1 = 증례보고 = 대한안과학회지제 49 권제 6 호 2008 J Korean Ophthalmol Soc 49(6): , 2008 DOI : /jkos 토피라메이트에의해발생한급성근시 2 예 김성욱 1 서성관 1 허준 1 안기수 2 최봉준 2 인제대학교의과대학부산백병원안과학교실 1, 부산이안과 2 목적 : 간질, 편두통, 비만치료제로처방되고있는토피라메이트 (Topamax R, 한국얀센 ) 를복용한뒤발생한양안의급성근시환자 2 예를경험하여보고하고자한다. 증례요약 : 평소안과적질환이없던 24 세, 25 세여자가각각체중감소를위해토피라메이트를복용하였고, 이후발생한양안의시력저하를주소로본원외래에내원하였다. 자세한문진과세극등검사를포함한안과적검사를시행하였다. 결론 : 세극등검사상두환자모두에서수정체 - 홍채격막의앞방이동이있었고, 이로인해앞방이얕아져근시가유발되었음을확인할수있다. Topiramate 를중단하고조절마비제투여로증상이소실되었다. < 한안지 49(6): , 2008> 설파제제인토피라메이트는다양한약리학적기전을가진광범위항간질치료제로서편두통, 양극성장애, 외상후스트레스장애, 대상포진후신경통등에도효과를나타내며. 드물지만급성근시및급성폐쇄각녹내장등의안과적합병증을유발한다. 1,2 저자들은 topiramate 를복용한뒤양안에급격한굴절력의변화가발생한환자 2 명의증례를경험하였기에문헌고찰과함께보고하고자한다. 증례 1 증례보고 24 세여자가당일아침갑자기발생한양안의시력저하를주소로내원하였다. 환자는체중감소를위해내원 7 일전부터 topiramate 25 mg 을하루에한번복용하고있었다. 평소안경을착용한적도없었으며본인이나가족중에안과질환의병력도없었다. 나안시력은우안 0.02, 좌안 0.02 이고, 자동굴절계로우안 (-7.25)(-0.75) cyl 5, 좌안 (-8.5)(-0.25) cyl 165 < 접수일 : 2007 년 9 월 11 일, 심사통과일 : 2008 년 2 월 26 일 > 통신저자 : 서성관부산시부산진구개금동 인제대학교부산백병원안과 Tel: , Fax: seossg@hanmail.net 의굴절이상을보였으며양안모두 1.0 으로교정되었다. 안압은우안 13 mmhg, 좌안 17 mmhg 로측정되었다. 세극등검사상결막의특이소견은보이지않았으며, 각막상피와기질에약간의부종이관찰되었다. 양안앞방내염증반응은없었으며중심앞방깊이는각막두께의 3 배, 주변부앞방은얕아져있었다 (Fig. 1). 수정체는투명했고, 안저에특이소견은없었다. 즉시 topiramate 의복용을중지시키고조절마비제를처방하여하루 2 회점안하게하였다. 4 일뒤, 환자의증상은소실되었고나안시력은우안 0.8, 좌안 0.8 이었으며자동굴절계로우안 (+0.25) sph, 좌안 (-0.50) cyl 45 로심한굴절이상은보이지않았다. 안압은우안 9 mmhg, 좌안 11 mmhg 이었다. 양안중심전방깊이는각막두께 6 배로깊게유지되었다 (Fig. 2). 증례 2 25 세여자가이틀전부터시작된양안의시력저하, 결막의충혈, 눈물흘림을주소로내원하였다. 환자는체중감소를위해내원 7 일전부터 topiramate 25 mg 을하루에한번복용하고있었다. 시력은우안 0.02, 좌안 0.02 이었고, 자동굴절계로우안 (-7.50)(-0.50) cyl 46, 좌안 (-7.50)(-0.75) cyl 89 의굴절이상을보였으나, 양안모두 0.6 으로교정되었으며, 핀홀을사용하였을경우양안모두 0.8 로교정되었다. 안압은우안 17 mmhg, 좌안 19 mmhg 이었다. 1033

