To resolve this problem, the liquid based cytology (LBC) method was developed. The LBC method reduces the number of false positive and false negative smear results because LBC achieves an even distribution of monolayered cells, eliminated the obscuring effects of inflammation and blood. Although the LBC method has many advantages, there are several drawbacks. The LBC method requires an adaptation period for cytopathologists and cytotechnicians. Another drawback is the expense of the method. Thus, the LBC method has been questioned, and criticism has been raised regarding the design of the studies that assert its superiority. With a focus on the cytomorphologic and technical differences of LBC compared with conventional cervical smears, a review of the clinical and cost effectiveness of LBC, a brief comparison of two popular LBC methods, and the basic concepts of study design with respect to LBC are presented in this review. Key Words : Cervix uteri; Cytology; Papanicolaou test; Liquid based cytology 자궁경부세포진검사는자궁경부암의발생빈도를줄이는데큰역할을해온중요한탈락세포검사이다. 이러한자궁경부세포진검사는오랜기간동안고전적인세포도말법을이용하여시행되어왔으나위음성률이높은단점을가지고있다. 도말과정중세포가건조되거나변성또는중첩되는현상, 혈성또는점액성삼출물과같은세포관찰에방해가되는물질들로인한부적절한검체생성률의증가등과같은단점을가지고있다. 액상세포검사법 (liquid based cytology, LBC) 은자동화선별검사방법 (automated screening devices) 의발달에따라세포의중첩현상을최소화함으로써고전적세포도말법의단점을보완한방법이라할수있는데이는액상고정액을이용하여세포를단층으로펴서슬라이드상의중앙원안에붙도록하는방법이다. 액상세포검사법은고전적세포도말법에비해세포소견을자세히살펴볼수있고, 혈성삼출물의제거로깨끗한배경을갖게되며선별검사시간 (screening time) 의단축, 검사방법과진단기준의표준화와같은장점이있다. 그외에도부적절한검체수의감소, 진단율의정확도상승및이에따른재검사방지로인한진단소요일수의단축, 의료비감소효과와같은장점도있다. 그러나액상세포검사법의단점도동시에지적이되고있는데액상고정액에따른개개의세포모양변형, 세포군집의형태변형, 배경물질의소실등과같은변화에따라기존의세포도말소견에친숙한세포병리사나세포병리진단의사에게위양성, 위음성률을높일가능성이있으며검사비용이비싸다는단점을가지고있다. 그럼에도불구하고앞서언급한여러가지장점에힘입어현재액상세포검사법은국내에서도많은병원에서도입하여이용되고있으며 2006년부터는자궁경부세포진검사의액상세포검사법에대하여건강보험급여가인정되고있다. 본논문에서는액상세포검사법의종류및원리에대한간단한비교, 액상세포검사법과고전적세포도말법의기술적및형태학적인차이점, 액상세포검사법의임상적유용성및경제적효용성에대한과거연구들의고찰, 액상세포검사법의장단점, LBC 방법을이용한연구유형에대한간단한고찰을하고자한다. 본론액상세포검사법의종류및비교이제까지널리이용되고있는액상세포검사법에는 ThinPrep (TP) 방법 (Cytyc Corp., Boxborough, MA, USA) 과 AutoCyte 291

