A unilateral adrenalectomy in hypertensive and hypokalemic patients, with a well-documented adrenal adenoma, is usually followed by the correction of hypokalemia in all subjects, with the cure of hypertension in 60 to 87% of patients. Here, a unique case, in which a unilateral adrenalectomy for the removal of an adrenal adenoma was followed by severe hyperkalemia, low levels of plasma renin activity and serum aldosterone, suggestive of chronic suppression of the renin-aldosterone axis, is reported. In a follow-up Lasix stimulation test on the 70th day after surgery, the suppression of the renin-aldosterone axis was resolved, indicating the suppression was transient. Patients undergoing a unilateral adrenalectomy for an aldosterone-producing adenoma should be closely followed up to avoid severe hyperkalemia (J Kor Soc Endocrinol 20:502~506, 2005). ꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏ Key Words: Aldosterone-producing adenoma, Adrenalectomy, Hypoaldosteronism 8) 서론 원발성알도스테론증은사구층에서생긴선종 ( 드물게암종 ) 또는증식증에의해비정상적으로알도스테론생성이증가하여나트륨저류에의한고혈압과고알도스테론증에의한저칼륨혈증을보이는질환이다. 이중편측성알도스테론분비선종이전체원인의 60% 정도를차지하며편측부신선종제거술을할경우거의대부분에서저칼륨혈증이교정되고 60~80% 에서고혈압이정상으로회복된다 [1~6]. 접수일자 : 2005 년 8 월 8 일통과일자 : 2005 년 8 월 23 일책임저자 : 고관표, 건양대학교의과대학내분비내과 편측부신선종절제술을받은 37명을대상으로한전향적연구에서 Gordon 등 [7] 은수술후 2년까지혈관내용적이감소하고혈청알도스테론수치가낮아지고혈장레닌활성도는증가함을보고하였고, 33명중 5명이 1년후에도저혈압을보였고, 29명중 2명은 2년후에도저혈압을보였는데이는부신절제로인해코티솔과알도스테론등의부신피질호르몬과아드레날린의분비가감소하였기때문이라고하였다. 하지만드물게편측부신선종제거술후일과성으로저레닌성저알도스테론증을보일수있는데미국에서는 Gadallah 등 [8] 이 1례를보고한적이있으나우리나라에서는아직보고된적이없기에문헌고찰과함께 1례를보고하는바이다

2 - 서정호외 9인 : 알도스테론분비선종에서편측부신제거술후발생한일과성저레닌성저알도스테론증 1례 - 증례환자 : 최 자, 50세여자주소 : 양측상하지의쇠약감현병력 : 환자는 5년전부터 3가지이상의혈압약을복용하였으나내원시혈압 160/90 mmhg로잘조절되지않았고내원 1년전부터간헐적으로양하지근무력증이있다가내원 2개월전부터증상이악화되어 2004년 5월 3일경희의료원재활의학과를방문하였다가내과로전과되었다. 과거력 : 5년전부터고혈압으로 3가지이상의혈압약을복용하였으나잘조절되지않음가족력 : 특이사항없음이학적소견 : 신체검사소견 : 혈압은 160/90 mmhg, 맥박은분당 85 회, 신장은 156 cm이었고체중은 47kg으로체질량지수는 19.2 kg/m 2 이었다. 