Until now, the ideal management for subdural hygromas in adults seems controversial, because of their differences between the effectiveness and the risk of complications. Thus, we evaluated the role of unvalved SPS for subdural hygromas in adults. Methods: From January 2001 to December 2007, fifteen patients who had undergone unvalved SPS for symptomatic subdural hygromas in adults at our hospital were retrospectively reviewed. We analyzed the age, gender, initial diagnosis, causes, symptoms, neurological signs, and radiological findings such as thickness of subdural hygroma. Outcome was evaluated according to the post operative Glasgow Outcome Scale (GOS), radiologic findings and complications. Results: The mean age of the patients is 61 years old (range from 29 to 86). We used the unvalved open-ended catheter to all patients. After SPS, all subdural hygromas disappeared on computed tomography scans within 14 days (mean 6.5 days) except 2 patients. Four patients had GOS 5. GOS 4 was present in three patients. GOS 3 was present in four patients. Three patients had GOS 2. One patient had GOS 1. Complications related to SPS occurred in 5 patients (33%). Conclusion: Our results suggest that unvalved SPS is one of the effective and safe treatment modality for symptomatic subdural hygromas in adults. The overall complication rate is relatively low, including infection and obstructions, compared to previous reports. 2 Ki Sung Yoon, et al. 하나이다. 5) 현재까지, 뇌경막하수종이있는영아혹은소아군의치료로 SPS에관한보고가일부발표되었다. 2-5,7-) 하지만, 본저자들이조사한바로는성인에서발생한뇌경막하수종의치료로 SPS에관한임상연구보고는국내에서뿐만아니라국외에서도발표되지않았다. 이에저자들은본원에서뇌경막하수종을가지고있는성인에서시행한 SPS의유용성과합병증을신경학적, 방사선학적소견을바탕으로후향적으로분석하여보고하고자한다. 대상및방법 2001년 1월부터 2007년 월까지본교실에서 SPS를시행한 15명의성인뇌경막하수종환자들을대상으로후향적으로분석하였다. 뇌경막하수종환자의나이, 성별, 초기진단, 증상및징후를분석하였다. 더불어뇌경막하수종의위치, 두께및 Hounsfield scale을측정하였고, SPS 시 행이전에환자들이받은치료를분석하였다 (Table 1). 본교실에서는뇌경막하수종환자중지속적인두통, 의식변화, 경련및편측마비등이있는환자에서 SPS를시행하였다. 또한추적관찰기간중뇌경막하수종이증가하여뇌경막하수종의두께가 10 mm가넘는경우에는증상에관계없이시행하였다. 뇌경막하수종의두께측정은뇌전산화단층촬영 (CT) 상가장두꺼운부분으로정하였다. 편측혹은양측의 SPS가뇌경막하수종의위치에따라시행되었다. 본교실에서는모든환자에서 valve가없는 distal open-end 도관을사용하였다. SPS는다음의방법으로시행하였다. 두피절개후에뇌경막하수종이가장두꺼운두개골부위에천공술을시행한후, 경막절개를시행하였다. 도관의 proximal tip을약 5~8 cm 경막하공간으로삽입하였다. Distal open-end 도관은 passer를통해서피하를통과한후배꼽주변에서복막하공간을통하여복강안으로삽입되었다. Subduro-Peritoneal Shunt in Adults imal 도관과 distal open-end 도관의연결에사용되었다. 수술후평가는 Glasgow Outcome Scale (GOS), 수술후추적관찰한 CT에서뇌경막하수종의두께를측정한뇌경막하수종의완해 (resolution) 정도및합병증발생여부를통해이루어졌다.

이중 10명 (67%) 의환자에서개두술이시행되었으며, 뇌실-복강단락술이시행된환자는 6명 (40%) 이었다. SPS 시행전에 8명 (53%) 의환자는뇌경막하수종에대해서일시적인천공술을통한배액술을시행받았다. 8명모두일시적인천공술을통한배액술이후에도뇌경막하수종의완해를보이지않았다. 7명 (47%) 은뇌경막하수종에대한선행치료없이 SPS가시행되었다.

입원일로부터 SPS가시행되기까지의기간은다양하였고, 평균 30일이었다 (median; 23일, 범위 ; 1~74 일 ). SPS 수술후평균추적관찰기간은평균 개월 ( 범위 ; 1~42 개월 ) 이었다.