2 292 이용희 Prep (SurePath, TriPath Imaging, Burlington, NC, USA) 이있다. 1996년미국의식약청 (FDA) 에서 ThinPrep을고전적자궁경부세포도말법을대체할방법으로승인한후연이어 1999년에는 SurePath (SP) 로현재알려져있는 AutoCyte Prep도미국의식약청의승인을받았다. 두가지검사법모두기구는다르나비슷한검사결과를도출하였다. 이들두제품의특징을비교하면다음과같다 (Table 1). 액상세포검사법과고전적세포도말법과의차이점 세포밀도 고전적자궁경부세포진에서의적정세포밀도는약 8,000-12,000개의잘보존되고잘보이는편평상피세포가슬라이드표면의 10% 이상에서도말되어있어야하는것으로규정되어있으나액상세포검사법의경우통상도말의세포밀도기준을적용할수없으므로 5,000개이상의잘보존되고잘보이는편평상피세포를최소의기준으로한다. 원의중앙을관통하여 10개 Table 1. LBC Conventional smear Quality Enhanced Variable Background Clean Yes No RBCs Reduced Present/usually obscure Neutrophils Reduced Present/usually obscure Necrosis Clumped Diffuse/usually obscure Cellularity Lower Higher Cell distribution Uniform Uneven, thick Cell size Smaller Larger Architecture Less preserved Preserved Cytomorphology Preserved Preserved Extracellular material Quantity Reduced Preserved Mitoses Preserved Preserved 이상의시야를계량하여야하며 5,000개미만의세포가도말된경우기술적문제로채취표본의세포가다도말되지않았다고생각되면다시슬라이드를제작하도록한다. 1 자궁내경부 / 변형대성분 (Endocervical/Transformation Zone Component) 고전적세포도말법에서는내경부세포가 5개이상인군집이 2 개이상이어야하는데반해액상세포검사법의경우세포군집보다는낱개로세포들이도말되므로최소한 10개이상의내경부세포가있어야적절한것으로판단한다. 폐경기여성, 산후, 프로게스테론제제사용시보이는방기저세포는화생세포와혼동하여변형대성분으로간주하여서는안되며편평세포화생은분명하게인지될경우만내경부세포성분의일부로서사용할수있다. 불확실한경우는변형대의존재를알기어렵다고보고하여야한다. 문헌상자궁내경부 / 변형대성분이있는경우에편평상피내병변 (squamous intraepithelial lesion, SIL) 이더많이발견된다. 기술적및형태학적차이점대부분의형태학적인특징은두가지검사법, 즉고전적인자궁경부세포진검사법과액상세포검사법모두유사하나다른점을열거하면표와같다 (Table 2, 3). 액상세포검사법의경우, 세포들은검체채취후액상고정액에즉시고정되어용액안에세포들이부유하는상태이므로세포들은전반적으로고정도가향상된다. 세포질이나핵의모습을좀더상세히관찰할수있어서고전적인자궁경부세포진검사법에의해고정된세포보다더나쁘게해석되는성향이있으므로검사법을전환할경우반드시훈련기간 (learning period) 이필요하다. 또한세포는좀더작게 Table 3. 293 자궁경부의 액상 세포검사 보이고, 흔히 개개로 흩어진 자궁 내경부 세포를 볼 수 있으며 이 들은 미숙한 편평상피화생세포(immature squamous metaplastic cells), 저장세포(reserve cells) 혹은 조직구와 유사하게 보일 수 있다.

형태학적인 차이점 외에 두 가지 검사법의 기 술적인 차이점을 살펴보면 다음 표와 같다(Table 4, Fig. 1). Table 4.

3 293 자궁경부의 액상 세포검사 보이고, 흔히 개개로 흩어진 자궁 내경부 세포를 볼 수 있으며 이 들은 미숙한 편평상피화생세포(immature squamous metaplastic cells), 저장세포(reserve cells) 혹은 조직구와 유사하게 보일 수 있다. 세포군집은 더욱 삼차원적으로 보이고 두껍게 도말된 세포 군집의 변연부에 존재하는 세포는 대개 세포군집 안에 존재하는 세포의 정체를 파악하는데 도움을 주는데 이들 세포군집의 변연 부 세포가 대개 특징적인 소견을 보이지 않는 성향이 있어 세포 의 정체를 판단하는 것이 고전적 자궁경부세포진 검사법에 비해 어려운 점이 있다. 형태학적인 차이점 외에 두 가지 검사법의 기 술적인 차이점을 살펴보면 다음 표와 같다(Table 4, Fig. 1). 294 이용희 편평상피내병변의 형태 액상세포검사법에서 관찰되는 편평상피화생세포는 판상 혹은 자갈돌(cobblestone) 모양으로 배열하며 세포질이 짙고 균질하 며 세포질 내 공포 형성이 많이 되는 성향이 있다(Fig. 2).