Note refractive error (-7.50/ ). 세극등검사상결막부종과충혈된소견을, 각막, 수정체는모두정상소견을보였고중심전방깊이는각막두께의 3 배, 주변부앞방은얕아져있었다. 수정체는투명했고안저에특이소견은없었으며 (Fig. 3), 눈초음파검사에서특이소견을발견할수없었다 (Fig. 4, 5). 즉시 topiramate 의복용을중지시키고, 조절마비제를처방하였다. 2 일뒤, 나안시력우안 1.0, 좌안 0.5 이며자동굴절계로우안 (-0.25)(-0.75) cyl 180, 좌안 (-1.00)(-0.50) cyl 90 의굴절이상을보였다. 안압은우안 10 mmhg, 좌안 10 mmhg 이었다. 양안중심앞방깊이는각막두께 4 배로유지되었다. 5 일후, 양안나안시력 1.0 이었으며, 자동굴절 계로우안 (-0.25)(-0.25) cyl 119, 좌안 (-0.50) (-0.25) cyl 92 으로심한굴절이상은보이지않았다 (Fig. 6, 7). 안압은우안 11 mmhg, 좌안 11 mmhg 이었다. 양안중심앞방깊이는각막두께 6 배로깊게유지되었다. 고 찰 Topiramate 는 1996 년에미국 FDA 의공인허가를받았으며, 난치성부분간질에추가치료제 (add-on drug) 로서그효과가증명되었다. 3 약리학적으로는세포내에서 kainite 유발전위를억제하고 GABA 활성을 1036

Right eye. 증강시키며 voltage activated sodium channel 과 calcium channel 을억제하고 carbonic anhydrase isoenzyme 을억제하는등최소한 5 가지의독립적인작용을가지고있으므로다양한발작형태에도광범위한효과를나타낸다. 4 뿐만아니라편두통의예방및비만과제 2 형당뇨병의치료에도사용되며, 용량반응체중감량의효과가있다. 5,6 초기 topiramate add-on 임상실험에서정신서행, 주의력결핍, 언어문제, 기억력저하등중추신경계와관련된주관적인인지장애가비교적흔한, 중요한부작용으로대두되었다. 7 최근의 topiramate 임상시험에서도주관적인환자의호소에근거한결과, 인지장 애가중요한부작용으로보고되었으며, topiramate 가투여된환자의일부에서동공차단없이급성폐쇄각녹내장과근시를유발한다는사실이증례보고를통해알려지게되었다 Fraunfelder et al 13 은세계보건기구 (WHO) 를통하여 topiramate 와연관된급성폐쇄각녹내장 86 예와 ( 그중 17 명은 8.75 diopter 에달하는급성근시가발생 ), 상맥락막삼출을동반한 9 예의증례를포함한안과적부작용 115 예의증례를검토하여, 급성폐쇄각녹내장, 급성근시, 상맥락막삼출등이 topiramate 치료에있어서같은범주의합병증에속한다는것을제시하였다. 한국에서도 Jeon and Kee 14 는 topiramate 에의한급성폐쇄각녹내장이발 1037

Left eye. 생한 2 예를보고한바있다. 급성근시는토피라메이트에의한드문이상반응으로써 Hook et al 15 에의해최초로보고되었으며 Acetazolamide, sulfamethoxazole, trimethoprim, indapamide, promethazine, spironolactone, isosorbide dinitrate, bromocriptine, tetracycline, corticosteroids, hydro chlorothiazide, penicillamine, quinine, metronidazole, isotretinoin, aspirin 등의약제들은근시유발과관련이있는약제로알려져있다. 16,17 Sen et al 18 은 topiramate 복용에있어수정체의삼투압조절장애에따른수정체부종으로앞방이얕아짐으로써급성근시가발생한다고생각하였으나, 최근의 연구에서는 topiramate 가약한 carbonic anhydrase inhibitor 의효과를가지고있거나, prostaglandin 과연관된효과를가지고있는것으로생각된다. 17 더불어낮은농도에서근시를유발하지않는것으로보아알러지성과민반응으로생각하기는어렵다. 19 초음파를이용한연구에서도섬모체 - 맥락막삼출과섬모체의부종으로인한수정체 - 홍채격막이동에의해앞방이얕아지며, 이로인해급성근시가유발된다고보고하였다. 9,20,21 환자들은대개급격한시력저하를주소로내원하게되며, 안과검사상시력저하, 안압상승, 급성근시, 눈떨림, 폐쇄각, 얕은앞방등이관찰되며, 눈꺼풀연축, 1038

