This study evaluated the efficacy and relative toxic effects of four immunosuppressive regimens. Methods: We assigned 299 renal-transplant recipients to receive group A (standard-dose cyclosporine, mycophenolate mofetil, and corticosteroids), group B (low-dose cyclosporine, basiliximab induction, mycophenolate mofetil, and corticosteroids), group C (standard-dose tacrolimus, mycophenolate mofetil, and corticosteroids), or group D (low-dose tacrolimus, basiliximab induction, mycophenolate mofetil, and corticosteroids) regimens. We compared the groups according to graft function through estimated glomerular filtration rate (GFR), acute rejection, and allograft survival. Results: The mean calculated GFR in patients receiving low-dose tacrolimus (76.4 ml per minute) was higher than in the other three groups (range, 66.3 to 73.8 ml per minute). The rate of biopsy-proven acute rejection was lower in patients receiving low-dose tacrolimus (14.3%) than in those receiving standard-dose cyclosporine (29.6%), low-dose cyclosporine (19.8%), or standard-dose tacrolimus (23.8%). Allograft survival rates differed significantly among the four groups (P=0.006) and were highest in the low-dose tacrolimus group (99.9%). Serious adverse events were more common in the standard-dose tacrolimus group than in the other groups (51.2% vs a range of 41.4 to 42.3%), although a similar proportion of patients in each group had at least one adverse event during treatment (81.1 to 90.5%). Conclusions: A regimen of basiliximab, mycophenolate mofetil, and corticosteroids in combination with low-dose tacrolimus may be advantageous for renal function, allograft survival, and acute rejection rates, compared with regimens containing basiliximab induction plus either low-dose cyclosporine or standard-dose tacrolimus or with standard-dose cyclosporine without induction. Key Words: Kidney transplantation, Calcineurin Immunosuppressive agents, Tacrolimus, Graft survival 중심단어 : 신장이식, 칼시뉴린, 면역억제제, Tacrolimus, 이식편생존 서론 신장이식후단기생존율의성장에도불구하고, 장기적인이식신장의신독성과수용자의사망으로매년 3 5% 가량의이식신장이소실되고있다 (1). 