양하지의쇠약감과근력약화가있었고, 심부건반사는양쪽상하지모두정상이었으며기타신경학적이상소견은없었다. 검사실검사소견 : 말초혈액검사상 Hb 12.0g/dL, Hct 33.9%, 소변검사와간기능검사는정상이었다. BUN 12 mg/dl, Cr 0.6 mg/dl, calcium 9.5 mg/dl, phosphorus 3.6 mg/dl였으며, 혈청전해질검사상 Na 146 mmol/l, K 2.3 mmol/l, Cl 89 mmol/l로심한저칼륨혈증을보였지만 24 시간소변의칼륨양은 23.4 mmol/day로혈청칼륨치를고려할때상대적으로소변내칼륨배설은증가해있었다. 심전도소견역시심한저칼륨혈증에합당한 U-wave 소견을보였다 (Fig 1). 동맥혈가스검사에서는 ph 7.50, PaCO 2 45 mmhg, PaO 2 85mmHg, HCO 3 32mmHg로 metabolic alkalosis에합당한소견을보였다. 무작위로채혈한검체에서혈중갑상선자극호르몬농도는 2.4 U/mL였고, 코티솔농도는 10 μg/dl로모두정상범위안에있었으며, 무작위로채혈한검체에서혈장레닌활성도와혈청알도스테론은 각각 0.3 ng/ml/hr, 645 pgm/ml로일차성고알도스테론증이의심되었다. 입원후시행한 Lasix stimulation test에서혈청내 aldosterone 기저치는 800 pg/ml 이상으로크게증가해있었고, 혈장내 Renin 활성도의기저치는 0.33 ng/ml/hr로감소돼있었다. Furosemide 40 mg을정주하고 2시간동안걸어다니게한후측정한혈청내 aldosterone 자극치는 687 pg/ml로기저치보다오히려약간감소했으며, 혈장 rennn 활성도의자극치도 0.64 ng/ml/hr로여전히감소된결과를보여알도스테론분비선종에합당한소견을보였다 (Table 1). 복부전산화단층촬영에서도오른쪽부신에양성선종으로의심되는 cm크기의종괴가발견되었다 (Fig. 2-A, B). 치료및경과 : 환자는우측부신선종에의한원발성고알도스테론증진단하에 Spironolactone 50 mg/day, 및 Losartan 75 mg, Nifedipine 33 mg/day, 및 potassium supplement를수술전 1개월간투여하여혈압 140/80 mmhg, K 4.5 mmol/l를보였고비뇨기과로전과되어 Lateral transperitoneal approach로우측부신에대해 Laparoscopic adrenalrectomy를시행하였으며, 병리학적진단은부신피질선종이었다 (Fig. 3-A, B) 환자는수술후별문제없이퇴원하였으나수술 10일후외래에서측정한혈압이혈압약을복용하지않은상태에서 150/90 mmhg를보였고혈청내 K은 6.4 mmol/l로증가되어있었다. 당시혈장 rennin 활성도는 0.4 ng/ml/hr로여전히낮아져있었고혈청 aldosterone은 40 pg/ml로감소돼있어서혈압이조금높긴하였지만, 수술후일시적으로생긴저레닌성저알도스테론증으로생각하였다 (Table 1). 칼륨교환수지인 calcium polystyrene sulfonate (Kalimate R ) 를투여하여수술후 21일째는 K 5.0 mmol/l로낮아졌으나, Kalimate R 복용을중단한후에는다시 K 6.1 mmol/l Fig. 1. The wellmarginated circular mass is about 1cm in diameter and contains much lipid. Around the adenoma there is perirenal fat tissue. 로높아져, 수술후 40일째부터 mineralocorticoid 제제인 Fludrocortisone 0.

수술후 70 일째시행한 Lasix stimulation test 상에서혈장레닌활성도는기저치 1.6 ng/ml/hr, 2시간후 5.6 ng/ml/hr로정상적인반응을보였고혈청알도스테론도기저치 180 pg/ml, 2 시간후 315 pg/ml 로정상에가까운반응을보여수술후 Fig. 3-B.