7 대한안과학회지제 49 권제 6 호 2008 년 상맥락막삼출, 눈알주위부종및동통, 공막염, 복시, 안구진탕등을보일수도있다. 13 증례 1 과 2 는 topiramate 에의한안과적부작용과전형적임상경과를보인예이다. 약복용 1 주후갑자기증상이시작되었고 topiramate 를중단하고조절마비제를점안하면서현저히호전되어내원 1 주만에완전히이전상태로회복이되었다. 증례 2 의경우특히결막부종및충혈을보여동반된안와연조직염을생각할수도있으나이것은약의부작용으로나타난증세이며복용약의중지와함께호전되는양상을보였다. Jeon and Kee 14 의증례에서도내원당시급성폐쇄각녹내장을의심하여, 그에해당하는약물치료및홍채절제술을시행하였으나, 초음파생체현미경을시행하여모양체맥락막박리와모양체의전방회전으로좁아진전방각을확인한후조절마비제의점안과 topiramate 의중단으로관해를유도하였다. 토피라메이트에의한급성근시는명확하지는않으나이미기술한바와같이설파계약제나아세타졸라마이드와같은약물과동일하게섬모체의부종또는섬모체맥락막의분리에의한수정체 - 홍채격막이동에의해발생하며환자의굴절상태와는큰관계가없으며주로여성에게서토피라메이트에의한급성폐쇄각녹내장의발생이전의전구증상또는단독으로도발생가능하고토피라메이트에의한급성폐쇄각녹내장의소실이후에도지속가능한것으로알려져있다 본증례는급성폐쇄각녹내장의발생과무관하게 -7 디옵터이상의고도의급성근시만단독으로나타난경우로토피라메이트의사용시환자가시력저하를호소할때안압의상승유무와상관없이급성근시의발생을꼭고려하여약물에의한부작용을고려하여야할것이다. Topiramate 에의한근시및폐쇄각녹내장의치료로는기저질환이있는경우, 해당과의전문의와상의한뒤약을중단하고조절마비제를투여하면서안압상승이있을경우국소적베타차단제와경구용안압하강제를사용해야한다. 13 조절마비제는섬모체소대를긴장시켜수정체를뒤로당김으로써앞방이깊어지고홍채에밀려막혀있던섬유주를열어안압을낮춘다. 증례 2 와같은경우에서는갑작스런시력저하로안저소견의이상이보이지않아, 다른여러가지검사를시행하였는데, 중요한것은충분한병력청취, 특별히약물복용의경력을자세히조사하고, 세밀한안과적검사를통해시력저하의원인을밝히려는노력이있어야하겠다. Topiramate 를사용함에있어서가능한부작용에대하여환자를교육하고안통이나시력저하와같은안과적증상이나타난다면즉시보고를하도록교육하는 것은매우중요한일이며, 약의부작용에대한인식과즉각적인복용의중단은증상을호전시킬것이다. 참고문헌 1) Ziemann U. Phamacology of TMS. Case summary: A 24-year-old woman and a 25-year-old woman, both previously devoid of ocular problems, visited our hospital with blurry vision after taking topiramate for weight reduction. A thorough medical history review and ocular examinations, including a slit lamp examination, were performed. Conclusions: Slit-lamp examinations revealed forward displacement of the lens-iris diaphragm, which resulted in myopia and anterior chamber shallowing. Discontinuation of topiramate and the administration of cycloplegic agents successfully resolved the symptoms.
