Nevertheless, if ventricular fibrillation recurs, ICD implantable cardioverter-defibrillator therapy is necessary. ICD is an important nonpharmacological option in the treatment of malignant ventricular arrhythmias. 53-year-old man was admitted into our hospital with a severe chest pain of 9 days. On cardiac catheterization, a significant stenosis at the mid-left anterior descending coronary artery and a huge ventricular aneurysm were found. Then PTC percutaneous transluminal coronary angioplasty with a stent was taken. Three days later, two episodes of ventricular fibrillation developed and it was treated with electrical cardioversion and antiarrhythmic agents. On the second look of cardiac catheterization, a subacute closure of the stented site was detected and then the culprit leision was recanalized with repeated PTC and another stent was implanted. ut ventricular fibrillation recurred although the stented lesion still patent on the third look of cardiac catheterization. fter all, we implanted ICD on him. 46 Episodes of ventricular fibrillations developed at the same day and the next day of ICD implantation. ll of the episodes of ventricular fibrillation were successfully converted to sinus rhythm with shocks from ICD. Since then, the frequency of ventricular fibrillation decreased with IP intraaortic balloon pump therapy. We report the first case of malignant ventricular fibrillation after myocardial infarction which was uncontrolled with conventional therapy, but effectively treated with ICD. Reverse polarity로는 20 J로 낮아졌으나 광범위한 심근경색 부위등을 고려하여 MicroJewel Ⅱ 대신에 최고 35 J까지 줄 수 있는 Gem SR을 시술하게 되었다. 결정된 DFT 20 J에 안전역으 로 15 J을 더하여 35 J를 제세동 에너지로 설정하였다. 였고 마찬가지 방법으로 정상 동율동으로 호전되었다. Generator에 심실제세동기 전극도자의 근위부 끝을 연 이후 입원 14일째 삽입형 심실제세동기 시술전까지 심 결하여 피하층에 만든 포켓(pocket)에 삽입하였다. 삽 실빈맥 및 심실세동은 발생하지 않았다. 입원 14일째, 입형 심실제세동기 시술한 당일 수시간이 지나서 빠른 심근경색후 허혈상태의 교정 및 항부정맥제의 투여에 심실빈맥과 심실세동이 3회 발생하였고 다음날인 입원 도 재발하는 악성 심실세동에 대한 치료로서 삽입형 심 15일째, 약 10시간에 걸쳐 심실세동이 43회 발생하였 실제세동기를 시술하였다. 전신마취하에 좌측 쇄골하정 다. These electrocardiograms show three episodes of spontaneous fast ventricular tachycardia or ventricular fibrillation and their successful terminations by the shocks from ICD.

Procainamide는 환자의 혈압, 좌심실 퇴원후 외래에서 amiodarone 투여하면서 경과 관찰중 기능 저하 등을 고려하여 짧은 시간내에 부하용량의 투 퇴원후 8개월째 비지속성 심실빈맥이 2차례 발생하였 여없이 분당 1 mg의 정주속도로 추가투여하였으나 효 으나 각각 31초와 8초동안만 지속되어 generator로부 과가 없었다.

These electrocardiograms show three episodes of spontaneous fast ventricular tachycardia or ventricular fibrillation and their successful terminations by the shocks from ICD. 정주로 변경하여 150 mg 투여후 분당 1 mg씩 정주하 고 관상동맥내 혈류를 좋게 하였다. 이후로 심실세동은 였다. 그러나 빠른 심실빈맥이 매우 조기에 나오는 심실 다시 발생하지 않았다. 입원 18일째, 대동맥내 풍선펌 기외수축에 의하여 유발되어 procainamide를 추가하 프를 제거하였으나 심실세동은 다시 발생하지 않았다. 였다(Fig. 5).
