Methods: Forty S. epidermidis blood isolates, including 19 pathogens, 11 colonizers, and 1 contaminants, were tested. Biofilm production was determined in 96-well polystyrene microtiter plates using crystal violet staining. Relative units (ru), given by the optical density (OD) of the clinical isolates divided by the OD of a standard biofilm-negative strain, were used for quantification. Polymerase chain reactions for the icaabcd, atle, and aap genes were performed to evaluate biofilm-related genes. The diagnostic accuracy of biofilm-related genes was assessed using the sensitivity, specificity, and positive predictive value. Results: Biofilm production of the catheter-related isolates, including pathogens and colonizers, was significantly greater than that of contaminants (5% vs. 2%, p=.12). For catheter-related isolates, ru was significantly greater for ica-positive isolates than for ica-negative isolates (1.72±.84 vs..94±.22; p=.3). The prevalence of atle and aap did not differ between the three groups. Seven S. epidermidis were isolated together with other microorganisms from intravascular catheters, and their biofilm production was relatively low. The positive predictive value of icaabcd was 83% for discriminating between catheter-related isolates and contaminants. Conclusions: Pathogens and colonizers had similar biofilm production capacities and prevalences of related genes, whereas catheter-related isolates and contaminants differed. The biofilm production ability and presence of these genes cannot be used for clinical decision-making regarding true pathogens. The ica operon might be useful for differentiating between contaminants and catheter-related isolates in catheter-related infection (CRI). Further studies of other related genes and protein expression are needed to assess the clinical usefulness of biofilm production for clinical isolates. 