Clopidogrel along with aspirin was shown by the CURE trial to result in a 20% relative reduction of cardiovascular death, myocardial infarction or stroke. Ticlopidine has been shown to reduce the incidence of stent thrombosis compared with warfarin, but it may cause serious hematological side effects. However, the reported neutropenia has been similar to clopidogrel and aspirin treatment groups (0.01 vs 0.17%, respectively) with corresponding rates (0.05 vs 0.04%, respectively) of severe neutropenia. We treated a 72-year-old female patient with severe neutropenia who underwent percutaneous coronary intervention with drug-eluting stent placement and had no neutrophils in the peripheral blood at 11 days after clopidogrel use. Therefore, clinicians should be alerted to the possibility of severe neutropenia with clopidogrel treatment.(korean J Med 71: , 2006) Key Words : Clopidogrel, Neutropenia 서론 Clopidogrel은항혈소판제제로서 1) 급성관동맥증후군환자, 경피적관동맥성형술을시행받는환자, 뇌졸중, 심근경색등의경우에혈전형성의예방을위해사용이되고있다 2-5). 특히 aspirin과 clopidogrel의병합요법은관동맥스텐트시술후급성및아급성혈전형성을의미있게감소시킴으로서 5, 6) 광범위하게사용되고있다. 기존의 aspirin은 thromboxane A2으로유도된혈소판응집작용을억제하지만 clopidogrel의작용기전은 ADP로유도된혈소판응집을선택적으로억제하고, 또한 ADP에 의존하는증폭기전도차단시켜강력한혈소판응집억제작용을한다 1). 부작용으로는위장관증상과출혈및피부발진은상대적으로흔하지만호중구감소증은드문것으로보고되어지고있고특히 clopidogrel의사용으로인한 450/mm 3 이하의심각한호중구감소증의발생빈도는 0.05% 로서 aspirin 사용으로인한 0.04% 와큰차이가없으며, ticlopidine 사용으로인한 0.8% 보다는현저히낮은것으로보고가되었다 2). 후군의 72세여자환자에서 clopidogrel을복용 11일째호중구수가 0이된증례를경험하였기에문헌고찰과함께보고하는바이다. 증례 환자 : 김 남, 여자, 72세주소 : 전신통증, 무기력증현병력 : 당뇨병과고혈압병력이있는환자로내원 20일전좌측고관절통증으로좌측대퇴부경부골절을진단받고좌측인공관절반치환술을시행하였다.

(B) Angiogram after stent implantation (Cyper, mm). 후군의 72세여자환자에서 clopidogrel을복용 11일째호중구수가 0이된증례를경험하였기에문헌고찰과함께보고하는바이다. 증례 환자 : 김 남, 여자, 72세주소 : 전신통증, 무기력증현병력 : 당뇨병과고혈압병력이있는환자로내원 20일전좌측고관절통증으로좌측대퇴부경부골절을진단받고좌측인공관절반치환술을시행하였다. 수술 10일후발생한급성심근경색으로 clopidogrel 75 mg을하루에한번씩복용하기시작하였다. 본원에서시행한관상동맥촬영술에서좌전하행지관상동맥중위부에 90% 협착소견, 좌회선지관상동맥중위부에 95% 협착소견을보여각각풍선확장술및약물방출스탠트삽입술 ( 그림 1) 을시행하였고, clopidogrel 복용 7일부터전신통증과무기력증이시작되었고, 복용 11일후오한과 38.5 고열이발생하였다. 과거력 : 20년전당뇨병, 10년전고혈압진단받고경구용혈당강하제와항고혈압제복용중가족력 : 특이소견없음. 