35 기타예방법 비타민E의 항암효과에 대한 추정되는 작용기전은 아래와 같다. ① Vitamin E possesses significant antioxidant activity by which it protects cell membranes from free radical-induced damage thereby reducing carcinogenic potential ② Inhibition of prostaglandin and arachidonic acid metabolism reducing inflammatory intermediates, which may play a role in the interplay of chronic inflammation and carcinogenesis ③ Inhibition of protein kinase C, which appears to be independent of the antioxidant activity and is theorized to reduce cellular proliferation ④ Induction of apoptosis ⑤ Interaction with the enzymes involved in cholesterol biosynthesis can lead to decrease levels of steroid hormones, especially androgens. This may form the basis of its activity in prostate cancer * Selenium and Vitamin E Cancer Prevention Trial (SELECT) 2001년부터 진행되고 있는 대규모 무작위 이중맹검 위약대조군 연구로 비타민E와 셀레늄의 단독 혹은 혼합요법의 전립선암 예방에 대한 효과를 일차적으로 알아보는 전향적 연구이며, 2004년 6월까지 그 대상 자가 35,533명에 이른다 (Scott M et al, 2009). 50세 이상의 아프리카계 미국인, 55세 이상의 비아프리카계 미 국인을 대상으로 셀레늄+위약, 비타민E+위약, 셀레늄+비타민E, 위약+위약 4군으로 나누어 최소 7년에서 12년까지 연구를 진행하고 있다. 200μg l-selenomethionine, 400mg racemic 알파 토코페롤, 비타민E와 셀 레늄이 포함되지 않은 종합비타민을 이용하였으며, 연구의 목적은 전립선암의 발생률의 차이를 알아보는 Dietary Agents and Others 33

또한 셀레늄의 장기 투여가 당뇨 발생의 위험성을 증가시킬 수 있다는 결과가 나왔다. 많은 사람들의 관심을 받고 연구가 진행되었으나 결과는 효과나 안정성을 뒷받침하는 충분한 자료 없이 약제를 사용하는 것은 위험하다는 것을 다시 한 번 보여주는 연구이다. 2.

36 Other Preventive Measures 것이었다. 그러나 2008년 9월 중간보고 결과에서 전립선암 발생률에 효과가 없다는 잠정결론을 내려 연구를 중단 할 것을 권고하고 있다. 비타민E나 셀레늄 투여군에서 암 발생에 차이가 없었고, 비타민E 투여군에서는 의 미는 없지만 전립선암이 오히려 증가하는 경향을 보였으며, 셀레늄 투여군에서는 2형 당뇨의 위험이 증가 하는 것으로 보고하였다. 이러한 결과는 비타민E나 셀레늄이 전립선암을 예방하는 데 도움이 되지 않는다 는 강력한 증거이다. 또한 셀레늄의 장기 투여가 당뇨 발생의 위험성을 증가시킬 수 있다는 결과가 나왔다. 많은 사람들의 관심을 받고 연구가 진행되었으나 결과는 효과나 안정성을 뒷받침하는 충분한 자료 없이 약제를 사용하는 것은 위험하다는 것을 다시 한 번 보여주는 연구이다. 2. 콩 아시아에서 전립선암 발생률이 현저히 낮은 것은 서양보다 콩 섭취량이 많고 그에 따라 콩류의 주요 성 분인 이소플라본 (isoflavinoids) 요소인 genistein, daidzein과 그 대사물의 혈중 농도가 높기 때문으로 생각 할 수 있다. 아시아 남성을 대상으로 한 연구에서 콩 식품 섭취량이 많은 군은 전립선암 위험성이 48% 낮았 으나, 구미 각국의 남성을 대상으로 한 연구에서는 콩 식품에 의한 전립선암 위험성에 대한 차이를 거의 보 이지 않는 결과를 나타내었다. 이소플라본의 일차적인 작용은 에스트로겐 수용체에 작용하여 세포증식을 억제하며, 그외 세포전달체 계를 억제하거나, 세포고사를 유발하고 신생혈관생성을 억제하는 것이다. 이 종류의 물질은 단기간 사용 시 중대한 부작용이 없고, 심혈관질환 및 골다공증에도 효과가 있는 것으로 알려져 있으나, 전립선암 환자 34 Prevention of Prostate Cancer