2 - Kyung-Hwa Park, et al: Biofilm formation and associated genes in staphylococcus epidermidis blood isolates - 서론 Coagulase 음성 Staphylococcus (CNS) 의일종인 Staphylococcus epidermidis 는중심정맥관사용이증가하면서중심정맥관관련감염의중요한원인균으로알려져왔다. 미국의경우 198년대이후지속적으로 CNS의분리율이증가되고있으며, 국내에서도 22년도에한대학병원에서실시한정량혈액배양을이용한중심정맥관관련균혈증의원인균분석에서 CNS는 1.4% 로 Candida species, S. aureus에이어 3 번째로흔한원인균이었다 1). 이후의중심정맥도관에서만혈액을채취, 배양하여분석한보고에서는 CNS가 33.9% 로가장흔하였다 2). 그러나 CNS는혈액배양을할때빈번하게오염되어배양되는피부상재균이기도하여혈액배양에서분리될경우임상적의의를판정하기가쉽지않다. 따라서중심정맥관의집락화를균혈증으로오인하여중심정맥관을불필요하게제거하거나항생제를사용함으로써의료비상승의요인이될수있다 3, 4). 중심정맥관을일주일이상유치하면거의모든중심정맥관의내부에는세균에의한균막이형성된다 5). 또한 Peters 등은 CNS에의한인공삽입물감염에있어 slime 생산과균막 (biofilm) 형성을통한균집락화가중요하게작용함을보고하였다 6). 따라서최근에는 CNS의균막형성과이에관련된유전자를검사함으로써중심정맥관을비롯한인공삽입물감염의발생기전을밝히고, 병원균여부를감별하는데보조적진단법으로서의유용성등에대해연구되고있다 7). 본연구에서는 Staphylococcus epidermidis에의한중심정맥관감염에서오염균과중심정맥관의집락균및병원균간에균막형성능의차이와균막형성관련유전자의보유율에차이가있는지를알아보고임상적인판단에유용한지를분석하였다. 대상및방법 1. 대상균주전남대학교병원에입원한환자의혈액배양에서동정된 Staphylococcus epidermidis 4주를대상으로본연구를수행하였다. 전신감염증상이있는환자의중심정맥관을통해채취한혈액과말초혈액에서동일한항생제감수성을갖는균이동정되고, 중심정맥관이외의다른추정감염원이없는경우를병원균 (19주) 으로정의하였다. 집락균 (11주) 은중심정맥관을통한혈액배양에서는 2회이상반복적으로동일 한항생제감수성을갖는균이분리되나말초혈액에서는동정되지않는경우로정의하였다. 오염균 (1주) 은중심정맥관이없는환자에서 2쌍이상의말초혈액배양검사중 1쌍에서만 S. epidermidis 가동정되는경우로정의하였다. S. epidermidis 가동정된환자에서임상양상과중심정맥관과의관련성등의임상데이터는의무기록지를토대로후향적으로분석하였다. 동일균이여러번배양된경우처음중심정맥에서배양된균을임상균주로하였고, 확보된균주는 -8 에서보관되었다. 혈액배양방법은말초혈액과중심정맥에서각 1 ml를채혈하여호기성및혐기성배양병 (BACTEC plus Aerobic/F 및 BACTEC plus Anaerobic/F medium 혹은 BACTEC PEDS Plus/F medium) 에각각 5 ml를넣었고, 배양병은 35 에서배양하여 BACTEC 기계에서양성으로판정될때까지혹은 5 일간배양하였다. 양성인혈액배양병은배양액일부를취하여그람염색하고계대배양하였으며 S. epidermidis 의동정에는그람염색, coagulase 검사및 Microscan (Dade Microscan Walkaway 96, West Sacramento, Calif., USA) 을이용하였다. 균막형성의대조군으로는양성표준균주로 S. epidermidis ATCC (complete genome sequence accession no. NC 2976) 와 ATCC 35983를사용하였고, 음성표준균주로는 S. epidermidis ATCC 12228을사용하였다. 2. 균막 (Biofilm) 형성실험 1) 균주배양임상균주와표준균주는 5 ml Luria-Bertani (LB, Difco, USA) 배지를이용하여 37 진탕배양기에서 24시간동안배양하였다. 균체의보존에는 LB 배지에 1.5% agar를첨가한배지를사용하여 4 에보관하였고, 일주일간격으로계대배양하였다. 2) 균막형성, 처리및정량화 S. epidermidis 의균막은이전의방법대로, 소독처리된폴리스틸렌재질의바닥이평평한 96-well 마이크로플레이트 (Corning Inc., Corning, USA) 를사용하여형성시켰고, crystal violet (CV) 염색을시행하여반정량적으로측정하는 Christensen 의방법을이용하였다 8). 각실험균주는 5 ml LB에진탕배양기에서 37, 24시간동안배양하였다. 마이크로플레이트의 1번에서 7번 well까지각 well당 Luria-Bertani (LB) 용액을 18 μl 분주하였고 24시간배양한 S. epidermidis