3 Subduro-Peritoneal Shunt in Adults imal 도관과 distal open-end 도관의연결에사용되었다. 수술후평가는 Glasgow Outcome Scale (GOS), 수술후추적관찰한 CT에서뇌경막하수종의두께를측정한뇌경막하수종의완해 (resolution) 정도및합병증발생여부를통해이루어졌다. 결과 본원에서 SPS를시행받은환자들의평균나이는 61세 ( 범위 ; 26~86세 ) 였다. 그중 10명이남자, 5명이여자로남녀의비는 2:1 이었다. 뇌경막하수종에대해서 SPS를시행한환자들의선행진단은다음과같았다. 외상 4명 (26%), 뇌동맥류파열에의한뇌지주막하출혈 5명 (33%), 뇌졸중 3명 (20%), 그리고뇌종양 3명 (20%) 이었다. 이중 10명 (67%) 의환자에서개두술이시행되었으며, 뇌실-복강단락술이시행된환자는 6명 (40%) 이었다. SPS 시행직전의환자의증상및징후는의식저하 (66%), 두통 (20%), 경련 (7%) 그리고언어장애 (7%) 였다. 뇌경막하수종의위치는양측에모두있는경우가 2명 (13%) 이었으며, 편측의경우왼쪽에위치한경우가 5명 (34%), 오른쪽에위치한경우가 8명 (53%) 이었다. 뇌경막 하수종의두께는 10~19 mm (median; mm) 로측정되었다. 입원일로부터 SPS가시행되기까지의기간은다양하였고, 평균 30일이었다 (median; 23일, 범위 ; 1~74 일 ). SPS 수술후평균추적관찰기간은평균 개월 ( 범위 ; 1~42 개월 ) 이었다. 수술후 15명의환자중 4명 (26%) 의환자는 good recovery (GOS 5) 를보였고, moderate disability 는 3명 (20%) 의환자에서보였다 (GOS 4). Severe disability는 4명 (26%) 의환자에서보였으며 (GOS 3), 3 명 (20%) 의환자는 vegetative state (GOS 2), 1명 (7%) 은사망하였다 (GOS 1). GOS 1~3 에해당되었던 8명 (53%) 의환자들은수술전에도 severe disability 이상의신경학적장애소견을가지고있었다. SPS 시행후 2주이내시행한추적두부 CT에서, 10명 (67%) 의환자에서뇌경막하수종이완전히완해되었다 (Figure 1). 추적관찰기간중합병증은 5명 (33%) 의환자에서발생하였다. 2명 (13%) 의환자에서도관이막혔고, 2명 (13%) 에서감염이있었다. 1명 (7%) 의환자에서도관과관련된동통을호소하였다. 이중 GOS 5인환자가 1명 (25%), GOS 4인환자가 2명 (50%), GOS 2인환자가 2명 (77%) 이었다. 통계적인분석은시행하지못하였으나 GOS가낮을수록합병증발병률이높아지는것으로판단된다. 위의 5명의환자에서합병증발생후도관은모두제거되었다 (Table 2). 고찰 1932 년에 Dandy 가뇌경막하수종이라는용어를처음 A B C FIGURE 1. Admission to SPS (day) SPS Post SPS F/U period (month) Out come Last F/U CT finding Complication Unilateral No change 5 Obstruction Unilateral 01 No change 2 Obstruction Unilateral 39 Improved Unilateral 21 Improved Unilateral 01 Improved Unilateral 01 Improved 2 Infection Unilateral 18 Improved Unilateral 02 Improved Unilateral 02 Improved 4 Infection Unilateral 42 Improved 4 Catheter induced pain + 14 Bilateral 01 Improved Bilateral Improved Unilateral 03 Improved Unilateral 23 Improved Unilateral 09 Improved no.: number, SPS: subduroperitoneal shunt, F/U: follow up, CT: computed tomography, GOS: Glasgow Outcome Scale GOS Shunt remove 사용한이후에 2) SDFC, 뇌경막하수종및뇌경막하출혈등이정확한진단기준없이혼돈되어사용됐다.,) 다양한원인이뇌경막하수종을일으키는것으로알려져있으며, 두부손상, 뇌막염, 선행하는외과적수술 ( 개두술, 천공술, 뇌척수액단락술 ) 및선천성기형이포함될수있다. 