최근에는급성거부반응이줄어들고있으나장기간의면역억제요법과연관된부작용이증가하고있어, 면역억제제의독성반응을줄이는것이이식수용자의치료의주요목적이되고 책임저자 : 권오정, 서울시성동구행당동 17 번지한양대학교병원외과, Tel: , Fax: ojkwon@hanyang.ac.kr 접수일 : 2010 년 8 월 30 일, 심사일 : 2010 년 9 월 24 일게재승인일 : 2010 년 10 월 8 일 있다. 수년동안사용한 calcineurine 억제제인, cyclosporine 은그임상적효용성때문에여전히많은면역억제제중기본적으로사용하나, 표준적으로권장하는용량을장기간사용하면신독성과연관이있으며 (2), 고혈압과고지혈증도연관이있다 (3-6). 한연구에따르면, 중앙값 6개월부터시작하여, 이식후 cyclosporine 기반면역억제를시행받은신-췌장이식환자 99명은이식 10년후모두신장기능이상을보였다 (7). 최근개발된 calcineurine 억제제인 tacrolimus는 cyclosporine보다이식신장의생존율을높였으며 1년이내급성거부반응을줄이는데더욱효과적이라고발표되었다 (8). 그러나, 최근의권장용량으로는 tacrolimus도신독성, 신경독성, 감염증및지질대사이상등의부작용을 cyclosporine과공유한다 (9,10). 2 따라서, 이에저자들은이식후저용량의 cyclosporine 이나 tacrolimus를사용하여약물에의한부작용을줄이고이식신장기능을유지할수있음을알아보기위해서후향적으로의무기록을분석하였다. 대상및방법 1) 연구계획및환자분포본연구는 Helsinki 선언과 Good Clinical Practice guideline에의거하여동등한조건을갖춘성인신장이식환자를대상으로후향적연구를진행하였다. 1995년 1월부터 2009년 12월까지한양대학교병원에서신장이식을받고외래추적검사가가능했던 16 66세의 299명의환자를대상으로면역억제제의사용방법에따라서네그룹으로나누어각각의그룹에있어서의이식신장기능, 감염발생, 급성거부반응의발생, 이식신장생존율, 약제에대한부작용등을비교분석하였다. 첫번째그룹은표준용량의 cyclosporine을사용한군으로 (n=95) 표준용량의 cyclosporine (Sandimmune, 한국노바티스혹은 Cipol, 종근당 ), mycophenolate mofetil (Cellcept, 한국로슈혹은 Myfortic, 한국노바티스 ), corticosteroid를사용하였고, 두번째그룹은감량된 cyclosporine을사용한군으로 (n=132), 술전 basiliximab (Simulect, 한국노바티스 ) 를추가로사용하였다. 세번째그룹은표준용량의 tacrolimus (Prograf, 한국아스텔라스제약혹은 Tacrobell, 종근당 ) 를사용한군으로 (n=42), mycophenolate mofetil, corticosteroid를같이사용하였다. 네번째그룹은감량된 tacrolimus를사용한군으로 (n=30), 술전 basiliximab을사용하였다. Basiliximab은술전 2시간이내에 20 mg을 15 20분에걸쳐정주하였으며, 술후 4일에동일용량을추가로정주하였다. Cyclosporine과 tacrolimus는술전 2일부터투여하기시작하였으며, 네그룹은매일 g의경구 mycophenolate mofetil을복용하였다. 표준용량의 cyclosporine 그룹에서혈중농도 ng/ml를달성하기위하여첫경구용량은 3 5 mg/kg을하루 2번씩복용하였다. 감량된용량의 cyclosporine 그룹에서는혈중용량을 ng/ml에서유지할수있도록초기경구용량은 4 mg/kg으로하루 2번씩복용하였다. 표준용량의 tacrolimus 그룹은하루 0.2 mg/kg의용량을 2번에나누어투약하였고, 혈중용량은 5 15 ng/ml가되도록하였으며, 감량된 tacrolimus를사용한그룹은하루에 0.1 mg/kg 을 2번에나누어복용하여혈중농도는 4 7 ng/ml가되도록하였다. Corticosteroid의경우 prednisolone을수술직후 90 mg를복용하였으며, 다음날부터 10 mg씩감량하 여 5 mg의유지용량을지속하였다. Cyclosporine과 tacrolimus의혈중농도는원내의농도검사를이용하였다. 