Materials and Methods: Between September 2011 and September 2012, 12 patients who underwent percutaneous needle aspiration of the paraspinal lesions for suspected pyogenic spondylitis, tuberculous spondylitis, or metastasis were included. The procedure was performed on the XperGuide CBCT. Based on the initial CBCT, target points and entrance were determined, using dedicated guidance software (XperGuide), and the needle pathway was visualized in various reconstructed images. Needle aspiration was performed using Westcott needle and correct needle positioning was confirmed with the second CBCT. The technical success was defined as the needle tip reached within 5 mm of the target point. Culture, smear, and polymerase chain reaction of obtained samples were performed. Results: In all twelve patients, target areas could be determined based on XperCT data and achieved 100% technical success. Diagnosis could be made in 91.7% of cases and the median interventional procedure time was 27.8 minutes. There was no major complication in all patients. Conclusion: Percutaneous needle aspiration of paraspinal lesions using XperGuide CBCT is easy, accurate, safe, and useful in determining the treatment of direction. Tel Fax parkhiji@gmail.com Copyrights 2013 The Korean Society of Radiology 서론 척추체및척추주변부병변이있을때, 감염성척추염과악성을구분하고감염성척추염중에서결핵성척추염과화농성척추염을구분하는것은치료방침결정에중요하다. 척추체및척추주변부병변의경피적생검술은투시장치또는전산화단층촬영 ( 이하 CT) 의유도하에시행되고있으며, CT의발달로 CT 유도하경피적생검술이보편화되었다 (1-3). 콘빔 CT (cone beam computerized tomography; 이하 CBCT) 는 C-arm 투시장치를통해 3차원영상을획득할수있는신흥기술이다. CBCT 는뼈와연부조직의해상력이뛰어나기때문에두경부수술의술전계획에사용되었지만최근까지 CBCT 가 real-time tracking 과 navigation 에사용되지는않았다 (4, 5). XperGuide CBCT (XperCT, XperGuide, and Allura FD20 angiography interventional system, all from Philips Healthcare, The Netherlands) 는새롭게개발된 3차원투시유도장치로, 전용계획프로그램을가진 CBCT 와실시간투시장치를병합한장치이다. 최근 XperGuide CBCT 를이용한척추병변의바늘중재술이안전하고정확하다는연구들이있었다 (6, 7). 하지만 XperGuide CBCT 를이용하여척추주변부병변만을대상으로한바늘흡인술의정확도나안정성에관한연구는없었다. 저자들은 XperGuide CBCT 를이용한척추주변부병변에대한바늘흡인술의초기경험을보고하고자하였으며, 정확도, 안정성, 그리고진단적유용성에대하여알아보고자하였다. submit.radiology.or.kr 대한영상의학회지 2013;68(3):

대상과방법 연구대상 2011 년 9 월부터 2012 년 9 월까지, 척추자기공명영상 ( 이하 MRI) 에서감염성척추염혹은전이가의심되었던환자에서, XperGuide CBCT 를이용한흡인술을시행받은 12 명의환자 를대상으로하였으며, 남자가 9 명, 여자가 3 명이었고, 나이는 55 세에서 79 세로평균 70 세였다.

1B) 피부에표지를붙이고나서, 첫번째비조영 CBCT 가시행되었고얻어진영상은전용 workstation (Allura 3D-RA Release 5/XperCT Release 2, Philips Medical System, Best, the Netherlands) 으로보내졌다.

사용된 reconstruction matrix 는 512 3, voxel 크기는 0.20~0.49 mm였고, 절편두께 (slide thickness) 는조절가능하였다. 재구성된자료용적은등방성이며, 어느투사각도에서도최적의재구성영상을얻을수있었다.

또한주변구조물을확인함으로써시술에의한손상을최소화할수있었다. 예측된바늘경로영상은계획된목표점과입구점으로만들어졌으며, 시술을할때예측된바늘경로영상과실제바늘의경로를합성영상으로같은화면에서확인할수있었다 (Fig. 2). 영상유도하시술이시작되면 C-arm 은바늘경로영상에맞는위치로이동되었고, 바늘의방향과각도는바늘유도장치를 A B C Fig. 2. A 69-year-old male, pyogenic spondylodiscitis.