38 Other Preventive Measures 며, 녹차를 마시는 양이 많을수록, 녹차를 마신 기간이 길수록 발병위험이 크게 감 소한다고 하였다. 최근 60명의 고위험 PIN 환자를 대상으 로 200mg 녹차 성분을 하루 세 번 1년 동 안 섭취한 연구에서 녹차를 섭취한 군에 서는 3%에서 전립선암이 발견되었고, 섭 취하지 않은 군에서는 30%에서 전립선암 이 발견되었다고 하였으며, 섭취군에서 전립선암으로 진행되기까지 시간이 더 오 래 걸렸다고 하였다 (Bettuzzi et al, 2006). 무증상의 호르몬비의존성 전립선암 환자 42명을 대상으로 하루 6g의 차를 섭취한 임상연구에서 PSA 수치 가 50% 이상 감소하는 경우가 한 명의 환자에서 나타났으며 (Jatoi et al, 2003), 하루 두 번 250mg을 섭취한 또 다른 임상연구에서는 19명 중 6명에서 3~5개월까지 진행이 억제되는 효과를 보였다. 이는 녹차의 폴리 페놀 성분이 암예방 뿐 아니라 미약하지만 진행된 전립선암에서도 항암효과가 있는 것으로 생각된다. 지금까지의 보고를 볼 때 녹차에 얼마나 많은 항전립선암 물질이 들어 있는지는 확실하지 않으며, 녹차 를 많이 마시는 사람들에 있어서 실제로 전립선암의 발병률이 얼마나 감소하느냐에 있어서도 아직 명확하 게 규명되지 않았다. 다만 이미 수천 년간 마셔왔던 녹차는 그 사실만으로도 안전성은 증명되었다고 할 수 있으나 사람에 따라 알레르기 반응을 보일 수 있으며 카페인이 많이 들어 있어 과하게 섭취하면 부작용이 발생할 수도 있다. 4. 라이코펜 라이코펜 (Lycopene)은 토마토 등의 붉은색 채소나 과일의 색소 성분으로 항산화, 항암작용이 있다고 알 려져 있다. 라이코펜은 항산화 비타민으로 알려진 베타카로틴, 눈을 좋아지게 하는 루테인과 함께 카로티 노이드 삼총사로 유명하며, 토마토에 가장 많이 함유되어 있지만 딸기, 수박, 자몽, 살구 등에도 상당량 들 어 있다. 미국인의 경우 라이코펜의 약 80%를 토마토와 토마토 가공식품을 통해 공급받는다. 토마토의 라 이코펜은 지용성 물질이어서 체내 흡수가 잘 안 되며, 따라서 더 많은 양을 흡수하려면 토마토를 열처리하 거나 올리브유 등 기름과 함께 먹는 것이 좋다. 50,000명을 대상으로 한 Health Professionals Follow-up Study는 라이코펜을 많이 섭취한 군에서 전립선 36 Prevention of Prostate Cancer

기타예방법 7. Selective estrogen receptor modulators (SERMs) 에스트로겐은 전립선의 성장을 촉진하고, 전립선암 발생에 관여한다. SERMs도 미약한 에스트로겐 작용 을 가지고 있어 암억제작용을 보이고 전립선암 예방에 효과가 있을 것으로 생각된다.