하지만, 아직도대부분의경우에서선행원인을정확히알지못하고있다. 2,8) 뇌경막하수종의발생기전은아직충분히밝혀져있지않다. 2,3,7) 뇌경막하수종의발생은, 바깥쪽의수막 (dura mater) 과안쪽의지주막 (arachnoid membrane) 사이를나누는어떤면도존재하지않는수막-지주막경계가어떤원인에의해파손되면서공간이형성되고, 이로인해뇌경막하공간으로체액 (body fluid) 이고이게되면서발생한것으로보인다. 2,7,14) 파손된지주막은 ball-valve 장치형태로작동되어수막-지주막공간내의계면층 (interface layer) 의 cohesion 회복을방해하게된다. 13) 따라서, 뇌경막하수종의치료목표는수막-지주막공간의계면층의 cohesion의회복이라고할수있다. 6,7) 뇌경막하수종은경막하공간내뇌척수액이시간이지나면서자연적으로흡수되는경우에는호전되는것으로알려져있다. 6,7) 그러나, 만약뇌경막하수종이너무커서자연완해를기다리기어려운경우에는수술적치료가필요하다. 3) Lee 등 9) 은뇌경막하수종환자를추적관찰한결과, 3개월이내에반수이상의뇌경막하수종이감소되거나완해되는것을보고하였다. 완해나크기의감소가이루어지지않은뇌경막하수종환자의 61.3% 에서는만성뇌경막하 혈종으로변하는것으로보고하였다. 과거에는반복적인경피적경막하천자나개두술및막제거술등이뇌경막하수종의치료로많이시행되었지만감염및수술의위험성때문에현재는거의시행되고있지않다. 현재는일시적인체외배액술및영구적인 SPS가뇌경막하수종의치료로주로이용되고있다. 2) 양측으로뇌경막하수종이있는경우에 SPS를편측만시행할지, 양측모두시행할지에대해서는논란의여지가있다. Aoki 1) 는편측의 SPS만으로도뇌경막하수종이좋아진다고보고하였고, Litofsky 등 10) 은양측혹은편측으로 SPS를한경우에두군간의차이가없다고보고하였다. Vinchon 등 15) 은도관폐쇄가감염보다더많이일어난다고보고하면서, 양측으로 SPS를시행하는것이도관폐쇄의가능성을줄이기때문에더욱좋다고주장하였다. 결국편측혹은양측에 SPS를시행하는것은아직까지논쟁의여지가있다. 3) 본교실에서는뇌경막하수종의위치에따라서 SPS를시행하였다. 양측으로뇌경막하수종이있었던 2명의환자모두에게양측으로 SPS를시행하였으며편측 SPS만시행한경우는없어서두그룹간비교는시행되지못하였다. SPS에서도관의적절한크기및 valve의선택에주의가필요하다. 왜냐하면, 뇌경막하수종내고농도의단백질성분과이와연관된도관폐쇄의위험성때문이다. 8) Korinth 등 8) 은 SPS에서, 낮은압력의 valve를사용해서효율적이고유용한결과를얻었다고보고하였다. 또한 Erşahin 5) 은뇌경막하수종은과다배출 (over drainage) 이크 3

5 Subduro-Peritoneal Shunt in Adults 게문제없기때문에 valve 없이 14명에게 SPS를시행해서좋은결과를얻었다고보고하였다. 본교실에서도도관폐쇄의가능성이적은 valve가없는 distal openended 도관을사용했다. 가장흔히보고되는 SPS와관련된합병증은감염과도관폐쇄이다. 2-4) 감염발생률은 2.7~9% 2,10,15) 로보고되고있으며, 도관폐쇄의발생률은 9~23% 4,10,15) 로보고되고있다. 본교실의분석결과는감염과도관폐쇄가각각 13% 로소아및영아군에서주로이루어진다른저자들의 2,10,14,15) 결과와비교할때비슷한결과를보인것으로판단되었다. SPS를시행한후에삽입된도관의제거는감염혹은경련의위험성때문에이루어지는것으로보고되고있다. 2,4,10) 사실상, 뇌경막하공간에위치한도관주변의반흔형성은근접뇌피질에변화를줄수있고결과적으로 epileptic focus로발전할수있다. 2) 하지만 Litofsky 등 10) 은도관을제거하지않은 37명의경우에도부작용을경험하지않았다고보고하였다. 본교실에서도합병증이발생하지않는한정규적으로도관을제거하지않았고, 도관을가지고있는환자에서이에따른특별한합병증을경험하지않았다. 결론 본저자들의분석결과성인에서발생한잘치료되지않는뇌경막하수종에서 SPS는효과적이고비교적안전한치료방법으로판단된다. 감염과도관폐쇄를포함하는합병증발생률은비교적낮았다. 그러나본연구는비교적적은환자를대상으로하였고대조군간비교분석이시행되지않아향후더욱많은성인환자군에서의 SPS 연구가필요할것으로판단된다. 중심단어 : 경막하수종 경막하복강단락술 무밸브도관.