세포의 기계적 변형이 동반되 지 않는다(Fig. 3). 세포는 고정액 내에 부유하는 상태이므로 세포의 변연부가 둥글어지는 형태를 띠며 또한 염색양상이 다양 하게 나타날 수 있는 성향이 있어 같은 SIL이라고 하더라도 저 염색성(hypochromatic) 혹은 과염색성(hyperchromatic)으로 나타날 수 있다.

세포질의 경계는 좀 더 뚜렷하게 구분 되며 자궁내막 세포들의 핵에 비해 좀 더 커다란 핵을 보인다. 침윤성 편평세포암종의 경우 괴사성 배경이 덜 할 수는 있으나 세포군집의 변연부에 괴사성 물질이 붙는 성향이 있다(clinging diathesis, Fig. 2).

자궁내 막 샘상피세포는 촘촘한 삼차원적 구조의 세포군집 형태가 유지 되나 세포군집을 이루는 세포가 성글게 배열하며 세포질 내에는 공포가 관찰된다(Fig. 3). 고전적 자궁경부 세포진 검사법에 비 해 역시 낱개의 세포로 흩어지는 경향이 있다.

4 294 이용희 편평상피내병변의 형태 액상세포검사법에서 관찰되는 편평상피화생세포는 판상 혹은 자갈돌(cobblestone) 모양으로 배열하며 세포질이 짙고 균질하 며 세포질 내 공포 형성이 많이 되는 성향이 있다(Fig. 2). 저등 급 편평상피내병변(low grade squamous intraepithelial lesion, LSIL) 중 인체유두종바이러스 감염에 의한 공동세포 비정형 (koilocytotic atypia)과 공동성 변화(human papilloma virus cavitation)는 더욱 선명하게 나타나며 고전적 자궁경부세포진 검사법의 세포 압축 도말에 따른 세포의 기계적 변형이 동반되 지 않는다(Fig. 3). 세포는 고정액 내에 부유하는 상태이므로 세포의 변연부가 둥글어지는 형태를 띠며 또한 염색양상이 다양 하게 나타날 수 있는 성향이 있어 같은 SIL이라고 하더라도 저 염색성(hypochromatic) 혹은 과염색성(hyperchromatic)으로 나타날 수 있다. 고등급 편평상피내병변(high grade squamous intraepithelial lesion, HSIL)의 경우 통상의 자궁경부세포진처 럼 판상 혹은 합포체성(syncytial) 세포군집을 유지하기도 하나 개개의 미성숙한 세포형태로 흩어지는 성향이 있기도 하며 이를 clue cell 이라고도 한다. 세포질의 경계는 좀 더 뚜렷하게 구분 되며 자궁내막 세포들의 핵에 비해 좀 더 커다란 핵을 보인다. 침윤성 편평세포암종의 경우 괴사성 배경이 덜 할 수는 있으나 세포군집의 변연부에 괴사성 물질이 붙는 성향이 있다(clinging diathesis, Fig. 2).1 샘상피내병변의 형태 자궁 내경부 샘상피세포는 고전적 자궁경부세포진처럼 벌집 모양(honeycomb)과 관병식상(palisade)배열을 유지하며 이들 의 세포군집은 좀 더 촘촘히 모이며 접히는 성향을 띤다. 자궁내 막 샘상피세포는 촘촘한 삼차원적 구조의 세포군집 형태가 유지 되나 세포군집을 이루는 세포가 성글게 배열하며 세포질 내에는 공포가 관찰된다(Fig. 3). 고전적 자궁경부 세포진 검사법에 비 해 역시 낱개의 세포로 흩어지는 경향이 있다. 자궁경부 샘상피 내암종(adenocarcinoma in situ, AIS)의 경우 고전적 자궁경부 세포진에서 보이는 비정상적 구조(abnormal architecture)를 볼 수 있다. 즉 핵은 중첩되고 염색질은 진하게 염색되며 핵의 거 A B C D Fig. 2. 295 자궁경부의 액상 세포검사 짓중첩현상(pseudostratification), 세포군집 변연부의 깃털모양 형태(feathering), 샘상피내암종 세포들의 장미모양(rosette) 혹 은 관병식상 띠 모양 배열(palisaded strips)을 나타낸다. 또한 유사분열이나 세포자멸사체(apoptotic bodies)가 관찰되기도 한 다.