PCR 을통한유전자분석 1) DNA 검출균막형성에연관된유전자검출을위해서 S. epidermidis 를 LB broth에서증식시킨후 1 μl 를취하여 4, rpm에서 5분간원심분리한뒤, 침전물을증류수 1 μl 에혼합하였다. 부유액을끓는물에 1분간넣은다음 13, rpm에서 1분동안원심분리한뒤, 상층액을이용하였다. 2) PCR 반응추출한 DNA 1 μl와 primer (Bioneer, 대전, 한국 ) 각각.6 μl, 증류수 17.8 μl를 Premix 반응혼합물 (1 mm Tris-HCL (ph 9.), 1.5 mm MgCl2, 3 mm KCl, 1U Taq DNA polymerase, Bioneer, 대전, 한국 ) 에넣어잘섞었다. 유전자의 primer 는표 1과같다. 4. 통계적방법모든통계분석은 SPSS version 12. 을이용하였으며, 균막형성능및관련유전자의보유율과 S. epidermidis 의임상적의미와의연관관계를알아보기위하여연속형변수는비모수적검정을위한 Mann-Whitney U test를시행하였으며, 범주형변수는 chi-square test와비모수적검정을위한 Fisher s Exact test를시행하였다. 결과 1. 각군의임상적특징병원균과집락균을포함한중심정맥관관련균주가혈액에서동정되기까지중심정맥관의유치기간은 14.6±23.5일이었다. 병원균과집락균에서의중심정맥관유치기간은각각 8.4±5.2일과 25.4±36.7일로집락균에서유치기간이더길었으나, 유치기간과균혈증과의통계학적유의성은없었다 (p=.157). 병원균이배양된 19명의환자중 4명에서는중심정맥관에서 S. epidermidis 와함께다른균이배양되었다 ( 표 2). 2명

Microorganisms isolated together with Staphylococcus epidermidis from intravascular catheters Microorganism Candida albicans Candida parapsilosis Acinetobacter baumannii Serratia marcescens Number 의환자는 Candida albicans, 2명은 Candida parapsilosis가동정되어중심정맥관관련복합감염으로진단되었다. 집락균중 1명은 Acinetobacter baumannii, 다른 1명은 Serratia marcescens에의한중심정맥관연관혈류감염이동반되었고, 1명의환자에서는중심정맥관제거시시행한 tip 배양에서 C. albicans가동정되어 S. epidermidis 와동시에집락화가되어있었다. 2. 각균주에따른균막형성능의차이 중심정맥관과관련이있는 3 주중 15 주 (5%) 에서균막 이형성되었고, 15주 (5%) 는균막을형성하지않았다. 병원균에서는 9주 (47.4%) 에서균막을형성하였고, 집락균에서는 6주 (54.5%) 에서균막을형성하였다. 반면오염균에서는 2균주 (2%) 만이균막을형성하여중심정맥도관관련균주에비해균막형성능력이감소되어있었다 (p=.12) ( 표 3). 균막형성균주에서중심정맥관의평균유치기간은 18.8±32.8 일, 균막무형성균에서는 1.4±5.5일로균막형성과유치기간의통계학적인연관성은없었다 (p=.357). 혈액배양과 tip 배양에서 S. epidermidis 외에다른균주가동정된 7명의환자에서배양된 S. epidermidis 의 ru값은 1.18±.5이었고, Candida parapsilosis가배양된 1명을제외하면 ru는.75~1.5이었다. 3. 