4 초 록 소아신장이식환자에서칼시뉴린 억제제의약력학적모니터링 안요한 서울대학교대학원 의학과분자유전체의학전공 서론 : 신장이식환자에서면역억제제로칼시뉴린억제제를사용하면서이식신의생존율이증가하였다. 칼시뉴린억제제를혈중농도측정을통해조절함에도불구하고일부환자들에서는이식신장거부반응, 감염, 칼시뉴린억제제의독성이나타난다. 이연구는칼시뉴린억제제의약력학적모니터링이면역억제정도의평가에유용한지확인하기위하여약동학모니터및임상양상과비교하였다. 방법 : 신장이식을받은 64명의소아환자를대상으로약력학적모니터링을시행하였다. 환자의단핵구에서 Nuclear factor of activated T lymphocytes (NFAT)-regulated gene (IL-2, IFN-γ, GM- iii

5 CSF) 의발현정도를칼시뉴린억제제복용직전과복용 1.5시간후 qpcr를이용하여측정하였고 residual gene expression (RGE) 를계산하였다. Cylex-ImmuKnow assay를통해세포매개성면역반응정도를측정하였다. 칼시뉴린억제제의혈중농도는복용직전과복용 1.5시간후각각측정하였으며이식신거부반응과감염합병증은후향적으로조사하였다. 결과 : ImmuKnow 로측정된면역반응의정도는칼시뉴린억제제의 troough와 peak 농도와상관관계가없었다. 이식신거부반응과감염여부에따른 ImmuKnow 값의차이는없었지만감염의종류를나누어보았을때 EBV 감염이있는환자가감염이없는환자보다 ATP 수치가높았다. (515.4 ± ng/ml vs ± ng/ml, P=0.006) NFAT-regulated gene의 RGE는칼시뉴린억제제의 peak 농도와음의상관관계를보였다. EBV 이외다른감염이있는환자는 IFN-γ RGE가감염이없는환자보다낮았다. (34.0 ± 7.5 % vs ± 30.2 %, P<0.001) 다른 NFAT-regulated gene의 RGE는임상양상과연관성이없었다. 결론 : 본연구에서사용한기존의칼시뉴린억제제의약력학적모니터링방법은감염또는이식장기거부반응의위험성을평가하는데제한이있었다. 보다적절한칼시뉴린억제제약력학적모니터링법의개발을위한추가연구가필요하다 주요어 : 신장이식, 칼시뉴린억제제, 약력학적모니터링학번 : iv

8 서 론 소아신장이식환자에서이식신장의생존율이증가하면서 면역억제제의사용기간이길어지고있다. 이에따라감염, 이식후 종양발생, 면역억제제의약물독성의부작용의유병률이증가하게 되고이식환자의삶의질에악영향을미치게된다.(1-5) 특히 소아이식환자의경우, 이식장기의유지와함께성장과발달을 지속해야하며이식전거대세포바이러스, 엡스타인바바이러스, BK 바이러스등상재바이러스의감염력이없는경우가많으므로 과도한면역억제를회피하여정상적인성장발달을도모하고 기회감염을피해야한다.(6, 7) 한편, 소아이식환자는면역체계가 성인에비해공격적이라고여겨져좀더적극적인면역억제가 요구되므로, 개개의환자에서적절한면역억제가필요하다. Cyclosporine, tacrolimus 등의칼시뉴린억제제는중요한이식 면역억제제로대부분의이식환자에서사용되고있다. 칼시뉴린억제제는 T 림프구내칼시뉴린의활성을억제함으로써 면역억제작용을나타낸다. 칼시뉴린억제제의적정치료농도범위가 좁고약물복용량에대한혈중농도의상관관계가나쁘며, cytochrome P450 을통해대사되는다른약물과의상호작용이 1

