4 지방척수수막류의 수술적 치료결과 Fig. 3. Transitional type with dermal sinus tract. 전체 57례 중 39례는 신경학적 결손이 없는 정상상태에 서 수술적 치료를 받았고 수술후 추적관찰 기간도중 새로운 신경학적 결손이 생긴 경우는 39례 중 단 한 례도 없었다. Table 5. Preoperative neurological findings related with patient s age Age Neurological finding Normal Abnormal Total 이들이 수술을 시행받은 시점은 6개월 미만에서 25례로 가 0-6 month 장 많았고 76%인 30례가 적어도 세 미만에서 수술을 시 6-2 month month year year year year 행받았다. 반면 8례에서는 하지의 운동약화나 배뇨-배변장애중 적어도 한가지 결손을 동반하고 있었으며 이 중 8례에서는 자세한 병력청취결과 이러한 신경학적 결손이 수술이전까지 조금씩 악화, 진행되고 있다고 판단되었다. 0례에서는 진 Total 단당시의 신경학적 결손이 이미 출생시부터 있었던 상태로 세 미만의 환아들인 경우 대개 정형외과적 내지는 비뇨기 세에 이르러 갑자기 새로운 신경학적 결손이 생긴 경우였다 과적 기형을 동반하고 있으면서 결손의 정도가 심한상태였 (Table 5). 거나 2세 이상인 경우는 경미한 상태로 더 이상의 악화없이 하지의 운동약화는 모두 세 이전(2, 4, 6, 2개월)에 수 지내왔던 환자인 반면 결손의 진행, 악화가 있었던 8례중 5 술받은 4례에서 수술이후 뚜렷한 호전을 보였으나 배뇨-배 례는 출생당시부터 신경학적 이상소견이 있었던 환자로 본 변장애가 수술후 정상기능으로 회복된 례는 없었다. 원에서 진단을 받을 시점(6, 2, 5개월, 3, 9세)에는 각각 각각 개월과 8개월에 수술받은 환아에서 수술후 요실금 의 증상이 상당부분 악화되어 있는 상태였고 나머지 3례는 의 악화가 있는 것으로 평가되었는데 개월된 남아의 경우 출생당시 정상으로 간주되었다가 각각 6, 8개월, 그리고 7 수술이전에 배뇨-배변장애의 객관적인 평가를 시행하기 어 224 J Korean Neurosurg Soc/ Volume 28 / February, 999