2) 효용성제1효용성평가점에서이식 1개월후의 glomerular filtration rate (GFR) 을 Cockcroft-Gault 공식을사용하여혈중 creatinine을측정하여추정하였다 (11). 제2효용성평가점에서는이식 12개월후에측정된 GFR과급성거부반응, 이식 1개월과 12개월의전체생존율및이식신장생존율, 급성거부반응이처음일어난시기, 첫 12개월동안의치료실패의빈도를포함했다. 치료실패는다음중한가지이상의경우로정의하였다. 즉, 면역억제제의추가약제사용, 연속된 14일이상의면역억제제의중단, 총누적 30일이상면역억제제의중단, 이식신장소실, 사망으로정의하였다. 3) 안전성안전성은생체징후와부작용, 기회감염, 종양, 연구기간내의사망의발생을결정하는소견및신장이식후 6 개월내중재적시술을요하는 lymphocele의형성으로평가하였다. 4) 통계적분석네그룹간의비교를위한 1차적방법은치료와제공자의여러요인을포함한변수들의분석이었다. 변수변환후에도정규성에대한가정이종종벗어나는경우가있어, 연구분석할때전체그룹간비교에는 Kruskal- Wallis 검정을사용하였으며, 5% 의유의성으로그룹간둘씩짝지어 Wilcoxon rank-sum 검정을시행하였다. 급성거부반응및이식신장의소실, 치료실패, 사망시기는 Kaplan-Meier 방법을사용하여분석하였고, 그룹간차이는 log-rank 검정을사용하였다. 그래서이식후발생한당뇨, 설사, cytomegalovirus 감염을포함한기회감염과같은면역억제제와관련되어알려진몇가지선택된부작용의분석을위해추가적인 Kaplan-Meier 분석을사용하였다 (9). 결과 1) 환자 1995년 1월부터 2009년 12월까지, 본원에서시행한 299 명의신장이식환자를대상으로하였으며, 각그룹은인구학적, 임상적, 제공-수용자간의특성에차이는없었다 (Table 1). 모든그룹에서치료실패가투약중지의주된이유였으며, 면역억제제의추가 ( 그룹에따라, %), 면역억 J Korean Soc Transplant December 2010 Volume 24 Issue 4

(2) 급성거부반응 : 1개월과 12개월에감량된 tacrolimus 그룹이 14.3% 로표준용량의 cyclosporine 그룹 (29.6%) 에비해약 50%, 그리고감량된 cyclosporine 그룹 (19.8%) 및표준용량의 tacrolimus 그룹 (23.8%) 과비교하여약 60% 가량에서조직검사로확진된급성거부반응이적었다 ( 감량된 tacrolimus 그룹과둘씩짝지어비교하였을때 P<0.001) (Table 2). 4 그룹과둘씩짝지어비교하였을때, 전반적인생존율은모든그룹에서 90% 를상회하여의미있는차이를보이지않았다 (Table 2). (4) 치료실패 : 면역억제제를추가한경우표준용량의 cyclosporine을사용한그룹에서 mycophenolate mofetil 을추가한경우가 7예가있었으며주된원인으로는고지혈증, 이식후발생한당뇨등이며, cyclosporine에서 tacrolimus로변경한경우가 8예가있었다. 주원인으로는고혈압및고지혈증이었다. 감량된용량의 cyclosporine 을사용한그룹에서간독성으로인해 mycophenolate mofetil이감량된경우가 1예가있었으며, 고혈압, 고지혈증으로인한 cyclosporine에서 tacrolimus로변경이 11예가있었다. 표준용량의 tacrolimus 그룹에서 cytomegalovirus 감염에의해 mycophenolate mofetil의중지가 1예있었으며, 이식후발생한당뇨및 cytomegalovirus 감염에의해 tacrolimus에서 cyclosporine으로변경된경우가 10예가있었다. 감량된용량의 tacrolimus 그룹에서는 cytomegalovirus 감염과간독성으로인한 tacrolimus에서 cyclosporine으로의변경이각각 1예가있었다. 치료실패는감량된 tacrolimus 그룹에서가장낮았으며 (14.3%), 표준용량의 tacrolimus 그룹에서가장높았다 (45.2%, P<0.001) (Table 2). 감량된 tacrolimus 그룹에서복용중지로이어지는 14일이상연속된기간의면역억제제투여중단의치료실패는다른그룹에서 % 를보인반면, 1.5% 만나타났다. 