Around the adenoma there is perirenal fat tissue. 로높아져, 수술후 40일째부터 mineralocorticoid 제제인 Fludrocortisone 0.1 mg/day을투여하였고, 수술후 2개월째에는혈청 K 4.5 mmol/l, 혈압은혈압약을복용하지않고 155/105 mmhg를보였다. 환자의혈압이높은이유는수술로인해알도스테론이감소했더라도, 오랜기간에걸쳐고혈압이있었기때문에혈관경직도가증가하였거나원래부터원발성고혈압이있었던것으로생각하였다. 수술후 70 일째시행한 Lasix stimulation test 상에서혈장레닌활성도는기저치 1.6 ng/ml/hr, 2시간후 5.6 ng/ml/hr로정상적인반응을보였고혈청알도스테론도기저치 180 pg/ml, 2 시간후 315 pg/ml 로정상에가까운반응을보여수술후 Fig. 3-B. Microscopic finding of adrenal adenoma which shows lipid-abundant clear cells and giant cells. 생긴고칼륨혈증은일시적인저레닌성저알도스테론증때문인것으로생각되었다 (Table 1). 환자는현재혈압조절을위해칼슘길항제 Lacidipine 2 mg/day, 이뇨제 Hydrochlorothiazide 12.5 mg/day 만복용하고있으며, 혈압과혈청칼륨치는정상을유지하고있는상태로특별한문제없이외래추적관찰중이다. 고 본환자는조절되지않는고혈압, 심한저칼륨혈증, 기저치 renin 활성도저하및 aldosterone 증가소견과함께 Lasix stimulation test 후에오히려기저치보다 aldosterone 이더감소하였고, 혈장 renin 활성도역시감소되어알도스 찰

PRA: plasma rennin activity (2.47 ± 2.23) 315 (216 ± 122) 테론분비선종에의한일차성고알도스테론증이의심되었으며, 이는복부전산화단층촬영과수술을통해부신피질선종으로확인되었다. 앞서서론에서말한바와같이편측성부신선종에대하여부신절제술을할경우거의대부분에서저칼륨혈증이조절되고, 60~80% 에서고혈압이조절된다고한다 [1~6]. 그러나이환자의경우수술후오히려고칼륨혈증이생기고혈압은평균 20~30 mmhg 감소하는소견을보였다. 수술후 10일째외래에서측정한혈장레닌활성도와혈청알도스테론수치는각각 0.4ng/mL/hr, 40pg/mL로저레닌성저알도스테론증에합당한소견을보였으며, 수술 2개월째다시시행한 Lasix stimulation test에서레닌알도스테론축은정상으로회복되었고, 고칼륨혈증은호전되었다. 정상코티솔분비를보이는알도스테론부족증은저레닌증과함께나타나거나, 유전적생합성장애, 알도스테론분비선종절제술후, 지속적인헤파린투여, 심한체위성저혈압등에서나타날수있다. 대개저레닌증을동반하므로저레닌성저알도스테론증이라고한다 [13]. 이번증례처럼부신선종으로편측성부신절제술을받은환자에서일과성저레닌성저알도스테론증이오는경우는매우드물며, 1998 년에 Gadallah 등 [8] 이한예를발표한적이있다. Biglieri 등 [9] 과 Morimoto 등 [10] 은이것이 juxtaglomerular (JG) apparatus가지속적으로억제되어생겼기때문이라고하였다. JG apparatus가일차성고알도스테론증이있으므로지속적으로억제되어있고이로인해레닌분비가억제되고결과적으로알도스테론분비도억제된다는것이다. Biglieri 등 [9] 에따르면편측부신선종절제술을받고저레닌성저알도스테론증을보인 18명에서수술후처음 4주동안은소변내알도스테론수치가거의측정되지않을만큼낮았다가 16명에서 1개월후부터는점차로알도스테론분비가증가되었다고하며, 나머지 2명은정상수치로회복되는데 5개월, 7개월이걸렸다고한다. 한편 Morimoto 등 [10] 은수술전 spironolactone을하루 300~400 mg 투여했을때수술후저혈압이나고칼륨혈증을예방할수있었다고하며, spironolactone을투여했을때혈장레닌활성도와알도스테론은정상이었다고한다. 반면 spironolactone을투여하지않은환자에서는혈장레닌활성 도가낮았다고하는데이는부신선종에서분비된알도스테론에의해지속적인 hypervolemia가 JG apparatus를억제하였기때문이며다시 renin 분비를회복하는데는시간이걸린다고하였다. 비슷한결과를 1965년에 Conn 등 [11] 도발표하였는데최소한수술 2개월전 spironolactone 을투여하였을때 JG apparatus에서 renin 분비가이루어져수술후에도정상레닌알도스테론축을보였다고한다. 저레닌성저알도스테론증의치료는합성 mineralocorticoid인 fludrocortisol (fludrocortisone acetate) 을 1일 0.1~ 1.0 mg 경구투여하면약 90% 에서정상적인혈청칼륨치를유지할수있다고하며, 이때염류저류작용에의한부작용이동반되는경우이뇨제를병행하여투여하는것이좋다 [12]. 요약편측성부신선종에대하여부신절제술을할경우거의대부분에서저칼륨혈증이조절되고, 60~80% 에서고혈압이조절되는것으로알려져있다. 저자들은부신피질선종에의한원발성알도스테론증환자에서편측부신절제술후에일과성으로생긴저레닌성저알도스테론증 1예를경험하였기에문헌고찰과함께보고하는바이다. 참고문헌 1.