균막형성과관련된유전자와균막형성능의상관관계총 4균주중병원균 9주 (47.4%), 집락균 6주 (54.5%), 오염균 3주 (3%) 가 icaabcd 오페론을가지고있었다 ( 표 4). 중심정맥도관관련균주에서 icaabcd 오페론의유무에따른균막형성능력의차이를비교했을때 icaabcd 오페론을 Table 3. The role of PCR-based biofilm-associated gene detection in assessing the clinical significance Gene Colonizer versus Contaminant Colonizer versus Pathogen Pathogen versus Contaminant Pathogen & colonizer versus Contaminant Sensitivity 55% 47% 47% 5% icaabcd Specificity 7% 5% 7% 7% PPV 67% 6% 75% 83% Sensitivity 45% 63% 63% 57% aap Specificity 2% 55% 2% 2% PPV 38% 71% 6% 68% Sensitivity 91% 89% 11% 87% atle Specificity 3% 18% 3% 3% PPV 59% 65% 71% 76% PPV, Positive predictive value 가지고있는 15주의 ru는 1.72±.84였고, icaabcd 음성인 15주에서 ru는.94±.22로 icaabcd 양성군에서음성군에비해균막형성능이높았다 (p=.3). icaabcd 양성군에서표준편차가큰것은 icaabcd 양성군내에서도균막형성능의다양성을보여주나 icaabcd 음성군에서는대체적으로균막형성능력이떨어짐이관찰되었다. 오염균에서는 icaabcd 오페론에관계없이전반적으로균막형성능이저하되어있었다. 오염균 1주중 3주에서 icaabcd 오페론을가지고있었으나 1주만이 ru 값 1.72로중등도의균막이형성되었고, 다른 2주는균막이형성되지않았다. atle 유전자는병원균은 17주 (89.5%), 집락균은 1주 (9.9%), 오염균은 7주 (7%) 에서양성이었다. aap 유전자는병원균은 12주 (63.2%), 집락균은 5주 (45.5%), 오염균은 8주 (8%) 에서양성이었다. 중심정맥관관련균주중 icaabcd 양성군에서 aap와 atle를모두가진균주는 7균주였고, ru는 2.34±.65였다. icaabcd 양성군에서 atle는음성이나 aap가양성인균의 ru는 2.5였으나, 반대로 aap는음성이나 atle 가양성인균의 ru는.88로낮았다. icaabcd 음성군에서는 aap, atle 유무와상관없이낮은 ru값을보였다. 4. 균막형성유전자의진단적유용성중심정맥관관련감염을진단하는데 icaabcd와 aap, atle 유전자의유용성을보기위하여민감도, 특이도및양성예측도를조사하였다 ( 표 5). 중심정맥관관련균주와오염균을구분하는데 icaabcd가 83% 로비교적좋은양성예측도를보였으나민감도와특이도는각각 5% 와 7% 를보였다. 병원균과집락균을감별하는데 atle이민감하였지만 특이도와양성예측도가낮았고 icaabcd, aap도특이도가낮았다. 고찰중심정맥도관이정맥로확보가어려운만성질환자와중증환자에게다양한목적으로이용되면서이와관련된도관감염도증가하고있다 9). Thylefors 등의 1998년보고에의하면 medline 데이터베이스에서 198년부터 1998년까지의혈류감염의원인균을분석한결과 CNS가가장흔한혈액배양의오염균이지만, 실제 CNS에의한혈류감염의분율이의미있게증가하고있고진성균혈증의발생률도증가하고있다고보고되었다 1). 병독성이강한 S. aureus와는반대로 S. epidermidis 는인체피부에흔히상재하는균으로인공기구가삽입된경우에기회감염균으로병독성이발현되고병원내전파가증가하면서감염빈도가높아지고있다 11). 본연구에서는중심정맥도관과관련이있는병원균과집락균에서오염균에비해균막형성능이증가되어 S. epidermidis 의병독성에균막이관여한다는것을뒷받침해준다. 이러한결과는혈류감염균에서피부상재균에비해균막형성이증가되어있음을증명한기존에연구결과와일치하며이는중심정맥도관관련균주의기원이피부상재균과다를것임을암시한다 12). 인공삽입물과연관되어발생되는감염에서균은숙주의단백질과미생물의삼출물로이루어진기질안에많은감염균들의균막 (biofilm) 을형성한다. 균막은 quorum sensing 이라불리는세포사이의신호기전에의해생성되며, 부착된미생물들은기능적공동체를구성하고, 탐식세포, 항체, 보체등