사회력 : 음주 (-), 흡연 (-) 이학적소견 : 입원시혈압은 130/70 mmhg, 맥박은 70/min, 호흡수 20/min, 체온은 36.2 이었다. 흉부청진소견에서호흡음이나심음은정상적인소견을보였고, 복부진찰에서특이소견은관찰되지않았고, 기타이학 Figure 2. Peripheral blood smear shows neutropenia ( 200). 적검사는정상이었다. 검사실소견 : Clopidogrel 복용전시행한말초혈액검사상혈색소 12.8 g/dl, 헤마토크리트 36.4%, 백혈구수 7,500/mm 3 ( 호중구 54%, 임파구 29.9%, 단핵구 10.3%, 호산구 3.5%, 호염구 0.4%), 혈소판수는 312,000/mm 3 이었으며, 생화학검사상총 cholesterol 205 mg/dl, LDL cholesterol 133 mg/dl, 중성지방 184 mg/dl 공복시혈당은 126 mg/dl, 기타간기능과신기능검사, 전해질검사, 소변검사는정상이었다. Clopidogrel 복용 11일째에시행한말초혈액검사상혈색소 9.5 g/dl, 헤마토크리트 29.1%, 백혈구수 2,200/mm 3 ( 호중구 0%, 임파구 62%, 단핵구 33%, 호산구 2%, 호염구 2%), 혈소판수는 260,000/mm 3 이였고, 말초혈액도말검사소견상백혈구

Time-related course of absolute neutrophil count (ANC). 는현저히감소되어있었고, 과립구는관찰되지않았으며적혈구모양과혈소판수는정상이었다 ( 그림 2). 말초혈액추적검사는 ( 그림 3) 과같다. 생화학검사상 BUN 16.8 mg/dl, creatinine 0.8 mg/dl, 기타간기능검사, 소변검사는정상이었다. B형간염표면항원과 C형간염항체는음성이었고, 혈액배양과소변배양검사에서음성이었다. 치료및경과 : Clopidogrel의투여를중지하고광범위항생제와호중구회복을위한 granulocyte-colony stimulating factor 75 ug/day를호중수 1,000/mm 3 될때까지피하주사하였다. 3일후부터임상증상이호전되었으며 7일째시행한말초혈액검사상혈색소 10.5 g/dl, 헤마토크리트 31.5%, 백혈구수 7,600/mm 3 ( 호중구 47%, 임파구 33%, 단핵구 10%, 호산구 2%, 호염구 2%), 혈소판수는 438,000/mm 3 로백혈구수치가정상으로회복되었다. 고찰 관상동맥스텐트시술의가장큰문제점은급성, 아급성혈전증 (acute or subacute thrombosis) 및스텐트재협착이라고할수있다. 스텐트혈전증은최근 aspirin, ticlopidine 등의항혈소판제제의적극적인사용으로발병빈도가 1% 미만으로감소하였다 7). 새로운항혈소판제제인 clopidogrel도 ticlopidine과같은효과가있으면서 ticlopidine의심각한합병증인혈소판감소증및백혈구감소증등의합병증발병빈도가더적어안전하고효과적인약으로보고되고있다 8). 그러나 ticlopidine은위장장애및알레르기반응이비교적흔하고, 혈소판감소증및중성혈구감소증을일으킬수있기때문에현재는 clopidogrel로대치되고있는실정이다 9). Clopidogrel 은특이적으로 adenosine diphosphate (ADP) 에의해유도되는혈소판응집을억제하고경구투여된후간에서대사를받아활성대사체가되어선택적으로그리고비가역적으로혈소판에있는 ADP 수용체를변형시킴으로써 ADP가 ADP 수용체와결합하는것을방해하며, 또한 ADP에의해일어나는피브리노겐과 glycoprotein 복합체의결합을억제하고 ADP에의해유도되는혈소판응집증폭작용을적절히억제한다 10, 11). 경구투여 2시간후에유의한혈소판응집억제효과가나타나고투여시작후 3~7일에항정상태 (steady state) 의항응집효과가유지되며 12) 약제투여중지후에도일단혈소판에작용하게되면 ADP 수용체를비가역적으로변형시키므로이혈소판의수명이다할때까지약 7일간응집억제효과를나타내게된다 13). 