2 액상세포검사법의 임상적 유용성에 대한 고찰 1997년 Lee 등에 의한 연구를 근간으로 ThinPrep의 미국 식약청의 승인이 이루어졌는데 이 연구에서는 3개의 검진센터 (screening center), 3개의 대학병원 검사실으로부터 분할시료 (split sample)를 이용한 연구를 하였다.3 이 연구에서는 TP소 Table 5.

역시 두 가지 방법 모두에서 크게 다르지 않다(Fig. 4, 5). 자궁경부 액상세포검사시 흔히 감별해야 하는 진단의 세 포학적 소견을 살펴보면 다음과 같다(Table 5-7).

5 295 자궁경부의 액상 세포검사 짓중첩현상(pseudostratification), 세포군집 변연부의 깃털모양 형태(feathering), 샘상피내암종 세포들의 장미모양(rosette) 혹 은 관병식상 띠 모양 배열(palisaded strips)을 나타낸다. 또한 유사분열이나 세포자멸사체(apoptotic bodies)가 관찰되기도 한 다. 그러나 이 중 세포군집 변연부의 깃털모양 형태는 액상세포 검사법에서 고전적 자궁경부세포진 검사에 비해 덜 뚜렷한 성향 을 보인다고 하며 고전적 자궁경부세포진이나 SP에서는 커다랗 게 짙게 염색되는 세포군집과 낱개로 흩어진 종양세포들이 TP 보다 자주 나타난다는 보고가 있다(Fig. 3).2 액상세포검사법의 임상적 유용성에 대한 고찰 1997년 Lee 등에 의한 연구를 근간으로 ThinPrep의 미국 식약청의 승인이 이루어졌는데 이 연구에서는 3개의 검진센터 (screening center), 3개의 대학병원 검사실으로부터 분할시료 (split sample)를 이용한 연구를 하였다.3 이 연구에서는 TP소 Table 5. Ferenczy 등의 연구에서는 TP 방법이 고전적 자궁경부세포진 검사법에 비해 민감성과 특이도 에 있어서 경미한 정도의 향상은 있으나 통계학적으로 유의한

Hutchinson 등은 자궁 경부암의 빈도가 높은 Costa Rica지 역의 여성을 대상으로 분할시료를 이용한 연구를 한 바 TP 방법 이 고전적 자궁경부세포진 검사법에 비해 HSIL 검출률의 민감 도가 의의있게 상승하였으나 특이도는 유의하게 낮추었다는 결 과를 내놓았다.

Hutchinson 등은 자궁 경부암의 빈도가 높은 Costa Rica지 역의 여성을 대상으로 분할시료를 이용한 연구를 한 바 TP 방법 이 고전적 자궁경부세포진 검사법에 비해 HSIL 검출률의 민감 도가 의의있게 상승하였으나 특이도는 유의하게 낮추었다는 결 과를 내놓았다.7 이러한 양상은 Koss8나 Selvaggi와 Guidos9의 연구결과에서도 증명이 되었는데 액상세포검사법의 적정성은 시 료 채취기구(collection instruments)와 검사자의 기술(skills of the provider)에 의해 크게 좌우된다고 할 수 있겠다. Bishop 등 은 SP 방법으로 분할시료를 이용한 연구를 하여 미국 식약청의 승인을 받았는데 SP 방법이 고전적 자궁경부세포진 검사방법에 비해 LSIL 검출률의 의의있는 상승(31%)을 가져왔다고 하였으 며 시료의 적정성에 있어서도 유의한 향상을 보였다.10 이 밖에 도 분할시료 혹은 direct to vial sample을 이용하여 TP 또는 SP와 고전적 자궁경부세포진 검사방법에 관한 비교 연구가 많이 있었고 공통적인 결과는 HSIL 검출률이 약 60%까지도 향상된 다는 결과를 미국 식약청에 제출하였다는 점이다 자궁경부 및 내막 샘암종 병변에 있어서 고전적 자궁경부세포 진 검사방법에 비해 TP를 이용 시 비정상 세포를 검출하는 비율 이 향상된다는 연구들이 있으며20-22 최근 Belsley 등은 고전적 자궁경부세포진 검사방법과 TP, SP 세 가지 방법을 모두 이용 하여 자궁경부 샘암종에 대한 연구를 한 바 짙게 염색되는 양상 의 커다란 판상의 세포와 개개로 흩어지는 단일세포가 TP 방법 에 비해 SP와 CS에 더 흔히 관찰되며 세포군집 변연부에 나타 나는 깃털양상은 두 가지 액상세포검사법에 비해 고전적 자궁경 부세포진 검사방법에서 좀 더 자주 관찰된다고 보고하였다.23