9 나타나기때문에, 혈중약물농도측정을통한치료적약물농도 모니터링이필수적이다.(8) 하지만칼시뉴린억제제의치료효과의 근거로사용하고있는혈중농도측정방법은성인의약동학적 특성에따른적정농도를기준으로한것이므로, 성인과약물대사 반응이다른소아환자에서이를그대로이용하는것은적절하지 않다.(9) 또한칼시뉴린억제제의대사와흡수에중요한효소인 CYP3A 와 P-glycoprotein gene 의 polymorphism 의차이로 약물의대사에개인간의차이가있을뿐아니라인종간의차이가 있다고알려져있으므로,(10) 대상환자군에서의검증이필요하다 하겠다. 한편, 칼시뉴린억제제의혈중농도는면역억제정도와비례하지 않아 (11) 약동학적모니터링을통해서적절한농도를유지하는 중에도이식거부반응이나합병증을막을수없는경우가있다. 이에 칼시뉴린억제제의약력학적모니터링을시도하는연구들이 있어왔으며미국 Food and Drug Administration 에서는 면역억제환자에서세포면역기능을평가할수있는 Cylex- ImmuKnow (Cylex, Inc., Columbia, MD) 를 2002 년허가한바 있다.(12-14) 또한유럽에서는고형장기이식환자에서 칼시뉴린억제제의표적이되는 Nuclear factor of activated T cells 2

10 (NFAT)-regulated gene 의발현정도를평가하는연구들이 있다.(15-19) 하지만소아신장이식환자에서의칼시뉴린억제제의약동학, 약력학적모니터링연구는미미한실정이다. 이연구에서는 소아신장이식환자에서약력학적모니터링을시행하고, 이를약동학 및임상양상과비교하였다. 3

11 대상및방법 1. 대상 2010 년 6 월부터 2012 년 2 월까지서울대학교병원에서 신장이식을받은소아환자중칼시뉴린억제제를투여받은경우를 대상으로하였다. 칼시뉴린억제제를투여받으면서최소한 1 차례 이상칼시뉴린억제제의약력학적모니터링을시행하였던환자를 포함하였다. 본연구는서울대학교병원의학연구윤리심의위원회의 승인을받았으며모든환자에게동의서를받았다. 2. 임상분석 칼시뉴린억제제의약력학적모니터링을했던시점에서의대상 환자의병력, 투약력, 인구학적정보, 혈액검사등에관한정보를 전자의무기록조회를통해수집하였다. 면역억제에따른임상양상변화의추적은 1) 환아의혈액혹은 소변의거대세포바이러스 (cytomegalovirus, CMV), 엡스타인바 바이러스 (Epstein-Barr virus, EBV), BK 바이러스 titer 의변화를 측정하였고, 2) 감염은임상적평가와생화학적검사 (C-reactive 4

12 protein, 세균배양등 ) 을통해평가하였으며, 3) 신이식의경우 조직검사나신장초음파검사, 혈중 creatinine 등으로이식신의 상태를평가하였다. 거부반응은요량감소, 발열, 이식신의압통, 혈압상승, 혈청 blood urea nitrogen (BUN) 과 creatinine 상승과함께신초음파 및신생검을통하여진단하였다. 감염은거대세포바이러스, 엡스타인바바이러스, BK 바이러스 titer 의증가또는입원을 요하는감염으로정의하였다. 바이러스감염은각각엡스타인바바이러스 PCR viral load 10,000 copies/ml 이상인경우, BK 바이러스 viral load 10 copies/ml 이상인경우, 거대세포바이러스항원검사 (antigenemia assay) 상신장이식전 CMV IgG 양성인환자에서 400/2x10 5 WBC 이상또는신장이식전 CMV IgG 음성인환자에서 2/2x10 5 WBC 이상인경우로정의하였다. 3. 칼시뉴린억제제의약동학적모니터링 칼시뉴린억제제의혈중농도분석은칼시뉴린억제제복용직전과 복용 1 시간 30 분후시행하였으며서울대학교병원 소아진단검사의학과에서 Chemiluminescent Microparticle 5