최근중재시술을기본으로한 PCI-CURE 연구에서는경피적관상동맥중재술후 clopidogrel과위약을무작위로투여하여 ( 평균 8개월 ) 1년간임상경과를관찰하였다. Clopidogrel 군에서 glycoprotein IIb/IIIa 차단제의사용이적었는데도위약군에비해 30일시점의일차종결점 ( 사망, 심근경색증, 재시술 ) 의발생빈도는낮았으며 (4.5% vs 6.4%, p=0.03), 그효과는 1년후까지지속되었다 6). CREDO (Clopidogrel for the Reduction of Events During Observation) 연구는대상환자중 70% 가급성관동맥증후군환자로 clopidogrel과위약을경피적관상동맥중재술시작 3~24시간전에투여한후 1년간지속적으로투여하면서추적관찰하였다. 30일시점에서는 clopidogrel군과위약군사이에주요심질환 ( 사망, 심근경색증, 재시술 ) 의차이는없었으나, 적어도 6시간전에 clopidogrel loading한군에서는위약군에비해위험도가 38% 정도감소하였다. 1년시점에서는 clopidogrel군에서사망, 심근경색증및뇌졸중의발생위험도가 26.9% 정도감소하였다 3). 따라서최근 ACC/AHA 지침에서는 clopidogrel을반드시사용하도록권장하고있으며, 300 mg을부하용량으로투여하고이후일일 75 mg을유지용량으로 9~12개월동안 aspirin과같이투여하도록권장하고있다 14). Clopidogrel의사용으로인한부작용으로는복통, 소화불량, 설사, 오심과같은위장관계증상과피부발진, 출혈등이흔하며호중구감소증 (0.10%), 혈소판감소증 (0.26%), 재생불량성빈혈 (1예) 과같은혈액학적이상소견이드문것으로보고되어지고있고호중구수가

4 - 김민웅외 6 인 : Clopidogrel 에의해유발된호중구감소증 1 예 - 450/mm 3 이하인중증호중구감소증은 0.05% 로서 aspirin 사용으로인한 0.04% 와큰차이가없으며, ticlopidine 사용으로인한 0.8% 보다는현저히낮은것으로보고가되었다 2). 또한 CAPRIE (clopidogrel versus aspirin in patients at risk of ischemic events) 연구에서절대호중구수가 0인심각한호중구감소증 2예가보고가된적이있을정도로 clopidogrel의사용으로인한무과립구증의경우는지극히드문합병증으로알려져있으며, 그병인에대해서도아직까지잘알려져있지않다. 국외문헌에서재생불량성빈혈 15), 혈전성혈소판감소성자반증 16), 범혈구감소증 17) 등이매우드물게보고되고있다. 치료로는즉시 clopidogrel을중지해야하고 Akcay 등 18) 은 clopidogrel에의한호중구감소증환자에 granulocytecolony stimulating factor와광범위항생제를투여함으로써부작용없이치료한예를보고하였다. 본증례에서 clopidogrel 투여중단후지속적인백혈구감소증이계속될것으로예측되어경험적항생제와 granulocyte- colony stimulating factor를투여하여 3일후임상증상이호전되었고, 7일만에호중구수 1,000/mm 3 이상으로회복되었다. 본증례에서 clopidogrel 외에도다른약물이나감염에의한호중구감소증의가능성있을수있으나병력상고혈압과당뇨병으로 aspirin, valsartan, atenolol, torasemide, insulin 등을적어도 2년이상혈액학적부작용없이사용하였고, 최근호중구감소증을유발할만한다른약물을복용한적이없었고, 직접적인골수감염을의심할수있으나호중구의선택적인감소증만있는것으로보아 clopidogrel에의한호중구감소증으로추정된다. 중요한점은 clopidogrel은드물지만호중구감소증을유발할수있는약제로서투약후부작용발생에대해서관심을가져야하고말초혈액검사를하여호중구감소여부를감시하여야하며, 중증호중구감소증과발열이동반된경우신속한 clopidogrel 투여중단과적절한항생제투약및 granulocyte- colony stimulating factor의조기투여가호중구감소증의빠른회복과병기단축및치료에도움이되리라생각된다. 요약 Clopidogrel 사용으로유발되는무과립구증은매우드문합병증이다. 저자들은급성관동맥증후군의 72세여자환자에서 clopidogrel을복용 11일째발생한열을동반한중증호중구감소증에대하여투약중단과광범 위한항생제치료와 granulocyte-colony stimulating factor를투여하여호전되었던 1예를경험하였기에문헌고찰과함께보고하는바이다. 중심단어 : Clopidogrel, 호중구감소증 REFERENCES 1) Herbert JM, Savi P, Maffrand J-P.