8 298 이용희 액상세포검사법의장단점액상세포검사법의장점액상세포검사법은검체를채취하는의사들에게있어서취급이용이하며검사실로의검체수송또한용이하다. 검체가채취즉시고정되므로세포검체가마르지않는장점이있고검체내의혈성삼출물이나염증세포동반에의한부적절한시료의수가감소한다는장점이있다. 또한검사실마다보고의차이는있으나비정상세포수의검출률이증가되는장점과선별검사시간의단축으로세포병리업무의효율성을높일수있으며잔존시료를이용한보조적추가검사 ( 면역염색혹은분자병리검사 ) 가가능한최대의장점이있다. 또한 TP는 3주, SP는 4주간상온보관이가능하므로진단이후에도액체고정액내에남아있는세포검체로추가슬라이드제작및연구가가능하다. 액상세포검사법의단점액상세포검사법은세포병리전문판독의나세포병리사모두에게새로운검사법으로의전환에따른세포의형태학적변화에의적응까지어느정도훈련기간이필요하다는단점이있으며, 도말되는세포집단이여러층을형성하는경향이있으며촘촘하게자리잡은세포집단의경우해석이어려울경우가종종발생한다는단점이있다. 또한기존보고의경우고전적검사법에비해진단특이도가낮다는보고가있으며고전적검사법에비해비싸다는단점이있다. 액상세포검사법의경제적효용성에대한고찰액상세포검사법의경제적효용성에관하여서는두가지다른관점이있다. 개개의검사센터의입장과공중보건정책결정자의입장이다. 공중보건정책결정자의입장에서는병변세포검출률이향상되는이점, 검진기간간격의연장, 질경검사 (colposcopy) 이용의비용감소등의이점에반해, 그에상응하는액상세포검사법의경제적비용상승의정도를저울질해보아야하는것이다. 24 이에관련하여서세계각국에서많은연구들이시행되어왔다. Klinkhamer 등은과거 TP, SP, 고전적자궁경부세포진검사방법을이용한연구들을재분석하여 TP 방법이 SIL 검출에있어서민감도가향상되고 ASCUS 비율을감소시켰다고하였다. 또한고전적자궁경부세포진검사방법과비교시비슷한특이도를보였다는결론을내렸으며, TP 방법의비용과이점을고려할때네덜란드에서 TP 방법을대단위인구를대상으로한검진프로그램으로이용할지의여부에대해서는좀더면밀한연구가진행되어야할것을제시하였다. 25 Negri 등도비슷한결론을주장하였으며 26 Chacho 등은 TP 방법을이용시침윤성암종을놓칠가능성이있다고하였다. 27 또한최근자궁경부암검진을위한유럽연합의지원을받아이루어진 Arbyn 등의연구에서는액상세포검사법이고전적자궁경부세포진검사방법에 비해 HSIL 검출성적이민감도나특이도모두우수하다고볼수없다는연구결과를제시하였다. 28 개개의검사실의입장에서고려해야할사항을살펴보면다음과같다. 29 비용검사장비비용및재료비는시행되는검사건수, 병리사의검체준비소요시간을고려해야한다. TP는일회용품 (disposables) 이더비싸지만 SP는검체준비시간이더길게소요된다. 잠재적비용절약효과부적절한검체율이감소되며생산성은증가된다. 잠재적이득효과병변세포검출률의증가, indeterminate (ASC) 진단율의감소, 인체유두종바이러스의아형결정, 자동화선별검사이용의접근성이있다. LBC 시료를대상으로한연구계획시고려해야할사항 LBC 방법이비용상고전적방법에비해비싸므로 LBC의임상적유용성에대해많은연구가있어왔고또한진행중이다. 많은연구들이 LBC 방법이임상적으로유용하다는것을증명해왔다. LBC 도입에따른검체의적정성의향상및병변의검출률의향상은 LBC 시행이전의고전적자궁경부세포진의질에의해좌우된다. 고전적자궁경부세포진과 LBC 간의임상적유용성에대한연구는몇가지유형으로나누어볼수있으며이러한연구의평가에여러가지인자를고려하여야한다. 30 고려해야할인자로는다음과같은사항이있을수있다. 연구유형 (Study design) 동일환자를대상으로한연구 (Same patient type study) 비교해야할시료를동일한환자에게서채취할경우분할시료라고하며이경우는환자가병원을방문시두개의시료를따로얻거나혹은고전적자궁경부세포진을도말후채취기구상에남아있는잔존세포를 LBC 시료로사용할경우이다. 후자의경우는 LBC 방법의효과를보는연구방법으로서는바람직하지않은연구방법이라할수있겠다. 6 다른환자를대상으로한연구 (Different patient type study) 코호트연구 (Cohort study) 나전향적연구 (prospective study) 가여기에해당하는데비교해야할시료를각각다른환자로부터채취할경우로서 LBC 시료를과거에시행한고전적자궁경부세포진시료와비교하는경우는 Cohort study, 전향적인방식 (prospective manner) 으로무작위로환자에게서 LBC나고전적자궁경부세포진시료를채취하여시행하는연구방법은전향적연구에해당한다고볼수있겠다.