13 ImmunoAssay (CMIA) 법을이용하여측정하였다. 4. 칼시뉴린억제제의약력학적모니터링 1) T 세포면역능검사 상용화되어있는 Cylex-Immuknow 를이용하여말초혈액 단핵구내의 CD4 T 림프구 (CD4 cell) 에서 ATP 의합성능을 평가하였다 (12). 2) Nuclear factor of activated T cells (NFAT)-regulated gene monitoring 환자의말초혈액단핵구를 phorbol myristate acetate (PMA) 로 자극시킨후칼시뉴린억제제의표적이되는 nuclear factor of activated T cells (NFAT) 에의해조절되는유전자, 즉 interleukin-2 (IL-2), granulocyte-macrophage colony stimulating-factor (GM-CSF), interferon-gamma (IFN-γ) 의 발현정도를 real-time PCR 로측정하였다 (19). NFAT-regulated gene 발현정도는칼시뉴린억제제직전 (C0) 과투여 1.5 시간 후 (C1.5) 측정되었으며면역억제의정도는 residual gene expression (RGE, C1.5/C0 x 100) 로평가하였다 (18). 3 가지 유전자의발현정도를평균하여 mean RGE 로표현하였다. 6

14 5. 통계분석 SPSS version 21.0 패키지를이용하여통계분석을시행하였다. 나이와기간은중앙값과범위, 그외의연속변수는평균 ± 표준편차로표시하였다. 임상소견비교는 Student T test ( 연속 변수 ) 또는카이제곱검정 ( 비연속적변수 ) 를이용하였다. P 값이 0.05 미만일때유의한차이가있는것으로정의하였다. 7

15 결 과 1. 연구대상환자의특성 총 62 의소아신장이식환자들이연구에참여하였다. 남 / 여비율은 35:27 였으며연구당시중위연령은 12.9 세 ( 세 ) 였으며 신장이식기간은중앙값은 2.9 년 ( 년 ) 이었다. 말기신부전의 원인으로는콩팥황페증 23.4%, 국소성결절성사구체경화증 17.7%, 역류성신장병증 17.7% 였으며신장이식후면역억제제는 칼시뉴린억제제및 mycophenolate mofetil (MMF) 사용이 60.9% 로가장흔하였다. 총 128 회칼시뉴린억제제의혈중농도를 분석하였으며 Immuknow 99 회, NFAT-regulated gene 모니터링 95 회시행되었다. 검사당시대상환자가안정적인상태를유지한 경우는 104 회, 이식거부반응을보인경우는 8 회, 감염의증거가 있는경우는 16 회였다. (Table 1) 이식거부반응을보인환자들의 나이는안정된환자들의나이보다의미있게많았으며 (15.4 ± 3.4 세 vs ± 3.0 세, P=0.037) 감염이있는환자들의 나이와안정된환자들의나이는보다적었으나의미있는차이는 없었다. (10.8 ± 4.0 세 vs ± 2.9 세, P=0.051) 8

17 2. 혈중약물농도와약력학적모니터링비교 칼시뉴린억제제복용직전 /1.5 시간후혈중약물농도와 ATP 합성능 (ng/ml) 을비교시상관관계가없는것으로나타났다. (r 2 = 0.001, P=0.757, r 2 = 0.021, P=0.167) 칼시뉴린억제제복용 직전 /1.5 시간후혈중약물농도와 NFAT-related gene 의 RGE (%) 에서는 GM-CSF RGE 가복용직전 /1.5 시간후혈중약물농도와 의미있는상관관계가있었으며 (r 2 = 0.046, P=0.037, r 2 = 0.066, P=0.015) 평균 RGE 는 1.5 시간후혈중약물농도와상관관계가 있었다. (r 2 = 0.060, P=0.019) (Table 2) ATP 합성능과 NFATrelated gene 의 mean RGE 사이에는의미있는상관관계가없었다. (r 2 = 0.020, P=0.237) 3. 약력학적모니터링과임상지표의관계 감염여부에따라 ATP 합성능의차이를비교해본결과, 감염시 ± ng/ml, 안정시 ± ng/ml 로의미 있는차이가없었다. (P=0.148) 감염여부에따라 NFAT-related gene 의 RGE 를비교했을때도의미있는차이를없었다. (Table 3) 감염의종류를 EBV 감염과 EBV 이외다른감염으로구분하여 결과를분석해보았을때는, EBV 감염시환자의나이가감염이 10