6 - Kyung-Hwa Park, et al: Biofilm formation and associated genes in staphylococcus epidermidis blood isolates - 의숙주방어기전과항생제에대한저항성을갖는다 13, 14). 이러한균막은균이인공삽입물에접촉후몇초이내에시작하는부착과부착단계후다층화된세포집락이점액성의매트릭스를분비하여둘러싸이는축적, 이후지속적인감염이확립되는 3단계의과정을거쳐형성된다 15). S. epidermidis 에서각각의과정에관계하는여러단백질이알려져있는데, 부착의단계에는 AtlE, fimbria-like SSP-1,2, PS/A (capsular polysaccharide adhesion) 등이관여하고축적과정에는 PIA (polysaccharide intercellular adhesion) 와 AAP (accumulationassociated protein) 가중요하다고알려져있다 7). AtlE는 atle 에의해서 PS/A와 PIA는 ica 오페론에의해서 AAP는 aap에의해서발현되는단백질로본실험에서는 ica 오페론, atle, aap의세개의유전자에대해조사하였다. 본연구에서중심정맥도관관련주에서 ica 오페론양성균이균막을더잘형성되고 ica 오페론음성균은균막을거의형성되지않음을확인할수있었다. 이전의문헌에의하면 ica 오페론변이균주는폴리스티렌에서균막형성능력이현저히떨어져양성균주보다중심정맥관관련감염이발생할가능성은낮다고하여본연구와비슷한결과를보여주었다 16). 또한 ica 오페론을가진균주가음성균주보다중심정맥관등인공삽입물에경쟁적으로작용하여더집락화가잘된다고알려져있고중심정맥관에집락화된균의 8% 이상에서 ica 오페론은빈번히발견된다고보고하였다 17). 본연구에서오염균에서 ica 오페론보유율은 3% 로다른논문의 3~5% 와비슷하였으나중심정맥도관균에서는 5% 로외국보고보다낮은보유율을보였다 17, 18). 이는전신감염증상이있는환자의중심정맥도관혈액배양과말초혈액배양에서동일한항생제감수성을갖는균이동정되면서다른감염의증거가없는경우를병원균, 말초혈액에서는배양되지않으면서중심정맥도관에서 2회이상반복적으로동일한항생제감수성을갖는균이배양되었을때집락균으로대상균주를선택하여오염균일가능성을낮췄지만후향적인자료조사이기에표본바이어스가있을것으로생각된다. 또한, 혈액에서연속분리된 S. epidermidis 균주일지라도 PFGE (pulsed field gel electrophoresis) 등으로확인하였을때분리일의간격에따라상당수환자에서서로다른균으로증명되는경우가있어실험을위해선택된균주가병원균이아닐가능성을배제할수없을것으로생각된다 19). 반면오염균은유전자와관계없이균막형성이저하됨을알수있었다. 아마도오염균에서는 ica 오페론등의관련유전자의단백질발현이저하되어있을가능성이있으며추후에 PIA, PS/A 등의단백질발현이떨어져있는지를확인하는과정이필요하겠다. 균막형성과관련된유전자의진단적유용성에있어오염균과중심정맥도관관련균에서 ica 오페론의양성예측도는높았으나특이도와민감도는높지않았고 aap와 atle는양성예측도, 민감도, 특이도모두높지않았다. 결과적으로, 혈액에서 S. epidermidis 가배양된경우균막형성에관련된유전자를검사하는것이임상적판단에큰도움이되지는않는다. 그러나 ica 오페론을포함한유전자의낮은보유율이관련유전자의감별진단유용성을떨어뜨렸을수있고전향적으로더많은수의임상균주와자료를모아 PFGE 등의역학적분석을통하여실제동일균인지확인을하는과정을거친후집락균과병원균을정확히나누어분자역학적차이를알아볼필요가있겠다. 인공삽입물과관련된감염은일반적으로는한가지균에의한것이대부분이지만여러균에의한복합감염도보고되었다 2). 본연구에서중심정맥관을보유한환자 3명중 7명의환자에서 S. epidermidis 가중심정맥관에서동정될때동시에혹은짧은간격으로다른균이중심정맥관에서배양되었다. 