9 자궁경부의액상세포검사 299 Gold standard LBC 시료를비교할신뢰할만한참고검사법 (reference test) 으로자궁경부세포검체채취후연이어조직생검시행을통한확인, 임상적인추적검사, 세포병리의사에의한재검토 (adjudicated review) 가여기에해당한다. 연구모집단내에서의질병의이환율 (Prevalence of disease in the study population) SIL의이환율은연구모집단의유형에따라다양하다. 즉선별검사대상의모집단의자궁경부질환의위험도가낮은군, 높은군혹은이미자궁경부에병변이있어서큰병원으로전원된환자군인지에따라모집단의이환율은다를수밖에없고이를연구결과분석에고려해야한다. 결 부인과영역의액상세포검사법은경제적효용성면에서는아직도논란의여지가있으나우리나라는 2006년부터건강보험급여가됨에따라이미많은기관에서도입하여시행되고있다. 액상세포검사법이고전적자궁경부세포진보다임상적유용성면에서 HSIL 검출률증가의효과가있는지, 그리고특이도와민감도를향상시키는지에관해서는아직도논란의여지가있으나 LSIL의검출률증가, 적정시료비율의증가, 부적절시료비율의감소, 잔존시료를이용한인체유두종바이러스및기타감염균의분자병리학적검사의용이한접근도, 대량자동화장비에의접근성등과같은장점을가지고있어더욱많은기관으로의확산을예상할수있다. 경제적효용성면에서일치된견해는대학병원검사실보다는검진센터와같은대량시료를대상으로하는검사실에더유용하다는점이다. 진단적인측면에서볼때대부분의형태학적소견은고전적자궁경부세포진과유사한소견을보이나침윤성암종의경우괴사성물질의배경이덜뚜렷한점, 판상세포군집이없이개개의흩어지는형태로의비정형편평상피세포만이소수관찰될때 HSIL 진단을놓칠확률, 깃털양상의세포군집변연부양상이뚜렷하지않은성향으로자궁경부샘상피내암종 (adenocarcinoma in situ) 을놓칠확률을염두에두고자궁경부액상세포진단에임해야할것이다. 론 참고문헌 1.