또한감량된 tacrolimus 그룹에서면역억제제추가는 1.5% 가나타났으며, 타그룹에서는 % 로나타났다. 전체적인치료실패의비율은급성거부반응을거치지않은환자에서 11.3% 일어났으나급성거부반응이있던환자그룹에서는 26.1% 로나타났다. 3) 안전성약물에의한부작용은표준용량의 tacrolimus 그룹에서 51.2% 가나타났으며, 타그룹에서는 % 가나타났다 ( 표준용량의 tacrolimus 그룹과비교하여 P< 0.05). 비슷한경우로표준용량의 tacrolimus 그룹에서는부작용이나합병되어있는질환으로인해투약을중단하는경우가 5.8% 있었으며, 타그룹에서는 % 가있었다. 연구기간동안적어도한가지이상의부작용을경험한비율은모든그룹에서유사하였다 ( %). 네그룹에서가장흔했던부작용으로는혈액및림프계통, 소화기계통, 감염및대사이상이었다 (Table 3). 이식후 12개월에새로생긴당뇨는표준용량의 tacrolimus 그룹에서가장높았으며이는 Kaplan-Meier 측정에서그룹간의미있는차이를보였다 (Table 3, P=0.02). 설사또한 Kaplan-Meier 측정에서그룹간의미있는차이 (P<0.001) 를보였으며, tacrolimus를사용한두그룹에서낮게측정되었다 (Table 3). 5 기회감염 (P=0.03) 및가장흔한기회감염인 cytomegalovirus 감염 (P=0.003) 또한의미있는차이를보였는데, 일반적으로타그룹에비해표준용량의 cyclosporine 그룹에서더흔하였다 (Table 3). Cytomegalovirus에대한제공자-수용자상태에따른계층화는시행하지않았지만, cytomegalovirus 양성인제공자에대한 cytomegalovirus 음성인수용자의비율은그룹간의미있는차이는없었다 (Table 1). 종양은첫 12개월동안각그룹에서생기지않았거나 2명이생겼다. 이후추적관찰에서표준용량의 cyclosporine 그룹에서 5명이사망하였으며, 감량된용량의 cyclosporine 그룹에서는 2명이사망하였다. 표준용량의 cyclosporine 그룹에서는 1명이치료실패가선행된후사망하였으며, 감량된용량의 cyclosporine 그룹에서는 2명이사망하였다. 네명의환자는부작용이나동반질환으로사망하였는데, 표준용량의 cyclosporine 그룹에서 2명, 감량된용량의 cyclosporine 그룹에서 1명이사망하였다. 고찰본연구의 1차적목표는적은용량의 calcineurin 억제제를사용하여신독성을줄이기위함이다. 따라서 1차효용성평가점은신장기능이었다. 이차적목표는급성거부반응, 환자의전반적생존율및이식신장생존의기간내에효용성을유지하며전반적인독작용을줄이는데있었다. 본원의여건으로새로운면역억제제가수입되면이를중심으로약제를투여하게되고, 매년시행하는이식건수가많지않기때문에이들약제의병용방법에따른전향적연구를시행하기는힘들었다. 그러나후향적연구이지만최근의이식예에서거부반응의발생빈도가줄어드는것은 ( %) 최근개발된면역억제제와이들의병용요법의효과라고생각된다. 본연구에서감소된용량의 cyclosporine의경우표준용량의 cyclosporine의경우보다급성거부반응의빈도가줄었으며, 신장기능에있어서도 1년추적관찰결과 GFR 의상승소견을보여 Ekberg 등 (12) 의연구에비하여 cyclosporine의감량은충분한면역억제의효과를보이며, 신장기능의향상에도충분한도움을줄것으로생각한다. 이런조기에 calcineurin 억제제를감량하는효용성의기전은완전히밝혀지지않았지만, calcineurin 억제제를감량하는것이이식신장기능에이로운효과를설명하는이론적배경으로 tacrolimus 기반의면역억제제는 cyclosporine보다 GFR의감소가의미있게천천히진행되는데, 표준용량의두면역억제제로투약하게되면신혈관의수축과간질의파괴가더진행된다 (13). 게다가표준용량의 cyclosporine은신장이식후간질의섬유화에기여하는신장의 tumor necrosis factor (TGF)-β의 mrna의발현을증가시키는것과연관이있다 (14). 그러므로두 calcineurin 억제제의용량을감량시켜투여하는것이신장기능의보존과밀접히관계있으며, 수입동맥의수축을역전시키며, 안정화시키는것이조직검사로증명되었다 (15). 