2 대상과방법 연구대상 2011 년 9 월부터 2012 년 9 월까지, 척추자기공명영상 ( 이하 MRI) 에서감염성척추염혹은전이가의심되었던환자에서, XperGuide CBCT 를이용한흡인술을시행받은 12 명의환자 를대상으로하였으며, 남자가 9 명, 여자가 3 명이었고, 나이는 55 세에서 79 세로평균 70 세였다. 흡인술이후진단이내려지 기전척추 MRI 에근거한대상군의임상진단은결핵성척추염 (n = 3), 화농성척추염 (n = 6), 그리고전이성척추암 (n = 3) 이었다 (Table 1). XperGuide CBCT 를이용하였으며, 흡인용바늘은 20 gauge 의 Westcott 바늘을사용하였다 (Fig. 1A). 환자의자세는복와위로하고 (Fig. 1B) 피부에표지를붙이고나서, 첫번째비조영 CBCT 가시행되었고얻어진영상은전용 workstation (Allura 3D-RA Release 5/XperCT Release 2, Philips Medical System, Best, the Netherlands) 으로보내졌다. 한번의 CBCT 가시행될때, 환자주위로 C-arm 이 240 도회전하면서 624( 혹은 312) frames 을얻을수있었다. 재구성범위 (reconstructed coverage) 는처음정해진관찰영역 (field of view) 에의존하였으며대개 cube side length 는 100~250 mm 3 범위였다. 사용된 reconstruction matrix 는 512 3, voxel 크기는 0.20~0.49 mm였고, 절편두께 (slide thickness) 는조절가능하였다. 재구성된자료용적은등방성이며, 어느투사각도에서도최적의재구성영상을얻을수있었다. 결론적으로 CBCT 로약 5 Hounsfield unit 의대비해상도 (contrast resolution) 를얻을수있었고, 이것은척주주위연부조직의범위를확인하고, 연부조직으로의정확한바늘삽입궤도를계획하기에적절하였다. CBCT 에서얻어진데이터는전용 workstation 으로보내져목표점과입구점을계획할수있었고, 예측된바늘경로영상을볼수있었다. 또한주변구조물을확인함으로써시술에의한손상을최소화할수있었다. 예측된바늘경로영상은계획된목표점과입구점으로만들어졌으며, 시술을할때예측된바늘경로영상과실제바늘의경로를합성영상으로같은화면에서확인할수있었다 (Fig. 2). 영상유도하시술이시작되면 C-arm 은바늘경로영상에맞는위치로이동되었고, 바늘의방향과각도는바늘유도장치를 A B C Fig. 2. 3 김솔잎외 이용하여고정되었다. 바늘의경로는합성영상으로볼수있었고, 계획된바늘궤도에맞추기위해필요시 C-arm 의위치를바꿀수있었다. 바늘이목표점에도달하면, 정확한바늘의위치를보기위해두번째 CBCT 를찍었다. 기술적성공은계획된목표점의 5 mm 내에바늘끝이위치하였을때로정의하였고, 두번째 CBCT 에서확인된바늘의위치와첫번째 CBCT 에서의목표점을비교하였다. 마지막으로, 합병증을확인하기위한 CBCT 를찍고시술을마쳤다. 시술에서얻어진흡인체에대해배양, 도말, 그리고결핵균에대한중합연쇄반응 (polymerase chain reaction; 이하 PCR) 검사를시행하였다. 결과 총 12예에서척추주위병변의흡인위치는흉추 (n = 7), 요추 (n = 5) 였다. 12예중 11예 (91.7%) 에서흡인을통해얻은검체에서화농성척추염 (n = 8), 결핵성척추염 (n = 2), 전이 (n = 1) 와같은진단이가능하였다 (Table 1). 화농성척추염으로진단된 8예중 5예에서각각 Streptococcus agalactiae (n = 2), Streptococcus equisimilis (n = 1), Acinetobacter baumannii (n = 1), Klebsiella ozaenae (n = 1) 가배양되었고, 3예에서는배양은되지않았지만대부분호중구로이루어진염증세포를보여화농성척추염으로진단할수 있었다. 결핵성척추염으로진단된 2예에서는모두도말로항산균 (acid-fast bacilli; 이하 AFB) 확인이되었고 PCR 검사에서양성이었으나, 결핵균배양은 1예에서만되었다. 전이를의심했던 1예에서는세포도말검사에서악성세포가나와전이성척추암으로진단할수있었다. 12 예중흡인체에서진단이되지않았던 1명의환자는추후수술을통해얻은조직에서만성육아종성염증소견을보였고, 조직학적으로 AFB 가확인되었으며 PCR 검사에서도양성이나와결핵성척추염으로진단되었다. 처음 CBCT 를찍을때부터마지막 CBCT 를찍을때까지, 즉영상유도하흡인술을시행하는데걸린시간은 10분에서 30 분으로, 평균 21.8 분이었다. 환자를혈관조영술테이블에올린시간부터, 시술전후준비와마무리하는시간까지포함한총시술시간은 14분에서 33분으로, 평균 27.8분이었다. 12예에서 CBCT 의평균선량면적곱 (dose area product; 이하 DAP) 은 mgy/cm 2 ( 표준편차, ± 7313 mgy/cm 2 ), 투시검사의평균 DAP 는 mgy/cm 2 ( 표준편차, ± 4251 mgy/cm 2 ) 로시술의총 DAP 는평균 mgy/cm 2 ( 표준편차, ± 9875 mgy/cm 2 ) 였다 (Table 2). 