20 없는환자의나이보다의미있게적었으며 (8.2 ± 2.6 세 vs ± 2.9 세, P<0.001) ATP 합성능은 EBV 감염이있는환자가 감염이없는환자보다높았다. (515.4 ± ng/ml vs ± ng/ml, P=0.006) EBV 이외다른감염이있는환자는 IFN-γ RGE 가감염이없는환자보다낮았다. (34.0 ± 7.5 % vs ± 30.2 %, P<0.001) (Table 4) 이식거부반응에따라 ATP 합성능을비교해본결과, 거부반응시 ± ng/ml, 안정시 ± ng/ml 로의미 있는차이는없었다. 이식거부반응에따른 NFAT-related gene 의 RGE 비교시는 IL-2, INF-r, GM-CSF, mean RGE 모두의미 있는차이는없었다. (Table 3) 4. 혈중약물농도와임상지표의관계 감염여부, 이식거부반응여부에따라칼시뉴린억제제의 혈중약물농도는약물복용직전, 1.5 시간후혈중약물농도가모두 차이가없었다. 1.5 시간후혈중약물농도 / 약물용량비는감염시 5.86 ± 3.18 로안정시 4.62 ± 2.52 보다높았으나의미있는 차이는없었다. (P=0.082) (Table 3) 감염의종류에따라분석해보았을때, EBV 감염시 1.5 시간후 13

22 혈중약물농도 / 약물용량비가 6.67 ± 4.13 으로감염이없는 경우의 4.58 ± 2.48 보다의미있게높았다. (P=0.031)(Table 4) 15

23 고 찰 이연구는소아신장이식환자를대상으로하여칼시뉴린억제제의 약동학, 약력학적모니터링의임상적효용성을알아본연구로, 칼시 뉴린억제제의약력학적모니터링결과는감염또는이식거부반응 여부에따라뚜렷한차이를보이지않았다 년미국 Food and Drug Administration 의공인을받은 Cylex-Immuknow 는 mitogenc simulation 에반응하는 CD4+ T cell 이생산하는 ATP 양을측정하여 T-helper 림프구의능력을 평가할수있는도구이다.(13, 14, 20) ATP (ng/ml) 의양에따라 강한면역반응 ( 525), 중등도의면역반응 ( ), 약한 면역반응 ( 225) 으로나누어보고하고있다. 504 명의고형장기 이식환자를대상으로한다기관연구에서는이식신거부반응이 있는경우 ATP 수치가높았으며감염이있는환자에서는 ATP 수 치가낮다고보고하였다.(12) 하지만메타분석에서는감염예측에 대한 ATP 의민감도와특이도가낮고거부반응에대한특이도는 높으나민감도가낮으며각각의연구마다결과의차이가많이나서 16

