Cumulative dose-related cardiotoxicity is a well-known toxicity of anthracyclines. Particularly, at total doses of more than 550 mg/m 2, therapy with anthracyclines can produce irreversible cardiac injury. Anthracycline-induced cardiac toxicity usually manifests by congestive heart failure or arrhythmia. In contrast, acute myocardial infarction is a rare event of anthracycline-induced heart diseases. A 31-year-old man with non-hodgkin lymphoma NHL and a single cardiac risk factor, smoking, presented with chest pain after receiving a 2 nd round of CEOP-BLAM chemotherapy. An electrocardiogram revealed ST segment elevation in the inferior leads consistent with acute myocardial infarction. An echocardiogram revealed an ejection fraction of 60% and severe hypokinesia in the inferior wall. Three days later, coronary angiography revealed 50% of luminal stenosis of right coronary artery RCA and near total occlusion with a large thrombi in the m-rca. Following balloon angioplasty and stent insertion, the patient was transferred to the coronary care unit and continuous intravenous heparin infusion was started. On the 10th day, the patient was discharged in good condition. Six months later, follow-up coronary angiography showed no significant lesion in the right coronary artery. In a young man with NHL, we report an acute myocardial infarction after a 2nd course of CEOP-BLAM chemotherapy with a review of the relevant literature. 시행한 심초음파에서 하벽의 운동저하 소견은 계속적 으로 관찰되었으나 좌심실기능을 정상이었다. 6개월 후 에 다시 시행한 관상동맥조영술 검사에서 우관상동맥 에는 혈전이 발견되지 않았고 잔존협착도 발견되지 않 았다(Fig. 3). 현재 환자는 외래에서 추적관찰 중이며 증상 없이 정상적인 생활을 영위하고 있다.

3 시행한 심초음파에서 하벽의 운동저하 소견은 계속적 으로 관찰되었으나 좌심실기능을 정상이었다. 6개월 후 에 다시 시행한 관상동맥조영술 검사에서 우관상동맥 에는 혈전이 발견되지 않았고 잔존협착도 발견되지 않 았다(Fig. 3). 현재 환자는 외래에서 추적관찰 중이며 증상 없이 정상적인 생활을 영위하고 있다. 고 찰 Anthracyclines는 항종양 항생제로 doxorubicin, dafig. 2. no residual lesion after balloon angioplasty and stenting on right coronary angiogram. unorubicin, idarubicin, epirubicin, pirarubicin 등이 있으며, 이중 doxorubicin는 광범위한 효능을 보이는 항 종양제이다. 이 약물은 유방암, 방광암, 폐암, 위암, 전 립선암, 호즈킨과 비호즈킨 림프종 및 뼈와 연조직 육 종의 치료에 이용된다.2) 그러나 anthracyclines의 우수한 치료효과와 광범위 한 임상적응증에도 불구하고 장기간 투여할 경우 누적 용량 의존성 심독성이 나타나기 때문에 임상에서 그 적 응증을 확대하는데 제한이 있었다. anthracyclines의 심 독성은 임상적으로 세 가지 형태로 나타나는데, 첫 번 째 양상은 약물 투여중이나 투여 후 수일 내지 수주 후 에 나타나는 급성 또는 아급성 심독성의 형태로 일시적 Fig. 3. Follow-up coronary angiogram at 6 month after stenting revealed no significant lesion. 인 부정맥, 심외막염, 심근염, 급성 좌심실 부전증 등으 로 나타나며 드물게 나타난다. 두 번째는 anthracy- 행하였다. 유로키나제 투여 30분 후에 증상은 소실되었 clines이 만성적인 심독성을 일으켜 궁극적으로 심근병 고 ST분절의 하강을 보였다. 심근 효소는 다음날 최고 증을 발생시키며, 임상적으로는 가장 흔하게 나타나는 치를 보였고 3일후 정상화되었다. 심혈관계 중환자실에 형태이다. 세 번째는 anthracyclines을 투여하고 수 년 서 집중치료를 시행하고 3일 후 관상동맥조영술을 시 내지 수십년 경과후 나타나는 지발성 심독성의 형태가 행하였다. 우측 요골동맥 접근법을 통해 시행한 관상동 있는데 대개 장기간의 무증상 기간을 거쳐 나타나며 최 맥조영술 검사에서 우관상동맥 중간부위에 50%의 내 근 그 빈도가 증가하고 있다.3)4) 강협착이 있었고 내강을 거의 폐쇄시키는 거대한 혈전 이 발견되었다(Fig. 1). 본 증례에서처럼 anthracyclines의 심독성으로 심근 경색이 합병된 예는 극히 드물며, 외국 문헌에는 몇 례가 시술 전에 18 mg의 Reo-pro를 정주한 후 풍선도자 보고된 적이 있으나 국내에는 아직 보고된 예가 없다. 문 로 풍선 확장술을 시행하였고, 확장 후에도 50%의 잔 헌검색에서 찾은 adriamycin에 의한 급성 심근 경색의 존협착이 있어 금속망을 삽입하였다(Fig. 2). 우심실 분 첫 증례는 1984년에 Asamoto등이 28명의 원발성 폐 지와 후하행 분지에 혈전이 색전되어 시술이후에도 계 암환자를 대상으로 시행한 cisplatin 기초 복합화학요법 속적으로 Reo-pro를 투여하였고, 이후 심혈관 중환자 연구로, 선암과 대세포암 환자 4명에게 추가로 adria- 실에서 heparin을 계속 정주하면서 집중 치료를 시행하 mycin 30 mg/m2과 mitomycin C 4 mg을 투여한 후 였다. 이후 흉통은 없었으며 심전도 변화도 관찰되지 않 심근 경색으로 사망한 1례를 보고하였다.5) 이외에도 비 아 ticlopidine, atenolol, captopril, aspirin, nitrate, wa- 소세포성 페암환자, 진행성 위암 및 전이된 유방암 환자 rfarin, folic acid를 투약하고 퇴원하였다. 2주후에 다시 를 대상으로한 복합화학요법 연구에서 심근 경색이 합병 509