11명의환자에서 CBCT 는 3번시행되었고, 1예에서만정확한위치를확인하기위해 1번의 CBCT 가더시행되어 4번의 CBCT 가시행되었다. 12예모두에서계획된목표점의 5 mm 내에바늘끝이위치하여 100% 의기술적성공을얻을수있었다. 시술도중또는시술후에심한동통을호소하거나출혈, 감 Table1. 4 염, 신경학적후유증및내부장기손상등의합병증이있었던경우는없었고, 1명의환자가시술후경미한동통을호소하였으나경구진통제로완화되었다. 고찰 척추병변의정확한진단은치료방향결정에중요하다. 원발암이없는환자에서종종임상소견과영상소견만으로는척추전이와감염성척추염을구분하기어려운경우가있고, 감염성척추염중에서도화농성척추염과결핵성척추염을감별하기어려운경우가있다. 척추전이가의심되는경우에는원발암을모르는경우원발암을찾는검사를해야하고, 원발암이있는환자에서는방사선치료또는화학적치료를할수있으므로척추병변에서전이를배제하는것이매우중요하다. 또한감염성척추염이의심될때병원체에따른약물의선택을위해원인균을규명하는것이중요하겠다. 수술을통한척추병변의생검은정확도는높으나그자체로많은위험과합병증을수반할수있기때문에덜침습적인경피적생검혹은흡인술을위한방법들이모색되었다. 최근 CT의발달로척추체와척추주위병변의 CT 유도하경피적생검술이보편화되었다 (1-3). Rimondi 등 (8) 은 430 예의척추병변의 CT 유도하경피적생검에서 93% 의성공률을보고하였고, Nourbakhsh 등 (9) 은경피적척추생검의 meta-analysis 에서 CT 유도하경피적생검의정확도를 90.2% 로보고하였다. XperGuide CBCT 는새롭게개발된 3차원투시유도장치로, 전용계획프로그램을가진 CBCT 와실시간투시장치를병합한장치이다. XperGuide CBCT 를이용한척추병변의바늘중재술이안전하고정확하다는연구들이보고되고있다. Braak 등 (6) 은 XperGuide CBCT 를이용한다양한해부학적부위의바늘중재술에대한연구에서 100% 의기술적성공률과 91.4% 의조직학적진단율을보고하였고, 4.3% 의경미한합병증이있었다고보고하였다. Leschka 등 (7) 은 XperGuide CBCT 를이용한 12명의척추와골반의중재술에관한연구에서 92% 의기술적성공률을보고하였고, 합병증은한예도없었다. 본연구에서도모든예에서계획된목표점의 5 mm 내에바늘끝이위치하여 100% 의기술적성공을얻을수있었고, 심한합병증이발생한경우는없었기때문에 XperGuide CBCT 를이용한중재술이안전하고정확하다는것을확인할수있었다. 최근 CT와 CT 유도하중재술의사용이증가함에따라환자의방사선조사량이증가되고있다 (10, 11). Braak 등 (12) 은 CBCT 유도하바늘중재술과고식적 CT 유도하바늘중재술의유효선량을비교한연구에서, CBCT 유도하중재술이고식적 CT를이용한경우보다 13~42% 의유효선량을감소시킬수있었다고보고하였다. 또한 Strocchi 등 (13) 도 CBCT 와고식적 CT를이용한흉부경유생검비교에서, 고식적 CT에서유효선량이 CBCT 보다 50% 더높다고보고하였다. Leschka 등 (7) 이 CBCT 를이용한척추와골반의중재술에서총평균 DAP 를 mgy/cm 2 로보고하였으나, 본연구에서총평균 DAP 는 mgy/cm 2 ( 표준편차, ± 9875 mgy/cm 2 ) 로분석되었다. 또한 Leschka 등 (7) 이척추와골반의중재술에서평균시술시간을 111분 (60~180 분 ) 으로보고하였으나, 본연구에서평균시술시간은 27.8 분으로분석되었다. 그이유는 Leschka 등의연구에서는대부분의환자에서전신마취하에척추생검을시행하였기때문으로생각된다. 본연구에서는모든예에서척추주위병변에대해바늘흡인술을시행하였기때문에전신마취는필요하지않았다. Hwang 등 (14) 은감염성척추염에서 CT 유도하생검및흡인이악성을배제하는데에는유용하지만진단율을 50% 이하라고보고하였다. 본연구에서 12명의환자에서진단은 11명에서이루어져, 진단율은 91.7% 로분석되어 XperGuide CBCT 를이용한바늘흡인술이감염성척추염과전이를진단하는데유용하다는것을알수있었다. 이연구에서의제한점이몇가지있다. 첫째, 12예중에서종양성병변으로진단된경우는단 1예로종양성병변에대한진단의정확도를일반화시키기에는증례수가너무적었다. 둘째, 생검이아닌흡인술을시행했기때문에종양성병변의경우원발암을예측하는데무리가있었다. 셋째, 실제로고식적 CT 유도하시술과 CBCT 유도하시술을비교한연구가아니었기때문에유효선량감소에대한정확한정보를제공할수없었다. 결론적으로화농성척추염, 결핵성척추염, 그리고종양성병변을감별하는데 XperGuide CBCT 를이용한경피적흡인술은빠르고, 안전하고, 정확하고, 또한 CT 유도하시술에비해방사선조사량을감소시킬수있으며, 치료방향을결정하는데아주유용한검사로사료된다. 참고문헌 1.