24 고형장기이식환자에서감염또는거부반응의위험성을평가하 는데충분한증거가없다고하였다.(21) ATP 를반복적으로측정한 연구에서는개개인의차이가심하여각개인의반복적인 ATP 측정 이감염또는이식거부반응의위험성을예측하는데도움이될수 있다고주장하였다.(20) 본연구에서는감염의종류를나누어보았을때 EBV 감염이있 는환자에서 ATP 수치가높은것을확인하였다. 이전한연구에서 12 세이하소아환자에서 EBV 감염시 ImmuKnow 검사의 ATP 수 치가상대적으로높았으며소아신장이식환자를대상으로한또다 른연구에서도비슷한양상을보였다.(22, 23) 이전연구의저자들 은 EBV-trasformed B 세포가 CD4 T 세포를자극하여 EBV 감염 시 ATP 수치가높아졌을것이라고설명하였다.(23) 칼시뉴린억제제복용이후 NFAT-regulated gene 의발현감소 정도는면역억제의정도를반영할수있다.(19, 24) 이전연구들은 통해소아신장이식환자에서 NFAT-regulated gene 의발현감소 가감염의위험요인이된다고알려져있다.(16, 18) 칼시뉴린억제제 의용량과혈중농도가다르지않은경우에도감염여부에따른 17

25 NFAT-regulated gene 의발현정도의차이가있었다.(18) 한연 구에서는신장이식후피부암이생긴환자에서 NFAT-regulated gene 발현정도가낮았다고보고하였다.(17, 25) 신장이식환자뿐 만아니라소아간이식환자에서도 NFAT-regulated gene 발현 정도를이용한칼시뉴린억제제의약력학적모니터링이유효한것으 로보고하였다.(26) 본연구에서는감염또는거부반응과 NFAT-regulated gene 의 발현정도가연관성이없는것으로나타났다. 하지만감염의종류를 나누어보았을때 IFN-γ RGE 이 EBV 이외다른감염시낮은것 을확인하였다. 이는이전연구들에서는반복적인감염이있는환자 와없는환자를구별하였고감염의종류가주로바이러스장염, 폐 렴, 인후염, 신우신염등이었는데비해 (16) 본연구에서는연구당 시의감염상태를평가하였고감염의종류가주로 EBV, CMV 감염 이었다. 연구방법과환자군의차이가다른결과를유발하였을것이 다. 이연구는소아를대상으로한후향적연구이며칼시뉴린억제제 의약력학적모니터링이감염또는이식장기거부반응의위험성을 18

26 평가하는데제한이있었다. 칼시뉴린억제제의약력학적모니터링의 유효성을확인하기위해서는대규모의환자를대상으로하여반복 적인측정을통해평가하는전향적연구가필요할것으로판단된다. 19

32 Abstract Introduction: Introduction of calcineurin inhibitor (CNI) as immunosuppressant has markedly improved the outcome of kidney transplantation. While therapeutic drug monitoring (TDM) is used to adjust the dosage of CNI, some patients, especially children, still suffer from rejection or infection and CNI toxicity. This study was to assess the adequacy of immunosuppression using pharmacodynamic monitoring. Methods: Pharmacodynamic monitoring was done for 64 pediatric kidney allograft recipients. Expression of nuclear factor of activated T lymphocytes (NFAT)-regulated genes in patients' mononuclear cells was measured after activation, by qpcr of IL-2, IFN-γ, and GM-CSF before (trough) and 1.5hr (peak) after ingestion of tacrolimus and the residual gene expression (RGE) was calculated. Global immune response was assessed by Cylex- ImmuKnow assay. Trough and peak levels of tacrolimus were measured and clinical findings of rejection episodes and infectious complications were reviewed retrospectively. Results: Global immune response measured by ImmuKnow was not correlated with trough and peak levels of tacrolimus. ATP level of 25

33 ImmuKnow was higher in patients with EBV infection than in those without infectious complications (515.4 ± ng/ml vs ± ng/ml, P=0.006). Mean RGE of the three NFAT-regulated genes showed negative correlation with tacrolimus peak levels. RGE of IFN- γ was lower in patients with other infections except EBV than in those without infectious complications (34.0 ± 7.5 % vs ± 30.2 %, P<0.001). RGE of IL-2 and GM CSF was not significantly correlated with clinical manifestations. Conclusions: RGE of NFAT-regulated genes and ImmuKnow did not show significant correlation with clinical manifestation of under- or oversuppression of immune function in pediatric kidney allograft recipients.