급성거부반응은만성거부반응의주요선행인자이며이식신장소실의주요위험인자로알려져있다 (16-18). Ferguson(19) 은급성거부반응을경험한경우이식신장생존반감기가 3.9년인데반해급성거부반응을경험하지않았던경우생존반감기가 16.9년으로이식신장의장기생존율을예측하는데있어가장중요한인자는급성거부반응의발생여부라고하였다. 유럽의 tacrolimus 다기관공동연구그룹의성적에서 cyclosporine 그룹에비해 tacrolimus 그룹에서급성거부반응의빈도가유의하게낮음이보고되었으며 (20), 또한 Gjertson 등 (21) 은이식신장의생존반감기가 tacrolimus 그룹에서 14년으로 cyclosporine 그룹의 8 9년에비해보다길다고보고하였으며이는 tacrolimus 그룹에서급성거부반응의빈도가낮았기때문으로설명하였다. 본연구에서는급성거부반응의빈도는 tacrolimus 그룹이표준용량에서는 23.8%, 감량된용량에서는 14.3%, cyclosporine 그룹이표준용량에서 29.6%, 감량된용량에서 19.8% 로그빈도가 tacrolimus 그룹에서유의하게낮았다. 이는평균추적기간이일단 1개월과 12개월로한정되어있어보다장기적인추적관찰을하게된다면이식신장생존율에유의한차이가나타날것으로사료된다. 본연구는부작용의부담을줄이는전략의개발도고려하였는데, 감량된용량의 cyclosporine 뿐아니라 tacrolimus 또한 basiliximab과 mycophenolate mofetil 및 steroid를추가하였다. Margreiter(9) 는 tacrolimus 그룹에서혈당수치가유의하게높고당뇨병의이환율도높았다고보고하였고, Pirsch 등 (22) 은이식후첫 1년안에 30일이상인슐린치료가필요했던경우는 cyclosporine이 4.0% 이었고, tacrolimus 그룹은 19.9% 로보고하였으며, Mayer 등 (20) 은이식후발병한당뇨병은 tacrolimus 그룹에서 cyclosporine 그룹의 2.2% 보다 8.3% 로더높다고보고하였다. 또, Knoll과 Bell(23) 은 tacrolimus 그룹에서당뇨병의이환율이약 5배정도더높다고보고하였다. 이식후발병한당뇨는중요한문제로 tacrolimus와 steroid의사용으로당내성에이상으로면역억제제의사용과관련되어있 J Korean Soc Transplant December 2010 Volume 24 Issue 4

6 는것으로사료된다. 저자들의경우이식후발병한당뇨병의이환율은표준용량의 tacrolimus 그룹에서 13.3% 로감량된용량의 10.1% 에비하여높았으며, 이는감량된용량의 cyclosporine (10.8%) 과유사한수준을보였다. 또한, 538명을이식후 6개월간관찰한연구에따르면 (24), tacrolimus, mycophenolate mofetil, corticosteroid 를사용한그룹보다 tacrolimus, mycophenolate mofetil, daclizumab을사용한그룹이이식후당뇨발병률이더낮았다 (0.4% vs 5.4%, P=0.003). 이를보아저자들은아마도신장이식후발생하는당뇨병의경우 tacrolimus의용량보다는 corticosteroid의용량과더관련성이높을것으로생각한다. 그러므로 steroid의용량을줄여이식후발병한당뇨의위험을줄일수있을것으로예상한다. 그러나 steroid를중단한처방의경우 Hricik 등 (25) 의연구에의하면 681명을대상으로시행한 7개의무작위연구를대상으로한 meta-analysis에서단기간의추적결과급성거부반응이의미있게증가하였다고보고하였으며, 미국의다기관에서시행한전향적인연구에서는급성거부반응의증가로연구를중단하였다고보고하였다 (26). 그러므로 steroid의적절한용량의감소와중단의선택및시기에대한연구가더필요하며신중한임상적선택이더욱필요하다고하겠다. 또다른연구로 1,645명의신장이식환자를대상으로 1 년간시행한표준용량의 cyclosporine 및감량된용량의 cyclosporine, 감량된용량의 tacrolimus 와 sirolimus를사용하여무작위전향적연구에서감량된용량의 tacrolimus 그룹이신장기능의보존, 이식신장의생존, 급성거부반응에있어서유리하다는결론을얻었다 (12). 본연구도이와유사하게감량된용량의 tacrolimus를사용할경우급성거부반응의감소및설사와기회감염면에서의미있게감소되는것으로나타났다. 