41 기타예방법 7. Selective estrogen receptor modulators (SERMs) 에스트로겐은 전립선의 성장을 촉진하고, 전립선암 발생에 관여한다. SERMs도 미약한 에스트로겐 작용 을 가지고 있어 암억제작용을 보이고 전립선암 예방에 효과가 있을 것으로 생각된다. Phytoestrogen이 풍 부한 음식을 많이 섭취할 경우 전립선암 발생률이 낮아지는 것처럼 SERMs도 에스트로겐 수용체에 agonist-antagonist 기능을 갖고 있으며, 에스트로겐 수용체에 경쟁적으로 결합하여 에스트로겐 유도 세포 증식을 억제한다. 이는 전립선암 transgenic mouse 모델을 이용한 동물실험에서 실험적으로 증명되었다. 제2상 임상연구에서 SERM toremifene의 안정성이 확인되었고, 대상이 적지만 고위험 PIN 환자에서 toremifene 투여군에서 전립선암 유병률이 낮게 보고되었다 (Price et al, 2006). 8. Allium Vegetables (부추, 파, 마늘류) 마늘에 들어있는 톡 쏘는 맛을 내는 알리신은 성 기능 및 전립선암과 방광암을 예방하고 치료하는 데 큰 효과가 있는 것으로 알려져 있다. 또한 마늘의 알 리신 성분은 세포의 돌연변이를 억제하고 암의 크기 를 감소시키는 것으로도 알려져 있다. 중국 상하이 에서는 238명의 전립선암 환자와 471명의 대조군을 대상으로 인구기반 환자-대조군 연구를 시행하였다. 체계화된 설문지를 이용하여 122가지 음식섭취량에 대하여 조사하였으며, 부추, 파, 마늘류 채소를 하루 10g 이상 섭취하는 군에서 부추, 파, 마늘류 채소를 적게 섭취하는 군보다 전립선암 위험이 50% 가까이 감 소하였다고 하였다 (Hsing et al, 2002). 858명의 전립선암 환자를 대상으로 한 또 다른 인구기반 환자-대조군 연구에서도 마늘류 채소 섭취량이 증가할수록 전립선암 발생 비율이 감소하였다 (Hodge et al, 2004). 이탈리아와 스위스를 포함한 남부 유럽에서 시행한 환자-대조군 연구에서도 양파와 마늘의 섭취 빈도와 전립선암을 포함한 암의 발생 빈도가 반비례 한다고 보고하였다. 71개국을 포함한 국제 생태학 조사에 따 르면 전립선암 사망률과 allium 채소 소비량과는 역비례한다고 하였다 (Colli & Colli, 2006). 9. 와 인 1976년 프랑스에서 생활수준이 비슷한 구미 18개국을 조사하여 국민소득, 지방섭취량, 보건의료수준과 Dietary Agents and Others 39

특히 레스베라트롤이 강력한 항암작용을 갖고 있는 것으로 생각되며, 실험실에서 폐암, 대장암, 유방암, 난소암 그리고 전립선암 세포에 작용하여 암세포의 성장을 억제하는 것으로 알려졌다. 또한 동물 실험을 통해 발암과정의 여러 단계에서 항암작용을 하는 것이 증명되었다.

42 Other Preventive Measures 는 관계없이 포도주 소비량이 많은 나라일수록 관상동맥질환에 의한 사망률이 적다고 발표하면서 와인에 대한 관심이 증가하였다. 와인에는 안토시안 (anthocyans), 플라본 (flavones), 레스베라트롤 (resveratrol) 등과 같은 항산화 물질로 알려진 폴리페놀 성분이 존재하여, 혈소판 응집 억제, 저밀도지단백 (LDL)의 산화 방 지, 혈관내피세포의 기능 호전 등에 의해 관상동맥질환을 예방하고 사망을 줄인다는 의학적인 근거가 제 시되었다. 특히 레스베라트롤이 강력한 항암작용을 갖고 있는 것으로 생각되며, 실험실에서 폐암, 대장암, 유방암, 난소암 그리고 전립선암 세포에 작용하여 암세포의 성장을 억제하는 것으로 알려졌다. 또한 동물 실험을 통해 발암과정의 여러 단계에서 항암작용을 하는 것이 증명되었다. 몇몇 동물실험 보고에서는 레 스베라트롤이 안드로겐 수용체에 작용하여 세포고사를 유도하고, 세포분열과정을 억제하여 전립선암 발 생을 억제한다고 한다. 그러나 사람을 대상으로 한 임상연구 결과는 부족한 상태이다. 폴리페놀 성분은 화이트와인보다 레드와인에 많이 존재하며, 특히 레스베라트롤은 포도의 껍질에 주로 존재하여 레드와인에만 다량 포함되어 있다. 여러 역학조사에서 다소 이견이 있으나 알코올섭취량과 전 립선암 발생과는 관계가 없다고 한다. 와인 특히 레드와인 섭취와 전립선암에 대한 관계는 아직 명확하지 않다. 최근 Califonia Men's Health Study (CMHS) 대상이 되었던 45~69세의 남자 84,170명을 대상으로 한 역학 조사에서 레드와인 섭취량과 다른 종류의 알코올섭취량 모두 전립선암 발생과 연관이 없다고 하였 다 (Chao, 2009). 10. 기타 식품류 과거부터 버섯은 암에 효과가 있다고 생각되어 왔다. 암예방과 관련이 있다고 알려진 버섯 종류로는 잎 새버섯, 덕다리버섯, 표고버섯 등이 있다. 잎새버섯은 다이어트와 고혈압, 당뇨 치료 등에 효과가 있다고 알려져 있으며, 항암효과 또한 일반 버섯보다 뛰어난 것으로 알려져 일본에서 인기를 얻고 있다. 이 때문에 40 Prevention of Prostate Cancer
