 CRP의상대수행능곡선의아래면적 (95% 신뢰구간 ) 은 ( ) 과 ( ) 로유의한차이가있었다 (P=0.022). 결론 : 패혈증의심환자의신속한진단과경과관찰을위하여수일이소요되는혈액배양검사결과를기다리기전균혈증또는패혈증을판단하는데 PCT 검사가 CRP 검사보다더유용할것으로생각된다. 참고문헌 1.