신장이식에서 tacrolimus와 cyclosporine의부작용을비교한 Pirsch 등 (22) 의연구에의하면신독성, 위장관장애, 신경독성등의빈도는양그룹에서비슷하였고, 고콜레스테롤혈증, 다모증, 치은염, 치은증식등은 cyclosporine 그룹에서, 수지진전, 감각이상, 이식후당뇨병은 tacrolimus 그룹에서빈도가높다고하였다. 유럽의다기관공동연구그룹의성적도 Pirsch 등의성적과비슷하였으나 tacrolimus 그룹에서고칼륨혈증의빈도가더높았고, 고혈압은 cyclosporine 그룹에서더높다고하였다 (20) 본연구에서는설사의경우 10.9% 와 9.8% 로 cyclosporine 그룹에서 tacrolimus 그룹의 9.5% 와 9.0% 보다다소높았으며, 이식후당뇨병의경우표준용량의 tacrolimus 그룹의경우는 cyclosporine 그룹보다높았으 나감량된 tacrolimus 그룹의경우더낮아위의연구와약간상반되는결과를도출하였으며, 감량된용량의 tacrolimus를투약한경우이식후당뇨병이감소된결과를고찰하여볼때, tacrolimus 용량이감소될수록이식후당뇨병의빈도또한줄어들것으로사료된다. 이식후기회감염특히바이러스감염증의발생빈도는 cyclosporine 그룹의경우 23.3% 와 23.5% 로 tacrolimus 그룹의 16.7% 와 16.2% 에비해유의하게높았다. 이는유럽의다기관공동연구그룹 (20) 의성적 26.9% 및 26.7%, 그리고 Pirsch 등 (22) 의 19.3% 및 19.6% 에비하면상반된결과로, 용량감량특히 cytomegalovirus에대한기회감염에대하여 tacrolimus의용량감량이유리한결과를가져오는것으로사료되며, 잘계획된전향적연구가더필요하다고사료된다. 유도약제가있는경우, 급성거부반응이 1년이내에의미있게증가하지않으며 (27), 이전술후유도약제가사용되었던경우 12개월이후및지속적으로 mycophenolate 기반의약제를사용한경우 steroid의완전중단을하여도급성거부반응이새로생기지않으며, 이식신장의기능에무해한것으로증명한연구가발표되고있다 (28, 29). 그러므로 calcineurin 억제제에대한감량시도와유도약제그리고 mycophenolate 기반의면역억제가성공적인이식신장의유지에필수적이라할수있다 (30-32). 추가적으로유도약제및 mycophenolate mofetil에대한 steroid의현재의표준용량보다추가감량에대한연구가더욱필요할것으로생각한다. 결국, 신장이식환자를대상으로한본연구에서 basiliximab 유도와감량된용량의 cyclosporine 또는 basiliximab 유도없이표준용량의 cyclosporine을사용한것에비해감량된용량의 tacrolimus, basiliximab 유도, mycophenolate mofetil, corticosteroid의처방이적절한면역억제를유지하며, 신장기능을유지하고, 급성거부반응을덜일으키는데공헌한다하겠다. 또한, 감량된용량의 tacrolimus 처방은표준용량의 cyclosporine보다이식신장생존에더유리하다고생각한다. 결론 1995년 1월부터 2009년 12월까지 1차면역억제제로 cyclosporine과 tacrolimus를사용하여신장이식을받았던 299 명을대상으로그효과와부작용을후향적으로서로비교하였다. 1년이식신장생존율은네그룹간에유의한차이가없었다. 급성거부반응의빈도는 cyclosporine의경우표준용량은 29.6%, 감량된용량은 19.8%, tacrolimus의경우 J Korean Soc Transplant December 2010 Volume 24 Issue 4

7 표준용량은 23.8%, 감량된용량은 14.3% 로 tacrolimus 그룹특히감량된용량에서의미있게낮았다. 부작용으로는감량된 tacrolimus 그룹에서이식후당뇨, 설사, 기회감염, 특히 cytomegalovirus 감염에서의미있게낮았다. 이상의성적으로 cyclosporine에비하여 tacrolimus가특히, 감량된용량인것이이식신장의생존율과부작용에더나은역할을하였으며, basiliximab과같은유도약제및 mycophenolate mofetil을같이사용하였을때더나은역할을수행한다할수있다.