복합감염이있는 7명에서 S. epidermidis 의균막형성은 ru는 1.18±.5로, Candida parapsilosis가동시에동정된 1주를제외하면 ru는.75~1.5로균막형성은거의없었다. 즉, 복합감염을일으킨 S. epidermidis 는균막형성이미미함에도중심정맥관과관련하여배양되었다. 일반적으로다른종의미생물들은균막을형성할때서로유착을도와주거나혹은경쟁하면서상호작용한다. Adam 등의논문에따르면 Candida와 S. epidermidis 는생체막내에서광범위한상호작용을하는데주사전자현미경으로보았을때 Staphylococci는아래층에 Candida는위층에위치하면서더찐득찐득한매트릭스를형성한다고하였다 21). 이런소견은 slime을잘형성하지않는 S. epidermidis 에서더쉽게관찰되고두드러진소견이었다 22). 본연구에서복합감염환자에서동정된 S. epidermidis 가균막을잘형성하지않음에도중심정맥관감염이발생한이유를부분적으로설명할수있을것이다. 또한복합감염중균막형성능력이저하되어있음에도 S. epidermidis 가병원균이었던 3명의환자에서병원성이증가되는데기여하였을것으로추정된다. 결론적으로, 병원균과집락균간에균막형성능과균막형성관련유전자의보유율에는차이가없었지만오염균과는의미있게차이가있었다. 또한중심정맥관관련균주와오염균을감별하는데 icaabcd 유전자의확인이도움이될것
7 - 대한내과학회지 : 제 75 권제 4 호통권제 578 호 28 - 으로판단되나병원균과집락균을감별하는데는유용성이낮았다. 향후병원균과집락균을감별하기위해임상양상과대조를통한전향적인균주수집으로균막형성에관여하는다른유전자및단백질의발현등에대한연구가필요하리라생각된다. 요약목적 : 최근중심정맥관의사용이증가함에따라 Staphylococcus epidermidis에의한중심정맥관감염이증가하고있다. 저자들은혈액에서동정된 S. epidermidis 를대상으로중심정맥관에서분리되는병원균과집락균및피부오염균간에균막형성능과이에관련된유전자보유율의차이와임상적유용성에대해알아보고자하였다. 방법 : 전남대학교병원에입원한환자의혈액배양에서동정된 S. epidermidis 4주를병원균 (19주), 집락균 (11주) 및오염균 (1주) 로분류하였다. 균막형성능은각균주를 96-well 마이크로플레이트에균막을형성시킨후 crystal violet으로염색하여균막음성표준균주의 OD값과비교함으로써 ru 값을구하였다. 균막형성관련유전자로알려진 icaabcd, atle, aap 유전자의유무는중합효소연쇄반응을이용하여검출하였다. 균막형성관련유전자의진단적유용성을알아보기위해각각의민감도, 특이도, 양성예측도를구하였다. 결과 : 병원균과집락균을포함한중심정맥도관관련균주 (5%) 는오염균 (2%) 에비해균막형성능력이증가되어있었다 (p=.12). 혈액과 tip배양에서 S. epidermidis 외에다른균주가분리된 7명의환자에서배양된 S. epidermidis 의 ru값은 1.18±.5이었고, Candida parapsilosis가배양된 1명을제외하면 ru값이.75~1.5로낮았으나중심정맥도관과관련하여분리되었다. 중심정맥도관관련균주에서 icaabcd 오페론의유무에따른균막형성능력의차이를비교했을때 icaabcd 오페론을가지고있는균주는 icaabcd 음성인균주에비해균막형성능이높았다 (1.72±.84 vs..94±.22, p=.3). 중심정맥관관련균주와오염균을구분하는데 icaabcd의양성예측도는 83% 로비교적높았으나민감도와특이도는각각 5% 와 7% 를보였다. 결론 : 병원균과집락균간에균막형성능과균막형성관련유전자의보유율에는차이가없었지만오염균과는차이가있었다. 또한중심정맥관관련균주와오염균을감별하는데 icaabcd 유전자의확인이도움이될것으로판단되나병원균과집락균을감별하는데는유용성이낮았다. 향후병원균과집락균을감별하기위해임상양상과대조를통한전 향적인균주수집으로균막형성에관여하는다른유전자및단백질의발현등에대한연구가필요하리라생각된다. 중심단어 : 카테터관련감염 ; Staphylococcus epidermidis; 균막 REFERENCES 1) Kim KJ, Shin JH, Shin DH, Choi MJ, Cho D, Kee SJ, Suh SP, Ryang DW.
